The adverse outcomes of a ventricular heart failure(left,right or biventricular)caused by cardiogenic shock are aggravated by lung oedema and organ mal perfusion.Despite advances in medical sciences,revascularisation and mechanical hemodynamic support have proved ineffective in reducing the mortality rate in such patients.A thorough study of the data available about cardio-vascular diseases reveals that the application of conventional methods of treatment are least helpful to practically restore normal functions of heart when it experiences end-stage systolic ventricular failure.Thus,to overcome the challenges and find alternatives to address this issue,percutaneous ventricular support devices/machines were designed and successfully introduced.These devices have revolutionized the treatment of ventricular heart failures and are now in use all over the world.In this review paper a newer mechanical circulatory support(MCS)device,Impella,has been discussed and compared with a few other devices like(Intra-aortic Balloon Pump(IABP),Extracorporeal Circulation(ECLS)and Veno-arterial Extracorporeal Membrane Oxygenation(VA-ECMO).This article studies the challenges being faced during the treatment of cardiogenic shock,and thoroughly discusses the use and effectiveness of Impella Cardiac Axial Pump in each emergency.It can be said that mechanical circulatory support(MCS)device use during percutaneous coronary intervention(PCI)should be individualized based on multiple factors with a recommended use in patients with the greatest potential benefit and a relatively low risk of device-related complications.The current literature suggests that the outcomes of use of Impella and other mechanical circulatory support devices like IABP and VA-ECMO are comparable.Though there seem to be a few advantages of Impella over the others,sufficiently powered,multi-centric,randomised control trials are needed to establish its superiority.

目的:探讨早期血运重建及体外膜肺氧合(ECMO)治疗急性心肌梗死合并心原性休克患者的疗效,并分析影响其预后的因素。方法:回顾2018年1月至2019年12月郑州大学人民医院早期血运重建及ECMO救治的急性心肌梗死合并心原性休克66例患者的临床资料。收集并分析患者一般情况、临床特征、合并症、冠状动脉病变特点,ECMO前后血流动力学变化及应用时长、并发症及疗效,是否应用主动脉球囊反搏(IABP)及院内临床转归等。结果:66例患者平均年龄(56.6±11.7)岁,其中男性46例。患者入院均行早期血运重建:5例行冠状动脉旁路移植术(CABG);61例行经皮冠状动脉介入治疗(PCI),其中44例行入院即刻早期血运重建,17例行院内延迟早期血运重建;其中42例患者为多支冠状动脉病变。所有患者均应用ECMO辅助,其中39例使用了IABP。ECMO治疗后患者平均动脉压、中心静脉压、血乳酸水平及pH值均得到显著改善(P均<0.05)。66例患者中38例成功撤机,30例存活出院,36例死亡;并发症分别为20例肾功能不全或急性肾功能衰竭、11例消化道出血、5例脑出血、1例肺出血、9例下肢血栓及3例下肢缺血。多因素Logistic回归分析显示高血压,多脏器功能衰竭,发生至少2次呼吸心脏骤停、意识丧失及恶性心律失常事件为患者院内死亡的高危因素(P<0.05),应用IABP对院内结局无显著影响。结论:早期血运重建及ECMO用于救治急性心肌梗死合并心原性休克的患者是可行的。

心源性休克的常见原因包括急性心肌梗死、心肌病、心包疾病、心肌炎、开胸手术,其主要表现为心输出量下降、平均动脉压下降、终末靶器官灌注不足,休克进入死亡瀑布。尽管介入、溶栓技术取得很大进步,但ST段抬高型心肌梗死合并心源性休克患者住院病死率仍高达50%以上。Impella是心脏血流动力学辅助装置轴流泵,可稳定心源性休克患者血流动力学,减轻左心室负荷,增加器官的有效灌注,促进患者康复。本文就目前治疗心源性休克面临的挑战及Impella心脏轴流泵在心源性休克救治中作用的研究进展作一综述。

The first major outbreak of the severely complicated hand,foot and mouth disease(HFMD),primarily caused by enterovirus 71,was reported in Taiwan in 1998.HFMD surveillance is needed to assess the spread of HFMD.The parameters we use in mathematical models are usually classical mathematical parameters,called crisp parameters,which are taken for granted.But any biological or physical phenomenon is best explained by uncertainty.To represent a realistic situation in any mathematical model,fuzzy parameters can be very useful.Many articles have been published on how to control and prevent HFMD from the perspective of public health and statistical modeling.However,few works use fuzzy theory in building models to simulateHFMDdynamics.In this context,we examined anHFMD model with fuzzy parameters.A Non Standard Finite Difference(NSFD)scheme is developed to solve the model.The developed technique retains essential properties such as positivity and dynamic consistency.Numerical simulations are presented to support the analytical results.The convergence and consistency of the proposed method are also discussed.The proposed method converges unconditionally while the many classical methods in the literature do not possess this property.In this regard,our proposed method can be considered as a reliable tool for studying the dynamics of HFMD.

