Prevalence and clinical impact of sarcopenia in liver transplant recipients:A meta-analysis

BACKGROUND The prevalence of sarcopenia in patients undergoing liver transplantation(LT)remains to be determined partly because of different diagnostic *** has recently been recognized as a new prognostic factor for predicting outcomes in LT *** To estimate the prevalence of sarcopenia and evaluate its clinical effect on LT *** This systematic search was conducted in PubMed,Web of Science,Embase,and Cochrane Library for original English-language articles that investigated the prevalence and influence of sarcopenia in patients undergoing LT from database inception to November 30,*** studies of the definition of sarcopenia that estimate sarcopenia prevalence and evaluate its effect on clinical outcomes and the risk of mortality were *** Twenty-five studies involving 7760 patients undergoing LT were *** pooled prevalence of sarcopenia in patients undergoing LT was 40.7%[95%confidence intervals(95%CI):32.1-49.6].The 1-,3-,and 5-year cumulative probab-ilities of post-LT survival in patients with preoperative sarcopenia were all lower than those without sarcopenia(P<0.05).Sarcopenia was associated with an increased risk of post-LT mortality in patients undergoing LT(adjusted hazard ratio:1.58;95%CI:1.21-2.07).Patients with preoperative sarcopenia had a longer intensive care unit stay,a high risk ratio of sepsis,and serious post-LT complications than those without *** Sarcopenia is prevalent in a substantial proportion of patients undergoing LT and is strongly and independently associated with higher a risk of mortality risk.

Alcoholic liver disease(ALD)remains an important health problem *** of micronutrients has been broadly reported to be a common characteristic in patients with ALD,given the fact that micronutrients often act as composition or coenzymes of many biochemical enzymes responsible for the inflammatory response,oxidative stress,and cell *** the metabolic pattern and the function of these micronutrients is a prerequisite before targeted intervention can be delivered in clinical *** years have registered a significant improvement in our understanding of the role of micronutrients on the pathogenesis and progression of ***,how and to what extent these micronutrients are involved in the pathophysiology of ALD remains largely *** the current study,we provide a review of recent studies that investigated the imbalance of micronutrients in patients with ALD with a focus on zinc,iron,copper,magnesium,selenium,vitamin D and vitamin E,and determine how disturbances in micronutrients relates to the pathophysiology of ***,zinc,selenium,vitamin D,and vitamin E uniformly exhibited a deficiency,and iron demonstrated an elevated *** for copper,both an elevation and deficiency were observed from existing *** importantly,we also highlight several challenges in terms of low sample size,study design discrepancies,sample heterogeneity across studies,and the use of machine learning approaches.

Current understanding of mesenchymal stem cells in liver diseases

Liver diseases caused by various factors have become a significant threat to publichealth worldwide. Liver transplantation has been considered as the only effectivetreatment for end-stage liver diseases;however, it is limited by the shortage ofdonor organs, postoperative complications, long-term immunosuppression, andhigh cost of treatment. Thus, it is not available for all patients. Recently,mesenchymal stem cells (MSCs) transplantation has been extensively explored forrepairing hepatic injury in various liver diseases. MSCs are multipotent adultprogenitor cells originated from the embryonic mesoderm, and can be found inmesenchymal tissues including the bone marrow, umbilical cord blood, adiposetissue, liver, lung, and others. Although the precise mechanisms of MSC transplantationremain mysterious, MSCs have been demonstrated to be able toprevent the progression of liver injury and improve liver function. MSCs can selfrenewby dividing, migrating to injury sites and differentiating into multiple celltypes including hepatocytes. Additionally, MSCs have immune-modulatoryproperties and release paracrine soluble factors. Indeed, the safety and effectivenessof MSC therapy for liver diseases have been demonstrated in ***, pre-clinical and clinical trials are largely required to confirm its safetyand efficacy before large scale clinical application. In this review, we will explorethe molecular mechanisms underlying therapeutic effects of MSCs on liverdiseases. We also summarize clinical advances in MSC-based therapies.

Hepatocellular carcinoma(HCC)is one of the most prevalent cancers worldwide,accounting for approximately 75%-85%of primary liver *** alterations have been labeled as an emerging hallmark of ***,the last decades have registered a significant improvement in our understanding of the role of metabolism in driving the carcinogenesis and progression of *** this paper,we provide a review of recent studies that investigated the metabolic traits of HCC with a specific focus on three common metabolic alterations involving glycolysis,lipid metabolism,and glutamine addiction which have been gaining much attention in the field of ***,we describe some representative diagnostic markers or tools,and promising treatment agents that are proposed on the basis of the aforementioned metabolic alterations for ***,we present some challenges and directions that may promisingly speed up the process of developing objective diagnostic markers and therapeutic options underlying ***,we recommend future investigations to carefully take into account the influence of heterogeneity,control for study-specific confounds,and invite the validation of existing biomarkers.

