Objective:To assess the epidemiology of urinary schistosomiasis and soil-transmitted helminthiasis among women of reproductive age in Mwaluphamba,Kwale County,Kenya.Methods:A community-based cross-sectional study design was employed to randomly sample 422 women of reproductive age(15-<50 years)from four villages in Mwaluphamba location.Stool specimens were collected and examined using the Kato-Katz method,while filtration technique was used to analyze urine specimens.Participants’sociodemographic details were obtained using a standardized questionnaire.Results:Urinary schistosomiasis prevalence was at 4.7%(20/422,95%CI 2.8%-6.9%)while the prevalence of soil-transmitted helminthiasis infection was 4.5%(19/422,95%CI 2.6%-6.7%).The infection intensities of urinary schistosomiasis among the study participants ranged from 1 to 120 eggs/10 mL of urine with median egg count of 18.45 eggs/10 mL.The patients were diagnosed with light infection,of 56.16 egg/gram and 48.48 egg/gram for Trichuris trichiura and hookworms,respectively.Women without latrines had 15.7 times higher risk of having urinary schistosomiasis compared to those with a latrine.Similarly,use of surface water(aOR=1.0,95%CI 0.2-1.4,P=0.010)and crossing the river to go to a place(aOR=1.1,95%CI 0.3-1.6,P=0.009)were statistically significant risk factors for getting urinary schistosomiasis.In bivariable regression analysis,defecating around the water source(OR=4.3,95%CI 1.5-12.9)had a statistically significant association with the prevalence of soil-transmitted helminthiasis(P=0.008).Conclusions:This study has given an insight on the prevalence and intensity of urinary schistosomiasis and soil-transmitted helminthiasis in Mwaluphamba location that form a basis for strengthening the control and elimination programmes for these neglected tropical diseases.

An improved understanding of biodiversity-productivity relationships(BPRs)along environmental gradients is crucial for effective ecosystem management and biodiversity conservation.The stress-gradient hypothesis suggests that BPRs are stronger in stressful environments compared to more favorable conditions.However,there is limited knowledge regarding the variation of BPRs along elevational gradients and their generality across different landscapes.To study how BPRs change with elevation,we harnessed inventory data on 6,431 trees from152 plots surveyed twice in eight to ten year intervals in mountain forests of temperate Europe and subtropical Asia.We quantified the relationship between aboveground productivity and different biodiversity measures,including taxonomic,functional,and phylogenetic diversity.To elucidate the processes underlying BPRs,we studied the variation of different functional traits along elevation across landscapes.We found no general pattern of BPRs across landscapes and elevations.Relationships were neutral for all biodiversity measures in temperate forests,and negative for taxonomic and functional diversity in subtropical forests.BPRs were largely congruent between taxonomic,functional and phylogenetic diversity.We found only weak support for the stress-gradient hypothesis,with BPRs turning from negative to positive(effect not significant)close to the tree line in subtropical forests.In temperate forests,however,elevation patterns were strongly modulated by species identity effects as influenced by specific traits.The effect of traits such as community-weighted mean of maximum plant height and wood density on productivity was congruent across landscapes.Our study highlights the context-dependence of BPRs across elevation gradients and landscapes.Species traits are key modulating factors of BPRs and should be considered more explicitly in studies of the functional role of biodiversity.Furthermore,our findings highlight that potential trade-offs between conserving biodiversity and fostering ecos

Mechatronic product development is a complex and multidisciplinary field that encompasses various domains, including, among others, mechanical engineering, electrical engineering, control theory and software engineering. The integration of artificial intelligence technologies is revolutionizing this domain, offering opportunities to enhance design processes, optimize performance, and leverage vast amounts of knowledge. However, human expertise remains essential in contextualizing information, considering trade-offs, and ensuring ethical and societal implications are taken into account. This paper therefore explores the existing literature regarding the application of artificial intelligence as a comprehensive database, decision support system, and modeling tool in mechatronic product development. It analyzes the benefits of artificial intelligence in enabling domain linking, replacing human expert knowledge, improving prediction quality, and enhancing intelligent control systems. For this purpose, a consideration of the V-cycle takes place, a standard in mechatronic product development. Along this, an initial assessment of the AI potential is shown and important categories of AI support are formed. This is followed by an examination of the literature with regard to these aspects. As a result, the integration of artificial intelligence in mechatronic product development opens new possibilities and transforms the way innovative mechatronic systems are conceived, designed, and deployed. However, the approaches are only taking place selectively, and a holistic view of the development processes and the potential for robust and context-sensitive artificial intelligence along them is still needed.

As basic facts of life,illness and healing occur frequently and in a variety of patterns in Chinese non-medical literature,starting from the earliest sources inscribed on oracle bones and continuing throughout literary history up to the present day.This article looks at illness narratives in early medieval anecdotal literature(3rd to 6th century CE)to understand how the experience of being sick or of attending to the sick was reflected in these socio-literary environments and what rhetorical and ideological roles these narratives played in their larger narrative contexts.By focusing on the experiences of the sick and those around them,this article aims at“Honoring the Stories of Illness,”in Rita Charon’s words,that are hiding in plain sight in much of Chinese non-medical literature.

