Severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)has been spreading globally since its initial outbreak in 2019.Substantial evidence has revealed that children who are receiving antitumor therapy or hematopoietic stem cell transplantation(HSCT)present higher rates of severe illness and mortality[1].It is important to identify infected children with hematology and oncology diseases who have the tendency to develop worse outcomes of coronal virus disease 2019(COVID-19)during the early stage of COVID-19.Recent studies have demonstrated that vaccination against SARS-CoV-2 in adults can reduce the likelihood of infection and reduce the severity and mortality of COVID-19 when SARS-CoV-2 breakthrough infection occurs[2,3,4].However,the effectiveness of SARS-CoV-2 vaccination in children with hematology and oncology diseases is poorly defined due to the lack of relevant data,which might lead to vaccine hesitancy[5,6].

Background Congenital heart disease(CHD)is one of the main supportive diseases of extracorporeal membrane oxygena-tion in children.The management of extracorporeal membrane oxygenation(ECMO)for pediatric CHD faces more severe challenges due to the complex anatomical structure of the heart,special pathophysiology,perioperative complications and various concomitant malformations.The survival rate of ECMO for CHD was significantly lower than other classifica-tions of diseases according to the Extracorporeal Life Support Organization database.This expert consensus aims to improve the survival rate and reduce the morbidity of this patient population by standardizing the clinical strategy.Methods The editing group of this consensus gathered 11 well-known experts in pediatric cardiac surgery and ECMO field in China to develop clinical recommendations formulated on the basis of existing evidences and expert opinions.Results The primary concern of ECMO management in the perioperative period of CHD are patient selection,cannulation strategy,pump flow/ventilator parameters/vasoactive drug dosage setting,anticoagulation management,residual lesion screening,fluid and wound management and weaning or transition strategy.Prevention and treatment of complications of bleeding,thromboembolism and brain injury are emphatically discussed here.Special conditions of ECMO management related to the cardiovascular anatomy,haemodynamics and the surgical procedures of common complex CHD should be considered.Conclusions The consensus could provide a reference for patient selection,management and risk identification of periop-erative ECMO in children with CHD.

A birth cohort study is an epidemiological study that enrolls a group of people who were born during a specifc period of time and follows up with them from the prenatal period to adulthood and even old age.The establishment of a birth cohort study enables the exploration of the etiology of diseases and enriches the understanding and awareness of health status among the reproductive population.Additionally,it provides evidence-based support for the launch of public health policies and programs and thus practically promotes the health care of women and children,facilitating the early prevention of disease in childhood and adulthood[1].

Early prediction of acute kidney injury in neonates with cardiac surgery

Background Acute kidney injury(AKI)occurs in 42%-64%of the neonatal patients experiencing cardiac surgery,contributing to postoperative morbidity and mortality.Current diagnostic criteria,which are mainly based on serum creatinine and hourly urine output,are not sufficiently sensitive and precise to diagnose neonatal AKI promptly.The purpose of this review is to screen the recent literature,to summarize the novel and cost-effective biomarkers and approaches for neonatal AKI after cardiac surgery(CS-AKI),and to provide a possible research direction for future work.Data sources We searched PubMed for articles published before November 2019 with pertinent terms.Sixty-seven articles were found and screened.After excluding 48 records,19 articles were enrolled for final analysis.Results Nineteen articles were enrolled,and 18 possible urinary biomarkers were identified and evaluated for their ability to diagnose CS-AKI.Urinary neutrophil gelatinase-associated lipocalin(uNGAL),serum cystatin C(sCys),urinary human kidney injury molecule-1(uKIM-1),urinary liver fatty acid-binding protein(uL-FABP)and interleukin-18(uIL-18)were the most frequently described as the early predictors of neonatal CS-AKI.Conclusions Neonates are vulnerable to CS-AKI.UNGAL,sCys,uL-FABP,uKIM-1 and uIL-18 are potential biomarkers for early prediction of neonatal CS-AKI.Renal regional oxygen saturation by near-infrared spectroscopy is a non-invasive approach for early identification of neonatal AKI.Further work should focus on exploring a sensitive and specific combined diagnostic model that includes novel biomarkers and other complementary methods.

