Effects of bilirubin on perioperative myocardial infarction and its long-term prognosis in patients undergoing percutaneous coronary intervention

BACKGROUND Although bilirubin is known to be an antioxidant,any relationship with coronary heart disease remains controversial.To the best of our knowledge,no previous study has investigated the association between bilirubin and perioperative myocardial infarction(PMI),including its long-term prognosis.AIM To investigate the impact of bilirubin levels on PMI in patients undergoing percutaneous coronary intervention(PCI),and long-term prognosis in post-PMI patients.METHODS Between January 2014 and September 2018,10236 patients undergoing elective PCI were enrolled in the present study.Total bilirubin(TB)and cardiac troponin I(cTnI)levels were measured prior to PCI and cTnI at further time-points,8,16 and 24 h after PCI.Participants were stratified by pre-PCI TB levels and divided into three groups:14.4μmol/L.PMI was defined as producing a post-procedural cTnI level of>5×upper limit of normal(ULN)with normal baseline cTnI.Major adverse cardiovascular events(MACEs)included cardiac death,MI,stroke and revascularization during a maximum 5-year follow-up.RESULTS PMI was detected in 526(15.3%),431(12.7%)and 424(12.5%)of patients with pre-PCI TB levels of14.4μmol/L(P=0.001),respectively.Multivariate logistical analysis indicated that patients with TB 10.2-14.4 and>14.4μmol/L had a lower incidence of PMI[TB 10.2-14.4μmol/L:Odds ratio(OR):0.854;95%confidence interval(CI):0.739-0.987;P=0.032;TB>14.4μmol/L:OR:0.846;95%CI:0.735-0.975;P=0.021]compared with patients with TB14.4μmol/L was associated with a reduced risk of MACEs compared with a TB level<10.2μmol/L(hazard ratio 0.667;95%CI:0.485-0.918;P=0.013).CONCLUSION Bilirubin was a protective factor in PMI prediction.For post-PMI patients,el

Comparison of low-density lipoprotein cholesterol/high-density lipoprotein cholesterol and total cholesterol/high-density lipoprotein cholesterol for the prediction of thin-cap fibroatheroma determined by intravascular optical coherence tomography

Background The correlation among the ratios of low-density lipoprotein cholesterol/high-density lipoprotein cholesterol(LDL-C/HDL-C), total cholesterol/high-density lipoprotein cholesterol(TC/HDL-C) and thin-cap fibroatheroma has not yet been established. Methods It was a single center, retrospective observational study. In total, we recruited 421 patients(82.4% men;mean age 65.73 ± 10.44 years) with one culprit vessel which determined by intravascular optical coherence tomography(OCT). The thinnest-capped fibroatheroma(TCFA) group was defined as lipid contents in > 2 quadrants, with the thinnest fibrous cap measuring less than 65 μm. Univariate and multivariate logistic regression were carried out to explore the relationship between lipoprotein ratios, TCFA and other characteristics of plaque. To compare different ratios, the area under curve(AUC) of receiver-operating characteristic(ROC) curve was assessed. Results OCT was performed in 421 patients(TCFA group(n = 109), non-TCFA group(n = 312)). LDL-C/HDL-C in the TCFA group was significantly higher than in the non-TCFA group(2.95 ± 1.20 vs. 2.43 ± 0.92, P both LDL-C/HDL-C(OR: 1.002(1.002-1.003), P < 0.05) and TC/HDL-C(OR: 1.001(1.001-1.004), P < 0.05) were considered independent factors for the prediction of TCFA according to the logistic regression. Based on the AUC comparison, LDL-C/HDL-C and TC/HDL-C had no significant difference statistically(LDL-C/HDL-C AUC: 0.63;TC/HDL-C AUC: 0.61;P = 0.10) for the prediction of TCFA. Conclusions LDL-C/HDL-C and TC/HDL-C could be the independent factors for predicting the presence of TCFA, indicating coronary plaque vulnerability in CAD patients. Moreover, TC/HDL-C also showed a comparative performance for the prediction of TCFA as LDL-C/HDL-C.