BACKGROUND Abnormal colonic pressure profiles have been associated with an increased risk of colonic diverticulosis.A surgical history is a known risk factor for abdominal adhesions and these may lead to increased intraluminal colonic pressure.AIM To assess whether previous abdominal surgery is associated with colonic divertic-ulosis or diverticulitis.METHODS We analyzed data from a study of patients undergoing colonoscopy for different indications from 2020 through 2021.Patients completed a structured question-naire concerning previous abdominal surgeries,dietary and lifestyle exposures including smoking,alcohol use and co-morbidities.RESULTS Three hundred and fifty-nine patients were included in the study.The mean age was 67.6 and 46%were females.Diabetes mellitus,hypertension,ischemic heart disease,chronic obstructive pulmonary disease,chronic renal failure,and body mass index were similar in the diverticulosis and control groups.The overall prevalence of colonic diverticulosis was 25%(91/359)and 48%of the patients had previous abdominal surgery.As expected,the prevalence of diverticulosis increased with age.There was no difference in the rate of previous abdominal surgery between patients with or without diverticulosis(49%vs 47%,P=0.78).In regards to specific surgeries,inguinal hernia repair was significantly associated with diverticulosis(52%vs 20%,P=0.001),but not diverticulitis.In contrast,appendectomy was not associated with diverticulosis(6%vs 14%,P=0.048).CONCLUSION These findings suggest that post-operative abdominal adhesions inducing high colonic intraluminal pressures do not appear to be the mechanism for diverticula formation.Rather,inguinal hernia and diverticulosis may share similar connective tissue pathologies with no causative relationship between them.

BACKGROUND Inflammatory bowel disease(IBD)patients’expectations of treatment outcomes may differ by ethnicity.AIM To investigate treatment preferences of Jewish and Arabs patients.METHODS This prospective survey ranked outcomes treatment preferences among Arab IBD patients,based on the 10 IBD-disk items compared to historical data of Jews.An anonymous questionnaire in either Arabic or Hebrew was distributed among IBD patients.Patients were required to rank 10 statements describing different aspects of IBD according to their importance to the patients as treatment goals.Answers were compared to the answers of a historical group of Jewish patients.RESULTS IBD-disk items of 121 Arabs were compared to 240 Jewish patients.The Jewish patients included more females,[151(62.9%)vs 52(43.3%);P<0.001],higher education level(P=0.02),more urban residence[188(78.3%)vs 54(45.4%);P<0.001],less unemployment[52(21.7%)vs 41(33.9%);P=0.012],higher income level(P<0.001),and more in a partnership[162(67.8%)vs 55(45.4%);P<0.001].Expectations regarding disease symptoms:abdominal pain,energy,and regular defecation ranked highest for both groups.Arabs gave significantly lower rankings(range 4.29-6.69)than Jewish patients(range 6.25-9.03)regarding all items,except for body image.Compared to Arab women,Jewish women attached higher priority to abdominal pain,energy,education/work,sleep,and joint pain.Multivariable regression analysis revealed that higher patient preferences were associated with Jewish ethnicity(OR 4.77;95%CI 2.36-9.61,P<0.001)and disease activity.The more active the disease,the greater the odds ratio for higher ranking of the questionnaire items(1-2 attacks per year:OR 2.13;95%CI 1.02-4.45,P=0.043;and primarily active disease:OR 5.29;95%CI 2.30-12.18,P<0.001).Factors inversely associated with higher patient preference were male gender(OR 0.5;95%CI 0.271-0.935,P=0.030),UC(OR 0.444;95%CI 0.241-0.819,P=0.009),and above average income level(OR 0.267;95%CI:0.124-0.577,P=0.001).CONCLUSION The highest priority for treatment outcome

Calcium phosphate cements (CPC) are currently widely used bone replacement materials with excellent bioactivity, but have considerable disadvantages like slow degradation. For critical-sized defects, however, an improved degradation is essential to match the tissue regeneration, especially in younger patients who are still growing. We demonstrate that a combination of CPC with mesoporous bioactive glass (MBG) particles led to an enhanced degradation in vitro and in a critical alveolar cleft defect in rats. Additionally, to support new bone formation the MBG was functionalized with hypoxia conditioned medium (HCM) derived from rat bone marrow stromal cells. HCM-functionalized scaffolds showed an improved cell proliferation and the highest formation of new bone volume. This highly flexible material system together with the drug delivery capacity is adaptable to patient specific needs and has great potential for clinical translation.

The Philippines was in the 1960s a model of development in Asia and second to Japan,but occupies presently only the 11th position under South-East and East Asian countries in terms of GDP-per capita.The article explores why this important Asian country with a long colonial past and enormous economic potential still ranks under lower-income countries and has in the last decades let pass by many other Asian countries.In answering this question,the approach of external triggers for accelerated development is being applied.In stark contrast to the success stories of the strongly outward-looking Asian countries like the four Tigers,later of Thailand and Vietnam the Philippines never developed a vision of an open economy connecting pro-actively to the world markets.Trade is hampered by a non-competitive and highly protected national economy.The existing FDI is more oriented to the profitable local markets.Foreign debts were never effectively used and international tourism was never well promoted.Linking these failures to the existing power structures in the country,it seems very much that the backward forces like the big landowners,the local producers and industrialists never wanted and continue not to want to open up the economy to international competition and governments are complacent with these groups.Various indicators demonstrate the long-term decline of the Philippines:Among them the slow growth of the GDP and the continuously high poverty rates.As the alliance of big business and policy holds firm no change in the failing nationalistic economic model can be detected leaving the bleak outlook that the economic decline will continue.
