＜正＞ We have prospective evaluated the potential beneficial effects of chronotherapy on renal function in patients with resistant hypertension. We studied 213 patients who were receiving 3 antihypertensive drugs in a single morning dose. Patients were randomly assigned to one of two groups according to the modification in their treatment strategy: 1) Changing one of the drugs,but keeping all 3 in the morning. 2) The same approach but administering the new drug at bedtime. BP was measured for 48h at baseline and after 3 months of intervention. The diurnal/nocturnal BP ratio was slightly reduced with all drugs on awakening,but significantly increased in patients receiving one drug at bedtime （P ＜ 0.001）. The percent decrease from baseline in urinary albumin excretion （ UAE） and increase in glomerular filtration rate （GFr） after treatment were significantly correlated with the increase in diurnal/nocturnal BP ratio （P ＜ 0.001）. Chronotherapy allows increasing the diurnal/nocturnal BP ratio towards a more dipper profile. This change in the circadian BP pattern is correlated with the improvement of renal function associated to reverting the high-risk non-dipper pattern into a dipper BP profile.

＜正＞ The MAPEC study was designed to investigate whether normalizing the circadian blood pressure （BP） profile towards a more dipper pattern reduces cardiovascular risk. This prospective study investigates 2643 subjects （1328 men）, 51.9±14.1 years of age. At inclusion and yearly thereafter, BP was measured at 20 - min intervals from 07:00 to 23: 00h and at 30 - min intervals at night for 48 hours. The median time of follow up so far was 3.2 years. Based on the baseline BP profile, cardiovascular morbidity was similar for extreme-dippers （1.23 events/100 patient-years） and dippers （1.14）, but significantly higher for non-dippers （2.81） and mainly for risers （8.70）. When morbidity was analyzed on the basis of the BP profile closer to the event, results indicate a diminished morbidity in extreme-dippers （0.38） and dippers （0.89）, and an increased morbidity in non-dippers （3.23） and risers （10.70）. The probability of event-free survival is markedly correlated with the day/night BP ratio. Most important, results suggest that increasing this ratio towards a more dipper pattern by Chronotherapy decreases cardiovascular risk, while decreasing the day/night BP ratio is associated with increased morbidity and mortality.

Prevalence of visual impairment and estimation of refractive errors among school children in Kakamega,Kenya

AIM:To investigate the prevalence of visual impairment(VI)and provide an estimation of uncorrected refractive errors in school-aged children,conducted by optometry students as a community ***:The study was *** 3343 participants were included in the *** initial examination involved assessing the uncorrected distance visual acuity(UDVA)and visual acuity(VA)while using a+2.00 D *** inclusion criteria for a subsequent comprehensive cycloplegic eye examination,performed by an optometrist,were as follows:a UDVAr)and/or a VA with+2.00 D≥0.8 decimal(0.96 logMAr).rESULTS:The sample had a mean age of 10.92±2.13y(range 4 to 17y),and 51.3%of the children were female(n=1715).The majority of the children(89.7%)fell within the age range of 8 to *** the ethnic groups,the highest representation was from the Luhya group(60.6%)followed by Luo(20.4%).Mean logMAr UDVA choosing the best eye for each student was 0.29±0.17(range 1.70 to 0.22).Out of the total,246 participants(7.4%)had a full eye *** estimated prevalence of myopia(defined as spherical equivalent≤-0.5 D)was found to be 1.45%of the total *** around 0.18%of the total sample had hyperopia value exceeding+1.75 *** astigmatism(cilrevalence was 1.26%of the total *** our cases of VI,76.2%could be attributed to uncorrected refractive *** was detected in 0.66%(22/3343)of the screened *** was no statistically significant correlation observed between age or gender and refractive ***:The primary cause of VI is determined to be uncorrected refractive errors,with myopia being the most prevalent refractive error *** findings underscore the significance of early identification and correction of refractive errors in school-aged children as a means to alleviate the impact of VI.

The complete and incomplete fusion cross section as well as one-neutron stripping process of ^(6)Li+^(94)Zr system were measured at the energies around the Coulomb barrier by onlineγ-ray *** addition to a 30%suppression factor when compared with the measured total fusion process,the complete fusion cross section in ^(6)Li+^(94)Zr system was observed to be significantly lower than those in the nearby ^(6)Li+^(90,96)Zr *** new experimental result implies that the coupling with breakup channel in the ^(6)Li-induced fusion processes can be affected by the inner structure of the target,which is still not clear in any available model *** the one-neutron stripping process,the direct production cross sections for each level in 95Zr were extracted and compared with the coupled reaction channel calculation,offering a unique opportunity to examine the single-particle nature of the produced excited *** the fact that an overall overestimation of the production cross section for 954-keV and1618-keV levels was observed in the comparison,further investigation is highly demanded in order to understand the full reaction mechanism for the one-neutron stripping process induced by ^(6)Li.