Numerical Study on Local Flow and Heat Transfer Characteristics of Supercritical CO_(2) in PCHE with Sinusoidal Channels

Local heat transfer and flow characteristics,which is crucial to the overall performance of supercritical CO_(2) recuperators,is rarely examined in details in reported studies.In this paper,the local heat transfer and flow characteristics of supercritical CO_(2) in sinusoidal channel printed circuit heat exchangers are numerically investigated under the working conditions of recuperators.Based on the simplified physical model constituted by10 pitches,the variations of Re,heat flux,Nu,secondary flow and other relevant parameters along the flow direction are analyzed firstly.Comparison is further made between the high and low temperature recuperators(HTR and LTR).It's observed that the local heat transfer and flow characteristics vary greatly from the inlet to the outlet,especially in the LTR.Differences of 127.5% and 61.7% can be observed on the cold side of the LTR for Re and heat transfer coefficient h, respectively.Results indicate that the temperature difference and heat transfer coefficient h should not be regarded as constant and the distribution of h should be carefully considered in the design of the LTR.The complicated interactions among the varying thermophysical properties,buoyancy and the periodically changed centrifugal force are believed to be the key that shapes the flow fields.Furthermore,the changes of the wavy angle θ are found to have greater influence than the changes of mass flow rate in reshaping the flow field when θ>25°.Though gravity direction strongly affects the local heat transfer and flow characteristics,it's also found that the effects on the overall thermal-hydraulic performance are relatively minor.Yet the installation direction that yields a_y=g should better be avoided for the sinusoidal channel printed circuit heat exchanger.

Tumor-associated macrophages(TAMs)are emerging as targets for tumor therapy because of their primary role in promoting tumor progression.Several studies have been conducted to target TAMs by reducing their infiltration,depleting their numbers,and reversing their phenotypes to suppress tumor progression,leading to the development of drugs in preclinical and clinical trials.However,the heterogeneous characteristics of TAMs,including their ontogenetic and functional heterogeneity,limit their targeting.Therefore,in-depth exploration of the heterogeneity of TAMs,combined with immune checkpoint therapy or other therapeutic modalities could improve the efficiency of tumor treatment.This review focuses on the heterogeneous ontogeny and function of TAMs,as well as the current development of tumor therapies targeting TAMs and combination strategies.

Tumor hypoxia severely limits the therapeutic efficacy of photodynamic therapy(PDT) for solid tumors,which is highly dependent on tissue oxygen concentration.In this study,we developed a platinum(Ⅱ)-acetylide-based metallacycle compound bearing six 1,4-dimethylnaphthalenes(DMN) groups,and controlled the photodynamic and photothermal effects of the compound by adjusting the power of 730 nm laser to achieve reversible sensitization,storage and release of 1~O_(2) within a single molecule.The compound formed nanoparticles by self-assembly and exhibited good water solubility and biocompatibility.Under laser irradiation,the strong spin-orbit coupling of platinum atoms in the metallacycle facilitated 1~O_(2) generation.The produced 1~O_(2) was captured by the DMN carriers and transported into the hypoxic tumor,where 1~O_(2) release was triggered owing to the good photothermal effect of the extended conjugation of the metallacycle.During therapy,the metallacycle serving as a photosensitizer,1~O_(2) carrier,and photothermal reagent,achieved the synergistic therapy of PDT/PTT,demonstrating the versatility of the metallacycle.This study proposes a new strategy to develop phototherapy agents that are suitable for hypoxic tumors.

An adaptive finite-difference method for seismic traveltime modeling based on 3D eikonal equation

3D eikonal equation is a partial differential equation for the calculation of first-arrival traveltimes and has been widely applied in many scopes such as ray tracing,source localization,reflection migration,seismic monitoring and tomographic imaging.In recent years,many advanced methods have been developed to solve the 3D eikonal equation in heterogeneous media.However,there are still challenges for the stable and accurate calculation of first-arrival traveltimes in 3D strongly inhomogeneous media.In this paper,we propose an adaptive finite-difference(AFD)method to numerically solve the 3D eikonal equation.The novel method makes full use of the advantages of different local operators characterizing different seismic wave types to calculate factors and traveltimes,and then the most accurate factor and traveltime are adaptively selected for the convergent updating based on the Fermat principle.Combined with global fast sweeping describing seismic waves propagating along eight directions in 3D media,our novel method can achieve the robust calculation of first-arrival traveltimes with high precision at grid points either near source point or far away from source point even in a velocity model with large and sharp contrasts.Several numerical examples show the good performance of the AFD method,which will be beneficial to many scientific applications.

