Objective: Acute kidney disease develops 16% - 30% of patients with preoperative impaired renal functions. Our aim of this study was to compare postoperative renal outcomes using two open heart surgery techniques, on-pump beating heart (OPBH) and conventional on-pump (COP), in patients who have preoperative low glomerular filtration rate (eGFR) as an indicator of creatinine clearance. methods: From 2004 to 2015, 341 patients with preoperative creatinine clearance were lower than 90 ml/min/1.73m2 were selected for this study. On-Pump beating heart was performed in 111 patients (Group I). Conventional on-pump technique was used in 200 patients. In the remaining patients, we measured postoperative Tumor Necrosis Factor Alpha (TNF-alpha), cardiac troponin I (cTn-I), Brain natriuretic peptid (NT-Pro- BNP), creati-nine (Cr), blood urea nitrogen (BUN) and postoperative eGFR daily until day four after surgery. Results: There were no differences in baseline levels of TNF-alpha, NT-Pro-BNP, BUN, cTn-I, Cr levels between the groups. Cardiopulmonary bypass (CPB) time were much longer and cumulative inotrope use was significantly higher in patients underwent COP (P Conclusion: Our study showed that on-pump beating heart CABG technique provides a morbidity benefit and is associated with a lower risk of peroperative LOS in patients with mild renal disfunction. Our study results suggest that OPBH is superior to the COP in terms of postoperative renal injury and cardiac mediators.

The acute abdomen is one of the most frequent causes for presentation to the emergency department. Imaging plays an important role for an accurate diagnosis, which in turn diminishes morbidity and mortality. The aim of this study was to demonstrate optimum CT aspects and emphasize on the important features of CT for those patients presenting with an acute abdominal pain at the Emergency Department both in general and in a number of selected conditions (appendicitis, small-bowel obstruction, acute pancreatitis, and diverticulitis). The reported data by this study are based on the author working experience, which forms a continuous protocol adjustment process. The present study provides evidence that CT would result in definite diagnosis of patients with abdominal pain in terms of the detection of some urgent conditions.

Objective: To review the neurologic, neuroradiologic, and electrophysiologic features of autosomal recessive horizontal gaze palsy and progressive scoliosis (HGPPS), a syndrome caused by mutation of the ROBO3 gene on chromosome 11 and associated with defective decussation of certain brainstem neuronal systems. methods: The authors examined 11 individuals with HGPPS from five genotyped families with HGPPS. Eight individuals had brain mRI, and six had electrophysiologic studies. Results: Horizontal gaze palsy was fully penetrant, present at birth, and total or almost total in all affected individuals. Convergence, ocular alignment, congenital nystagmus, and vertical smooth pursuit defects were variable between individuals. All patients developed progressive scoliosis during early childhood. All appropriately studied patients had hypoplasia of the pons and cerebellar peduncles with both anterior and posterior midline clefts of the pons and medulla and electrophysiologic evidence of ipsilateral corticospinal and dorsal column-medial lemniscus tract innervation. Heterozygotes were unaffected. Conclusions: The major clinical characteristics of horizontal gaze palsy and progressive scoliosiswere congenital horizontal gaze palsy and progressive scoliosis with some variability in both ocular motility and degree of scoliosis. The syndrome also includes a distinctive brainstem malformation and defective crossing of some brainstem neuronal pathways.

研究比较了来自正常和白化处理的皇家嘎拉 (RoyalGala)苹果 (malusdomesticBorkh)新梢节段外植体不定芽的离体再生。结果表明 ,白化处理的新梢顶部第一节段外植体所产生的不定芽数比第二、第三、第四节段分别高 2、8、73倍 ;白化新梢的外植体不定芽再生数目比同一部位正常新梢高 7倍。培养基添加较低浓度的TDZ有利于不定芽再生 ,对白化外植体来说 ,1 μmol/L比 1 0 μmol/L高 2 .5倍 ,非白化外植体则高 7倍 ,平均每个外植体的不定芽再生数高达 68.8个。白化第一节段外植体的不定芽再生能力比常用的叶片外植体高 4倍。

