Azvudine(FNC)is a nucleoside analog that inhibits HIV-1 RNA-dependent RNA polymerase(RdRp).Recently,we discovered FNC an agent against SARS-CoV-2,and have taken it into Phase III trial for COVID-19 *** monophosphate analog inhibited SARS-CoV-2 and HCoV-OC43 coronavirus with an EC_(50) between 1.2 and 4.3 μM,depending on viruses or cells,and selective index(SI)in 15-83 *** administration of FNC in rats revealed a substantial thymus-homing feature,with FNC triphosphate(the active form)concentrated in the thymus and peripheral blood mononuclear cells(PBMC).Treating SARS-CoV-2 infected rhesus macaques with FNC(0.07 mg/kg, qd,orally)reduced viral load,recuperated the thymus,improved lymphocyte profiles,alleviated in flammation and orga n damage,and lessened grou nd・glass opacities in chest *** gle-cell seque ncing suggested the promotion of thymus function by FNC.A randomized,single-arm clinical trial of FNC on compassionate use(n=31)showed that oral FNC(5 mg,qd)cured all COVID-19 patients,with 100%viral ribonucleic acid negative conversion in 3.29±2.22 days(range:1-9 days)and 100%hospital discharge rate in 9.00±4.93 days(range:2-25 days).The side-effect of FNC is minor and transient dizziness and nausea in 16.12%(5/31)***,FNC might cure COVID-19 through its anti-SARS-CoV-2 activity concentrated in the thymus,followed by promoted immunity.

Background Acinetobacter baumanii （A. baumanii ） remains an important microbial pathogen resulting in nosocomialacquired infections with significant morbidity and mortality. The mechanism by which nosocomial bacteria, like A. baumanii, attain multidrug resistance to antibiotics is of considerable interest. The aim in this study was to investigate the spread status of antibiotic resistance genes, such as multiple 13-1actamase genes and aminoglycoside-modifying enzyme genes, from A. baumanii strains isolated from patients with lower respiratory tract infections （LRTIs）. Methods Two thousand six hundred and ninety-eight sputum or the bronchoalveolar lavage samples from inpatients with LRTIs were collected in 21 hospitals in the mainland of China from November 2007 to February 2009. All samples were routinely inoculated. The isolated bacterial strains and their susceptibility were analyzed via VITEK-2 expert system. Several kinds of antibiotic resistant genes were further differentiated via polymerase chain reaction and sequencing methods. Results Totally, 39 A. baumanii strains were isolated from 2698 sputum or bronchoalveolar lavage samples. There was not only a high resistant rate of the isolated A. baumanfi strains to ampicillin and first- and second-generation cephalosporins （94.87%, 100% and 97.44%, respectively）, but also to the third-generation cephalosporins （ceftriaxone at 92.31%, ceftazidine at 51.28%） and imipenem （43.59%） as well. The lowest antibiotic resistance rate of 20.51% was found to amikacin. The OXA-23 gene was identified in 17 strains of A. baumanii, and the AmpC gene in 23 strains. The TEM-1 gene was carried in 15 strains. PER-1 and SHV-2 genes were detected in two different strains. Aminoglycoside-modifying enzyme gene aac-3-1a was found in 23 strains, and the aac-6＂lb gene in 19 strains, aac-3-1a and aac-6＂lb genes hibernated in three A. baumanfi strains that showed no drug-resistant phenotype. Background Data on the epidemiology of hypertension in Chinese non-dialysis chronic kidney disease （CKD） patients are *** aim of the present study was to investigate the prevalence,awareness,treatment,and control of hypertension in the non-dialysis CKD patients through a nationwide,multicenter study in *** The survey was performed in 61 tertiary hospitals in 31 provinces,municipalities,and autonomous regions in China （except Hong Kong,Macao,and Taiwan）.Trained physicians collected demographic and clinical data and measured blood pressure （BP） using a standardized *** was defned as systolic BP ≥140 mmHg and/or diastolic BP ≥90 mmHg,and/or use of antihypertensive *** 〈140/90 mmHg and 〈130/80 mmHg were used as the 2 thresholds of hypertension *** multivariate logistic regression with adjustment for sex and age,we analyzed the association between CKD stages and uncontrolled hypertension in non-dialysis CKD *** The analysis included 8927 non-dialysis CKD *** prevalence,awareness,and treatment of hypertension in non-dialysis CKD patients were 67.3%,85.8%,and 81.0%,*** hypertensive CKD patients,33.1% and 14.1% had controlled BP to 〈140/90 mmHg and 〈130/80 mmHg,*** successive CKD stages,the prevalence of hypertension in non-dialysis CKD patients increased,but the control of hypertension decreased （P〈0.001）.When the threshold of BP 〈130/80 mmHg was considered,the risk of uncontrolled hypertension in CKD 2,3a,3b,4,and 5 stages increased 1.3,1.4,1.4,2.5,and 4.0 times compared with CKD 1 stage,respectively （P〈0.05）.Using the threshold of 〈140/90 mmHg,the risk of uncontrolled hypertension increased in advanced stages （P〈0.05）.Conclusions The prevalence of hypertension Chinese non-dialysis CKD patients was high,and the hypertension control was *** successive CKD stages,the risk of uncontrolled hypertension increased.

