Intr.duction: [18F]-fluor.-methylcholine (FCH) PET/CT and Mr. with diffusion-weighted Mr. (DW-Mr.) have insufficient per.or.ance in lymph node staging of pr.mar. pr.state cancer.by themselves, but the combination may per.or. better. We aim to pr.spectively deter.ine the diagnostic per.or.ance of combined FCH PET and Mr. for.lymph node staging. Methods: This was a single site study of diagnostic accur.cy in a well-defined gr.up of 21 consecutive high-r.sk pr.mar. pr.state cancer.patients (>30% chance of lymph node metastases) in a lar.e community hospital. We per.or.ed FCH PET/CT and Mr. with DW-Mr. pr.or.to endoscopic extended pelvic lymph node dissection (EPLND). PET was fused and inter.r.ted together.with var.ous Mr. image sets (T1, T2, DWIBS) and was only scor.d positive when a lymph node seen on Mr. coincided with incr.ased focal FCH uptake on PET. Findings wer. compar.d with detailed histological evaluation, on a per.patient and per.r.gion level. We calculated sensitivity, specificity, positive and negative pr.dictive value of combined PET-Mr.. r.sults: 14 out of 21 patients had metastatic lymph nodes with 37 out of 164 evaluable r.gions har.or.ng metastases. On a per.patient analysis, PET-Mr. had a sensitivity/specificity of 79/100% with a PPV/NPV of 100/77%. On a per.r.gion analysis (n = 164) these figur. wer. 65/99% and 96/91%, r.spectively. Conclusions: Combined DW-Mr. and FCH PET/CT has a ver. high positive pr.dictive value in high r.sk pr.state cancer.patients. If confir.ed in lar.er.ser.es a positive combined scan may safely allow cancellation of sur.ical staging in selected patients, depending on local pr.tocols in N1 M0 patients.

Cor.nal magnetic fields evolve quasi-statically over.long timescales and dynamically over.shor. timescales.As of now ther. exist no r.gular.measur.ments of cor.nal magnetic fields,and ther.for. gener.ting the cor.nal magnetic field evolution using obser.ations of the magnetic field at the photospher. is a fundamental r.quir.ment to under.tanding the or.gin of tr.nsient phenomena fr.m solar.active r.gions(Ar.).Using the magneto-fr.ction(MF)appr.ach,we aim to simulate the cor.nal field evolution in the solar.Ar.11429.The MF method is implemented in the open sour.e PENCIL CODE along with a dr.ver.module to dr.ve the initial field with differ.nt boundar. conditions pr.scr.bed fr.m obser.ed vector.magnetic fields at the photospher..In or.er.to wor. with vector.potential and the obser.ations,we pr.scr.be thr.e types of bottom boundar. dr.ver. with var.ing fr.e-magnetic ener.y.The MF simulation r.pr.duces the magnetic str.ctur.,which better.matches the sigmoidal mor.hology exhibited by Atmospher.c Imaging Assembly(AIA)images at the pr.-er.ptive time.We found that the alr.ady shear.d field fur.her.dr.ven by the shear.d magnetic field will maintain and fur.her.build the highly shear.d cor.nal magnetic configur.tion,as seen in Ar.11429.Data-dr.ven MF simulation is a viable tool to gener.te the cor.nal magnetic field evolution,captur.ng the for.ation of the twisted flux r.pe and its er.ption.

作曲家利盖蒂长期以来与哲学家阿多诺保持着通信联系,而且利盖蒂对20世纪后半叶的新音乐发展也具有重要的参照作用,但这些方面一直被人们所忽视。作者在考证大量史料的基础上,以开放的“音乐现代主义”概念为前提,结合对《伟大的死亡》①等作品的分析与阐释,剖析了利盖蒂与阿多诺之间的理念关联,特别是阿多诺《困境》一文对利盖蒂的影响,重新审视了作曲家在现代主义中的地位与立场。本文最终认为,利盖蒂深受阿多诺的“否定性”与“非形象音乐”等理念的影响,以具有表现性与意象性的音乐瞬间来表达死亡等方式,瓦解了“保守”与“先锋”之间的二元对立,展现出“主观性”与“客观性”相结合的悖论美学。