Utilization of rNA sequencing to investigate olfactory dysfunction in chronic rhinosinusitis without nasal polyps:A pilot study

Objectives:Prior research on olfactory dysfunction in chronic rhinosinusitis(CrS)has focused on patients with polyps and suggests that direct inflammation of the olfactory cleft mucosa plays a contributory *** purpose of this study was to evaluate gene expression in superior turbinate mucosal specimens,comparing normosmic and dysosmic CrS patients without polyps(CrSsNP).Methods:Tissue samples were obtained from the superior turbinates of patients with CrSsNP at the time of endoscopic sinus *** subsequently underwent rNA sequencing and functional analysis to investigate biological pathways associated with differentially expressed genes between dysosmic(n=7)and normosmic(n=4)***:Differential gene expression analysis comparing dysosmic and normosmic CrSsNP patients showed upregulation of 563 genes and downregulation of 327 *** stringent criteria for multiple comparisons,one upregulated gene(Immediate Early response 3[IEr3])had an false discovery rate(FDr)correction adjustedP value considered statistically significant(Preactome functional analysis revealed eight biological pathways significantly different between dysosmic and normosmic patients(Pr correction)including IL-4 and IL-13 signaling,IL-10 signaling,and rhodopsin-like ***:rNA sequencing of the superior turbinates in patients with CrSsNP can provide valuable information regarding biological pathways and genes involved in olfactory *** study supports literature suggesting that Type 2 inflammation may play a role in olfactory dysfunction in at least some patients with *** study also prompts questions regarding the role of IL-10,rhodopsin-like receptors,andIEr3 in the pathogenesis of olfactory dysfunction.

Comparison of trigeminal lateralization with differing stimulants

Trigeminal-specific stimulants have been shown to activate different receptors preferentially and this likely accounts for variation in sensory *** is unclear whether trigeminal sensitivity is similar across different transient receptor potential(TrP)receptors or if dysfunction of different receptors results in differing patient ***,a prospective cohort study was conducted,consisting of trigeminal lateralization testing with three different stimulants(eucalyptol,isothiocyanate,acetic acid),olfaction testing with Sniffin'Sticks,and measurement of various patient‐reported outcome measures(PrOMs).A total of 50 participants were enrolled across the olfactory *** TDI score was 27.1±8.3(range 7.0–39.5)with 38%normosmic and 62%*** trigeminal lateralization scores out of 20 in the overall cohort were 16.18(2.78)for eucalyptol,14.94(3.49)for mustard oil,and 15.28(3.68)for *** showed a significant correlation with threshold scores of Sniffin'Sticks.A significant correlation was found between acetic acid and various *** of the lateralization scores of the trigeminal stimulants correlated to each other significantly and there was no correlation to *** lack of correlation suggests that the measured sensitivity of one type of TrP receptor may not translate to similar sensitivity of the other *** investigations with TrPV1 and TrPA1 agonists are needed to corroborate our findings.

AIM：To evaluate the distribution of refractive error in young subjects in a rural area of Paraguay in the context of an international cooperation campaign for the prevention of blindness. METHODS：A sample of 1466 young subjects（ranging from 3 to 22 years old）,with a mean age of 11.21±3.63 years old,were examined to assess their distance visual acuity（VA）and refractive error. The first screening examination performed by trained volunteers,included visual acuity testing,autokeratometry and non-cycloplegic autorefraction. Inclusion criteria for a second complete cycloplegic eye examination by an optometrist were VA 〈20/25（0.10 log MAr or 0.8 decimal）and/or corneal astigmatism ≥1.50 D. rESULTS：An uncorrected distance VA of 0 log MAr（1.0decimal）was found in 89.2% of children. VA 〈20/25 and/or corneal astigmatism ≥1.50 D was found in 3.9% of children（n=57）,with a prevalence of hyperopia of 5.2%（0.2%of the total）in this specific group. Furthermore,myopia（spherical equivalent ≤-0.5 D）was found in 37.7% of the refracted children（0.5% of the total）. The prevalence of refractive astigmatism（cylinder ≤-1.50 D）was 15.8%（0.6% of the total）. Visual impairment（VI）（0.05≤VA≤0.3）was found in 12/114（0.4%）of the refracted eyes. Main causes for VI were refractive error（58%）,retinal problems（17%,2/12）,albinism（17%,2/12）and unknown（8%,1/12）.CONCLUSION：A low prevalence of refractive error has been found in this rural area of Paraguay,with higher prevalence of myopia than of hyperopia.

He eventually observed spermatozoa in the semen of men, dogs, horses, birds, fishes, amphibians, molluscs, and many insects, and concluded that they must be a universal feature of male reproduction.

In their recently published study in Nature,Jenson et al.1 provide compelling,unexpected evidence that bacterial cGAMP synthase(cGAS)and other cGAS/DncV-like Nucleotidyltransferases(CD-NTase)undergo ubiquitin-like conjugation(ULC)in type-II cyclic-oligonucleotide-based anti-phage signaling systems(CBASS).cGAS conjugation promotes cyclic-nucleotide(CN)formation and thus antiphage ***,the authors also reveal a new class of phage-encoded CBASS inhibitor,T4 Vs.4,a CN-sponge which limits CBASS effector function and promotes phage replication,revealing another weapon in the“arms race”between phages and their bacterial hosts.