A Nonstandard Computational Investigation of SEIR Model with Fuzzy Transmission, Recovery and Death Rates

In this article,a Susceptible-Exposed-Infectious-Recovered(SEIR)epidemic model is considered.The equilibrium analysis and reproduction number are studied.The conventional models have made assumptions of homogeneity in disease transmission that contradict the actual reality.However,it is crucial to consider the heterogeneity of the transmission rate when modeling disease dynamics.Describing the heterogeneity of disease transmission mathematically can be achieved by incorporating fuzzy theory.A numerical scheme nonstandard,finite difference(NSFD)approach is developed for the studied model and the results of numerical simulations are presented.Simulations of the constructed scheme are presented.The positivity,convergence and consistency of the developed technique are investigated using mathematical induction,Jacobean matrix and Taylor series expansions respectively.The suggested scheme preserves all these essential characteristics of the disease dynamical models.The numerical and simulation results reveal that the proposed NSFD method provides an adequate representation of the dynamics of the disease.Moreover,the obtained method generates plausible predictions that can be used by regulators to support the decision-making process to design and develop control strategies.Effects of the natural immunity on the infected class are studied which reveals that an increase in natural immunity can decrease the infection and vice versa.

Nonlinear modelling has a significant role in different disciplines of sciences such as behavioral,social,physical and biological sciences.The structural properties are also needed for such types of disciplines,as dynamical consistency,positivity and boundedness are the major requirements of the models in these fields.One more thing,this type of nonlinear model has no explicit solutions.For the sake of comparison its computation will be done by using different computational techniques.Regrettably,the aforementioned structural properties have not been restored in the existing computational techniques in literature.Therefore,the construction of structural preserving computational techniques are needed.The nonlinearmodel for cervical cancer is constructed by parametric perturbation technique.Well-known computer methods are considered for the computation of cervical cancer dynamics.The well-known existing methods in literature are Euler Maruyama,Euler and Runge Kutta.Nonstandard finite difference method or Implicitly driven explicit method is first time considered for aforesaid model under the assumptions given byMickens in a stochastic way.Unfortunately,the aforementioned existing methods did not reinstate structural properties of cervical cancer dynamics in the human population.Our plannedmethod is structural preserving and a powerful tool for all nonlinear models of biomedical engineering problems.We have verified that existing computational methods do not preserve dynamical properties.But,the implicitly driven explicit method is a good device for dynamical properties.In the support of assertions,convergence analysis of implicitly driven explicit method is presented.

In 2020,the reported cases were 0.12 million in the six regions to the official report of the World Health Organization(WHO).For most children infected with leprosy,0.008629 million cases were detected under fifteen.The total infected ratio of the children population is approximately 4.4 million.Due to theCOVID-19 pandemic,the awareness programs implementation has been disturbed.Leprosy disease still has a threat and puts people in danger.Nonlinear delayed modeling is critical in various allied sciences,including computational biology,computational chemistry,computational physics,and computational economics,to name a few.The time delay effect in treating leprosy delayed epidemic model is investigated.The whole population is divided into four groups:those who are susceptible,those who have been exposed,those who have been infected,and those who have been vaccinated.The local and global stability of well-known conclusions like the Routh Hurwitz criterion and the Lyapunov function has been proven.The parameters’sensitivity is also examined.The analytical analysis is supported by computer results that are presented in a variety of ways.The proposed approach in this paper preserves equilibrium points and their stabilities,the existence and uniqueness of solutions,and the computational ease of implementation.

Most developing countries such as Afghanistan,Pakistan,India,Bangladesh,and many more are still fighting against poliovirus.According to the World Health Organization,approximately eighteen million people have been infected with poliovirus in the last two decades.In Asia,still,some countries are suffering from the virus.The stochastic behavior of the poliovirus through the transition probabilities and non-parametric perturbation with fundamental properties are studied.Some basic properties of the deterministic model are studied,equilibria,local stability around the stead states,and reproduction number.Euler Maruyama,stochastic Euler,and stochastic Runge-Kutta study the behavior of complex stochastic differential equations.The main target of this study is to develop a nonstandard computational method that restores dynamical features like positivity,boundedness,and dynamical consistency.Unfortunately,the existing methods failed to fix the actual behavior of the disease.The comparison of the proposed approach with existing methods is investigated.

The application of fuzzy theory is vital in all scientific disciplines.The construction of mathematical models with fuzziness is little studied in the literature.With this in mind and for a better understanding of the disease,an SEIR model of malaria transmission with fuzziness is examined in this study by extending a classicalmodel ofmalaria transmission.The parametersβandδ,being function of the malaria virus load,are considered fuzzy numbers.Three steady states and the reproduction number of the model are analyzed in fuzzy senses.A numerical technique is developed in a fuzzy environment to solve the studied model,which retains essential properties such as positivity and dynamic consistency.Moreover,numerical simulations are carried out to illustrate the analytical results of the developed technique.Unlike most of the classical methods in the literature,the proposed approach converges unconditionally and can be considered a reliable tool for studying malaria disease dynamics.