目的:分别对蒙药复方那如三味丸组方药物不同配伍方式和不同煎煮方式的样品进行了整体宏观的红外谱学研究以获得其相互作用和药物剂量与复方配伍之间的关系。方法:采用傅里叶变换红外光谱结合2D-IR同步相关谱技术。结果:组方各单味药水煎液的FT-IR谱图中草乌、诃子、荜拨均具有不同的红外谱峰特征。在分辨率较高的二阶导数谱中,各单味药的谱峰特征明显增强,原一维谱图中的不明显的特征峰也显现出来。对不同配方样品在指纹区2D-IR同步相关谱的分析可知,原方配伍比例样品中的自相关峰的峰数、峰强均较其他配伍方式的样品明显增多和增强,说明原方配伍比例样品中不仅含有较多的对热敏感的物质,且这些物质成分对温度微扰的敏感程度也很高。结论:FT-IR技术用于蒙药复方配伍研究,不仅可以鉴别出各单味药对复方配伍的整体贡献,而且根据谱峰的变化规律可以证明原方配伍的合理性。

目的研究北京市朝阳区三里屯社区卫生服务中心9年强化管理对2型糖尿病(T2DM)患者糖尿病微血管并发症的影响,探索血糖、血压、血脂水平及联合达标≥3次与微血管并发症预后的关系。方法将三里屯社区2008年入选的224例T2DM患者随机分为强化组(113例)和标准组(111例),对其连续管理并随访9年,以收缩压、舒张压、糖化血红蛋白(Hb A1c)、低密度脂蛋白胆固醇(LDL-C)为主要管理指标,比较管理前后两组上述指标的差异;收集终点事件(糖尿病视网膜病变和糖尿病肾病),比较管理后两组终点事件的发生情况;探索分组管理方式和主要指标达标次数是否≥3次与微血管并发症的关系。结果入组时两组患者性别、年龄、病程、吸烟情况、BMI、收缩压、舒张压、空腹血糖(FPG)、餐后2 h血糖(2 h PG)、LDL-C、Hb A1c及尿微量清蛋白排泄率(UAER)比较,差异无统计学意义(P>0.05);管理后强化组FPG、2 h PG、LDL-C、Hb A1c、UAER水平低于标准组,差异有统计学意义(P0.05)。标准组发生或加重的糖尿病视网膜病变6例(5.4%),糖尿病肾病13例(11.7%);强化组发生或加重的糖尿病视网膜病变1例(0.9%),糖尿病肾病5例(4.4%)。两组视网膜病变发生率、糖尿病肾病发生率比较,差异有统计学意义(P2DM强化管理可有效减少糖尿病视网膜病变和糖尿病肾病的发生;Hb A1c达标、联合达标次数≥3次,更可明显减少上述并发症的发生,提高患者生存质量。

目的分析循环肿瘤细胞(CTC)在胃癌辅助诊断中的作用及其与胃癌临床病理特征的相关性,探讨其临床意义。方法纳入98例胃癌患者和38例胃部良性疾病患者,采用阴性富集联合免疫荧光原位杂交(imFISH)检测平台检测患者CTC,分析CTC及其核型特征与临床病理特征的关系。结果胃癌患者中CTC检出率(CTC≥1)为94.9%,而胃部良性疾病患者中CTC检出率为28.9%,两者差异显著(P2个细胞/3.2ml血时,CTC在胃癌诊断中的灵敏度为89.8%,特异性为89.5%(AUC=0.945,95%CI, 0.908-0.982,P2和T3～T4期患者中CTC检出率分别为81.3%和96.7%,在N0、N1～N3患者中CTC检出率分别为81.3%和96.5%,在上述分组中检出率有显著差异(p2表达、Ki-67表达及M分期等分组中,CTC阳性率差异不显著。8号染色体多体CTC在Ⅲ～Ⅳ期患者中的阳性率为69.6%,显著高于Ⅰ～Ⅱ期患者(44%,P<0.05)。结论阴性富集联合imFISH技术在胃癌CTC检测中具有较高的灵敏度和特异性,对于胃癌的辅助诊断和早期检测具有重要意义。同时,CTC总数及8号染色体多体CTC与胃癌TNM分期密切相关,或可用于胃癌分期的辅助判断和预后评估。

JUNO is a multi-purpose neutrino observatory under construction in the south of *** publication presents new sensitivity estimates for the measurement of the △m_(31)^(2),△m_(21)^(2),sin^(2)θ_(12),and sin^(2)θ_(13) oscillation parameters using reactor antineutrinos,which is one of the primary physics goals of the *** sensitivities are obtained using the best knowledge available to date on the location and overburden of the experimental site,the nuclear reactors in the surrounding area and beyond,the detector response uncertainties,and the reactor antineutrino spectral shape constraints expected from the TAO satellite *** is found that the △m_(21)^(2) and sin^(2)θ_(12) oscillation parameters will be determined to 0.5%precision or better in six years of data *** the same period,the △m_(31)^(2) parameter will be determined to about 0.2%precision for each mass ordering *** new precision represents approximately an order of magnitude improvement over existing constraints for these three parameters.

目的探究中国年轻骨髓增殖性肿瘤(MPN)患者的临床和基因突变特征。方法通过横断面研究,在全国范围内向MPN患者发放调查问卷,根据诊断时年龄分为年轻组(≤40岁)、中年组(41~60岁)和老年组(>60岁),在各疾病类型中比较三组的差异。结果共收集到1727份可供分析的问卷,其中年轻组453例(26.2%),包括原发性血小板增多症(ET)274例、真性红细胞增多症(PV)80例、骨髓纤维化(MF)99例(原发性MF 45例,PV后MF 20例,ET后MF 34例);男性178例(39.3%),中位年龄31(18~40)岁。与中年、老年MPN受访者相比,年轻MPN受访者中未婚、高学历、无合并症、无合并用药、较低危险度分层占比较高(P2;PV:P=0.063)最高,而血栓事件的发生率在初诊(ET:P2突变比例最低(ET:P2;PV:P=0.043;MF:P=0.004)和高分子风险(HMR)突变(ET:P=0.024;PV:P=0.023;MF:P=0.001)的检出率均最低。结论与中、老年患者相比,年轻MPN患者有着特有的临床表现和基因突变特征。
