BACKGROUND Early adenocarcinoma mixed with a neuroendocrine carcinoma(NEC)component arising in the gastroesophageal junctional(GEJ)region is rare and even rarer in young ***,we report such a case in a 29-year-old Chinese *** SUMMARY This patient presented to our hospital with a 3-mo history of dysphagia and *** endoscopy revealed an elevated nodule in the distal esophagus 1.6 cm above the GEJ line,without Barrett’s esophagus or involvement of the gastric *** nodule was completely resected by endoscopic submu-cosal dissection(ESD).Pathological examination confirmed diagnosis of intra-mucosal adenocarcinoma mixed with an NEC component,measuring 1.5 ***,both adenocarcinoma and NEC components were positive for P53 with a Ki67 index of 90%;NEC was positive for synaptophysin and ***-generation sequencing of 196 genes demonstrated a novel germline mutation of the ERCC3 gene in the DNA repair pathway and a germline mutation of the RNF43 gene,a common gastric cancer driver gene,in addition to pathogenic somatic mutations in P53 and CHEK2 *** patient was alive without evidence of the disease 36 mo after *** Early adenocarcinoma with an NEC component arising in the distal esophageal side of the GEJ region showed evidence of gastric origin.

BACKGROUND The efficacy of Vonoprazan-amoxicillin dual therapy(VAT)in the treatment of Helicobacter pylori(***)is *** To evaluate the efficacy of VAT in the Chinese *** This prospective,multicenter,randomized,open-label,and two-stage study was conducted at 23 centers in Fujian,China(May 2021-April 2022).***-infected patients were randomized to bismuth quadruple therapy(BQT),BQT-Vonoprazan(BQT-V),seven-day VAT(VAT-7),ten-day VAT(VAT-10),and fourteen-day VAT(VAT-14)*** primary endpoint was the *** eradication *** secondary endpoint was the frequency of adverse *** study was registered with the Chinese Clinical Trial Registry,*** In the first stage,VAT-7 and BQT-V groups were selected for early termination because less than 23 among 28 cases were *** the second stage,the eradication rates for BQT,VAT-10,and VA-14 were 80.2%[95%confidence interval(95%CI):71.4%-86.8%],93.2%(86.6%-96.7%),92.2%(85.3%-96.0%)in the intention-to-treat(ITT)analysis,and 80.9%(95%CI:71.7%-87.5%),94.0%(87.5%-97.2%),and 93.9%(87.4%-97.2%)in the per-protocol *** ITT analysis showed a higher eradication rate in the VAT-10 and VAT-14 groups than in the BQT group(P=0.022 and P=0.046,respectively).The incidence of adverse events in the VAT-10 and VAT-14 groups was lower than in the BQT group(25.27%and 13.73%vs 37.62%,respectively;P<0.001).CONCLUSION VAT with a duration of 10 or 14 days achieves a higher eradication rate than the BQT,with a more tolerable safety profile in ***-infected patients in *** XP et *** for *** eradication.

Organic room temperature phosphorescence(RTP)materials have potential applications in information technology and ***,the precise control of the afterglow in reversible manners remains challenging for organic ***,we report a kind of organic RTP material fabricated by simple heating mixtures of tartaric acid(TA)and aromatic acids,which can switch their phosphorescence by *** mixtures show tunable phosphorescence from indigo to orange with phosphorescence efficiency of up to 53.99% due to locking different organic luminogens by the TA-formed matrix through the noncovalent *** afterglow of those materials lasts a few seconds and disappears by water fumigation,which can be repeated in response to wet/heat *** drop-casting those materials on glass slides,a laser-repatternable phosphorescence is achieved by facile laser direct writing and quenched by water *** results open the opportunity for the design of smart stimuli-responsive phosphorescence materials from sustainable natural products.

Cobalt doping amount determines dominant reactive species in peroxymonosulfate activation via porous carbon catalysts co-doped by cobalt and nitrogen

Switching the reaction routes in peroxymonosulfate(PMS)-based advanced oxidation processes have attracted much attention but remain ***,a series of Co-N/C catalysts with different compositions and structures were prepared by using bimetallic zeolitic imidazolate frameworks based on ZIF-8 and ZIF-67(x Zn/Co-ZIFs).Results show that Co doping amount could mediate the transformation of the activation pathway of PMS over CoN/*** Co doping amount was less than 10%,the constructed x Co-N/C/PMS system(x≤10%)was singlet oxygen-dominated reaction;however further increasing Co doping amount would lead to the generation and coexistence of sulfate radicals and high-valent cobalt,besides singlet ***,the nitrogen-coordinated Co(Co-NX)sites could serve as main catalytically active sites to generate singlet *** excess Co doping amount caused the formation of Co nanoparticles from which leached Co ions were responsible for the generation of sulfate radicals and high-valent *** to undoped N/C,Co doping could significantly enhance the catalytic *** 0.5%Co-N/C could achieve the optimum degradation(0.488 min^(-1))and mineralization abilities(78.4%)of sulfamethoxazole among the investigated Co-N/C catalysts,which was superior to most of previously reported *** addition,the application prospects of the two systems in different environmental scenarios(pH,inorganic anions and natural organic matter)were assessed and showed different degradation *** study provides a strategy to regulate the reactive species in PMS-based advanced oxidation process.

