大家好。很开心能够回到母校参加小说家讲堂。我2017年到2020年在这里学习,师从张清华老师。跟张莉老师认识了很多年,是一起成长的小伙伴。对我来说,最怯于去的两个地方,一是回到老家,二是回到母校。近乡情怯,近校也情怯。我跟朋友开玩笑说,我要有一种豁出来的勇气才能够坐到这儿跟大家做一个汇报,当然也希望跟更年轻的学弟学妹们做一个交流。今天来和大家聊聊写作。

Background:Pneumoconiosis is the most widely distributed occupational disease worldwide.China is currently the largest coal producer and consumer and the country with the most coal miners and cases of coal workers’pneumoconiosis(CWP).Despite more than 70 years of effort,the problem of CWP and silicosis remains serious.There is a lack of analysis of direct data on coal miners’pneumoconiosis from all over the country.This study aimed to describe the epidemiology of coal miners’pneumoconiosis and reveal some important clues regarding its social determinants.Methods:The annual incidence rate,20-year prevalence rate,and incidence rate of coal miners’pneumoconiosis per million tons in China from 1949 to 2021 were calculated by using the data of annual number of coal miners’pneumoconiosis diagnosed and reported from the coal mining and dressing industry,the number of coal miners,and the raw coal production,and the relationship between the incident cases of coal miners’pneumoconiosis and the death toll from coal mine safety accidents was analyzed using Pearson correlation analysis,with the aim of exploring the relationship between the incident cases of coal miners’pneumoconiosis and its social determinants with an ecological study.Results:From 1949 to 2021,there have been more than 462,000 patients with coal miners’pneumoconio-sis in China,showing double U-shaped distributions with an increasing trend,accounting for about 50.5%(462,000/915,000)of all diagnosed pneumoconiosis in China,while the incidencet rate of coal miners’pneu-moconiosis presents a large W shaped distribution with three peaks over a time span of more than 50 years.From 1949 to 1986,there was a strong correlation between the incident cases of coal miners’pneumoconiosis and raw coal production,the number of coal miners,and the number of deaths from coal mine accidents(r=0.849,P<0.001;r=0.817,P<0.001;r=0.697,P<0.001,respectively),but there was no such correlation found from 1987 to 2006.It was estimated that the annual incidence rate of coal miners’pneumo

Background： Idiopathic pulmonary fibrosis （IPF） is an age-related and progressive interstitial lung disease. Up to 20% of cases of IPF cluster in families, genetic factors contribute significantly to the pathogenesis of the disease. This study aimed to explore the association between rare genetic variants and IPF in Chinese Han families. Methods： A Han family, comprising three IPF patients and five unaffected their first-degree relatives, and 100 ethnically matched control individuals from North China were enrolled in this study. Peripheral blood was collected, and genomic DNA was extracted. To elucidate if rare genetic variants are associated with the familial IPF, we performed whole-exome sequencing of affected members from a Chinese Han IPF family. Candidate rare variants were then confirmed by Sanger sequencing. Results： We identified a potentially damaging rare variant-a heterozygous mutation c.2146G〉A in exon 6 of the gene encoding for telomerase reverse transcriptase （TERT）, which results in an amino acid substitution （p.Ala716Thr）. We confirmed the missense mutation by Sanger sequencing in all the affected family members but did not detect this mutation in 100 ethnically matched healthy controls. Patients carried this mutation were characterized by the frequently acute exacerbation of IPF phenotype, with poor prognosis. The mean time to death was 2.8 years after diagnosis. Conclusion： Using next-generation sequencing technology in familial IPF patients, we identified the heterozygous rare variant in TERT gene, and strengthened the importance of genetic variants in telomere-related pathogenesis in Chinese IPF patients.

Objective： This review aimed to examine the effectiveness of clinical nurse specialist （CNS） interventions in patients with chronic obstructive pulmonary disease （COPD）. COPD significantly affects people＇s health worldwide. With the development in nursing, CNSs are playing increasingly important roles in different departments. However, the studies on the effectiveness of CNSs in COPD are not as well organized as the studies on the effectiveness of CNSs in bronchiectasis and asthma. Therefore, this review aims to find some updated evidence on the CNS interventions for patients with COPD and on whether these interventions are effective.Methods： A narrative analysis of the data was performed for the eligible studies. Four databases were chosen： CINAHL, MEDLINE, British Nursing Index, and Cochrane Library. Other websites such as the National Institute for Health and Clinical Excellence, National Health Service Evidence, Association of Respiratory Nurse Specialists, and National Association of Clinical Nurse Specialist were searched as well. Two reviewers performed study identification independently, and all the retrieved articles were stored using the EndNote X7 software. The risk of bias in the included studies was assessed using the Cochrane Collaboration＇s risk of bias tool. Results： A total of nine studies were included in this review. There were five current interventions by CNSs for patients with COPD. These interventions were home nursing support, CNS]s supported discharge, multidisciplinary cooperation programs, nurse-led care programs, and self-care management education. The effectiveness of these five interventions was evaluated individually. There is low- to moderate-quality evidence indicating that home nursing support interventions may have a positive effect on mortality and quality of life.
