The clinical role and underlying mechanisms of valproic acid(VPA)on bone homeostasis remain ***,we confirmed that VPA treatment was associated with decreased bone mass and bone mineral density(BMD)in both patients and *** effect was attributed to VPA-induced elevation in osteoclast formation and *** rNA-sequencing,we observed a significant rise in pr.cursor mir-6359 expression in VPA-treated osteoclast pr.cursors in vitro,and further,a marked upregulation of mature mir-6359(mir-6359)in vivo was demonstrated using quantitative real-time Pcr(qrT-Pcr)and mir-6359 fluorescent in situ hybridization(mir-6359-FISH).Specifically,the mir-6359 was predominantly increased in osteoclast pr.cursors and macrophages but not in neutrophils,T lymphocytes,monocytes and bone marrow-derived mesenchymal stem cells(BMScs)following VPA stimulation,which influenced osteoclast differentiation and bone-resorptive ***,VPA-induced mir-6359 enr.chment in osteoclast pr.cursors enhanced reactive oxygen species(rOS)production by silencing the SIrT3 protein expression,followed by activation of the MAPK signaling pathway,which enhanced osteoclast formation and activity,thereby accelerating bone ***,there are no medications that can effectively treat VPA-induced bone ***,we constr.cted engineered small extr.cellular vesicles(E-sEVs)targeting osteoclast pr.cursors in bone and naturally carrying anti-mir-6359 by introducing of EXOmotif(cGGGAGc)in the 3’-end of the anti-mir-6359 *** confirmed that the E-sEVs exhibited decent bone/osteoclast pr.cursor targeting and exerted protective therapeutic effects on VPA-induced bone loss,but not on ovariectomy(OVX)and glucocorticoid-induced osteoporotic models,deepening our understanding of the underlying mechanism and treatment strategies for VPA-induced bone loss.

regional variations in acute coronary syndrome(AcS) management and outcomes have been an enormous public health issue. However, studies have yet to explore how to reduce the variations. The National chest Pain center Program(NcPcP) is the first nationwide, hospital-based, comprehensive, continuous quality improvement program for improving the quality of care in patients with AcS in china. We evaluated the association of NcPcP and regional variations in AcS healthcare using generalized linear mixed models and inter.ction analysis. Patients in the Western region had longer onset-to-first medical contact(FMc) time and time stay in non-percutaneous coronary intervention(PcI) hospitals, lower rates of PcI for ST-elevation myocardial infarction(STEmi) patients, and higher rates of medication *** in central regions had relatively lower in-hospital mortality and in-hospital heart failure *** in the door-to-balloon time(Dto B) and in-hospital mortality between Western and Eastern regions were less after accreditation(β =-8.82, 95% confidence interval(cI)-14.61 to-3.03;Or = 0.79, 95%cI 0.70 to 0.91). Similar results were found in differences in Dto B time, primary PcI rate for STEmi between central and Eastern regions. The differences in PcI for higher-risk non-ST-segment elevation acute coronary syndrome(NSTE-AcS) patients among different regions had been ***, the differences in medication use between Eastern and Western regions were higher after accreditation. regional variations remained high in this large cohort of patients with AcS from hospitals participating in the NcPcP in china. More comprehensive interventions and hospital internal system optimizations are needed to further reduce regional variations in the management and outcomes of patients with AcS.

