BACKGrOUND: Prophylaxis against deep- vein thrombosis in hospitalized patients remains underused. We hypothesized that the use of a computer- alert program to encourage prophylaxis might reduce the frequency of deep- vein thrombosis among high- risk hospitalized patients. METHODS: We developed a computer program linked to the patient database to identify consecutive hospitalized patients at risk for deep- vein thrombosis in the absence of prophylaxis. The program used medical- record numbers to randomly assign 1255 eligible patients to an intervention group, in which the responsible physician was alerted to a patient s risk of deep- vein thrombosis, and 1251 patients to a control group, in which no alert was issued. The physician was required to acknowledge the alert and could then withhold or order prophylaxis, including graduated compression stockings, pneumatic compression boots, unfractionated heparin, low- molecular- weight heparin, or warfarin. The primary end point was clinically diagnosed, objectively confirmed deep- vein thrombosis or pulmonary embolism at 90 days. rESULTS: More patients in the intervention group than in the control group received mechanical prophylaxis(10.0 percent vs. 1.5 percent, Pr pharmacologic prophylaxis(23.6 percent vs. 13.0 percent, Primary end point occurred in 61 patients(4.9 percent) in the intervention group, as compared with 103(8.2 percent) in the control group; the Kaplan- Meier estimates of the likelihood of freedom from deep- vein thrombosis or pulmonary embolism at 90 days were 94.1 percent(95 percent confidence interval, 92.5 to 95.4 percent) and 90.6 percent(95 percent confidence interval, 88.7 to 92.2 percent), respectively(Pr alert reduced the risk of deep- vein thrombosis or pulmonary embolism at 90 days by 41 percent(hazard ratio, 0.59; 95 percent confidence interval, 0.43 to 0.81; P=0.001). Long-Term Postoperative Perfusion Status in Giant retinal Tears: A Preliminary Case report

Background: Information on the long-term perfusion status of patients after successful surgery for giant retinal tear (GrT) macula-off rhegmatogenous retinal detachment (rrD) is limited. Purpose: To examine long-term structural, functional, and perfusion outcomes in normal control eyes and eyes treated for different degrees of GrT-associated extensions of rrD. Methods: One emmetropic normal eye (control), one healthy highly myopic eye (control myopic), and three eyes surgically treated for GrT (surgical), were included in the study for a long-term comparison of study outcomes. The surgical eyes were classified based on the degree of GrT-associated rrD extension as follows: one eye with GrT-associated rrD extension ˚one eye with GrT-associated rrD extension between 180˚- 270˚and one eye with GrT-associated rrD extension > 270˚. Structural, functional, and perfusion outcomes were compared with those of the control eyes. results: All three eyes were phakic and the condition was monocular. The mean age of the patients was 48.67 ± 8.50 years (range, 39 - 55 years). All three eyes had GrT macula-off rrD. The mean preoperative time for GrT surgery was 1.2 weeks. The mean pre- and postoperative best corrected visual acuities (BCVA) were 1.87 logMAr and 0.46 logMAr, respectively. The mean postoperative follow-up period was 19.67 ± 5.69 months. Proliferative vitreoretinopathy resulted in multiple surgeries in one eye (31.5%). Long-term postoperative optical coherence tomography (OCT) showed abnormal retinal thickness, ellipsoid zone disruption, and external limiting membrane line discontinuities in one eye. OCT angiography yielded abnormal perfusion indices in the surgically treated eyes. Conclusions: Our data showed multiple structural alterations in spectral-domain OCT biomarkers. Degradation of Gesaprim Herbicide by Heterogeneous Photocatalysis Using Fe-Doped TiO2

Fe-doped TiO2 was prepared by the sol gel method and characterized by X-ray diffraction. All the Fe-doped TiO2 were composed of an anatase crystal form. The activity of the Fe-doped TiO2 for the degradation of the gesaprim commercial herbicide (which contains atrazine as active compound and formulating agents) was studied by varying the iron content during UV (15 W), visible light and solar irradiations. The visible light came from commercial saving energy lamps (13, 15 and 20 Watts). The gesaprim degradation rate depended on the iron content in the photo catalyst. The Fe-doped TiO2 (0.5% by weight of TiO2) showed higher TOC removal under visible light and was more active than the undoped TiO2 photo catalyst under the light irradiation sources tested. Over 90% of chemical oxygen demand abatement was achieved with both UV and visible light but less time was required to decrease the chemical oxygen demand content by using the catalyst doped with iron at 0.5% under visible light. It was observed that the degradation of gesaprim increased by increasing the iron content in the catalyst under visible light.

Effect of split applications of urea on protein size distribution, physical dough properties, and baking performance of five experimental bread wheat lines

Five experimental bread wheat lines (BWL) were grown at the Yaqui Valley Experimental Station in Sonora, México during two consecutive growing cycles. The effect of five nitrogen fertilization (NF) treatments on protein size distribution (PSD), physical dough properties (PDP), and baking performance of the BWL was evaluated. Polymeric and monomeric proteins were evalu-ated by SE-HPLC. PDP were carried out by the National Mixograph and the TA-XT2 Texture Analyzer. Baking performance was evaluated using the straight dough method. Protein content (PC), main protein fractions (glutenins, gliadins, albumins-globulins), and mixograph development time (MDT) were significantly influenced by NF treatment and BWL but not by their interaction. On the other hand, PDP measured by the Kieffer rig, and baking performance were significantly influenced by the main factors (NF and BWL) and their interaction. The amount and timing of fertilizer applied to the BWL modified the PC, PSD, PDP, and bread loaf volume. PDP exhibited a larger variation in comparison to the PSD of glutenins. The split application of 150 kg of urea/ha (50-50-50) to all BWL showed a better loaf volume response than the same amount of urea applied at sowing (150-0-0). The application of 300 kg of urea/ha to all BWL, either at sowing or at three split applications of 100 kg of urea/ha each, resulted in higher flour unextractable polymeric protein (FUPP). On the other hand, the split application of 100-100-100 kg of urea/ha to three of the BWL represented the higher total unextractable polymeric protein (TUPP). Differences on PC and PSD were reflected on differences on PDP and bread loaf volume observed among the BWL.
