●AIM:To explore the clinical efficacy and safety of stromal lenticule addition keratoplasty(SLAK)with corneal crosslinking(CXL)on patients with corneal ectasia secondary to femtosecond laser-assisted in situ keratomileusis(FS-LASIK).●METHODS:A series of 5 patients undertaking SLAK with CXL for the treatment of corneal ectasia secondary to FS-LASIK were followed for 4-9mo.The lenticules were collected from patients undertaking small incision lenticule extraction(SMILE)for the correction of myopia.Adding a stromal lenticule was aimed at improving the corneal thickness for the safe application of crosslinking and compensating for the thin cornea to improve its mechanical strength.●RESULTS:All surgeries were conducted successfully with no significant complications.Their best corrected visual acuity(BCVA)ranged from 0.05 to 0.8-2 before surgery.The pre-operational total corneal thickness ranged from 345-404μm and maximum keratometry(Kmax)ranged from 50.8 to 86.3.After the combination surgery,both the corneal keratometry(range 55.9 to 92.8)and total corneal thickness(range 413-482μm)significantly increased.Four out of 5 patients had improvement of corneal biomechanical parameters(reflected by stiffness parameter A1 in Corvis ST).However,3 patients showed decreased BCVA after surgery due to the development of irregular astigmatism and transient haze.Despite the onset of corneal edema right after SLAK,the corneal topography and thickness generally stabilized after 3mo.●CONCLUSION:SLAK with CXL is a potentially beneficial and safe therapy for advanced corneal ectasia.Future work needs to address the poor predictability of corneal refractometry and compare the outcomes of different surgical modes.

OBJECTIVES Stress-related glycemic indicators,including admission blood glucose(ABG),stress-hyperglycemia ratio(SHR),and glycemic gap(GG),have been associated with worse outcomes after acute myocardial infarction(AMI).However,data regarding their prognostic value in the oldest old with AMI are unavailable.Therefore,this study aimed to investigate the association of stress-related glycemic indicators with short-and long-term cardiovascular mortality(CVM)in the oldest old(≥80 years)with AMI.METHODS In this prospective study,a total of 933 consecutive old patients with AMI admitted to FuWai hospital(Beijing,China)were enrolled.On admission,ABG,SHR,and GG were assessed and all participants were classified according to their quartiles.Kaplan-Meier,restricted cubic splines(RCS),and multivariate Cox regression analyses were performed to evaluate the association between these glycemic indicators and CVM within 30 days and long-term follow-up.RESULTS During an average of 1954 patient-years of follow-up,a total of 250 cardiovascular deaths were recorded.Kaplan-Meier analyses showed the lowest CVM in quartile 1 of ABG and in quartile 2 of SHR and GG.After adjusting for potential covariates,patients in quartile 4 of ABG,SHR,and GG had a respective 1.67-fold(95%CI:1.03-2.69;P=0.036),1.80-fold(95%CI:1.16-2.79;P=0.009),and 1.78-fold(95%CI:1.14-2.79;P=0.011)higher risk of long-term CVM risk compared to those in the reference groups(quartile 1 of ABG and quartile 2 of SHR and GG).Furthermore,RCS suggested a J-shaped relationship of ABG and a Ushaped association of SHR and GG with long-term CVM.Additionally,we observed similar associations of these acute glycemic parameters with 30-day CVM.CONCLUSIONS Our data first indicated that SHR and GG consistently had a U-shaped association with both 30-day and longterm CVM among the oldest old with AMI,suggesting that they may be useful for risk stratification in this special population.

BACKGROUND The recently introduced ultrasonic flow ratio(UFR),is a novel fast computational method to derive fractional flow reserve(FFR)from intravascular ultrasound(IVUS)images.In the present study,we evaluate the diagnostic performance of UFR in patients with intermediate left main(LM)stenosis.METHODS This is a prospective,single center study enrolling consecutive patients with presence of intermediated LM lesions(diameter stenosis of 30%-80%by visual estimation)underwent IVUS and FFR measurement.An independent core laboratory assessed offline UFR and IVUS-derived minimal lumen area(MLA)in a blinded fashion.RESULTS Both UFR and FFR were successfully achieved in 41 LM patients(mean age,62.0±9.9 years,46.3%diabetes).An acceptable correlation between UFR and FFR was identified(r=0.688,P50%(AUC=0.66,P<0.001)and numerically higher than IVUS-derived MLA(AUC=0.82;P=0.09).Patient level diagnostic accuracy,sensitivity and specificity for UFR to identify FFR≤0.80 was 82.9%(95%CI:70.2-95.7),93.1%(95%CI:82.2-100.0),58.3%(95%CI:26.3-90.4),respectively.CONCLUSION In patients with intermediate LM diseases,UFR was proved to be associated with acceptable correlation and high accuracy with pressure wire-based FFR as standard reference.The present study supports the use of UFR for functional evaluation of intermediate LM stenosis.

