高通气综合征是由于通气过度超过生理代谢所需而引起的一组征候群,其特征是临床症状可以用过度通气激发试验诱发出来[1].传统的观念认为,焦虑和应激反应等心因性因素诱发了超生理代谢需要的过度通气,而临床症状都可以用过度通气和呼吸性碱中毒来解释.高通气综合征的临床症状累及多器官系统,常见的有气短、憋气、胸部不适或胸痛、呼吸深或快、心慌或心悸、头晕、四肢麻木、精神紧张、焦虑、恐惧等.这些所谓的心身症状不伴有相应的器质性病因,症状的发生与呼吸控制系统异常(很可能是下丘脑、大脑边缘系统、大脑皮层的高位神经调节异常)、自主呼吸调节丧失稳定性有关.当患者的呼吸受到刺激时,呼吸调节功能发生一过性紊乱,出现过度通气.

The purpose of the present study was to investigate the diagnostic specificity of bodily symptoms and respiratory behavior at rest and after a hyperventilation provocation test (HVPT) in patients that were either grouped according to the DSM classification or diagnosed as suffering from hyperventilation syndrome. Nine hundred three anxiety and somatoform patients, showing symptoms supposedly caused by psychogenic hyperventilation, and 170 healthy subjects, were studied. Breathing pattern and end tidal CO 2 concentration were recorded during breathing at rest and following a HVPT. Subjective symptoms in daily life and after HVPT were measured. A principal components analysis was performed on both the symptoms and breathing variables and their specificity levels were compared in the two classifications of patients. Some symptoms in daily life were gruped together with the same symptoms after the HVPT, other symptoms were not. This suggests that the HVPT elicited partly specific symptoms, and partly reproduced the symptoms experienced in daily life. Similar findings were observed with respect to the breathing variales. Patients with panic differed from other patients with anxiety disorders by an increased level of symptoms and a FETCO 2 decline at rest. The HVPT may be informative for diagnosis because it provokes some of the typical somatic and psychological symptoms, and it identifies the breathing instability that is characteristic of both patients with HVS and with anxiety. The same symptoms and breathing variables characterized the patients, whatever their classification. Overall, the specificity of breathing variables is rather low. c 1998 Elsevier Science Inc.

Background:Bile leaks are one of the most common complications after liver *** International Study Group of Liver Surgery(ISGLS)established a uniform bile leak definition including a severity ***,a risk factor assessment according to ISGLS grading as well as the clinical implications has not been studied sufficiently so ***:The incidence and grading of bile leaks according to ISGLS were prospectively documented in 501 consecutive liver resections between July 2012 and December 2016.A multivariate regression analysis was performed for risk factor *** with other surgical complications,90-day mortality as well as length of hospital stay(LOS)was ***:The total rate of bile leaks in this cohort was 14.0%:2.8%grade A,8.0%grade B,and 3.2%grade C bile leaks were *** chemotherapy or biliary intervention,diagnosis of hilar cholangiocarcinoma,colorectal metastasis,central minor liver resection,major hepatectomy,extended hepatectomy or two-stage hepatectomy,were some of the risk factors leading to bile *** multivariate regression analysis revealed that preoperative chemotherapy,major hepatectomy and biliodigestive reconstruction remained significant independent risk factors for bile *** C bile leaks were associated not only with surgical site infection,but also with an increased 90-day mortality and prolonged ***:The preoperative treatment as well as the surgical procedure had significant influence on the incidence and the severity of bile *** C bile leaks were clinically most relevant,and led to significant increased LOS,rate of infection,and mortality.

The objectives of this study are to present a new concept of the bone anchorage using long implants in remote bone sites and to discuss four cases treated with this method. Our patients were treated with long implants with a distant anchorage in the skull bone. The planning procedure, the construction of the drill guide, and the surgical protocol are described. In the clinical cases described, all four patients were rehabilitated with the remote bone anchorage concept using long implants anchored in the skull base. Patients were followed for 5-12 years and the implants remained present and stable in these time periods. The skull base implant is a new concept of bone anchorage using long implants. It can be a solution for complicated clinical situations (often failed bone reconstructions and implant placements) or an alternative for bone grafting and maxillary augmentation procedures. There is effective implant retention in the skull base, an anatomical area that is often overlooked for implant placement.
