Early prediction of acute kidney injury in neonates with cardiac surgery

Background Acute kidney injury(AKI)occurs in 42%-64%of the neonatal patients experiencing cardiac surgery,contributing to postoperative morbidity and mortality.Current diagnostic criteria,which are mainly based on serum creatinine and hourly urine output,are not sufficiently sensitive and precise to diagnose neonatal AKI promptly.The purpose of this review is to screen the recent literature,to summarize the novel and cost-effective biomarkers and approaches for neonatal AKI after cardiac surgery(CS-AKI),and to provide a possible research direction for future work.Data sources We searched PubMed for articles published before November 2019 with pertinent terms.Sixty-seven articles were found and screened.After excluding 48 records,19 articles were enrolled for final analysis.Results Nineteen articles were enrolled,and 18 possible urinary biomarkers were identified and evaluated for their ability to diagnose CS-AKI.Urinary neutrophil gelatinase-associated lipocalin(uNGAL),serum cystatin C(sCys),urinary human kidney injury molecule-1(uKIM-1),urinary liver fatty acid-binding protein(uL-FABP)and interleukin-18(uIL-18)were the most frequently described as the early predictors of neonatal CS-AKI.Conclusions Neonates are vulnerable to CS-AKI.UNGAL,sCys,uL-FABP,uKIM-1 and uIL-18 are potential biomarkers for early prediction of neonatal CS-AKI.Renal regional oxygen saturation by near-infrared spectroscopy is a non-invasive approach for early identification of neonatal AKI.Further work should focus on exploring a sensitive and specific combined diagnostic model that includes novel biomarkers and other complementary methods.

We examined the global biogeography of the Scytodes thoracica group of spitting spiders based on 23 years of sampling at the species level(61 species in the thoracica group and 84 species of Scytodes)using DNA data from six loci.Our results indicated that the thoracica group initially dispersed from Southeast Asia to East Africa between 46.5 and 33.0 million years ago,and dispersal events intensified between Southeast/South Asia and East/South Africa from the early to late Miocene.The timing of these events indicates that Asian-African faunal exchange of the thoracica group was driven by the Indian monsoon,and the pattern of dispersal suggests that colonialization took root when the Indian monsoon shifted from a North-South direction to an East-West direction from the middle Eocene.

Cenozoic tectonic evolution in the Tethyan region has greatly changed the landforms and environment of Eurasia,driving the evolution of animals and greatly affecting the diversity patterns of Eurasian animals.By combining the latest Tethyan paleogeographic models and some recently published Eurasian zoological studies,we systematically summarize how tectonic evolution in the Tethyan region has influenced the evolution and diversity patterns of Eurasian animals.The convergence of continental plates,closure of Tethys Sea,and Tethyan sea-level changes have directly affected the composition and spatial distribution of Eurasian animal diversity.The topographic and environmental changes caused by Tethyan tectonics have determined regional animal diversity in Eurasia by influencing animal origin,dispersal,preservation,diversification,and extinction.The ecological transformations resulted in the emergence of new habitats and niches,which promoted animal adaptive evolution,specialization,speciation,and expansion.We highlight that the Cenozoic tectonic evolution of the Tethyan region has been responsible for much of the alteration in Eurasian animal distribution and has been an essential force in shaping organic evolution.Furthermore,we generalize a general pattern that Tethyan geological events are linked with Eurasian animal evolution and diversity dynamics.

Background We investigated the sex-specific differences in ossification patterns of the first two cervical vertebrae in Chinese children.Methods A retrospective computed tomography(CT)study was performed between June 2016 and December 2020.Patients younger than 16 years with cervical CT images acquired<1.5 mm slice thickness were included.All eligible patients were stratified into 2 sex groups and 16 age groups based on 1-year intervals.The ossification status of each synchondrosis and ossification variants were evaluated.Results A total of 910 subjects(518 males and 392 females)were included in the study.For the Cl vertebra,the neurocentral synchondroses closed at a median age of 8 years in males and 6.3 years in females,and the posterior synchondrosis fused at 5.4 years in males and at 4.4 years in females.Multifocal anterior arch ossification centers were present in 74 of 411(18%)subjects,whereas posterior arch variants were observed in 18 of 258(7%)subjects.For the C2 vertebra,the sequence of complete fusion was as follows:posterior synchondrosis,neurocentral synchondroses,and dentoneural synchondrosis.Uniquely,a fusion line was observed in the dentocentral synchondrosis through adolescence.Anterior arch variants of the C2 vertebra occurred in 17 of 248(6.9%)subjects.There was no significant difference between the sexes in ossification variants.Conclusions All synchondroses of the first two cervical vertebrae fuse slightly earlier in females.The sequence of fusion follows a posterior-to-anterior and caudal-to-cephalad pattern in both sexes.Congenital variants are not rare and should not be confused with trauma.

