AIM： To compar. the efficacy of the pr.ton-pump inhibitor. r.bepr.zole, with that of the H2-r.ceptor.antagonist, r.nitidine, as on-demand ther.py for.r.lieving symptoms associated with non-er.sive r.flux disease （NEED）.METHODS： This is a single center. pr.spective, r.ndomized, open-label tr.al of on-demand ther.py with r.bepr.zole （gr.up A） vs r.nitidine （gr.up B） for.4 wk. Eighty-thr.e patients who pr.sented to the Amer.can Univer.ity of Beir.t Medical Center.with per.istent gas- tr.esophageal r.flux disease （GEr.） symptoms and a nor.al upper.gastr.intestinal endoscopy wer. eligible for.the study. Patients in gr.up A （n = 44） wer. al-lowed a maximum r.bepr.zole dose of 20 mg twice daily, while those in gr.up B （n = 39） wer. allowed a maximum r.nitidine dose of 300 mg twice daily. Ef- ficacy was assessed by patient evaluation of global symptom r.lief, scor.s of the SF-36 quality of life （QoL） questionnair.s, total number.of pills used, and number.of medication-fr.e days.r.SULTS： Among the 83 patients who wer. enr.lled in the study, 76 patients （40 in the r.bepr.zole gr.up and 36 in the r.nitidine gr.up） completed the 4-wk tr.al. Baseline char.cter.stics wer. compar.ble between both gr.ups. After.4 wk, ther. was no significant differ.nce in the subjective global symptom r.lief between the r.bepr.zole and the r.nitidine gr.ups （71.4% vs 65.4%, r.spectively; P = 0.9）. Ther. wer. no statistically significant differ.nces between mean cumulative scor.s of the SF-36 QoL questionnair. for.the two study gr.ups （r.bepr.zole 22.40±27.53 vs r.nitidine 17.28 ± 37.06; P = 0.582）. Ther. was no significant differ.nce in the mean number.of pills used （r.bepr.zole 35.70±29.75 vs r.nitidine 32.86±26.98; P = 0.66）. Ther. was also no statistically significant differ.nce in the mean number.of medication-fr.e days between both gr.ups.CONCLUSION： r.bepr.zole has a compar.ble efficacy compar.d to r.nitidine when given on-demand for.the tr.atment of NEr.. BACKGr.UND In-hospital car.iac ar.est(IHCA)constitutes a significant cause of mor.idity and *** data is scar.e in the Middle East and Lebanon,we devised this study to shed some light on it to better.infor. both hospitals and policymaker. about the magnitude and quality of IHCA car. in *** We analyzed r.tr.spective data fr.m 680 IHCA events at the Amer.can Univer.ity of Beir.t Medical Center.between July 1,2016 and May 2,*** var.ables included age and sex,in addition to the comor.idities listed in the Char.son comor.idity *** event var.ables wer. day,event location,time fr.m activation to ar.ival,initial car.iac r.ythm,and the total number.of IHCA *** also looked at the months and *** consider.d the r.tur. of spontaneous cir.ulation(r.SC)and sur.ival to dischar.e(StD)to be our.outcomes of *** The incidence of IHCA was 6.58 per.1,000 hospital admissions(95%CI:6.09−7.08).Non-shockable r.ythms wer. 90.7%of *** IHCA cases occur.ed in the closed units(87.9%)(intensive car. unit,r.spir.tor. car. unit,neur.logy car. unit,and car.iology car. unit)and on weekdays(76.5%).r.SC followed mor. than half the IHCA events(56%).However.only 5.4%of IHCA events achieved *** r.SC and StD wer. higher.in cases with a shockable *** outcomes wer. not significantly differ.nt between day,evening,and *** was not significantly differ.nt between weekdays and weekends;however.StD was higher.in events that happened dur.ng weekdays than weekends(6.7%vs.1.9%,P=0.002).CONCLUSIONS The incidence of IHCA was high,and its outcomes wer. lower.compar.d to other.developed *** outcomes wer. better.for.patients who had a shockable r.ythm and wer. similar.between the time of day and days of the *** findings may help infor. hospitals and policymaker. about the magnitude and quality of IHCA car. in Lebanon.

AIM: To study the efficacy of low-dose imipr.mine in r.lieving symptoms associated with the ir.itable bowel syndr.me (IBS). METHODS: A r.ndomized, double-blind tr.al of 25 mg imipr.mine vs matched placebo for.12 wk was per.or.ed. Doubling the dose was allowed once at week 2 in case of an unsatisfactor. ear.y r.sponse. Pr.mar. efficacy var.ables wer. subjective global symptom r.lief and quality of life (QoL) using SF-36 at week 12. r.SULTS: One hundr.d and seven patients wer. enr.lled by adver.isement or.r.fer.al by gener.l pr.ctitioner. and 56 (31 imipr.mine: 25 placebo) completed the 16-wk study. Baseline char.cter.stics wer. compar.ble. A high over.ll dr.pout r.te was noted in the imipr.mine and placebo ar.s (47.5% vs 47.9%, P > 0.05), a mean of 25.0 and 37.4 d fr.m enr.llment, r.spectively (P r. was a significant differ.nce in global symptom r.lief with imipr.mine over.placebo (per.pr.tocol: 80.6% vs48.0%, P = 0.01) and a tr.nd on intent-to-tr.at (ITT) analysis (42.4% vs 25.0%, P = 0.06). This impr.vement was evident ear.y and per.isted to week 16 (P = 0.024 and 0.053 by per.pr.tocol and ITT analyses, r.spectively). Mean cumulative and componentspecific SF-36 scor.s impr.ved in the imipr.mine gr.up only (per.pr.tocol, P r.g-r.lated adver.e events leading to patient dr.pout wer. mor. common in the imipr.mine gr.up (25.4% vs 12.5%, P > 0.05). CONCLUSION: Imipr.mine may be effective in the tr.atment of IBS patients and is associated with impr.ved QoL. Car.ful patient selection, initiation of a low dose with gr.dual escalation and monitor.ng for.side effects may r.sult in an impr.ved ther.peutic r.sponse.

The wor. pr.sented in this paper.aims at investigating the ability of acoustic noise cor.elation technique for.r.ilway infr.str.ctur. health monitor.ng. The pr.nciple of this technique is based on impulse r.sponses r.constr.ction by cor.elation of r.ndom noise pr.pagated in the medium. Since wheel-r.il inter.ction constitutes a sour.e of such noise, cor.elation technique could be convenient for.detection of r.il defects using only passive sensor.. Exper.ments have been car.ied out on a 2 m-long r.il sample. Acoustic noise is gener.ted in the sample at sever.l positions. Dir.ct compar.son between an active emission-r.ception r.sponse and the estimated noise cor.elation function has confir.ed the validity of the equivalence r.lation between them. The quality of the r.constr.ction is shown to be str.ngly r.lated to the spatial distr.bution of the noise sour.es. High sensitivity of the noise-cor.elation functions to a local defect on the r.il is also demonstr.ted. However. inter.r.tation of the defect signatur. is mor. ambiguous than when using classical active r.sponses. Application of a spatiotempor.l Four.er.tr.nsfor. on data r.cor.ed with var.able sensor.defect distances has allowed over.oming this ambiguity.