BACKGROUND Esophageal chromoendoscopy with iodine solution is important for detecting early esophageal cancer.The effect of routine treatment for lesions lightly stained with Lugol’s iodine solution is limited,and the addition of natural substances to a regular diet is becoming increasingly common.Vinegar has antitumor effects as reported in previous studies.AIM To evaluate whether vinegar supplementation could improve the prognosis of patients with lightly stained esophageal lesions.METHODSThis prospective single-centre trial included consecutive patients with lightly stained lesions between June 2020 and April 2022.Patients in the experimental group received increased amounts of vinegar for 6 months.The primary outcome of the study was the clinical therapeutic effect.Complications related to vinegar ingestion and adverse events were also recorded in detail.RESULTS A total of 166 patients were included in the final analysis.There was no significant difference in the baseline data between the two groups.Intention-to-treat(ITT)analysis demonstrated that the rates at which endoscopic characteristics improved were 33.72%in the experimental group and 20.00%in the conventional group(P=0.007);and the rates at which biopsy pathology improved were 19.77%and 8.75%,respectively(P=0.011).Additional vinegar consumption had a statistically protective effect on the rate at which endoscopic characteristics improved[hazard ratio(HR)_(ITT)=2.183,95%CI:1.183-4.028;HR_(per-protocol(PP))=2.307,95%CI:1.202-4.426]and biopsy pathology improved(HR_(ITT)=2.931,95%CI:1.212-7.089;HR_(PP)=3.320,95%CI:1.295-8.507).No statistically significant effect of increased vinegar consumption on preventing high-grade intraepithelial neoplasia or early cancer was observed(HR_(ITT)=0.382,95%CI:0.079-1.846;HRPP=0.382,95%CI:0.079-1.846).The subgroup analyses indicated that the overall therapeutic improvement of endoscopic characteristics and biopsy pathology seemed more obvious in older(age>60)male patients with small lesions(lesion size≤0.5 cm).Three pati

In this paper， we have demonstrated an Er-doped ultrafast laser with a single mode fiber-gradient index multimode fiber-single mode fiber (SMF-GIMF-SMF， SMS) structure as saturable absorber， which can generate not only stable single-pulse state， but also special mode-locked pulses with the characteristics of high energy and noisy behaviors at proper pump power and cavity polarization state. In addition， we have deeply investigated the real-time spectral evolutions of the mode-locked pulses through the dispersive Fourier transformation (DFT) technique. It can be found that the pulse regime can actually consist of a lot of small noise pulses with randomly varying intensities. We believe that these results will further enrich the nonlinear dynamical processes in the ultrafast lasers.

Wide-angle imaging and spectral detection play vital roles in tasks such as target tracking,object classification,and anti-camouflage.However,limited by their intrinsically different architectures,as determined by frequency dispersion requirements,their simultaneous implementation in a shared-aperture system is difficult.Here,we propose a novel concept to realize reconfigurable dual-mode detection based on electrical-control tunable metasurfaces.As a proof-of-concept demonstration,the simultaneous implementation of wide-angle imaging and polarization-spectral detection in a miniature sharedaperture meta-optical system is realized for the first time via the electrical control of cascaded catenary-like metasurfaces.The proposed system supports the imaging(spectral)resolution of approximately 27.8 line-pairs per millimeter(lp·mm^(-1);~80 nm)for an imaging(spectral)mode from 8 to 14 lm.This system also bears a large field of view of about 70,enabling multi-target recognition in both modes.This work may promote the miniaturization of multifunctional optical systems,including spectrometers and polarization imagers,and illustrates the potential industrial applications of meta-optics in biomedicine,security,space exploration,and more.

Bouc-Wen-Baber-Noori(BWBN)模型是改进的Bouc-Wen模型。BWBN模型可以较好地描述钢筋混凝土结构在强震作用下的捏缩效应,但由于BWBN模型中“捏缩效应”函数复杂且参数较多,其计算效率较低。针对该问题,该文基于近似狄拉克δ函数构建更简单、高效的“捏缩效应”函数,进而提出了新的改进Bouc-Wen(MBW)模型,它可以用更少的参数来实现捏缩效应。基于预制RC桥墩的拟静力试验数据来验证MBW模型的有效性,利用MATLAB遗传算法工具箱对BWBN模型和MBW模型进行参数识别,结果表明,该文提出的MBW模型具有更高的计算精度。

柴达木盆地地处青藏高原东北部,为高原型巨大封闭盆地,植被以荒漠为主,兼具沼泽湿地、盐生草甸和山地草原等。为更精细地展示柴达木盆地地区植被的物种组成、群落特征及其分布格局,该研究利用2022年夏季第二次青藏高原综合科学考察及2014年青藏高原资料匮乏区综合科学考察的野外数据,共157个样地,458个样方,整合为柴达木盆地植物群落样方数据集。通过数据的整理与汇编,共得到185条物种信息,其中物种数最多的科依次为菊科(39种)、禾本科(33种)、豆科(17种)、苋科(16种)及十字花科(10种),包含物种数最多的属依次为针茅属(Stipa)、蒿属(Artemisia)、黄耆属(Astragalus)、棘豆属(Oxytropis)和风毛菊属(Saussurea)等。植物生活型组成以草本居多,占78.37%;区系地理成分以亚洲中部种占优势,占41.62%。基于群落学-生态学分类原则,157个样地可以归为4个植被型组,7个植被型,11个植被亚型,40个群系。该数据集可为深入研究柴达木盆地植被特征、《中国植被志》编研、青藏高原及全国植被图绘制提供最原始的基础数据。
