Prognostic Significance of Absolute Lymphocytic Count Recovery in Pediatric Patients with Acute Lymphoblastic Leukemia at South Egypt Cancer Institute

Background: Leukemia is the most common cause of disease-related death in childhood despite significant improvement in survival and modern risk stratification. Prognostic significance of absolute lymphocytic count (ALC) recovery was studied in pediatric patients with acute lymphoblastic leukemia?(ALL). Methods: One hundred twenty one patients newly diagnosed asALL ≤ 18 yearstreated at pediatric oncology department, South Egypt Cancer Institute from 2011 to 2015 were enrolled. Age, sex, initial total leucocytic count, immunophenotyping, central nervous system (CNS) involvement, mediastinal mass, bone marrow (BM) response on day fifteen and after finishing induction remission were compared to ALC recovery on day 15 (ALC-15) and day 29 (ALC-29) of induction response. Also, prognostic impact of ALC recovery on event free survival (EFS) and overall survival (OS) was evaluated. Results: ALC didn’t show any significant relation to known patients’ prognostic factors. Patients with low ALC didn’t show significant lower response on day 15 and post induction evaluation from those with high ALC (p = 0.74, 0.6 respectively). ALC-15 ≥ 500 cells/uL showed a prognostic significance as patients with ALC-15 500 cells/uL had a poorer outcome (4- EFS 47.8% ± 0.12%) than patients with ALC-15 ≥ 500 cells/uL who carried a better outcome (4-EFS 66.3% ± 0.05%). Neither ALC-500 (day 29) nor ALC-350 (day 15 & day 29) had impact on OS or EFS. In multivariate analysis, ALC-15 500 cells/uL still had a poor impact on outcome (OR 2.56, 95% CI 1.27 - 5.14) with d15-BM response (OR 2.99, 95% CI 1.35 - 6.61) and age 10 (OR 1.96, 95% CI 1.01 - 3.69). Conclusions: ALC-15 has an independent prognostic impact on outcome as well as simple, inexpensive, and easy to interpret. Although, it’s utility in risk stratification of ALL still need a confirmation through a multicenter study with uniform variables, large cohort and long duration of follow up.

Function, kinetic properties, crystallization, and regulation of microbial malate dehydrogenase

Malate dehydrogenase （MDH） is an enzyme widely distributed among living organisms and is a key protein in the central oxidative pathway. It catalyzes the interconversion between malate and oxaloacetate using NAD＋ or NADP＊ as a cofactor. Surprisingly, this enzyme has been extensively studied in eukaryotes but there are few reports about this enzyme in prokaryotes. It is necessary to review the relevant information to gain a better understanding of the function of this enzyme. Our review of the data generated from studies in bacteria shows much diversity in their molecular properties, including weight, oligomeric states, cofactor and substrate binding affinities, as well as differences in the direction of the enzymatic reaction. Furthermore, due to the importance of its function, the transcription and activity of this enzyme are rigorously regulated. Crystal structures of MDH from different bacterial sources led to the identification of the regions involved in substrate and cofactor binding and the residues important for the dimer-dimer interface. This structural information allows one to make direct modifications to improve the enzyme catalysis by increasing its activity, cofactor binding capacity, substrate specificity, and thermostability. A comparative analysis of the phylogenetic reconstruction of MDH reveals interesting facts about its evolutionary history, dividing this superfamily of proteins into two principle clades and establishing relationships between MDHs from different cellular compartments from archaea, bacteria, and eukaryotes.

Labeo victorianus(Boulenger,1901)is one of the endemic fishes in Lake Victoria Basin(LVB)but is now threatened by multiple stressors caused by human *** investigated spatial and temporal variability in food composition and condition of *** in influent rivers of Lake Victoria,*** was done during the dry and wet seasons by *** composition analysis showed that *** is a benthophagus and omnivorous species whose diet is dominated by detritus,periphyton and *** were differences in food composition among rivers,with significant river X season interactions(PERMANOVA F=11.6,df=4,p=0.001),suggesting that the diet depended on prevailing environmental *** turbid rivers,the diet was dominated by detritus while in less turbid rivers it was dominated by insects and *** and mud also formed a significant part of the diet,which was an indication of a limited occurrence of preferable food *** were ontogenetic shifts in food composition(PERMANOVA F=4.6,df=3,p=0.001),but also with a spatial interaction(PERMANOVA F=5.6,df=7,p=0.001),further indicating the role of environmental conditions in determining the diet for different size ***,the fish condition did not differ among *** study shows that turbidity and organic matter and nutrient loading determine the diet of *** in LVB rivers,and provides further justification for the maintenance of water quality as a conservation measure for threatened species.