Objectives The aims of the present study were to explore the causes of minimally invasive surgical ventricular septal defect(VSD)closure failure under transesophageal echocardiography guidance and thus to improve the success rate of surgical VSD closure.Methods From January 2015 to December 2019,522 children with VSD underwent minimally invasive surgical closure.Nineteen procedures(3.64%)were unsuccessful.The failure causes,VSD locations and surgical incision approaches were retrospectively analyzed.Results Among the 19 patients(3.64%)with unsuccessful outcomes,18 were switched to cardiopulmonary bypass(CPB)surgery,and 1 was closed successfully using an occlusion device a year later.The causes of failure included occlusion device shedding or shifting(n=6),failure of the guidewire(or the sheath)to pass through a small defect(n=5),device-related valve regurgitation(n=4),significant residual shunt(n=2),ventricular fibrillation(n=1),and continuous sharp blood pressure decreases(n=1).Patients with high VSD had a slightly higher failure rate than those with perimembranous VSD(p=0.049),and its key reason is the high proportion of occlusion device shedding or shifting(p=0.005).No significant difference in the failure rate was found between patients with different surgical incision approaches.Conclusions Minimally invasive surgery has a high success rate for perimembranous VSDs.Occlusion device shedding or shifting is the most common cause of failure.The shedding or shifting risk of eccentric occlusion devices being used only for high VSDs is much greater than that of concentric occlusion devices being used for perimembranous VSDs,which increases the risk of conversion to CPB surgery for high VSDs.

Although widely thought to be aggressive, solitary,and potentially cannibalistic, some spider species have evolved group-living behaviors. The distinct transition provides the framework to uncover groupliving evolution. Here, we conducted a comparative transcriptomic study and examined patterns of molecular evolution in two independently evolved group-living spiders and twelve solitary species. We report that positively selected genes among groupliving spider lineages are significantly enriched in nutrient metabolism and autophagy pathways. We also show that nutrient-related genes of group-living spiders convergently experience amino acid substitutions and accelerated relative evolutionary rates. These results indicate adaptive convergence of nutrient metabolism that may ensure energy supply in group-living spiders. The decelerated evolutionary rate of autophagy-related genes in group-living lineages is consistent with an increased constraint on energy homeostasis as would be required in a group-living environment. Together, the results show that energy metabolic pathways play an important role in the transition to group-living in spiders.

In this paper we present a necessary and sufficient condition to guarantee that the zeroextended function of the solution for the heat equation in a smaller cylinder is still the solution of the corresponding extension problem in a larger cylinder.We prove the results under the frameworks of classical solutions,strong solutions and weak solutions.Moreover,we generalize these results to uniformly parabolic equations of divergence form.

Study of risk factors for intraoperative hypothermia during pediatric burn surgery

Background Intraoperative hypothermia(core temperature<36.0°C)is common during the perioperative period and can result in adverse consequences,especially in children.We aimed to determine the incidence of intraoperative hypothermia and its risk factors in pediatric patients during burn surgery.Methods In the present study we enrolled 197 pediatric patients with burn injury undergoing surgical debridement and skin grafting.Factors,such as total burn surface area(TBSA),were collected and analyzed to identify the potential risk factors for intraoperative hypothermia.Results The incidence of intraoperative hypothermia among all patients was 17.8%.Compared with patients with normothermia,children with hypothermia were associated with larger TBSA(25%vs 15%,p<0.001)and with less intraoperative active warming(34.28%vs 54.93%,p<0.05).In addition,compared with patients with moderate-degree burn,patients with severe and extremely severe burn were associated with much higher risk of intraoperative hypothermia[severe:odds ratio(OR)=3.805,95%confidence interval(CI)=1.396-10.368,p=0.009;extremely severe:OR=6.933,95%CI=2.604-18.462,p<0.001].TBSA was the only independent risk factor that emerged as being strongly associated with intraoperative hypothermia(OR=1.068,p=0.001)and could be used to predict the occurrence of hypothermia when combined with other factors.TBSA for predicting intraoperative hypothermia by receiver operating characteristic(ROC)curve analysis showed a good predictive ability with an area under the ROC curve of 0.758.Conclusion TBSA is an important risk factor for intraoperative hypothermia in pediatric patients with burn.

Online monitoring of the curing temperature field is essential to improving the quality and efficiency of the manufacturing process of composite parts.Traditional embedded sensor-based technologies have difficulty monitoring the full temperature field or have to introduce heterogeneous items that could have an undesired impact on the part.In this paper,a non-contact,full-field monitoring method based on deep learning that predicts the internal temperature field of composite parts in real time using surface temperature measurements of auxiliary materials is proposed.Using the proposed method,an average temperature monitoring accuracy of 97%is achieved in various heating patterns.In addition,this method also demonstrates satisfying feasibility when a stronger thermal barrier covers the part.This method was experimentally validated during the self-resistance electric heating process,in which the monitoring accuracy reached 93.1%.This method can potentially be applied to automated manufacturing and process control in the composites industry.
