Circulating tumor cells with epithelial-mesenchymal transition markers as potential biomarkers for the diagnosis of lung cancer

BACKGROUND Circulating tumor cells(CTCs) can be clustered into three subtypes according to epithelial-mesenchymal transition(EMT) markers: CTCs with epithelial markers(E-CTCs), CTCs with mesenchymal markers(M-CTCs), and CTCs with both markers(E&M-CTCs). CTC detection has clinical implications in the diagnosis of lung cancer(LC).AIM To clarify the diagnostic value of CTCs categorized by EMT markers in *** The study included 106 patients with lung adenocarcinoma, including 42 groundglass opacities(GGO) and 64 solid lesions, who underwent surgery between July 2015 and December 2019. Eleven patients with benign tumors and seventeen healthy controls were included. CTCs in peripheral blood and associated EMT markers were detected preoperatively using the CanPatrol TM technique. The diagnostic power of CTCs for discriminating LC cases from controls was analyzed by the receiver operating characteristic(ROC) curve. The CytoploRare technique was used in 20 cases and 18 controls for validation, and Kappa values were calculated to evaluate consistency between *** Of the 106 LC cases, 94(89.6%) had at least one CTC. CTCs were detectable in 35(83.3%) of 42 GGO cases. Total CTCs and E&M-CTCs were significantly more frequent in LC cases than in benign or healthy controls. The proportion of MCTCs plus E&M-CTCs increased gradually from healthy controls, to benign controls, to LC cases. The area under the ROC curve of total CTCs and E&M-CTCs was > 0.8 and > 10.75, respectively. The combined sensitivity of total-CTCs and E&M-CTCs was 85.85% for LC patients(80.95% for GGO patients) and the specificity was 78.57%.The Kappa value was 0.415,indicating relative consistency between CanPatrol TM and *** CTC detection is valuable for distinguishing LC from controls,and particularly E&M-CTC detection warrants further study.

Diagnosis of chest pain with foregut symptoms in Chinese patients

AIM:To evaluate the diagnosis of chest pain with foregut symptoms in Chinese patients. METHODS:Esophageal manometric studies, 24-h introesophageal pH monitoring and 24-h electrocardiograms(Holter electrocardiography)were performed in 61 patients with chest pain. RESULTS:Thirty-nine patients were diagnosed with non-specific esophageal motility disorders(29 patients with abnormal gastroesophageal reflux and eight patients with myocardial ischemia).Five patients had diffuse spasm of the esophagus plus abnormal gastroesophageal reflux(two patients had concomitant myocardial ischemia),and one patient was diagnosed with nutcracker esophagus. CONCLUSION:The esophageal manometric studies, 24-h intra-esophageal pH monitoring and Holter electrocardiography are significant for the differential diagnosis of chest pain,particularly in patients with foregut *** cases of esophageal motility disorders,pathological gastroesophageal reflux may be a major cause of chest pain with non-specific esophageal motility *** of the esophageal smooth muscle might affect the heart-coronary smooth muscle,leading to myocardial ischemia.

81例短期初次使用苯二氮类药物的撤药观察

目的:观察、比较短期(4-6周)初次使用苯二氮类的撤药过程,为合理应用苯二氮类药物(BZDs)提供一些参考。方法:本研究为前瞻性队列研究。选取连续使用5羟色胺再摄取抑制剂(SSRIs)+BZDs方案治疗4-6周临床痊愈的初诊初治广泛性焦虑障碍(GAD)患者共81名,按BZDs分组(劳拉西泮41人,使用氯硝西泮组40人)。在自愿情况下,进行苯二氮卓类药物撤药(撤药时BZDs均为0.5mg/天),用汉密尔顿焦虑量表(HAMA-14)评定撤药过程中焦虑反应最严重时的焦虑水平,用t检验或χ2检验比较撤药前后两组患者HAMA得分情况、撤药症状的发生率等方面的差异。结果:两组患者在撤药后HAMA评分均增高(P<0.01);组间对比,在HAMA总分、精神性焦虑、害怕、失眠、撤药症状发生率等方面,氯硝西泮组均高于劳拉西泮组。结论:4-6周短期连续使用BZDs可出现撤药症状;氯硝西泮组撤药后焦虑水平较劳拉西泮组高,其中失眠、精神焦虑更突出。更短期或按需使用BZDs,优先选择短效低亲和力的BZDs药物,可能有助于减轻撤药症状、减少依赖的产生。

Quantitative analysis of 3-isopropylamino-1,2-propanediol as a degradation product of metoprolol in pharmaceutical dosage forms by HILIC-CAD

Aryloxypropanolamine is an essential structural scaffold for a variety of b-adrenergic receptor antagonists such as *** with such a structural motif tend to degrade into α,β ehydroxypropanolamine impurities via a radicaleinitiated oxidation *** impurities are typically polar and nonchromophoric,and are thus often overlooked using traditional reversed phase chromatography and UV *** this work,stress testing of metoprolol confirmed the generation of 3-isopropylamino-1,2-propanediol as a degradation product,which is a specified impurity of metoprolol in the European Pharmacopoeia(impurity N).To ensure the safety and quality of metoprolol drug products,hydrophilic interaction chromatography(HILIC)methods using Halo Penta HILIC column(150mm×4.6 mm,5 μm)coupled with charged aerosol detection(CAD)were developed and optimized for the separation and quantitation of metoprolol impurity N in metoprolol drug products including metoprolol tartrate injection,metoprolol tartrate tablets,and metoprolol succinate extended-release *** HILIC-CAD methods were validated per USP validation guidelines with respect to specificity,linearity,accuracy,and precision,and have been successfully applied to determine impurity N in metoprolol drug products.

Optimal Dividend Strategy in Compound Binomial Model with Bounded Dividend Rates

We consider the compound binomial model, and assume that dividends are paid to the shareholders according to an admissible strategy with dividend rates bounded by a *** company controls the amount of dividends in order to maximize the cumulative expected discounted dividends prior to ruin. We show that the optimal value function is the unique solution of a discrete HJB equation. Moreover, we obtain some properties of the optimal payment strategy, and offer a simple algorithm for obtaining the optimal strategy. The key of our method is to transform the value function. Numerical examples are presented to illustrate the transformation method.

Cyclopeptidic Constituents from Arenaria oreophila J.D,Hooker(Caryophyllaceae)

To further investigate the cyclopeptldes of the Caryophyllaceae family, two new cyclopeptldes, named Arenarlphilin E （compound 1） and Arenariphilin F （compound 2）, were obtained from Arenaria oreophUe J. D. Hooker using some Isolation methods, e. g. normal and reverse silica gel. By detailed spectroscopic analysis, such as FAB^＋-MS, 1 D NMR, 2D NMR, the structures of Arenariphilin E （compound 1） and Arenariphilln F （compound 2） were determined as cyclo（lle^1-Gly-Val^1-Ala-Leu-lle^3-lle^2-Val^2-Pro） and cyclo（Pro^2-Pro^1-Gly^2-lle-Val-Leu-Gly^1-AiaThr- Gly^3）, respectively.
