Predictive Factors of renal Failure in COVID-19 Patients at the Anti-COVID Center in Lome, Togo

Background: Angiotensin-converting enzyme 2 has been identified as the receptor that allows the entry of SarsCov2 into the human cell. Its expression in the kidney is 100 times higher than in the lung;thus, making the kidney an excellent target for SarsCov2 infection manifesting as renal failure (rF). The objective of this study was to determine the predictive factors of rF during COVID-19 in the Togolese context. Patients and Methods: This was a retrospective descriptive and analytical study conducted at the Lomé Anti-COVID Center including the records of patients hospitalized for COVID-19, of age ≥ 18 years and having performed a creatinemia. rF was defined by a GFr 2 calculated according to the MDrD formula. Patients were randomized into 2 groups according to GFrresults: 482 patients were selected for this study with a mean age of 58.02 years. Sixty-five percent of the patients were men, i.e., a sex ratio of 1.88. Fifty-two patients had rF, i.e., a frequency of 10.8%.There were 65% men (315 cases), for a sex ratio (M/F) of 1.88. risk factors for renal failure in COVID-19 were age ≥ 65 years (Ora 2.42;CIa95% [1.17 - 4.95];p = 0.016), anemia (Ora 2.49;CIa95% [1.21 - 5.26];p = 0.015), moderate (Ora 13;CIa95% [2.30 - 2.44];p = 0.017), severe (Ora 26.2;CIa95% [4.85 - 4.93];p = 0.002) and critical (Ora 108;CIa95% [16.5 - 21.76];p Conclusion: renal failure would therefore be related to the severity of COVID-19 and is the most formidable factor, conditioning the course of the disease and the patient’s vital prognosis.

目的判定静脉内注射钆塞酸二钠与钆贝葡胺后引起急性短暂性呼吸困难和(或)动脉期影像劣化的多少。材料与方法此项研究经机构审查委员会的批准且参与实验的病人均签署了参与前瞻性研究的知情同意书。对198例有肝胆适应证的192例病人行含钆对比剂注射[99例采用钆塞酸二钠(10mL,n=97;8mL,n=1;16mL,n=1);99例用钆贝葡胺(0.1mmol/kg体质量;最大剂量,20mL)],并对其进行评估。主观评估病人主诉。客观评估T1加权像在注射对比剂前、后(动脉期,静脉期,或动态晚期,或细胞外期)呼吸运动的下降,Mr成像数据以随机盲法由5名读片者用5分制模式独立评估,平均分值为4或更高提示严重位移。因子间对照分别适用χ2检验或Fisher确切检验。结果注射钆塞酸二钠后病人主诉急性短暂性呼吸困难显著多于注射钆贝葡胺后者[14%(14/99)∶5%(5/99),P=0.05]。动脉期数据质量严重下降,注射钆塞酸二钠组较注射钆贝葡胺组显著增多,在一般人群中为[17%(17/99)∶2%(2/99),P=0.0007],在肝硬化亚群中为[19%(14/72)∶3%(1/37),P=0.02]。这一现象并不影响静脉期[1%(1/99)∶2%(2/99),P>0.99(所有人群)]或动态晚期或细胞外期[2%(2/99)∶0(0/99),P=0.5(所有人群)]。没有病人要求治疗自限性呼吸困难。结论静脉内注射钆塞酸二钠可导致急性自限性呼吸困难,对动脉期Mr影像质量有不利影响,且其发生率显著高于静脉内注射钆贝葡胺者。

Objective and Impact *** present a fully automated hematological analysis framework based on single-channel(single-wavelength),label-free deep-ultraviolet(UV)microscopy that serves as a fast,cost-effective alternative to conventional hematology *** analysis is essential for the diagnosis and monitoring of several diseases but requires complex systems operated by trained personnel,costly chemical reagents,and lengthy ***-free techniques eliminate the need for staining or additional preprocessing and can lead to faster analysis and a simpler *** this work,we leverage the unique capabilities of deep-UV microscopy as a label-free,molecular imaging technique to develop a deep learning-based pipeline that enables virtual staining,segmentation,classification,and counting of white blood cells(WBCs)in single-channel images of peripheral blood *** train independent deep networks to virtually stain and segment grayscale images of *** segmented images are then used to train a classifier to yield a quantitative five-part WBC *** virtual staining scheme accurately recapitulates the appearance of cells under conventional Giemsa staining,the gold standard in *** trained cellular and nuclear segmentation networks achieve high accuracy,and the classifier can achieve a quantitative five-part differential on unseen test *** proposed automated hematology analysis framework could greatly simplify and improve current complete blood count and blood smear analysis and lead to the development of a simple,fast,and low-cost,point-of-care hematology analyzer.

