Comparative analysis of breast and lung cancer survival rates and clinical trial enrollments among rural and urban patients in Georgia

Objectives:Rural patients have poor cancer outcomes and clinical trial(CT)enrollment compared to urban patients due to attitudinal,awareness,and healthcare access *** of cancer survival disparities and CT enrollment is important for designing interventions and innovative approaches to address the stated *** study explores the potential disparities in cancer survival rates and clinical trial enrollments in rural and urban breast and lung cancer *** hypotheses are that for both cancer types,urban cancer patients will have longer 5-year survival rates and higher enrollment rates in clinical trials than those in rural ***:We compared breast and lung cancer patients’survival rates and enrollment ratios in clinical trials between rural(RUCC 4-9)and urban counties in Georgia at a Comprehensive Cancer Center(CCC).To assess these differences,we carried out a series of independent samples t-tests and Chi-Square ***:The outcomes indicate comparable 5-year survival rates across rural and urban counties for breast and lung cancer patients,failing to substantiate our *** clinical trial enrollment rates demonstrated a significant difference between breast and lung cancer patients at CCC,no significant variation was observed based on rural or urban ***:These findings underscore the need for further research into the representation of rural patients with diverse cancer types at CCC and other cancer ***,the findings have considerable implications for the initiation of positive social change to improve CT participation and reduce cancer survival disparities.

Background:Information on the association between physical activity(PA)and the risk of chronic kidney disease(CKd)is *** aimed to explore the associations of total,domain-specific,and intensity-specific PA with CKd and its subtypes in ***:The study included 475,376 adults from the China Kadoorie Biobank aged 30-79 years during 2004-2008 at *** interviewer-administered questionnaire was used to collect the information about PA,which was quantified as metabolic equivalent of task hours per day(MET-h/day)and categorized into 4 groups based on *** regression was used to analyze the association between PA and CKd ***:during a median follow-up of 12.1 years,5415 incident CKd cases were documented,including 1159 incident diabetic kidney disease(dKd)cases and 362 incident hypertensive nephropathy(HTN)*** PA was inversely associated with CKd risk,with an adjusted hazard ratio(HR,95%confidence interval(95%CI))of 0.83(0.75-0.92)for incident CKd in the highest quartile of total PA as compared with participants in the lowest *** results were observed for risk of dKd and HTN,and the corresponding HRs(95%CIs)were 0.75(0.58-0.97)for dKd risk and 0.56(0.37-0.85)for HTN *** nonoccupational PA,low-intensity PA,and moderate-to-vigorous-intensity PA were significantly associated with a decreased risk of CKd,with HRs(95%CIs)of 0.80(0.73-0.88),0.85(0.77-0.94),and 0.85(0.76-0.95)in the highest quartile,***:PA,including nonoccupational PA,low-intensity PA,and moderate-to-vigorous-intensity PA,was inversely associated with the risk of CKd,including dKd,HTN,and other CKd,and such associations were dose dependent.

In Africa, the prevalence of diabetes is escalating and remains a concern due to the numerous complications it causes. Vascular damage associated with diabetes leads to a prothrombotic state observed in diabetic individuals. diabetes is a complex and multifactorial disease involving genetic components. With the aim of preventing complications and contributing to an efficient management of diabetes, we investigated genes likely to lead to a risk of thrombosis, in particular the C677T of MTHFR, G20210A of prothrombin, and R506Q of factor V Leiden in type 2 diabetics in Abidjan receiving ambulatory care. A descriptive cross-sectional study was carried out on consenting type 2 diabetic patients. Mutation detection was carried out using the PCR-RFLP method employing restriction enzymes. Hemostasis tests (fibrinogen, d-dimers, fibrin monomers, and von Willebrand factor) were performed using citrate tubes on the Stage? Star Max automated system. Plasminogen activator inhibitor was assayed by ELISA method, and biochemical parameters were determined using the COBAS C311. The study population consisted of 45 diabetic patients, 51.1% of whom presented vascular complications, mainly neuropathy. disturbances in hemostasis parameters were observed, with 15.5% of patients showing an increase in fibrin monomers. Mutation analysis revealed an absence of factor V mutation (factor V Leiden) and of G20210A mutation of the prothrombin gene. However, 15.6% of subjects had a heterozygous C677T mutation of MTHFR, with 57% of them being anemic. The exploration of biological and genetic factors associated with thrombotic risk is of significant interest in the optimal management of African type 2 diabetics.

Pregnane X receptor(PXR)is a ligand-activated nuclear receptor that transcriptionally upregulates drug-metabolizing enzymes[e.g.,cytochrome P4503A4(CYP3A4)]and *** the regulation of PXR target genes is well-characterized,less is known about the regulation of PXR protein *** screening an RNAi library,we identified the F-box-only protein 44(FBXO44)as a novel E3ligase for *** abundance increases upon knockdown of FBXO44,and,inversely,decreases upon overexpression of *** analysis revealed that FBXO44 interacts with PXR,leading to its ubiquitination and proteasomal degradation,and we determined that the F-box associated domain of FBXO44 and the ligand binding domain of PXR are required for the functional *** summary,FBXO44 regulates PXR protein abundance,which has downstream consequences for CYP3A4 levels and drug-drug *** results of this study provide new insight into the molecular mechanisms that regulate PXR protein level and activity and suggest the importance of considering how modulating E3ubiquitin ligase activities will affect PXR-mediated drug metabolism.

dear editor,dysregulated inflammatory responses primarily account for damage to the central nervous system,however,neurodegenerative processes likely contribute to the pathophysiology of progressive forms of multiple sclerosis(MS).These pathophysiological aspects include axonal energy depletion and impaired remyelination capacity(Levin et al.,2014).The clinical efficacy of most disease-modifying therapies in MS to date has been achieved by targeting functions of the adaptive immune system(Cree et al.,2019;Szepanowski et al.,2021).
