Background The utility of routine extensive molecular profiling of pediatric tumors is a matter of debate due to the high number of genetic alterations of unknown significance or low evidence and the lack of standardized and personalized decision support *** drug assignment(DDA)is a novel computational method to prioritize treatment options by aggregating numerous evidence-based associations between multiple drivers,targets,and targeted *** has been validated to improve personalized treatment decisions based on the outcome data of adult patients treated in the sHIVA01 clinical *** aim of this study was to evaluate the utility of DDA in pediatric *** Between 2017 and 2020,103 high-risk pediatric cancer patients(rs)were involved in our precision oncology program,and samples from 100 patients were eligible for further *** or blood samples were analyzed by whole-exome(WEs)or targeted panel sequencing and other molecular diagnostic modalities and processed by a software system using the DDA algorithm for therapeutic decision ***,a molecular tumor board(MTB)evaluated the results to provide therapy *** Of the 100 cases with comprehensive molecular diagnostic data,88 yielded WEs and 12 panel sequencing *** identified matching off-label targeted treatment options(actionability)in 72/100 cases(72%),while 57/100(57%)showed potential drug *** reached 88%(29/33)by 2020 due to the continuous updates of the evidence *** approved the clinical use of a DDA-top-listed treatment in 56 of 72 actionable cases(78%).The approved therapies had significantly higher aggregated evidence levels(AELs)than dismissed *** of WEs results for targeted panels missed important mutations affecting therapy *** DDA is a promising approach to overcome challenges associated with the interpretation of extensive molecular profiling in the routine car

A major roadblock in achieving substantial building energy reduction is the low performance of old buildings that account for a significant portion of the building energy *** low-cost energy retrofit solutions that do not disrupt occupants’daily life during the retrofitting is the key to successful building energy retrofit *** this paper,a novel and low-cost exterior wall retrofitting solution is introduced,and its performance in reducing space cooling load was quantitatively evaluated to demonstrate its feasibility and *** primary goal of this paper is to provide a quantitative assessment of the cooling-energy savings potential by using the proposed new wall *** intended retrofitting targets are the large amount of existing cavity-wall buildings located in hot climate *** quantification of the before-after heat-flux reduction was conducted through a 3-dimensional steady-state low turbulence computational fluid dynamics(CFD)model,which is validated by benchmarking its prediction against the published experimental case *** outcomes of the investigation suggest that this simple low-cost solution has great potentials in reducing buildings’summer cooling load in hot climate *** applicability of this solution is not limited to retrofitting existing *** energy-efficient building designs can also adopt this solution in their envelope systems.

Irritable bowel syndrome (IBs) is a long-lasting, relapsing disorder characterized by abdominal pain/discomfort and altered bowel habits. Intestinal motility impairment and visceral hypersensitivity are the key factors among its multifactorial pathogenesis, both of which require effective treatment. Voltage-gated calcium channels mediate smooth muscle contraction and endocrine secretion and play important roles in neuronal transmission. Antispasmodics are a group of drugs that have been used in the treatment of IBs for decades. Alverine citrate, a spasmolytic, decreases the sensitivity of smooth muscle contractile proteins to calcium, and it is a selective 5-HTsub>1Asub> receptor antagonist. Alverine, in combination with simethicone, has been demonstrated to effectively reduce abdominal pain and discomfort in a large placebo-controlled trial. Mebeverine is a musculotropic agent that potently blocks intestinal peristalsis. Non-placebo-controlled trials have shown positive effects of mebeverine in IBs regarding symptom control; nevertheless, in recent placebo-controlled studies, mebeverine did not exhibit superiority over placebo. Otilonium bromide is poorly absorbed from the GI tract, where it acts locally as an L-type calcium channel blocker, an antimuscarinic and a tachykinin NK2 receptor antagonist. Otilonium has effectively reduced pain and improved defecation alterations in placebo-controlled trials in IBs patients. Pinaverium bromide is also an L-type calcium channel blocker that acts locally in the GI tract. Pinaverium improves motility disorders and consequently reduces stool problems in IBs patients. Phloroglucinol and trimethylphloroglucinol are non-specific antispasmodics that reduced pain in IBs patients in a placebo-controlled trial. Antispasmodics have excellent safety profiles. T-type calcium channel blockers can abolish visceral hypersensitivity in animal models, which makes them potential candidates for the development of novel therapeutic agents

Copper(I)selenide-nanocrystalline semiconductor was synthesized via one-step mechanochemical synthesis after 5 min milling in a planetary ball *** kinetics of synthesis was followed by X-ray powder diffraction analysis and specific surface area measurements of milled 2Cu/se *** X-ray diffraction confirmed the orthorhombic crystal structure of Cu_(2)se with the crystallite size∼25 *** surface chemical structure was studied by X-ray photoelectron spectroscopy,whereby the binding energy of the Cu 2p and se 3d signals corresponded to Cu^(+)and se^(2−)oxidation *** electron microscopy revealed agglomerated nanocrystals and confirmed their orthorhombic structure,as *** optical properties were studied utilizing ultraviolet-visible spectroscopy and photoluminescence *** direct bandgap energy 3.7 eV indicated a blue-shift phenomenon due to the quantum size *** type of Cu_(2)se synthesis can be easily adapted to production dimensions using an industrial vibratory *** advantages of mechanochemical synthesis represent the potential for inexpensive,environmentally-friendly,and waste-free manufacturing of Cu_(2)se.

We consider Leray＇s problem on stationary Navier stokes flows with arbitrary large fluxes in an unbounded cylinder with several exits to infinity. For a stationary Navier stokes flow with large fluxes in the unbounded cylinder, we prove that, if the difference between the pressure of the main flow and the pressure of the Poiseuille flow with the same flux in a branch of the cylinder remains bounded at ｜x｜→∞, then the flow behaves at infinity of the branch like the Poiseuille flow.

We present some regularity criteria for the Leray–Hopf weak solutions to the Cauchy problem for 3D Navier–stokes equations based on energy distribution at wavenumber bands. We show sufficient conditions for regularity based on the ratios of energy dissipation rates at sufficiently high wavenumber bands and neighboring medium wavenumber bands. Moreover, we give a regularity criterion based on relations between energy at high and low wavenumber bands.

Experimental investigations of non-equilibrium spontaneous condensation in transonic steam flow were carded out in linear blade cascade. The linear cascade consists of the stator blades of the last stage of low pressure steam turbine. The applied experimental test section is a part of a small scale steam power plant located at silesian Uni- versity of Technology in Gliwice. The steam parameters at the test section inlet correspond to the real conditions in low pressure part of 200MWe steam turbine. The losses in the cascade were estimated using measured static pressure and temperature behind the cascade and the total parameters at inlet. The static pressure measurements on the blade surface as well as the schlieren pictures were used to assess the flow field in linear cascade of steam turbine stator blades.