On-treatment quantitative hepatitis B e antigen predicted response to nucleos(t)ide analogues in chronic hepatitis B

AIMTo investigate potential predictors for treatment response to nucleos(t)ide analogues (NAs) in hepatitis B e antigen (HBeAg)-positive chronic hepatitis B (CHB) patients. METHODSSeventy-six HBeAg-positive CHB patients received 96-wk NAs optimized therapy (lamivudine and adefovir dipivoxil) were studied retrospectively. Serum hepatitis B surface antigen, HBeAg, hepatitis B core antibody, hepatitis B virus (HBV) DNA and alanine aminotransferase levels were quantitatively measured before and during the treatment at 12 and 24 wk. Stepwise logistic regression analyses were performed to identify predictors for treatment response, and areas under the receiver operating characteristic curves (AUROC) of the independent predictors were calculated. RESULTSForty-three CHB patients (56.6%) achieved virological response (VR: HBV DNA ≤ 300 copies/mL) and 15 patients (19.7%) developed HBeAg seroconversion (SC) after the 96-wk NAs treatment. The HBeAg level (OR = 0.45, P = 0.003) as well as its declined value (OR = 2.03, P = 0.024) at 24-wk independently predicted VR, with the AUROC of 0.788 and 0.736, respectively. The combination of HBeAg titer 1.6 lg PEIU/mL at 24-wk predicted VR with a sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV) of 85%, 100%, 100% and 83%, respectively, and the AUROC increased to 0.923. The HBeAg level (OR = 0.37, P = 0.013) as well as its declined value (OR = 2.02, P = 0.012) at 24-wk also independently predicted HBeAg SC, with the AUROC of 0.828 and 0.814, respectively. The HBeAg titer 2.2 lg PEIU/mL at 24-wk predicted HBeAg SC with a sensitivity, specificity, PPV, NPV of 88%, 98%, 88% and 98%, respectively, and the AUROC reached 0.928. CONCLUSIONThe combination of HBeAg level and its declined value at 24-wk may be used as a reference parameter to optimize NAs therapy.

Identifying the risk factors for pancreatic fistula after laparoscopic pancreaticoduodenectomy in patients with pancreatic cancer

BACKGROUND Laparoscopic pancreaticoduodenectomy(LPD)is a surgical procedure for treating pancreatic cancer;however,the risk of complications remains high owing to the wide range of organs involved during the surgery and the difficulty of *** fistula(PF)is a major complication that not only increases the risk of postoperative infection and abdominal hemorrhage but may also cause multi-organ failure,which is a serious threat to the patient’s *** study hypothesized the risk factors for PF after *** To identify the risk factors for PF after laparoscopic pancreatoduodenectomy in patients with pancreatic *** We retrospectively analyzed the data of 201 patients admitted to the Fudan University Shanghai Cancer Center between August 2022 and August 2023 who underwent LPD for pancreatic *** the basis of the PF’s incidence(grades B and C),patients were categorized into the PF(n=15)and non-PF groups(n=186).Differences in general data,preoperative laboratory indicators,and surgery-related factors between the two groups were compared and analyzed using multifactorial logistic regression and receiver-operating characteristic(ROC)curve *** The proportions of males,combined hypertension,soft pancreatic texture,and pancreatic duct diameter≤3 mm;surgery time;body mass index(BMI);and amylase(Am)level in the drainage fluid on the first postoperative day(Am>1069 U/L)were greater in the PF group than in the non-PF group(P24.91 kg/m²[odds ratio(OR)=13.978,95%confidence interval(CI):1.886-103.581],hypertension(OR=8.484,95%CI:1.22-58.994),soft pancreatic texture(OR=42.015,95%CI:5.698-309.782),and operation time>414 min(OR=15.41,95%CI:1.63-145.674)were risk factors for the development of PF after LPD for pancreatic cancer(all P<0.05).The areas under the ROC curve for BMI,

Abstract The development of physically crosslinked hydrogels with excellent mechanical and sensing properties is of importance for expanding the practical applications of intelligent soft hydrogel ***,after copolymerization of hydroxyl-containing amino acid derivative N-acryloyl serine （ASer） with acrylamide （AM）,we introduce Zr4+through an immersion strategy to construct metal ion-toughened non-covalent crosslinked hydrogels （with tensile strength of up to 5.73 MPa）.It is found that the synergistic coordination of hydroxyl and carboxyl groups with Zr4+substantially increases the crosslinking density of the hydrogels,thereby imparting markedly superior mechanical properties compared to hydroxyl-free Zr4+-crosslinked hydrogels,such as N-acryloyl alanine （AAla） copolymerized with AM hydrogels （with tensile strength of 2.98 MPa）Through the adjustment of the composition of the copolymer and the density of coordination bonds,the mechanical properties of the hydrogels can be modulated over a wide ***,due to the introduction of metal ions and the dynamic nature of coordination bonds,the hydrogels also exhibit excellent sensing performance and good self-recovery properties,paving the way for the development of flexible electronic substrates with outstanding comprehensive performances.

The degree and scale of underground space development are growing with the continuous advancement of urbanization in *** lack of research on the change of the groundwater flow field before and after the development of underground space has led to various problems in the process of underground space development and *** paper took the key development zone of the Xiong’an New Area as the study area,and used the Groundwater modeling system software(GMS)to analyse the influence on the groundwater flow field under the point,line,and surface development *** main results showed that the underground space development would lead to the expansion and deepening of the cone of depression in the *** groundwater level on the upstream face of the underground structure would rise,while the water level on the downstream face would ***“line”concurrent development has the least impact on the groundwater flow field,and the maximum rise of water level on the upstream side of the underground structure is expected to be approximately 3.05 ***“surface”development has the greatest impact on the groundwater flow field,and the maximum rise of water level is expected to be 7.17 m.

The paper investigates the escape probability for isotropic emitters near a Kerr black *** propose a new approach to obtain the escape probability in a general manner,going beyond previous case-by-case *** approach is based on studies of the black hole shadow with astrometric observable and can be applied to emitters with an arbitrary 4-velocities and locations,even to the emitters outside of the equatorial *** also consider representative examples illustrating how escape probabilities vary with distance,velocity,and inclination ***,this new approach provides an effective method for studying escape probabilities near Kerr black holes.