Giant Angiomyolipoma of the kidney Presenting as Anaemia—A rare Presentation

Angiomyolipoma presenting with severe anaemia is a rare entity. Urgent operative intervention is warranted in such patients especially if the tumour is “Giant” i.e. greater than 10 cm and involves the whole kidney. We chronicle the case of a 35 year old lady who presented with a rapidly enlarging renal lump in the left loin associated with severe anaemia. MrI was highly suggestive of angiomyolipoma. She underwent nephrectomy in view of continuous intratumoral haemorrhage. The tumour measured 18 × 13 × 8 cm and weighed approximately 1.6 kilograms after removing blood and blood clots. The histopathology was suggestive of angiomyolipoma. Angioembolization may be considered for tumours involving a portion of the kidney, as a part of the nephron sparing approach.

Congenital Absence of the Cystic Duct: A rare but Significant Anomaly

Cholecystectomy is the most common digestive tract surgery performed worldwide and injury to the bile duct leads to both acute and chronic sequelae. The incidence of bile duct injury is increased in the presence of severe inflammation and is compounded by congenital abnormalities of the biliary tract. Congenitally absent cystic duct is one such rare anomaly with significant surgical implications. So far only nine clear cases of congenitally absent cystic duct have been reported. In this report we describe two additional cases of a congenitally absent cystic duct and provide a comprehensive discussion of the clinical significance, and appropriate surgical management of this anomaly.

Emphysematous Pyelonephritis in a Black African Woman Managed with Antibiotics Alone

Introduction: Emphysematous pyelonephritis (EPN) is a severe, life-threatening infection of the renal parenchyma. This condition is characterized by the production of intrarenal and perinephric gas. In the world, the EPN is currently limited to case series reported. No cases have been described in black Africa. Aims: related a first case of EPN diagnosed and treated in the University Hospital Center Sylvanus Olympio of Lomé in Togo. Observation: A 40 years old woman, with antecedent of diabetes presented pyelonephritis emphysematous class 2. She was treated successfully with antibiotic alone without using percutaneous drainage or nephrectomy. Conclusion: As reported in every case series, it was a young diabetic patient with a clinical features of acute pyelonephritis which CT scan had helped lay the EPN class 2 diagnosed. The germ was klebsiella pneumoniae. She was treated with adapted antibiotic alone.

risk Factors of renal Failure in HIV Patients at Initiation of ArV Treatment: retrospective Study of 3118 Patients Followed in Infectious Diseases Department at LoméUniversity Hospital

HIV infection is a major cause of chronic kidney disease, associated with high morbidity and mortality in sub-Saharan Africa. The objective of this study is to assess the prevalence and risk factors of renal disease at initiation of antiretroviral therapy. This was a descriptive and analytical retrospective study carried out in the infectious and tropical diseases department at Sylvanus Olympio University Hospital. The data have been extracted from the ESOPE software. kidney disease was defined by a GFr, estimated by MDrD (Modification of Diet in renal Disease) formula, less than 60 ml/min/1.73 m2. risk factors associated with kidney disease were assessed using univariate and multivariate analysis. There were 3118 HIV-infected patients included in our study. The median estimated filtration rate was 94.7 ml/min/1.73 m2: 2.9% had an eGFr 2. 1303 had kidney disease (41.8%). Most patients (30.8%) were in the WHO clinical stage 1. The median CD4 count was 165/μL [IQr = 72 - 274/μL];the median hemoglobin level was 10.4 g/dL [IQr = 8.8 - 11.9 g/dL];all patients had thrombocytopenia less than 100.000/mm3;8.5% had leukocytosis greater than 10.000/mm3. Most of patients had HIV1. In the multivariate analysis, age greater than 40 years (p 3 were significantly associated with renal disease. The prevention of kidney disease must go through the identification of its risk factors in the target populations.
