Background: Unstable atherosclerotic plaques activate blood cells which may adhere to the coronary endothelium causing vessel occlusion. However, it is unknown if different clinical syndromes associated with plaque rupture induce similar blood cell activation and adhesion to the endothelium. Methods: We studied changes in adhesion molecule expression of platelets(GPIIb/IIIa), neutrophils -CD18, CD11b and L-selectin -and monocytes(CD14) after interaction with active lesions of patients with stable angina subjected to PTCA and patients with unstable angina(UA). Generation of superoxide(SO) radicals from PMns and PMn sequestration in the coronary circulation were also assessed. Blood samples were collected from the aorta(Ao) and coronary sinus(CS) before and 15 min after PTCA(n=13) and within the first 48 h of UA(n=12). results: PTCA induced a marked up-regulation of CD18, CD11b, CD14 and GPIIb/IIIa with L-selectin shedding and reduced SO formation, whereas only minor L-selectin down-regulation and decreased SO production indicated activation in UA. However, a significant decrease in neutrophil count in the CS compared to the Ao was only observed in UA. Conclusions: The magnitude of cellular activation depends on the underlying clinical setting and just partially contributes to cell adhesion to the endothelium which might be modulated by different extent of vascular occlusion and shear forces.

Impaired swallowing mechanics of post radiation therapy head and neck cancer patients: A retrospective videofluoroscopic study

AIM: To determine swallowing outcomes and hyolaryngeal mechanics associated with post radiation therapy head and neck cancer(rt HnC) patients using videofluoroscopic swallow studies. METHODS: In this retrospective cohort study, video-fluoroscopic images of rt HnC patients(n = 21) were compared with age and gender matched controls(n = 21). Penetration-aspiration of the bolus and bolus residue were measured as swallowing outcome variables. Timing and displacement measurements of the anterior and posterior muscular slings elevating the hyolaryngeal complex were acquired. Coordinate data of anatomical landmarks mapping the action of the anterior muscles(suprahyoid muscles) and posterior muscles(long pharyngeal muscles) were used to calculate the distance measurements, and slice numbers were used to calculate time intervals. Canonical variate analysis with post-hoc discriminant function analysis was performed on coordinate data to determine multivariate mechanics of swallowing associated with treatment. Pharyngeal constriction ratio(PCr) was also measured to determine if weak pharyngeal constriction is associated with post radiation ***: The rt HnC group was characterized by poor swallowing outcomes compared to the control group in regards to: Penetration-aspiration scale(P normalized residue ratio scale(nrrS) for the valleculae(P = 0.002) and nrrS for the piriform sinuses(P = 0.003). Timing and distance measurements of the anterior muscular sling were not significantly different in the two groups, whereas for the PMS time of displacement was abbreviated(P = 0.002) and distance of excursion was reduced(P = 0.02) in the rt HnC group. A canonical variate analysis shows a significant reduction in pharyngeal mechanics in the rt HnC group(P r was significantly higher in the test group than the control group(P = 0.0001) indicating reduced efficiency in pharyngeal clearance. Background: Trichoepithelioma (TE) is a benign neoplasm that shares both clinical and histologic features with basal cell carcinoma (BCC). However, it is important to distinguish these neoplasms. Limited immunohistochemical stains are available to separate these two tumors. Methods: CD10 protein immunohistochemistry was performed on paraffin-embedded biopsies of 13 TE and 23 BCC diagnosed by routine microscopy. Cases were analyzed for pattern of CD10 expression by tumor cells and surrounding stroma. results: Twelve of 13 (92% ) TE showed positive stromal immunoreactivity. Of these, eight cases also demonstrated positivity of the papilla, and two also showed positivity of the basaloid cells. no TE demonstrated epithelial expression alone. On the other hand, expression of CD10 by basaloid cells was identified in 20 (87% ) cases of BCC. Stromal positivity was also identified in three cases of BCC. Condensation of CD10-positive stromal cells around basaloid nests was statistically significant in differentiating TE from BCC (p nversely, CD10-positive basaloid cells were seen predominantly in BCC (p nclusions: This study demonstrates a statistically significant difference in CD10 staining pattern between TE and BCC. Thus, CD10 may be a useful adjunct marker in distinguishing these tumors.

