Laparoscopic management of remnant gall bladder with stones: Lessons from a tertiary care centre's experience

Objective:Laparoscopic cholecystectomy is currently the gold standard for treating symptomatic gallstone disease.Despite its success,approximately 10%of patients may experience persistent biliary symptoms,leading to the post-cholecystectomy syndrome.A remnant gallbladder with cystic duct or bile duct stones is one of the causes of this syndrome.The objective of this study was to shed light on the clinical manifestations,evaluation,therapeutic strategies,and outcomes associated with laparoscopic management of symptomatic remnant gallbladders.Methods:This was a retrospective study,conducted over a five-year period(January 2017 to December 2022)at Apollo Hospitals in South India.All patients who underwent laparoscopic completion cholecystectomy for a remnant gall bladder were included.The following data were collected:patient demographics,symptoms,preoperative investigations,intraoperative details and post operative outcomes.results:In total,36 patients were included and analysed.The majority of patients were male(25,69.4%),with a mean age of 50.7±12.1 years.The most common presentation was pain in the upper abdomen or right upper quadrant region(24,66.7%).The laparoscopic approach was attempted in all patients,with a success rate of 94.4%.Two patients required conversion to open surgery.Cholecystoenteric fistula to the colon was observed in one patient.Choledocholithiasis was observed in 7 patients(19.4%),and stone clearance was successfully achieved using endoscopic retrograde cholangiopancreatography in all patients preoperatively.Conclusion:Incomplete gall bladder removal either intentionally or unintentionally leaves a remnant gall bladder that is at risk for stone formation and infection.Patients who have this clinical entity with symptoms require a redo or complete cholecystectomy,a complex procedure associated with certain risks.This study highlights the feasibility and safety of laparoscopic completion cholecystectomy for the management of remnant gallbladder with cystic duct or bile duct stones.

Fuzzy reliability and Availability of System under a Calendar-based Inspection Involving Multiple Failures and Its Application to Wind Turbine System

Uncertainty is an important factor that needs to be considered while analyzing the performance of any engineering system.In order to quantify uncertainty,fuzzy set theory is frequently used by most of researchers,including energy system experts.According to the classical reliability theory,component lifetimes have crisp parameters,but due to uncertainty and inaccuracy in data,it is sometimes very difficult to determine the exact values of these parameters in real-world systems.To overcome this difficulty in the current research,failure and repair rates were taken as triangular fuzzy numbers to determine the fuzzy availability of a system undergoing calendar-based periodic inspection subject to multiple failure modes(FMs).It was assumed that each component in the system had an exponential failure rate and repair rate with fuzzy parameters.System FMs were explicitly taken into account when a functional state of the system was considered.Each FM had a random failure time.On the occurrence of any failure,a random time was selected for the relevant corrective repair work.The proposed research was studied for one of the major sources of green energy,namely a wind turbine system wherein all the derived propositions have been implemented on it.

Enhancing fatigue performance of AZ31 magnesium alloy components fabricated by cold metal transfer-based wire arc directed energy deposition through LPB

Cold Metal Transfer-Based Wire Arc Directed Energy Deposition(CMT-WA-DED)presents a promising avenue for the rapid fabrication of components crucial to automotive,shipbuilding,and aerospace industries.However,the susceptibility to fatigue of CMT-WA-DED-produced AZ31 Mg alloy components has impeded their widespread adoption for critical load-bearing applications.In this study,a comprehensive investigation into the fatigue behaviour of WA-DED-fabricated AZ31 Mg alloy has been carried out and compared to commercially available wrought AZ31 alloy.Our findings indicate that the as-deposited parts exhibit a lower fatigue life than wrought Mg alloy,primarily due to poor surface finish,tensile residual stress,porosity,and coarse grain microstructure inherent in the WA-DED process.Low Plasticity Burnishing(LPB)treatment is applied to mitigate these issues,which induce significant plastic deformation on the surface.This treatment resulted in a remarkable improvement of fatigue life by 42%,accompanied by a reduction in surface roughness,grain refinement and enhancement of compressive residual stress levels.Furthermore,during cyclic deformation,WA-DED specimens exhibited higher plasticity and dislocation density compared to both wrought and WA-DED+LPB specimens.A higher fraction of Low Angle Grain Boundaries(LAGBs)in WA-DED specimens contributed to multiple crack initiation sites and convoluted crack paths,ultimately leading to premature failure.In contrast,wrought and WA-DED+LPB specimens displayed a higher percentage of High Angle Grain Boundaries(HAGBs),which hindered dislocation movement and resulted in fewer crack initiation sites and less complex crack paths,thereby extending fatigue life.These findings underscore the effectiveness of LPB as a post-processing technique to enhance the fatigue performance of WA-DED-fabricated AZ31 Mg alloy components.Our study highlights the importance of LPB surface treatment on AZ31 Mg components produced by CMT-WA-DED to remove surface defects,enabling their widespread use in load-bea