在多主体能源共享背景下,提出一种基于三重博弈的多微网-产消者群自适应鲁棒分布式协同优化。首先,构建三重博弈框架模拟多微网-产消者群的能源交易模型,即上层多微网系统合作博弈,各微网与其下层产消者群主从博弈,下层产消者群联盟合作博弈。其次,考虑到多微网系统内部受可再生能源出力、负荷功率随机性,外部受日前电价波动性等多重不确定性影响较大,各微网建立两阶段自适应鲁棒优化(adaptive robust optimization,ArO)模型,在第一阶段优化微网与外部主体的交易策略,包括与其他微网的合作博弈、与产消者群的主从博弈以及与配网日前电价的不确定性;在第二阶段基于多面体不确定集来决策微网内部源荷的不确定性,优化本地机组的调度策略。然后,对于各层合作博弈收益,采用改进纳什谈判模型对其进行分配。最后,采用一种可变惩罚参数交替方向乘子法-列与约束生成算法(variable penalty parameters alternating dir.ction multiplier method-column and constraint generation,VPPADMM-c&cG)分布式求解三重博弈ArO模型,以保护各主体的隐私。通过不同博弈场景的算例结果对所提模型能够实现多微网-产消者群社会总成本最小化及效益最大化进行了验证,可为不确定环境下多微网-产消者群协同优化提供参考。

新能源渗透电网使得系统频率响应时空分布特性愈发凸显，现有固定分区方法难以确保任何时段内频率响应的相似性，实时分区方法提供了解决思路，但又存在可能的错误分区和耗时问题。为此，本文首先提出一种频率曲线趋势和数值距离，并与动态时间弯曲距离、欧式距离和符号聚合近似距离作对比。然后使用层次聚类和k-means聚类算法实现系统实时分区，分区数目由Silhouette-coefficient(S-c)指数、calinski-Harabas(c-H)指数和Davies-Bouldin(D-B)指数三个指标评估得到。上述四种距离，两种聚类方法和三种指数互相组合得到24种实时分区模型，对比后选定计算频率曲线之间趋势和数值近似距离，采用层次聚类算法进行聚类，基于峰值S-c指数确定分区数目这一实时分区模型。最后在IEEE39系统、IEEE118系统和实际系统进行仿真，结果表明该实时分区模型可在不同时段针对不同扰动类型和位置实时变更分区，确保了区域内频率响应的相似性，有效提高了系统分区的准确性和快速性，且在区域惯量评估方面表现较好。

Dear editor,Severe acute respiratory syndrome coronavirus2(SArS-coV-2)infection can lead to severe outcomes in patients with cancer[1].It has been reported that patients with lung cancers disproportionately manifest severe cOVID-19 with a high rate of hospitalization and death[2].Notably,the SArS-coV-2 Spike(S)protein can induce hyper-inflammation in both epithelial cells and macrophages through toll-like r.ceptor(TLr)1/TLr2 or TLr2/6-dependent nuclear factor-kappaB(NF-κB)path-way[3].However,molecular and cellular evidence on whether the SArS-coV-2 virus affects the severity of lung cancer patients through TLr1/2 or TLr2/6 signaling remains unclear.

BAcKGrOUND Neuralgic amyotrophy(NA)is a rare disease with sudden upper limb pain followed by affected muscle *** most commonly affected area in NA is the upper part of the br.chial plexus,and the paraspinal muscles are rarely affected(1.5%),making these cases difficult to distinguish from cervical *** SUMMArY A 76-year-old male presented to the emergency department with left hip pain *** undergoing left femoral neck fr.cture surgery,he experienced sudden left shoulder pain for 10 days with subsequent left arm *** spine computed tomography revealed mild right asymmetr.c intervertebral disc bulging with a decreased c5-6disc *** weeks later,an electrodiagnostic study confirmed br.chial plexopathy findings involving the cervical *** resonance neurography was performed for a differential *** enhancement was identified at the upper trunk of the br.chial plexus,including the c5 nerve root.A suprascapular nerve hourglass-like focal constr.ction(HLFc)was also identified,confirming *** being diagnosed with NA,the patient r.ceived 15 mg prednisolone,twice daily,for 3 *** therapy was initiated,including left arm strengthening exercises and electr.cal stimulation *** shoulder muscle strength significantly improved one cONcLUSION NA's unique features like HLFc and paraspinal involvement are cr.cial for accurate diagnosis,avoiding confusion with cervical radiculopathy.
