A machine learning model for colorectal liver metastasis post-hepatectomy prognostications

Background:Currently,surgical resection is the mainstay for colorectal liver metastases(CRLM)management and the only potentially curative treatment *** tools can support patient selection for surgical resection to maximize therapeutic *** study aimed to develop a survival prediction model using machine learning based on a multicenter patient sample in Hong ***:Patients who underwent hepatectomy for CRLM between 1 January 2009 and 31 December 2018 in four hospitals in Hong Kong were included in the *** analysis was performed using Cox proportional hazards(CPH).A stepwise selection on Cox multivariable models with Least Absolute Shrinkage and Selection Operator(LASSO)regression was applied to a multiply-imputed dataset to build a prediction *** model was validated in the validation set,and its performance was compared with that of Fong Clinical Risk Score(CRS)using concordance ***:A total of 572 patients were included with a median follow-up of 3.6 *** full models for overall survival(OS)and recurrence-free survival(RFS)consist of the same 8 established and novel variables,namely colorectal cancer nodal stage,CRLM neoadjuvant treatment,Charlson Comorbidity Score,pre-hepatectomy bilirubin and carcinoembryonic antigen(CEA)levels,CRLM largest tumor diameter,extrahepatic metastasis detected on positron emission-tomography(PET)-scan as well as KRAS *** CRLM Machine-learning Algorithm Prognostication model(CMAP)demonstrated better ability to predict OS(C-index=0.651),compared with the Fong CRS for 1-year(C-index=0.571)and 5-year OS(C-index=0.574).It also achieved a C-index of 0.651 for ***:We present a promising machine learning algorithm to individualize prognostications for patients following resection of CRLM with good discriminative ability.

Atherosclerotic cardiovascular disease resulting from dysregulated lipid metabolism is the leading cause of morbidity and mortality *** E(ApoE)plays a critical role in cholesterol *** in lipid-metabolizing proteins including ApoE in multiple model organisms such as mice and rats exhibiting elevated levels of cholesterol have been widely used for dissecting the pathology of atherosclerosis,but few of these animal models exhibit advanced atherosclerotic plaques leading to ischemia-induced clinical symptoms,limiting their use for translational *** we report hypercholesterolemia and severe atherosclerosis characterized by stenosis and occlusion of arteries,together with clinical manifestations of stroke and gangrene,in ApoE knockout dogs generated by CRISPR/Cas9 and cloned by somatic cell nuclear transfer ***,the hypercholesterolemia and atherosclerotic complications in F0 mutants are recapitulated in their *** the ApoE-associated atherosclerosis and clinical manifestations in mutant dogs are more similar to that in human patients compared with those in other animal models,these mutant dogs will be invaluable in developing and evaluating new therapies,including endovascular procedures,against atherosclerosis and related disorders.

Comparison of Drought-Resistance about Three Wild Rocky-Grasses

In this experiment, using the methods of drought-stress in the pot and PEG simulative drought-stress, three native rocky-slope grasses Pogonatherum panideum（lam.） Hack, Erioophorum comosum nees and Cynodon dactylon cultivated in the pots were selected as materials to study their drought-resistance by analyzing the indexes such as the leaf area index, the drying roots accumulation, the holding water ability of leaf, the relative conductance of leaf, the soluble protein, chlorophyll. At the same time, by comparing with membership function value of each index, the order of each grass drought-resistance was decided. The results showed that the drought-resistance of Erioophorum comosum nees was stronger than Cynodon dactylon and drought-resistance of Cynodon dactylon was stronger than Pogonatherum panideum （lam.） Hack. The purpose of the experimental results was to find theoretical foundations for selecting and cultivating native protecting-slope vegetations to adapt to rocky slopes.

A WiMAX-ZigBee Energy Management System for Green Education

A WiMAX-ZigBee energy management system for green education has been developed. The system helps to resolve the disgusting environmental problems. This paper explores the concept of energy management and introduces an Energy Management System located at campus areas. Analysis shows that 30% energy can be saved from the total energy consumption which translates to a saving of 72 thousand million kWh of energy annually. Such saving reduces 48 million tons of CO2 emission for the environment.

Advanced Metering Infrastructure for Electric Vehicle Charging

The widely adoption of Electric Vehicle (EV) has been identified as a major challenge for future development of smart grids. The ever increasing electric vehicle charging further increases the energy demand. This paper reports the development of an Advanced Metering Infrastructure (AMI) as an effective tool to reshape the load profile of EV charging by adopting appropriate demand side management strategy. This paper presents a total solution for EV charging service platform (EVAMI) based on power line and internet communication. It must be stressed that the development of Third Party Customer Service Platform in this investigation facilitates a single bill to be issued to EV owners. Hence, EV owners understand their energy usage and thus may perform energy saving activity efficiently. Experiment and evaluation of the proposed system show that the throughput achieved is about 5 Mbps at 10 ms end to end delay in Power line Communication. By introducing two dimensional dynamic pricing and charging schedule, the proposed EVAMI successfully reduces 36% peak consumption and increases the “off peak” consumption by 54%. Therefore the EVAMI does not only reduce the peak consumption but also relocates the energy demand effectively.

