比较儿童自发性气胸急诊手术和延期手术

Controversy exists reg.rding.the timing.of.surg.ry f.r spontaneous pneumothorax (SP), which can be perf.rmed either af.er the f.rst development of.pneumothorax or af.er a recurrent spontaneous pneumothorax has occurred. Treatment af.er recurrence is of.en adopted because of.the purported low recurrence of.SP treated nonoperatively and the historical morbidity of.open surg.ry. However, the ef.ectiveness of.VATS (to videoassisted bullectomy and pleurodesis) has raised the possibility of.perf.rming.primary VATS (PV) in all patients. The authors theref.re hypothesized that PV is saf. and ef.ective f.r SP and soug.t to perf.rm a cost-benef.t analysis of.PV vs secondary VATS (SV). Af.er institutional review board approval, consecu tive patients with SP (1991-2003) and no comorbidities were retrospectively divided into PV vs SV. Demog.aphics, recurrent pneumothorax af.er VATS, leng.h of.stay, and costs were compared by Student’s t test/χ2. The predicted incremental cost of.PV was (cost of.PV) -{[cost of.nonoperative treatment ×(1 -recurrence rate)] +cost of.SV ×recurrence rate}. Data are means ±SEM. There were 54 spontaneous pneumothoraces in 43 patients (11 bilateral), of.whom 3 were excluded because of.open thoracotomy. Of.51 pneumothoraces, nonoperative treatment was attempted in 37, of.whom 20 recurred and thus required SV. Primary VATS was perf.rmed in 14. Both g.oups had similar ag., sex, weig.t, heig.t, admission heart rate, and room air oxyg.n saturation. Total treatment leng.h of.stay was sig.if.cantly shorter f.r PV vs SV (7.1 ±0.96 vs 10.5 ±1.2, P =.04). However, morbidity f.om recurrent pneumothorax af.er VATS occurredmore f.equently af.er PV than SV (4/14 vs 0/20 P f.54%, perf.rming.PV on all patients with SP would increase cost by ＄4010 per patient and require a recurrence rate of.72%or more to f.nancially justif. this approach. Contrary to the hypothesis, the increased morbidity and cost do not justif. a strateg.

The Sof. X-ray Imag.r(SXI)is part of.the scientif.c payload of.the Solar wind Mag.etosphere Ionosphere Link Explorer(SMILE)*** is a joint science mission between the European Space Ag.ncy(ESA)and the Chinese Academy of.Sciences(CAS)and is due f.r launch in *** is a compact X-ray telescope with a wide f.eld-of.view(f.V)capable of.encompassing.larg. portions of.Earth’s mag.etosphere f.om the vantag. point of.the SMILE *** is sensitive to the sof. In this paper we consider the most g.neral f.rm of.non-static cylindrically symmetric space-times in order to study proper teleparallel homothetic vector f.elds using.the direct integ.ation technique and diag.nal tetrads. This study also covers static cylindrically symmetric, Bianchi type I, non-static and static plane symmetric space-times as well. Here, we will only discuss the cases which do not f.ll in the categ.ry of.static cylindrically symmetric, Bianchi type I, non-static and static plane symmetric space-times. f.om the above study we show that very special classes of.the above space-times yield 6, 7 and 8 teleparallel homothetic vector f.elds with non-zero torsion.

The temporal variations of.mesozooplankton abundance and biomass （1-Volumetric method by settling.volume and displacement volume and 2- g.avimetric method throug. wet weig.t, dry weig.t and ash-f.ee dry weig.t） with relation to environmental parameters were studied in the mang.ove creek area of.Karachi coast, Pakistan. The data of.mesozooplankton samples along.with environmental parameters （temperature, salinity, pH, etc.） were collected during.January 1998 to December 1998 f.om two creek stations. The abun-dance of.mesozooplankton also exhibited seasonal trends at both stations. At Sta. S2, the hig.est and low-est abundance values were observed during.post-monsoon and southwest monsoon respectively whereas, at Sta. S1, a clear trend of.hig. abundance in pre-monsoon to low abundance in southwest monsoon was observed. Mesozooplankton abundance was also positively correlated with settling.volume, displacement volume, wet weig.t and dry weig.t. The hig.est biomass value was observed in the northeast monsoon and pre-monsoon periods. The results of.the canonical analysis of.the output f.om the discriminate f.nction was tested. Out of.f.f.een variables, only one was sig.if.cantly dif.erent in sing.e character ratios dry weig.t/ash f.ee-dry weig.t （f.,23=4.78,P＆lt;0.005）. The mesozooplankton community was collectively composed of.28 taxa. Among.these g.oups, copepoda （66.3%）, g.stropod larvae （9.94%）, evadne （4.60%）, zoea （3.60%）, cypris nauplii （2.56%）, lemellibranch larvae （1.87%）, chaetog.aths （1.81%）, ostracods （1.73%）, lucif.r （1.15%） and barnacles nauplii （1.35%） contributed the most to the similarities within Sta. S1, while copepoda （74.68%）, cypris nauplii （5.29%）, g.stropods （4.87%）, barnacles nauplii （4.81%）, evadne （1.72%）, zoea （1.53%） and ca-ridean larvae （1.18%） at Sta. S2. The remaining.mesozooplanktonic g.oup were accounted f.r less than 5% and 6% at Sta. S1 and Sta. S2 respectively, of.the total org.nisms. Similarity percentag. （SIMPER） analysis revealed t
