There are four steps in the interaction between intestinal microbes and mucosal inflammation in genetically predisposed individuals from the viewpoints of basic and clinical aspects of inflammatory bowel disease （IBD）. The first step is an interaction between intestinal microbes or their components and intestinal epithelial cells via receptors, the second step an interaction between macrophages and dendritic cells and mucosal lymphocytes, the third step an interaction between lymphocytes and vascular endothelial cells, and the fourth step an interaction between lymphocytes and granulocytes producing proinflammatory cytokines or free radicals and mucosal damage and repair. recent therapeutic approaches for IBD aim to block these four steps in the intestinal inflammation of patients with IBD.

We present a case of fetal liver failure caused by the activation of larnivudine-resistant hepatitis B virus （HBV） nine months after lamivudine treatment. A 57-year old man visited our hospital for the treatment of decornpensated chronic hepatitis B. Lamivudine was started in December 2001. Subsequently, serum HBV was negative for HBV DNA with seroconversion from HBeAg to anti-HBe and improvement of liver function. However, HBV DNA and HBeAg were again detected in September 2002. He was complicated by breakthrough hepatitis and admitted to our hospital in November for severely impaired liver function. Vidarabine treatment was started and serum HBV DNA and alanine aminotransferase （ALT） decreased transiently. However, after the start of m-interferon treatment, HBV DNA level increased and liver function deteriorated. He died 1 mo after admission. An analysis of amino acid sequences in the polymerase region revealed that rtM204I/V with rtLSOI/V occurred at the time of viral breakthrough. After the start of antiviral treatment, rtL180M was detected in addition to rtM204I/V and rtLSOI/V, and became predominant in the terminal stage of the disease. HBV clone with a high replication capacity may be produced by antiviral treatment leading to the worsening of liver function. Antiviral therapy for patients with breakthrough hepatitis in advanced liver disease should be carefully performed.

Gastrointestinal stromal tumor (GIST) of the stomach was treated by endoscopic enucleation in five patients. They were three men and two woman, aged 36-56 years. Tumors located in the cardia were completely enucleated endoscopically without any serious complication. The largest diameter of removed tumors ranged from 1.2 to 2.5 cm. Histopathological diagnosis was GIST with low risk of malignancy (mitotic index r field) in all cases. The patients were disease-free for 10.5-42.2 mo after endoscopic enucleation.

This study aimed to ascertain the current status of Japanese sperm banking for young cancer patients. During 2015, we mailed the directors of 695 institutes where sperm cryopreservation might be performed with questionnaires requesting information on the number of patients, age, precryopreservation chemotherapy, semen analyses results and diagnoses, cryopreservation success rate, and causes of unsuccessful cryopreservation. Of these 695 institutes, 92 had cryopreserved sperm before chemotherapy within the study period. In all, 820 cancer patients （237 testicular, 383 hematological, 46 bone and soft tissue, 20 brain, and 134 other malignancy） consulted the responding institutes for sperm cryopreservation. Except for testicular tumor, the number of patients whose sperm was preserved before cancer treatment was low compared to that of young cancer patients. Approximately 20% of patients with malignancies other than testicular tumor underwent chemotherapy before cryopreservation. The success rate of cryopreservation in hematological malignancy was 82.5%, significantly lower than that of both the testicular cancer （93.6%） and other malignancy groups （95.6%）（P 〈 0.05）. The primary reasons for preservation failure were azoospermia and poor semen quality. Patients with hematological malignancies had a higher rate of unsuccessful cryopreservation compared to those in other groups, possibly due to the large number of patients requesting sperm cryopreservation after chemotherapy induction. In Japan, information regarding sperm banking prior to cancer treatment appears to be lacking. Information regarding sperm preservation before chemotherapy should be provided to all Japanese oncologists.

A novel iodonium-ylide compound 2 that appends a trifluoromethylthio （SCF3） group is disclosed as a new, shelf-stable electrophilic trifluoromethylation reagent. Unlike known shelf-stable electrophilic trifluoromethylation reagents, 2 has a stable SCF3 group which is activated by appending iodonium ylide under copper catalysis via sul- fonium ylide to generate a cationic trifluoromethyl （CF3） species. reagent 2 was found to be an efficient electro- philic trifluoromethylation reagent for a wide range of silyl enol ethers 3 under copper catalysis. Cyclic and acyclic a-trifiuoromethyl ketones 4 were obtained by reagent 2 in moderate to good yields. On the other hand, a difluoro- methylthio analogue 5 did not affect intermolecular transfer difluoromethylation to substrates. Instead, intramolecu- lar 1,4-migration proceeded similar to the Stevens rearrangement to provide 6 in 21% yield, independent of the presence of nucleophiles 3.

We report a case of hemangiopericytoma of the soft palate of 60-year-old patient, who noticed a mass of the soft palate and experienced difficulty in speaking. We found a pediculate, hard, elastic mass measuring 38 mm （cross-sectional diameter）. Computed tomography （CT） scans and dynamic magnetic resonance imaging （MrI） confirmed irregularly shaped mass and revealed a heterogeneous internal composition, consistent with vascular tumors. We excised the tumor under general anesthesia. Histopathological diagnosis was based on positive immunoreactivity of CD99 and vimentin and weak, positive staining of CD34. Three and half years following tumor excision, there is no recurrence or metastasis.

Organic solar cells based on copper naphthalocyanine(CuNc) and fullerene(C60) were fabricated, and their photovoltaic properties were investigated. C60 and CuNc were used as n-type and p-type semiconductors, respectively. In addition, the effect of Au nanoparticle addition on a hole transfer layer was investigated, and the power conversion efficiency of the devices was improved after blending the Au nanoparticles into the hole transport layer. Nanostructures of Au nanoparticles were investigated by transmission electron microscopy and X-ray diffraction. Energy levels of molecules were calculated by molecular orbital calculations, and the nanostructure and electronic properties were discussed.& 2014 Chinese Materials research Society. A 78-year-old woman presented with fever, severe abdominal pain, and *** had been institutionalized for depression and senile *** examinations disclosed a leucocytosis(WBC:12 500/μL) and elevated levels of serum C-reactive protein(2.8 mEq/L).Diagnosis of acute cecal volvulus was made from a"coffee bean sign"on an abdominal computed tomography and a"beak sign"on a gastrographin *** emergent laparotomy confirmed the diagnosis and an ileo-colectomy with primary anastomosis was carried *** patient recovered after intensive respiratory care and fluid therapy, and then returned to her former institution.A review of Japanese literature disclosed that:(1) a marked increase of aged patients with mental disability presenting with cecal volvulus,(2) adoption of ileo-colectomy as the standard surgical procedure, and(3) improved survival of the patients, were observed in the last decade.

Carcinosarcomas are rare,malignant,biphasic tumors simultaneously comprising carcinoma and sarcoma in a single *** present an extremely rare case of gastric carcinosarcoma with an osteoblastic component that drastically changed its shape within 2 mo.A 59-year-old male patient presented to the emergency outpatient unit with a complaint of black *** endoscopy showed an ulcerated mass in the cardia of the lesser curvature of the *** specimens revealed only *** months later,the ulcerated lesion drastically changed its shape into an exophytic *** gastrectomy was *** the resected specimen,the gastric tumor contained both adenocarcinoma and sarcoma components with lace-like *** patient died 7 mo after the operation,and an autopsy was *** the autopsy,widespread metastases were present in the liver,lung,lymph nodes and *** this report,we describe a case of gastric carcinosarcoma and presume its tumorigenesis based on the autopsy findings.
