目的：评价电针联合其他药物在甲状腺全切除术中提供镇痛和稳定血流动力学参数的有效性和安全性。方法：该随机对照试验囊括了100例患者，这些患者于2022年10月至2023年10月在越南国立针灸医院接受了甲状腺全切除术。患者被随机分为了两组。电针镇痛组（EA）对合谷穴（LI 4）、内关穴（PC 6）、水突穴（ST 10）、缺盆穴（ST 12）、翳风穴（SJ 17）5个穴位进行了电针刺激，对照组患者进行了双侧浅表颈丛神经阻滞。主要结局包括两组患者的镇痛水平和围手术期生命体征。此外，测定了EA组患者电针前后的疼痛阈值和血清β-内啡肽水平。结果：EA组和对照组分别有86%和76%的患者达到完全镇痛（A级），两组间无显著差异（P=1.00）。EA组患者接受EA后平均疼痛阈值翻倍（648.7(77.4) g/s vs. 305.3 (45.3) g/s, P m L(P <0.001)。所有患者在整个手术过程中的血流动力学维持稳定。结论：EA联合其他药物刺激了5个穴位，LI 4、PC 6、ST 10、ST 12和SJ 17，在甲状腺全切除术中耐受良好，有效地维持了适当的镇痛水平和血流动力学稳定性。

As a green environmentally-friendly material,rubberised concrete(ru C),which has the characteristics of low elastic modulus,large deformation capacity,high damping,good energy dissipation and good crack resistance,has attracted extensive attention and research in the civil engineering ***,most of existing studies are based on experimental tests on ru C material properties,and there has been no numerical study based on meso-scale modelling of ru C *** more comprehensively investigate the ru C dynamic material properties without conducting intensive experimental tests,this study developed a high-fidelity meso-scale model considering coarse and fine aggregates and rubber crumbs to numerically investigate the mechanical properties of rubberised concrete under different strain *** meso-scale model was verified against both quasi-static compressive testing data and Split Hopkinson Pressure Bar(SHPB)dynamic testing *** the verified numerical model,the dynamic properties of rubberised concrete with various rubber content(0%-30%)under different strain rates were *** numerical results show that the developed meso-scale model can use to predict the static and dynamic properties of rubberised concrete with high *** dynamic compressive strength of the rubberised concrete increases with the increment of the strain rate,and the strain rate sensitivity increases with the rubber content ranging from 0 to 30%.Based on intensive numerical simulation data,empirical DIFs is used as a function of strain rate and rubber content to predict the dynamic strength of rubberised concrete.

The availability of many high-degree Global Geopotential models(GGms), namely EGm2008, EIGEN-6C4,GECO, SGG-UGm-1, SGG-UGm-2, XGm2019e_2159, and GGmPlus, challenges users regarding which model is best for Vietnam. This study, therefore, evaluates their performance by comparing them with GNSS/leveling data over Vietnam. results show that their absolute and relative performances are largely independent of topographic conditions and geographical location and can be ranked into three classes:(1)XGm2019e_2159 has the highest accuracy,(2) the models EIGEN-6C4, GECO, SGG-UGm-1, SGG-UGm-2, and GGmPlus, have a very similar level of medium accuracy, while(3) EGm2008 is found to be the least accurate. In an absolute sense, the differences between GNSS/leveling and EGm2008-based height anomalies have a standard deviation(STD) of 0.290 ± 0.010 m, whereas, for XGm2019e_2159, this is 0.156 ± 0.006 *** other models have STDs of(0.18-0.19) ± 0.007 m. regarding relative performance without fitting, all GGms have comparable accuracies for baseline length of 5-20 km, while for baselines longer than 20 km,the STD of XGm2019e_2159 is 1.5 ppm-0.5 ppm(approximately 19%-40%) lower compared with EGm2008, and 0.5 ppm-0.25 ppm(approximately 7%-36%) lower compared with EIGEN6C4, GECO,SGG-UGm-1, SGG-UGm-2, and GGmPlus. In addition, the STDs decrease significantly from 20 to 12 ppm in the range of 5-10 km, slightly from 12 to 6 ppm for 10-35 km, very slightly from 6 to 2.5 ppm for35-200 km, and then remain almost unchanged for longer baselines. After fitting, the relative accuracies of all GGms are at the same level with negligible STD/rmSE values. Furthermore, only EGm2008 experiences significant regional differences, while other GGms show more homogeneous spatial variation of absolute accuracy over Vietnam. These findings can contribute to the development of local quasigeoid models in Vietnam and may be helpful with the improvement of GGms in the future.