Aggregate-induced emission luminogens（AIEgens） have been widely used in biological imaging, chemical sensing, and disease treatments. The rational design and construction of AIEgens have received considerable research interests during the last few years. Herein, molecular docking-aided AIEgen design has been reasonably proposed and AIEgen TBQZY with excellent1O2generation ability has been synthesized. The newly developed TBQZY could efficiently kill S. epidermidis and methicillinresistant S. epidermidis（MRSE） by tightly binding to bacteria and triggering the accumulation of1O2in bacteria. TBQZY specifically regulated the immune system and polarized macrophages from M1 to M2 to accelerate the elimination of biofilm in vivo. In addition, healing acceleration was observed in chronic wounds treated with TBQZY, and side effects were negligible.Meanwhile, TBQZY had extraordinary potential for combating drug-resistant bacteria in the clinical setting. This research not only provided new concepts for the design of AIEgens, but also shed some lights on the discovery of drugs against drug-resistant bacteria.

BACKGROUND Fracture risk assessment in children with benign bone lesions of long bones remains poorly investigated.AIM To investigate the risk factors for pathological fracture in children with benign bone lesions and to propose a modified scoring system for quantitative analysis of the pathologic fracture risk.METHODS We retrospectively reviewed 96 pediatric patients with benign bone lesions.We compared radiographic and clinical features between 40 patients who had fractures through a benign bone lesion and 56 who had no fracture.Information including histological diagnosis,anatomical site,radiographic appearance,severity of pain,and lesion size was recorded for the patients.A modified scoring system was proposed to predict the risk of fracture.RESULTS The univariate comparisons showed a significant difference between the fracture and non-fracture groups in terms of lesion type,pain,lesion-to-bone width,and axial cortical involvement of the patients(P7 indicates a high risk of a pathologic fracture and is an indication for prophylactic internal fixation.

Background A number of clinical trials evaluated the efficacy and adverse effects of oral propranolol in the treatment of infantile hemangioma(IH),but the treatment has not yet been standardized.This meta-analysis aims to reevaluate the efficacy and adverse effects of oral propranolol in comparative studies and to provide a reliable basis for clinical administration in the therapy for IH.Methods Data were obtained from PubMed,Embase,Cochrane Library,Web of Science,China National Knowledge Infra-structure and Wanfang database,from inception to December 1st,2018.The pooled risk ratios(RR)with 95%confidence intervals(95%CI)were calculated and used to evaluate the effect size.The meta-analysis was performed using the random-effects model due to heterogeneity between the studies.The Cochrane Collaboration 6 aspects of bias,methodological index for non-randomized studies and the Newcastle-Ottawa Scale were used to assess the risk for bias.Sensitivity analysis,publication bias and subgroup analysis were performed.Results Eighteen unique studies involving 2701 unique children were included in the analysis.The response rate was reported in 18 trials,which compared oral propranolol with other treatments.The heterogeneity was statistically significant(P<0.00001,P=95%).The difference in the response rate was statistically significant(RR=1.40,95%CI 1.13-1.75)while compared with the controls.However,no significant difference in the adverse events rate(RR=0.78,95%CI 0.45-1.34)and relapse rate(RR=1.45,95%CI 0.66-3.16)were found.Otherwise,the subgroup analysis indicated that the RR was 1.64(95%CI 0.24-11.36)for low-dose propranolol(1 mg/kg/day),1.42(95%CI 1.12-1.80)for medium dose(2 mg/kg/day)and 1.46(95%CI 1.17-1.82)for high dose(3 mg/kg/day),but the high dose had higher adverse events rate than medium dose,with 3.60%and 86.22%,respectively.The effectiveness of propranolol therapy among cases of treatment duration less than 6 months(RR=1.24,95%CI 1.05-1.47)was inferior to that of treatment duration greater than or equal to 6 mon

Background To summarize the experience of management of persistent pulmonary hypertension of the newborn (PPHN) with extracorporeal membrane oxygenation (ECMO) support.Methods We presented three neonates with PPHN supported by ECMO in our center.Medical records and patient management notes were retrospectively reviewed.Results For two neonates with congenital diaphragmatic hernia (CDH),diaphragm repair surgery was done under ECMO support.One patient was weaned from ECMO after 73 hours,and recovered well at the last follow-up after 1 year.The other patient was weaned from ECMO after 167 hours,and he died from septic shock 21 days after decannulation.For the neonate with idiopathic PPHN,ECMO was withdrawn successfully.Conclusions ECMO is an effective rescue means for refractory PPHN.Appropriate intervention timing,accurate coagulation,and volume management are important.