Inherited Thrombophilia and Early Recurrent Pregnancy Loss among Egyptian Women

Inherited thrombophilia has been implicated as a possible cause of recurrent pregnancy loss. Although numerous studies are available in literature, thrombophilia rate seems to vary fromstudy to another. The aim of our study was to determine the frequency of FII Prothrombin(G20210A), Factor V Leiden (G1691A), as well as methyl tetrahydrofolate reductase (MTHFR?C677T) polymorphisms, protein C, protein S and antithrombin III deficiency in a series of patients with unexplained RPL compared to control. Patients and Methods: 100 patients of unexplained RPL and 43 age-matched healthy controls were investigated for inherited thrombophilia. Results: MTHFR and Factor V Leiden were the commonest gene defects among cases studied (63%, 60% respectively) and control groups (41.9%, 41.9% respectively) (p = 0.019, p = 0.046 respectively). The least common deficiencies were protein S and protein C deficiency in cases (3%, 2% respectively) as well as in controls (1%, 0% respectively). 4 cases were homozygous for MTHFR and 2 cases homozygous for Factor V Leiden mutation. Odds ratio for MTHFR and Factor V mutation was 2.36 and 2.08 respectively (CI 95%). Combined defects were seen in cases and controls (p < 0.05). Conclusion: Our study found an association between MTHFR and Factor V Leiden mutations in patients with unexplained RPL among Egyptian women. Further studies are needed to define the management of genetic thrombophilia in cases of recurrent pregnancy loss.

The impacts of organizational culture and neoliberal ideology on the continued existence of incivility and bullying in healthcare institutions:A discussion paper

Countless research studies have demonstrated the detrimental effects of incivility and bullying in *** the abundance of proposed solutions to this issue,many healthcare leaders continue to fail in mitigating the existence of such negative behaviors in the *** attributes of perpetrators and victims have received attention,but much less research has examined the organizational and neoliberal causations of incivility and bullying in *** the largest occupational group in the health sector,nursing professionals have the greatest influence and are crucial in ending these *** discussion paper outlines the effects of incivility and bullying in healthcare and provides a critical analysis on how organizational culture and neoliberal ideology influence the pervasiveness and persistence of these negative *** analysis reveals that organizational cultures that misuse power,disregard equality,and facilitate oppression,foster the existence of incivility and bullying in the *** cultures permit perpetrators to misuse their authority to control resource allocation,ignorance to social inequalities,and the silence of ***,the neoliberal concept of deregulation,austerity,and individualism further these *** neoliberal reforms have led to underfunding of anti-bullying programs and policies,use of bullying behaviours as management strategies,and victim-blaming for profit *** cutbacks have resulted in denial and acceptance of uncivil and bullying behaviours in healthcare institutions,which endangers the rights of healthcare providers to a safe workplace *** curtail these negative behaviors,robust anti-bullying policies and programs must be strictly enforced and sustained in *** exploration on the association of organizational culture and neoliberal principles to incivility and bullying in healthcare is greatly warranted.

Characterisation of Micrometeorological Parameters in Ecuadorian Highland

The interaction among air and soil affects the dynamic in the atmospheric boundary layer, this research seeks to describe this variation in small scale, in the Ecuadorian Highlands Regions and the variations produced due to its elevation and geographical position, using as base the Van Ulden and Hostlang (1985) models. From this research, there was a conclusion that the zone has strong fluids of sensitive heat and superficial heat with small variations in the latent fluid heat. Besides, there were significant variations of the Obukov length between 17:00 and 19:00, when the sunset occurs, producing a thermal inversion that modifies the atmospheric dynamic state, these results average vary during the day hours and are regulars in the daily and monthly average, in the research year, given the zone geographical position. The meteorological data utilized were taken from the meteorological station from Universidad Nacional de Chimborazo located in the Ecuadorian Highlands.