Incense smoke(IS)inhalation exposure system:Physicochemical characterization,IS particle deposition and clearance in human airway using MPPD model

Incense smoke(IS)is source of indoor air pollution and key risk for diverse human diseases.Less in-formation is available regarding controlled IS rodent inhalation exposure system and IS particulate matter(PM)deposition in human airways.Study aimed to demonstrate stable ISPM physicochemical parameters of 10 incense products inside the customized whole body inhalation exposure chamber(without animal)connected to smoke generation unit via aerosol mixing device.IS analyzed for size segregated PM emission,ISPM in vitro aerodynamics(MMAD and GSD determination),fine and ultrafine particle's SEM,SEM-EDX and PAH analysis.Using real life exposure scenario by utilizing MMAD,GSD and PM concentration after Tier 1 exposure assessment as key input parameters,ISPM dosimetry in infant(3 months)and adult(21 years male and female)human airways was calculated using multiple-path particle dosimetry(MPPD 3.04)modeling.Mass median aerodynamic diameter(MMAD)and geo-metric standard deviation(GSD)ranged between 0.55 and 2.10μm and 1.22 to 1.77(polydisperse)respectively.PM1.0 and PM0.1 showed multiple morphology and presence of heavy and trace elements.PAH like acenaphthylene,anthracene,fluorene,naphthalene and phenanthrene were detected(0.84-143.17μg/g).MPPD results showed higher ISPM deposition in pulmonary region and lowest in trachea bronchial region.ISPM deposition in tissue was higher in lower,peripheral lung as compared to upper and central lung tissue.Whole body inhalation exposure system showed stable IS atmosphere(physi-cochemical parameters)indicating the device suitability in future inhalation studies.MPPD ISPM deposition fraction and clearance data showed deep lung penetrating and retention behavior with higher risk in infant followed by female and then male.These modeled particle deposition and clearance data may be useful in risk assessment analysis of IS.

Cool white light and tunable multicolor emission from Tb^(3+)/Dy^(3+)co-activated glasses under different excitations for WLEDs

A set of co-doped(Tb^(3+)/Dy^(3+))lithium zinc borate(LZB)glasses were developed by melt quenching.The structural evaluation was performed for synthesized glassy matrices.The Dy^(3+)and Tb^(3+)individually doped glasses exhibit intense yellow and green luminescence bands at 575 nm(^(4)F_(9/2)→^(6)H_(13/2))and543 nm(^(5)D_(4)→^(7)F_(5)),respectively.The sensitization effect of Dy^(3+)on Tb^(3+)was analyzed by increasing the Tb^(3+)content with respect to the optimum Dy^(3+)content(0.5 mol%)in Dy^(3+)/Tb^(3+).The spectral overlay of Dy^(3+)luminescence and Tb^(3+)absorption profiles,Dy^(3+)/Tb^(3+)PL spectra under different excitations 352,362,376,and 385 nm,shortening decay lifetimes of Dy^(3+)in Dy^(3+)/Tb^(3+)co-activated glasses,energy transfer(ET)parameters,chromaticity coordinates and their corresponding correlated temperatures all help to explain ET from Dy^(3+)to Tb^(3+).At 385 nm of Dy^(3+)excitation,the optimized co-activated(0.5Dy^(3+)+1.0Tb^(3+)):LZB glass displays cool white light emission.The non-radiative ET from Dy^(3+)to Tb^(3+)is dominated by electric dipole-dipole interaction and its ET efficiency was calculated to be 63%.At the same time,reverse ET from Tb^(3+)to Dy^(3+)was also analyzed.The shift in color coordinates from dominant yellow to greenish-yellow,green and white light emission suggests that Dy^(3+)/Tb^(3+)coactivated LZB glasses can be a potential candidate for UV converted multicolor and white light emitting devices.