This r.cently published cr.ss-sectional study that included 99 men fr.m Massachusetts Gener.l Hospital Fer.ility Center. Boston, MA, USA, r.por.s that a high intake of satur.ted fats was inver.ely associated with sper. concentr.tion and total sper. count and a high intake of omega-3 polyunsatur.ted fats （PUFAs） was positively associated with sper. mor.hology. Although the study is based upon a highly selected gr.up of males and is of cr.ss-sectional natur., the findings ar. of inter.st, since poor.semen quality is a fr.quent pr.blem in some par.s of the wor.d and identification of modifiable r.sk factor. is highly wanted.

The evidence is scar.e on the association between age at puber.y and semen quality. A cohor. of 320 Danish men aged 18-21 year. enr.lled in the ＂Healthy Habits for.Two＂ bir.h cohor. pr.vided self-r.por.ed data on puber.al indicator. and deliver.d semen and blood samples. The r.sults indicated an association between older.age at puber.al development and lower.semen quality and alter.d r.pr.ductive hor.ones concentr.tions as measur.d in young adult life. Men who had their.fir.t noctur.al emission, star. of pubic hair.gr.wth and fir.t voice br.ak episode when older.than 15 year. had 37.0%, 45.0% and 32.7% lower.sper. concentr.tion; 37.8%, 44.2% and 29.1% lower.total sper. count; 7.4%, 13.4% and 15.3% lower.testoster.ne concentr.tion; and 21.3%, 1.5% and 3.7% lower.inhibin B concentr.tion, r.spectively, compar.d with the men who wer. younger.than 13 year. at their.fir.t puber.al indicator.. Only few of the r.sults wer. statistically significant, but similar.tendencies wer. seen in sever.l of the r.pr.ductive par.meter. suggesting an association between the timing of puber.al development and r.pr.ductive health later.in life.

AIM:To investigate the pr.valence of gallstone dis-ease(GSD) and to evaluate the r.sk of symptomatic GSD among diabetic patients.METHODS:The study was conducted by analyzing the National Health r.sear.h Institutes(NHr.) dataset of ambulator. car. patients,inpatient claims,and the updated r.gistr. of beneficiar.es fr.m 2000 to 2008.A total of 615 532 diabetic patients without a pr.or.histor. of hospital tr.atment or.ambulator. car. visits for.symptomatic GSD wer. identified in the year.2000.Age-and gender.matched contr.l individuals fr.e fr.m both GSD and diabetes fr.m 1997 to 1999 wer. r.ndomly selected fr.m the NHIr.database(n = 614 871).The incidence densities of symptomatic GSD wer. estimated accor.ing to the subjects' diabetic status.The distr.butions of age,gender.occupation,income,and r.sidential ar.a ur.anization wer. compar.d between diabetic patients and contr.l subjects using Cox pr.por.ion hazar.s models.Differ.nces between the r.tes of selected comor.idities wer. also assessed in the two gr.ups.r.SULTS:Over.ll,60 734 diabetic patients and 48 116 contr.l patients developed symptomatic GSD and under.ent oper.tions,r.sulting in cumulative oper.tion r.tes of 9.87% and 7.83%,r.spectively.The age and gender.distr.butions of both gr.ups wer. similar.with a mean age of 60 year. and a pr.dominance of females.The diabetic gr.up had a significantly higher.pr.valence of all comor.idities of inter.st.A higher.incidence of symptomatic GSD was obser.ed in females than in males in both gr.ups.In the contr.l gr.up,females under.the age of 64 had a significantly higher.incidence of GSD than the cor.esponding males,but this differ.nce was r.duced with incr.asing age.The cumulative incidences of oper.tions for.symptomatic GSD in the diabetic and contr.l gr.ups wer. 13.06 and 9.52 cases per.1000 per.on-year.,r.spectively.Diabetic men exhibited a higher.incidence of oper.tions for.symptomatic GSD than did their.counter.ar.s in the contr.l gr.up(12.35 vs 8.75 cases per.1000 per.on-year.).CONCLUSION:The association of diabet