BACKGrOUnD AnD PUrPOSE-To describe baseline clinical features and outcomes of adults with moyamoya phenomenon treated at a single north American ***-We identified 34 adults with moyamoya phenomenon by review of angiographic *** presentation and baseline stroke risk factors were obtained by chart ***-up was obtained prospectively.A 5year Kaplan-Meier stroke risk was ***-The median age was 42(range 20 to 79)***-five were *** initial symptom was ischemia,hemorrhage,or asymptomatic in 24,7,and 3 patients,***-two had bilateral involvement and 12 had unilateral moyamoya *** stroke risk factors were similar between *** median follow-up in 31 living patients was 5.1(range 0.2 to 19.6)*** patients were treated with surgical revascularization(20 total hemispheres).In medically treated symptomatic hemispheres,the 5-year risk of recurrent ipsilateral stroke was 65%after the initial symptom and 27%after angiographic *** with bilateral involvement presenting with ischemic symptoms were at the highest risk of subsequent stroke(n=17,5-year risk of stroke with medical treatment after first symptom of 82%).In surgically treated hemispheres,the 5-year risk of perioperative or subsequent ipsilateral stroke or death was 17%.This was significantly different compared with medical treatment after first symptom(P=0.02)but not after angiographic ***-Moyamoya phenomenon in north American adults is associated with a high risk of recurrent stroke,particularly those with bilateral involvement and ischemic *** data suggest a potential benefit with surgery if diagnosis could be made earlier.

In this paper,algorithms of rOI segmentation,feature selecting and classifying were studied,and a novel scheme has been proposed to detect solitary pulmonary nodules on CT *** are segmented based on multi-scale morphological filtering method,features of rOI are selected using separability of probability,and rOIs are classified to nodule or non-nodule by improved Mahalanobis *** clinical cases were tested in this study,the sensitivity of nodule detection is 94.6%.Experiment results indicated that lung nodule detection using the proposed algorithms is with high sensitivity and low false positive rate,it can provide helpful information for automatic detection of pulmonary nodules in a computer-aided diagnosis(CAD) system.

背景：尽管血管近距离放射治疗是唯一批准用于治疗裸金属支架植入后再狭窄的方法，但是药物洗脱支架目前亦用于再狭窄的治疗。比较上述两种治疗方法各自优势的资料还很有限。目的：与血管近距离放射治疗裸金属支架内再狭窄比较，评价西罗莫司（雷帕霉素）洗脱支架的安全性及有效性。设计、地点及研究对象：于384例支架内再狭窄患者中进行前瞻性、多中心、随机试验。患者于2003年2月2004年7月入选自26家学院及社区医疗中心。本文资料均随访至2005年6月30日。干预：血管近距离放射治疗（n=125）或植入西罗莫司洗脱支架（n=259）。主要观测指标：术后9个月靶血管治疗失败（心源性死亡、心肌梗死或靶血管血运重建）。结果：两组患者的基线特征匹配良好。血管近距离放射治疗组与西罗莫司洗脱支架组患者病变长度相似（均值[SD]分别为16．76[8．55]laln和17．22[7．97]mln；P=0．61）。血管近距离放射治疗组手术成功率为99．2％（124／125），西罗莫司（雷帕霉素）洗脱支架组为97．3％（250／257）（P=0．28）。血管近距离放射治疗组靶血管治疗失败率为21．6％（27／125），西罗莫司洗脱支架组为12．4％（32／259）（相对危险度[relativerisk，rr]，1．7；95％可信区间[confidence interval，CI]，1．1～2．8；P=0．02）。血管近距离放射治疗组靶病变需要血运重建发生率为19．2％（24／125），西罗莫司洗脱支架组为8．5％（22／259）（rr，2．3；95％CI，1．3～3．9；P=0．004）。在血管造影随访中，血管近距离放射治疗组分析节段血管造影再狭窄率为29．5％（31／105），西罗莫司洗脱支架组为19．8％（45／227）（rr，1.5；95％CI，1.0—2．2；P=007）。6个月随访时，西罗莫司洗脱支架组最小管腔直径大于血管近距离放射治疗组（均值[SD]，1．52[0．63]mm比1．80[0．63]mm；P〈0．001），表明前者分析节段血管管腔净获益更大，其主要原因是植入支架以及无边缘再狭窄（0．68[0．60]mm比1．0[0．61]mm；P〈0．001）。结论：与血管近距离放射治疗裸金属支架内再狭窄比较，西罗莫司洗脱支架具有更好的临床及血管造影效果。