Tuning the coordination atoms of central metal is an effective means to improve the electrocatalytic activity of atomic catalysts.Herein,iridium(Ir) is proposed to be asymmetrically anchored by sp-N and pyridinic N of hydrogen-substituted graphdiyne(HsGDY),and coordinated with OH as an Ir atomic catalyst(Ir_(1)-N-HsGDY).The electron structures,especially the d-band center of Ir atom,are optimized by these specific coordination atoms.Thus,the as-synthesized Ir_(1)-N-HsGDY exhibits excellent electrocatalytic performances for oxygen reduction and hydrogen evolution reactions in both acidic and alkaline media.Benefiting from the unique structure of HsGDY,IrN_(2)(OH)_(3) has been developed and demonstrated to act as the active site in these electrochemical reactions.All those indicate the fresh role of the sp-N in graphdiyne in producing a new anchor way and contributing to promote the electrocatalytic activity,showing a new strategy to design novel electrochemical catalysts.

Recovery of cyanophycin granule polypeptide from activated sludge: carbon source dependence and aggregation-induced luminescence characteristics

In the sewage treatment process,facilitating the conversion of pollutants into value-added resources holds great potential for reducing the amount of greenhouse gas emissions and promoting economic circulation.Cyanophycin granule polypeptide(CGP),a recently discovered high value-added biopolymer present in activated sludge,has provided new avenues for the recovery of resources.However,the mechanisms that regulate CGP synthesis and the characteristics of this biopolymer in activated sludge remain unclear thus far.This study investigated the synthesis of CGP,polyhydroxyalkanoates(PHA),and alginate-like exopolysaccharides(ALE)in various microbial aggregates under different carbon sources feeding conditions.Our results showed that the CGP yields was superior that of PHA and ALE when subjected to identical carbon source feeding conditions.Furthermore,biofilm was more conducive to CGP accumulation than floc sludge.Compared with glucose and methanol,sodium acetate significantly enriched the CGP synthetase-encoding gene(cphAabundance=~17419),resulting in the highest CGP yield(average 107.1 mg/g MLSS)in both biofilm and floc sludge.This study is the first to reported the characteristic fluorescence of CGP(Ex/Em=~360/450 nm)caused by the aggregated luminescence of arginine on the side chains.Overall,this study highlights the potential application of CGP as a fluorescent material and offers insights into CGP recovery from activated sludge in wastewater treatment plants.

A newly proposed heatstroke-induced coagulopathy score in patients with heat illness: A multicenter retrospective study in China

Purpose:In patients with heatstroke, disseminated intravascular coagulation (DIC) is associated with greater risk of in-hospital mortality. However, time-consuming assays or a complex diagnostic system may delay immediate treatment. Therefore, the present study proposes a new heatstroke-induced coagulopathy (HIC) score in patients with heat illness as an early warning indicator for DIC.Methods:This retrospective study enrolled patients with heat illness in 24 Chinese hospitals from March 2021 to May 2022. Patients under 18 years old, with a congenital clotting disorder or liver disease, or using anticoagulants were excluded. Data were collected on demographic characteristics, routine blood tests, conventional coagulation assays and biochemical indexes. The risk factors related to coagulation function in heatstroke were identified by regression analysis, and used to construct a scoring system for HIC. The data of patients who met the diagnostic criteria for HIC and International Society on Thrombosis and Haemostasis defined-DIC were analyzed. All statistical analyses were performed using SPSS 26.0.Results:The final analysis included 302 patients with heat illness, of whom 131 (43.4%) suffered from heatstroke, including 7 death (5.3%). Core temperature (OR = 1.681, 95% CI 1.291 - 2.189, p bosis and Hemostasis-DIC (r = 0.8848, p < 0.001). The incidence of HIC (27.5%) was higher than that of DIC (11.2%) in all of 131 heatstroke patients. Meanwhile, the mortality rate of HIC (19.4%) was lower than that of DIC (46.7%).