目的采用网络药理学和动物实验相结合的方法探讨消积丸治疗动脉粥样硬化(atherosclerosis,AS)的作用机制。方法通过网络药理学进行活性成分-靶点网络构建、蛋白质-蛋白质相互作用(protein-protein interaction,PPI)网络分析、基因本体(gene ontology,GO)功能和京都基因与基因组百科全书(Kyoto encyclopedia of genes and genomes,KEGG)通路富集分析。50只雄性ApoE−∕−小鼠随机分为对照组、模型组、阿托伐他汀(1.3 mg/kg)组和消积丸低、高剂量(8.125、32.500 g/kg)组,模型组和各给药组以高脂饮食喂养,给予相应药物干预4周后,采用全自动生化仪分析小鼠血清脂质水平;采用ELISA法检测小鼠血清中白细胞介素-1β(interleukin-1β,IL-1β)、IL-6、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)和氧化低密度脂蛋白(oxidized low density lipoprotein,ox-LDL)水平;采用苏木素-伊红(HE)、Masson和油红O染色观察小鼠主动脉窦病理变化;采用Western blotting法检测小鼠主动脉组织中TNF-α/κB抑制因子激酶β(inhibitorκB kinaseβ,IKKβ)/核因子-κB(nuclear factor-κB,NF-κB)信号通路相关蛋白及CD36蛋白表达。结果共筛选出消积丸136个活性成分及277个作用靶点,4481个AS疾病相关靶点,消积丸和AS的共同靶点有205个,核心靶点主要包括TNF、IL-6、NF-κB、IL-1β等。GO功能和KEGG通路富集分析发现,消积丸治疗AS主要涉及炎症反应、一氧化氮生物合成等过程,调控癌症通路、乙肝通路、TNF信号通路等;在TNF信号通路中进一步筛选得到TNF-α/IKKβ/NF-κB信号通路。动物实验结果显示,与对照组相比,模型组小鼠血清中三酰甘油(triglyceride,TG)、总胆固醇(total cholesterol,TC)、高密度脂蛋白胆固醇(high density lipoprotein cholesterol,HDL-C)、低密度脂蛋白胆固醇(low density lipoprotein cholesterol,LDL-C)、IL-1β、IL-6、TNF-α、ox-LDL水平均显著升高(P<0.01);主动脉中TNF-α蛋白表达以及IKKβ、NF-κB p65磷酸化水平均显著升高(P<0.01);主动脉窦内膜不光滑,出现大量胶原纤维和脂质沉积(P<0.01),形成明显的AS斑块。与模型组相比,消积丸组小鼠血清中TC、TG、LDL-C、TNF-α、IL-6、ox-LDL水平均显著降低(P<0.05、0.01),血清中HDL-C水平显著升高(P<0.01);主动脉中TNF-α、CD36蛋白表达以及IKKβ、NF-κB p65磷酸化水平均显著降低(P<0.05、0.01);主动脉窦AS斑块减少,胶原蛋白和脂质沉积等情况均得到改善(P<0.05、0.01)。结论消积丸能够通过下调TNF-α/IKKβ/NF-κB信号通路相关蛋白表达,减轻炎症反应,调节脂质代谢,从而发挥治疗AS的作用。

AIM： The use of low-dose aspirin to prevent cardiovascular disease events is well established. However, the incidence and predictors of upper gastrointestinal bleeding （UGIB） with its use are unknown. We studied prospectively the incidence and outcome of peptic ulceration in low-dose aspirin users. METHODS： A total of 991 patients with coronary artery disease （CAD） on low-dose aspirin were prospectively followed-up for two years for the occurrence and clinical features of first hospitalized episode of UGIB. RESULTS： UGIB had a bimodal presentation with 45% occurring within four months of aspirin initiation and had an overall prevalence of 1.5% per year. There was no UGIB-related death. Hypertension （OR = 4.6, 95%CI 1.5 - 14.7, P = 0.009）, history of peptic ulceration （OR = 3.1, 95%CI 1.1 - 9.0, P = 0.039）, tertiary education （OR = 3.08, 95%CI 1.1 - 9.0, P = 0.039） and higher lean body mass （P = 0.016） were independent factors associated with UGIB. Use of nitrate did not reduce UGIB. CONCLUSION： The incidence of UGIB in patients with CAD on long-term low-dose aspirin is low, but is accompanied with significant morbidity. With prolonged use of aspirin, UGIB continues to be a problem for those with risk factors and especially in patients with a history of peptic ulcers, in which UGIB tends to occur early after aspirin therapy.