Comparison of rare earths doping(neodymium and samarium) on structural and magnetic properties of Ni-Zn-Bi nanoferrites

Excellent magnetic properties in ferrites are required for high-frequency applications and for wastewater treatment. Thus, the present study shows the comparison of magnetic and structural properties of Nd and Sm substituted Ni-Zn-Bi ferrites with the series Ni_(0.5)Zn_(0.5)Bi_(0.04)Nd_(x)Fe_(1.96-x)O_4(with step size 0.002)and Ni_(0.5)Zn_(0.5)Bi_(0.04)Sm_(x)Fe_(1.96-x)O_4(with step size 0.02) prepared using citrate precursor method. The impact of the substitution of rare earth ions(Nd and Sm) on magnetic properties of the synthesized samples is observed using a vibrating sample magnetometer(VSM). The saturation magnetization values enhance considerably from 52 to 58 emu/g for Nd^(3+)ions and 39 to 57 emu/g for Sm^(3+) ions, thus, making these materials magnetically hard. Further, the higher value of coercivity is also observed ranging from133 to 167 Oe for Nd^(3+)ions and 81 to 155 Oe for Sm^(3+) ions. The shape of hysteresis loops indicates a super paramagnetic and ferromagnetic behavior in the obtained samples. The squareness ratio value isropy of particles and hence, these ferrites are suitable for microwaveabsorbing and in permanent magnetic materials. The X-ray diffraction(XrD) pattern shows the formation of pure cubic crystallites, where, lattice parameters range from 0.840 to 0.839 nm and from 0.838to 0.839 nm for Nd^(3+)and Sm^(3+) ions substitution, respectively. The crystallite size ranges between 28.63to 29.89 nm and 18.33 to 26.23 nm, for substitution of Nd^(3+)and Sm^(3+) ions, respectively. Field emission scanning electron microscopy(FESEM) shows the formation of homogeneous grains, whereas, energy dispersive spectrometer(EDS) counts describe the purity of the samples. The Sm concentration x = 0.10has the maximum surface area with value of 42.6 m^(2)/g which proves to be having good data storage application due to high surface area. Sustainable agriculture plays a crucial role in meeting the growing global demand for food while minimizing adverse environmental impacts from the overuse of synthetic pesticides and conventional fertilizers.In this context,renewable biopolymers being more sustainable offer a viable solution to improve agricultural sustainability and production.Nano/micro-structural supramolecular biopolymers are among these innovative biopolymers that are much sought after for their unique features.These biomaterials have complex hierarchical structures,great stability,adjustable mechanical strength,stimuli-responsiveness,and self-healing attributes.Functional molecules may be added to their flexible structure,for enabling novel agricultural uses.This overview scrutinizes how nano/micro-structural supramolecular biopolymers may radically alter farming practices and solve lingering problems in agricultural sector namely improve agricultural production,soil health,and resource efficiency.Controlled bioactive ingredient released from biopolymers allows the tailored administration of agrochemicals,bioactive agents,and biostimulators as they enhance nutrient absorption,moisture retention,and root growth.Nano/micro-structural supramolecular biopolymers may protect crops by appending antimicrobials and biosensing entities while their eco-friendliness supports sustainable agriculture.Despite their potential,further studies are warranted to understand and optimize their usage in agricultural domain.This effort seeks to bridge the knowledge gap by investigating their applications,challenges,and future prospects in the agricultural sector.Through experimental investigations and theoretical modeling,this overview aims to provide valuable insights into the practical implementation and optimization of supramolecular biopolymers in sustainable agriculture,ultimately contributing to the development of innovative and eco-friendly solutions to enhance agricultural productivity while minimizing environmental impact.

Hybanthus enneaspermus (L.) F. Muell belonging to the family Violaceae, popularly known as ratanpurus (Hindi) is a herb or a shrub distributed in the tropical and subtropical regions of the world. In the Ayurvedic literature, the plant is reported to cure conditions of "Kapha" and "Pitta", urinary calculi, strangury, painful dysentery, vomiting, burning sensation, wandering of the mind, urethral discharge, blood trouble, asthma, epilepsy, cough, and to give tone to the breasts. Phytochemically, the plant contains a considerable amount of dipeptide alkaloids, aurantiamide acetate, isoarborinol, and β-sitosterol, sugars, flavonoids, steroids, triterpenes, phenols, flavones, catachins, tannins, anthraquinones and amino acids. Pharmacologically, the plant is reported to possess antidiabetic, antiplasmodial, antimicrobial, anticonvulsant, nephroprotective, aphrodisiac, hepatoprotective, antiinflammatory, aldose reductase inhibitory and free radical scavenging activities. The information provided in this review will be worthwhile to know the applicability of H. enneaspermus for the treatment of various acute or chronic diseases with a diverse nature of phytoconstituents. The overall data in this review article were collected from various scientific sources on the research of H. enneaspermus.