为了在体外研究HPA-1抗原的多态性,同时建立一种更简便灵敏的检测抗HPA-1同种(异型)抗体的方法,将含有HPA-1a和HPA-1b的DNA质粒分别转染CHO细胞(中国仓鼠卵巢细胞)并获得稳定表达β_3HPA-1a和β_3HPA-1b的细胞株。流式细胞分析结果表明,CHO细胞结合抗HPAPA-1a同种(异型)抗体的能力与细胞表面β_3整合素表达量直接相关。同时,针对HPA-1a表位的单克隆抗体SZ21仅能抑制CHOβ_3HPA-1a细胞的粘附,对 CHOβ_3HPA-1b细胞却没有影响。抗HPA-1a参照血清能阻断CHOβ_3HPA-1a细胞的粘附,但对CHOβ_3HPA-1b细胞却影响甚微。结果表明,CHO细胞模型可以成功地用来表达并研究HPA-1a的多态性及其抗原特性,同时可以作为用于临床检测抗HPA-1a同种(异型)抗体的一种替代方法。

应用蛋白印迹(WB)、悬浮细胞免疫荧光流式细胞分析(SIFT)、单抗分析血小板抗体检测(MAIPA)以及限制性内切酶片段长度分析对新生儿血小板减少患儿母体作基因分型和血清分型比较研究。从两例新生儿血小板减少症患儿母亲的血清中检测到了抗β_3抗体。CHO-SIFT方法的敏感性和MAIPA分析相似。和间接免疫荧光或MAIPA分析相比,WB分析法敏感性稍低,可能是SDS使HPA-1a抗原决定簇发生了改变。以较温和条件对CHO细胞进行固定时,不但细胞表面的HPA-1a抗原表位未受影响,而且细胞的保存时间被大大的延长。这些结果表明,作为分型血小板的替代物,固定化的细胞可能更适合于一般妇幼保健门诊对大量血清样品进行筛查。研究结果表明,表达有重组β_3整合素的CHO细胞可以用来检测母体血清中的抗HPA-1a同种(异型)抗体。

Effect of Soaking Temperature on Water Absorption Characteristics of Selected Ghanaian Cowpea Varieties

The water absorption kinetics of three cowpea varieties (Asontem, Hewale and Asomdwee) was studied following the phenomenological models derived from Fick’s law of diffusion. Soaking of seeds from each cowpea variety was carried out for 10 h at four temperatures (30°C, 40°C, 50°C and 60°C). The saturation moisture content was higher for Asontem (106.9 g water/1000 g dry weight) and Hewale varieties (108.7 g water/1000 g dry weight) and lower for Asomdwee hybrid (100.7 g water/1000 g dry weight), respectively. The proposed Fick’s law of diffusion satisfactorily described the kinetics of water absorption regardless of the variety and temperature. The estimated values for water diffusion coefficient for Asontem, Hewale and Asomdwee varied from 5.12 × 10-10 m2/s to 6.64 × 10-10 m2/s, 3.96 × 10-10 m2/s to 5.12 × 10-10 m2/s, 4.93 × 10-10 m2/s to 6.08 × 10-10 m2/s, respectively. The strong influence of temperature on the water diffusion coefficient was adequately described by an Arrhenius-type equation with activation energy values for Asontem, Hewale and Asomdwee as 7.27 kJ/mol, 7.26 kJ/mol and 6.26 kJ/mol, respectively.

Land-cover change can affect inland fisheries,which underpin food security of millions of people *** of forests from very large floodplains has been found to decrease fish catch via loss of feeding and nursery habitat for ***,it is unknown if similar effects occur in smaller rivers with limited floodplain *** is also known about the mechanisms by which land-cover changes affect inland fish ***,we assessed land-use and land-cover(LULC)effects on fish catch,diet,and condition at sites in two medium-sized rivers of Cameroon,in Central *** found that LULC explained 30%of the variation in fish catch,with the catch of five species being positively,and of two species being negatively,related to floodplain *** niche breadth of fish diets was higher in the river surrounded by forests than in the river surrounded by agricultural ***,contrary to expectations,condition of the fish,as indicated by occurrence of diseases or deformities,eroded fins,lesions or tumors,was not related to *** results support the notions that floodplain forests support fish populations in rivers with limited floodplain areas in ways similar to that of rivers with large floodplains,and that LULC affects fish populations via changes in fish diets and instream habitat features(i.e.,riparian canopy closure,water clarity,substrate heterogeneity,and habitat volume).These effects imply that prevailing changes in LULC threaten the food and livelihood security services provided by inland fisheries,highlighting the importance of policies that maintain native vegetation along riverbanks and in floodplain areas.