Background: Heart failure (HF) is a common clinical syndrome mostly due to the impaired ability of the Left Ventricle (LV) to eject blood with reduced cardiac output. Heart failure is called systolic (SHF) if left ventricle ejection fraction on echo-cardiogram is low (LVEF of ≤50%). Aim: To assess the echocardiographic characteristics of patients with SHF with Atrial Fibrillation (AF) and compare with those with Sinus Rhythm (SR) on 12 leads ECG. Furthermore, to evaluate the clinical and biochemical markers for the prediction of AF in SHF. method: Over two years duration, each patient diagnosed with SHF was enrolled in the study (n = 354) based on admission code. AF or sinus rhythm on 12 leads ECG was documented on each patient. multiple logistic regression analysis was applied to assess the risk ratio of different clinical and Doppler derived variables for the development of AF in SHF. Results: Out of the total hospital medical admissions of 14,674 patients, there were 354 patients with diagnosis of SHF, a prevalence of 2.4%. The incidence of AF on ECG was 109 (31%) patients in the whole study population and 245 (69%) in SR. m mode echocardiogram in patients with SHF and AF compared with those in SR showed significant dilation of LV cavity in systole with LVESD of 5.72 ± 0.63 vs. 5.23 ± 0.76 cm, P m, P m showed a severe restrictive-pattern with shorter Decellration Time (DT) of 163.73 ± 7.42 vs. 214.9 ± 31.81msec, P m of 14.26 ± 1.34 vs. 9.99 ± 1.27, P , and the serum level of Brain Natriuretic Peptide (BNP) hormone of 723.72 ± 13.45 vs. 686.98 ± 72.57 pg/ml, P ment of AF in SHF were positive for high BNP > 500 pg/ml of 2.8, history of hypertension of 1.8, history of Dm of 1.7, BmI > 28 of 1.4, LV hypertrophy on ECG of 1.3. Efficient complementary metal-oxide semiconductor-based nonlinear optical devices in the near-infrared are in strong *** to two-photon absorption in silicon,however,much nonlinear research is shifting towards unconventional photonics *** this work,we demonstrate the generation of an octave-spanning coherent supercontinuum in a silicon waveguide covering the spectral region from the near-to *** input pulses of 18 pJ in energy,the generated signal spans the wavelength range from the edge of the silicon transmission window,approximately 1.06 to beyond 2.4μm,with a−20 dB bandwidth covering 1.124–2.4μ*** octave-spanning supercontinuum was also observed at the energy levels as low as 4 pJ(−35 dB bandwidth).We also measured the coherence over an octave,obtaining|g_()12^(1)(λ)>90%,in good agreement with the *** addition,we demonstrate optimization of the third-order dispersion of the waveguide to strengthen the dispersive wave and discuss the advantage of having a soliton at the long wavelength edge of an octave-spanning signal for nonlinear *** research paves the way for applications,such as chip-scale precision spectroscopy,optical coherence tomography,optical frequency metrology,frequency synthesis and wide-band wavelength division multiplexing in the telecom window.

In the production of compression springs,high forming velocities and grades of deformation during winding and setting may induce cracks that can lead to failure causing risks of an accident and *** AE(acoustic emission)technology,a non-destructive monitoring method,can detect acoustic signals reflected from *** establish this method in the production of technical springs,it was necessary,to find out whether the AE signal is influenced by material properties,phase fractions distribution from tempered martensite,retained austenite,and microstructure including crystallographic *** addition,it was investigated to what extent the detected AE signal can be useful to separate between an actual crack and other material *** an in-situ three-point bending test with the AE technology,macro-and micro-crack-typical AE signals were detected for five different spring steel wires(SH,VDSiCr,and FDSiCr according to EN-10270-1 and EN-10270-2).The relative energy related to the initiation,propagation,and growth of cracks caused by mechanical stress was measured using a piezoelectric *** a crack AE signal appeared for the first time,the bending tests were stopped *** results show that the frequency spectrum combined with the intensity of the acoustic signals generated during crack growth depends on the material properties and the crystallographic ***,it could be shown that it is possible to differentiate between micro-crack-typical AE signals and other signals that result from different material responses.