The identification of jets originating from quarks and gluons,often referred to as quark/gluon tagging,plays an important role in various analyses performed at the Large Hadron Collider,as Standard model measurements and searches for new particles decaying to quarks often rely on suppressing a large gluon-induced *** paper describes the measurement of the efficiencies of quark/gluon taggers developed within the ATLAS Collaboration,using √s=13 TeV proton–proton collision data with an integrated luminosity of 140 fb^(-1) collected by the ATLAS *** taggers with high performances in rejecting jets from gluon over jets from quarks are studied:one tagger is based on requirements on the number of inner-detector tracks associated with the jet,and the other combines several jet substructure observables using a boosted decision tree.A method is established to determine the quark/gluon fraction in data,by using quark/gluon-enriched subsamples defined by the jet *** in tagging efficiency between data and simulation are provided for jets with transverse momentum between 500 GeV and 2 TeV and for multiple tagger working points.

Artificial intelligence (AI) is the technique that enables computers to solve problems and perform tasks that traditionally require human intelligence. The availability of large amounts of medical data from electronic medical records and powerful modern microcomputers enables the development of AI in medicine. AI has proven its applicability in many different medical areas, such as drug discovery, diagnostic radiology and pathology, as well as interventional applications in cardiology and surgery. However, until today, AI is scarcely used in the clinical practice of anesthesiology. Although there has been a significant body of research published on AI applications for anesthesiology in the literature, the number of developed robot systems for commercial use or those ready for clinical trials remains limited. The limitations of AI systems are identified and discussed, which include incorrect medical data formatting, individual patient variability, the lack of ability of current AI systems, anesthesiologist inexperience in AI usage, system unreliability, unexplainable AI conclusions and strict regulations. In order to ensure anesthesiologists’ trust in AI systems and improve their implementation in daily practice, strict quality control of the systems and algorithms should be undertaken. Further, anesthesiology personnel should play an integral role in the development of AI systems before we are able to see more AI integration in clinical anesthesiology.

One-year survival in recipients older than 50 bridged to heart transplant with Impella 5.5 via axillary approach

BACKGrOUND Optimizing patients with advanced heart failure before orthotopic heart transplantation(OHT), especially in patients greater than 50 years old, is imperative to achieving successful post-transplant outcomes. Complications are well-described for patients bridged to transplant(BTT) with durable left ventricular assist device(LVAD) support. Given the lack of data available in older recipients after the recent increase in mechanical support use, we felt it crucial to report our center’s one-year outcomes in older recipients after heart transplantation with percutaneously placed Impella 5.5 as a *** Forty-nine OHT patients were supported with the Impella 5.5 intended as a bridge between December 2019 and October 2022 at mayo Clinic in Florida. Data were extracted from the electronic health record at baseline and during their transplant episode of care after Institutional review Boards approval as exempt for retrospective data *** Thirty-eight patients aged 50 or older were supported with Impella 5.5 as BTT. Ten patients underwent heart and kidney transplantation within this cohort. The median age at OHT was 63(58–68) years, with 32 male(84%) and six female patients(16%). Etiology was divided into ischemic(63%) and non-ischemic cardiomyopathy(37%). The baseline median ejection fraction was 19%(15–24). most patients were in blood group O(60%), and 50% were diabetic. The average duration of support was 27 days(range 6–94). The median duration of follow-up is 488 days(185–693). For patients that have reached the 1-year follow-up timeframe(22 of 38, 58%), the 1-year post-transplant survival is 95%.CONCLUSION Our single-center data provides awareness for using the Impella 5.5 percutaneously placed axillary support device in older heart failure patients in cardiogenic shock as a bridge to transplantation. One-year survival outcomes after heart transplantation are excellent despite the older recipient’s age and prolonged pre-transplant support.