The occurrence characteristics of shale oil are of great significance to the movability of shale *** this study,the occurrence characteristics of oil in the shale matrix at Funing Formation shale in Subei Basin were quantitatively evaluated by organic geochemistry and microscopic pore structure characterization *** Multiple Isothermal Stages Pyrolysis(MIS)experiment results show that the content of total oil,adsorbed oil,and free oil in the shales are 3.15-11.25 mg/g,1.41-4.95 mg/g,and 1.74-6.51 mg/g,*** which the silicon-rich shale has the best *** relative content of free oil shows an increasing trend in pores with pore diameters greater than 3 *** the relative content of free oil reaches 100%,the pore size of silicon-rich shale is about 200 nm,while that of calcium-rich shale,clay-rich shale,and siliceous mixed shale is about 10 *** occurrence law of adsorbed oil is opposite to that of free oil,which indicates that shale oil will occur in the pores and fractures in a free state in a more extensive pore size range(>200 nm).This study also enables us to further understand the occurrence characteristics of shale oil under the interaction of occurrence state and occurrence space.

Drying soil has been conducive to a high frequency of extreme high-temperature events over many regions worldwide in recent ***,changes in the intraseasonal variability of soil moisture can also influence the likelihood of extremely high *** previous investigators have examined the association between extremely high temperatures and large-scale atmospheric circulation variability,the role of land-atmosphere coupling dominated by soil moisture variability in extremely high temperatures,particularly over the Eurasian continent,is not well *** this study,on the basis of the Land Surface,Snow,and Soil Moisture Model Intercomparison Project,we found that land-atmosphere feedback amplified the variability of soil moisture in most regions of Eurasia during summer from 1980 to *** amplification of soil moisture variability is closely correlated with more intensive intraseasonal variability of surface air temperature and more frequent occurrences of extreme high-temperature events,particularly in Europe,Siberia,Northeast Asia,and the Indochina *** correlation implies that increasing the intraseasonal variability of soil moisture results in a high likelihood of heat extremes during summer in most parts of Eurasia except Asian desert *** the intraseasonal timescale,the land-atmosphere coupling increases the variability of surface sensible heat flux and net long-wave radiation heating the atmosphere by intensifying the soil moisture variability,thus amplifying the variability of surface air temperature and enhancing the extreme high-temperature *** finding demonstrates the importance of changes in intraseasonal soil moisture variability for the increasing likelihood of heat extremes in summer.

Analysis of metabolic characteristics of metabolic syndrome in elderly patients with gastric cancer by non-targeted metabolomics

BACKGROUND The relationship between metabolic syndrome(MetS)and gastric cancer(GC),which is a common metabolic disease,has attracted much ***,the specific metabolic characteristics of MetS in elderly patients with GC remain *** To investigate the differentially abundant metabolites and metabolic pathways between preoperative frailty and MetS in elderly patients with GC based on nontargeted metabolomics *** In this study,125 patients with nonfrail nonmeal GC were selected as the control group,and 50 patients with GC in the frail group were selected as the frail ***-five patients with GC combined with MetS alone were included in the MetS group,and 50 patients with GC combined with MetS were included in the MetS *** metabolomics techniques were used to measure plasma metabolite levels by ultrahigh-performance liquid chromatography-mass *** statistical analysis was performed by principal component analysis,orthogonal partial least squares,pattern recognition analysis,cluster analysis,and metabolic pathway *** A total of 125 different metabolites,including amino acids,glycerophospholipids,sphingolipids,fatty acids,sugars,nucleosides and nucleotides,and acidic compounds,were identified via nontargeted metabolomics *** with those in the control group,there were 41,32,and 52 different metabolites in the MetS group,the debilitated group,and the combined group,*** metabolites were significantly increased in the MetS *** the weak group,amino acids and most glycerol phospholipid metabolites decreased significantly,and fatty acids and sphingosine increased *** combined group was characterized by significantly increased levels of nucleotide metabolites and acidic *** alanine,aspartic acid,and glutamate metabolic pathways were obviously enriched in the asthenic group,and the glycerol and phospholipid metabolic pathways were obv

The mode-division multiplexing technique combined with a few-mode erbium-doped fiber amplifier(FM-EDFA)demonstrates significant potential for solving the capacity limitation of standard single-mode fiber(SSMF)transmission ***,the differential mode gain(DMG)arising in the FM-EDFA fundamentally limits its transmission capacity and ***,an innovative DMG equalization strategy using femtosecond laser micromachining to adjust the refractive index(RI)is *** mode-dependent attenuations can be achieved according to the DMG profile of the FM-EDFA,enabling DMG *** validate the proposed strategy,DMG equalization of the commonly used FM-EDFA configuration was *** results revealed that by optimizing both the length and RI modulation depth of the femtosecond laser-tailoring area,the maximum DMG(DMGmax)among the 3 linear-polarized(LP)mode-group was mitigated from 10 dB to 1.52 dB,whereas the average DMG(DMGave)over the C-band was reduced from 8.95 dB to 0.78 ***,a 2-LP mode-group DMG equalizer was experimentally demonstrated,resulting in a reduction of the DMGmax from 2.09 dB to 0.46 dB,and a reduction of DMGave over the C band from 1.64 dB to 0.26 dB,with only a 1.8 dB insertion ***,a maximum range of variable DMG equalization was achieved with 5.4 dB,satisfying the requirements of the most commonly used 2-LP mode-group amplification scenarios.

Objective:To confirm the efficacy and safety of Ganyushu Granule(GYSG)in treating premenstrual syndrome(PMS)in patients with Gan(Liver)depression and qi stagnation syndrome(GDQSS)and determine its effective ***:From June 2018 to March 2021,a total of 240 PMS women with GDQSS were included and randomly divided into 3 groups in a 1:1:1 ratio using central block randomization:high-dose GYSG group(n=78,GYSG 2 packs/time),low-dose GYSG group(n=82,GYSG and its simulant 1 pack/time),and placebo group(n=80,GYSG simulant 2 packs/time).Treatment with GYSG or placebo was given thrice daily and for up to 3 menstrual *** outcomes were PMS diary(PMSD)score and premenstrual tension syndrome self-rating scale(PMTS).Secondary outcomes were Chinese medicine(CM)syndrome ***,PMTS,and efficacy of CM were evaluated with menstrual cycles during the treatment *** indicators were analyzed after each menstrual *** analyses were performed using an intention-to-treat method,and clinical safety was ***:Of the 216 patients included in the effectiveness analysis,70,75,and 71 patients were in the high-,low-dose GYSG,and placebo groups,*** the 2nd treatment cycle,the change in PMSD scores in the high-and low-dose groups was lower than that in the placebo group(P<0.05).PMTS scores in the high-dose GYSG group after the 1st treatment cycle was lower than that in the placebo group(P<0.05),while after the 3rd treatment cycle,that in the low-dose group was lower than that in the placebo group(P<0.05).After the 2nd treatment cycle,the high-dose GYSG group had the best CM syndrome efficacy(P<0.05).No serious adverse reactions were ***:GYSG was safe and well-tolerated at both doses for treating PMS patients with ***-dose GYSG might be the optimal dose for a phaseⅢtrial.

A 110-year ensemble simulation of an ocean general circulation model(OGCM)was analyzed to identify the modulation of salinity interdecadal variability on El Niño-Southern Oscillation(ENSO)amplitude in the tropical Pacific during *** simulating results show that sea surface salinity(SSS)variation in the region exhibits notable and coherent interdecadal variability signal,which is closely associated with the Interdecadal Pacific Oscillation(IPO).As salinity increases or reduces,the SSS modulations on ENSO amplitude during its warm/cold events vary asymmetrically with positive/negative IPO ***,salinity interdecadal variability can enhance or reduce ENSO-related conditions in upper-ocean stratification,contributing noticeably to ENSO *** anomalies associated with the mixed layer depth and barrier layer thickness can modulate ENSO amplitude during positive and negative IPO phases,resulting in the asymmetry of sea surface temperature(SST)anomaly in the tropical *** positive IPO phases,SSS interdecadal variability contributes positively to El Niño amplitude but negatively to La Niña amplitude by enhancing or reducing SSS interannual variability,and vice versa during negative IPO ***,the results indicate that the modulation of the ENSO amplitude by the SSS interdecadal variability is 15%-28%during negative IPO phases and 30%-20%during positive IPO phases,***,the SSS interdecadal variability associated with IPO and its modulation on ENSO amplitude in the tropical Pacific are among factors essentially contributing ENSO diversity.
