目的提高对感染性心内膜炎并发症的临床认识和鉴别诊断能力。方法回顾分析2022年8月15日本院收治的1例感染性心内膜炎患者的临床、实验室、影像学资料及治疗转归,结合国内外相关文献进行复习。结果感染性心内膜炎可表现出类似于原发性系统性血管炎的临床表现及实验室指标特征,从而增加疾病诊断的挑战性。动态复查心脏彩超有助于感染性心内膜炎早期诊断,糖皮质激素及免疫抑制剂对感染性心内膜炎临床症状改善作用有限,对改善实验室指标有一定作用,抗生素治疗及心脏瓣膜手术是关键性治疗手段。结论以多系统受累为临床表现的疑似系统性血管炎患者,需要警惕感染性心内膜炎的可能,糖皮质激素及免疫抑制剂在感染性心内膜炎模拟系统性血管炎治疗中患者有一定获益,早期诊断、规范治疗是改善感染性心内膜炎预后的关键因素。

非酒精性脂肪性肝病是目前的多发病,为多因素所致,严重影响公共健康,目前对于该病的证治报道较多。该病的临床表现多数不典型,病机证候复杂,笔者汲取李东垣理论思想中升发脾之清气,以及黄元御理论思想中泻湿疏木的治法,把二者进行优化组合,拟定了升阳化湿,培土疏木为治疗大法运用于临床,立足于“湿”,分为“湿浊内蕴”和“脾虚湿困”2个证型,驭繁就简,取得了理想的临床疗效。

针对现有装配整体式楼盖的模板支架计算通常是等同于现浇结构,未计入预制底板的刚度和承载能力等有利因素,而造成模板支架拆除时间较晚,经济效益和工期效益不良的问题,结合工程实际进行了模板支架最佳拆除时间的研究。现场测试结果及理论计算表明,在一定跨度条件下,考虑预制底板的刚度和承载能力贡献,现场可以实现板底支架的提前拆除,相关研究结论可为类似工程提供参考。

目的探讨黄连素对糖尿病(diabetes mellitus,DM)大鼠血脂代谢及胰岛素抵抗(insulin resistance,IR)的作用。方法取10只正常大鼠作为正常对照组(Con)。将DM建模成功大鼠随机分为模型组(M)、阳性对照组(Pon)、黄连素低剂量组(L-Ber)和黄连素高剂量组(H-Ber),分别为9、9、9、8只,将剩余8只非DM模型大鼠纳入非DM组(N-DM)。经相应干预后,测定各组大鼠体质量、饮水量、摄食量、血糖、血脂指标和IR;HE染色观察胰腺组织形态学变化;蛋白印迹法(Western blotting)检测磷酸化胰岛素受体底物1(phosphorylated insulin receptor substrate 1,p-IRS-1)、磷酸化磷脂酰肌醇3激酶(phosphorylated phosphatidylinositol 3-kinase,p-PI3K)和葡萄糖转运蛋白-4(glucose transporter-4,GLUT-4)蛋白的表达水平。结果与Con组比较,M组的饮水量、摄食量、总胆固醇(total cholesterol,TC)、三酰甘油(triglyceride,TG)、低密度脂蛋白胆固醇(low density lipoprotein cholesterol,LDL-C)、空腹胰岛素(fasting insulin,FIns)、胰岛素抵抗指数(homeostasis model assessment of insulin resistance,HOMA-IR)、糖耐量和空腹血糖(fasting blood glucose,FBG)均升高,体质量、高密度脂蛋白胆固醇(high density lipoprotein cholesterol,HDL-C)、p-IRS-1、p-PI3K和GLUT-4蛋白相对表达量均降低(P<0.05)。与M组比较,L-Ber组、H-Ber组和Pon组的饮水量、摄食量、TC、TG、LDL-C、FIns、HOMA-IR、糖耐量和FBG均降低,胰腺组织形态学异常减轻,体质量、HDL-C、p-IRS-1、p-PI3K和GLUT-4蛋白相对表达量均升高(P<0.05)。结论黄连素可改善DM大鼠血脂代谢及IR,作用机制可能与调节IRS-1/PI3K/GLUT4通路有关。

There is no effective treatment for hemiplegia after hypertensive intracerebral *** that the branches of L4 nerve roots in the lumbar plexus root control the movement of the lower extremity anterior and posterior muscles,we investigated a potential method of nerve repair using the L4 nerve *** models of hindlimb hemiplegia after a hypertensive intracerebral hemorrhage were established by injecting autogenous blood into the posterior limb of internal *** L4 nerve root on the healthy side of model rats was transferred and then anastomosed with the L4 nerve root on the affected side to drive the extensor and flexor muscles of the *** investigated whether this method can restore the flexible movement of the hindlimbs of paralyzed rats after hypertensive intracerebral *** a beam-walking test and ladder rung walking task,model rats exhibited an initial high number of slips,but improved in accuracy on the paretic side over *** 17 weeks after surgery,rats gained approximately 58.2%accuracy from baseline performance and performed ankle motions on the paretic *** 9 weeks after surgery,a retrograde tracing test showed a large number of fluoro-gold-labeled motoneurons in the left anterior horn of the spinal cord that supports the L4-to-L4 nerve *** addition,histological and ultramicrostructural findings showed axon regeneration of motoneurons in the anterior horn of the spinal *** and paw print analysis showed that denervated hindlimb muscles regained reliable innervation and walking coordination *** findings suggest that the L4-to-L4 nerve root transfer method for the treatment of hindlimb hemiplegia after hypertensive intracerebral hemorrhage can improve the locomotion of hindlimb major joints,particularly of the distal *** from study support that the L4-to-L4 nerve root transfer method can effectively repair the hindlimb hemiplegia after hypertensive intracerebral hemorrh

BACKGROUND Sarcomatoid carcinoma of the pancreas(SCP) is a rare and aggressive epithelial tumor that has both epithelial and mesenchymal features. It is characterized by sarcomatous elements with evidence of epithelial differentiation. And the term"sarcomatoid carcinoma" is often confused with "carcinosarcoma".CASE SUMMARY We present a case of SCP with lymph node metastasis in a 59-year-old male patient. He had experienced darkening of the urine, scleral icterus, and fatigue for 4 weeks. Computed tomography and magnetic resonance imaging revealed a mass in the pancreatic head, and laboratory tests revealed elevated serum bilirubin levels. The patient underwent pancreaticoduodenectomy after biliary decompression. Histologically, spindle cells with marked nuclear atypia and brisk mitotic activity arranged in a storiform or fascicular pattern were present in the bulk of the tumor. Immunohistochemical analysis found that the spindle cells exhibited strong diffuse positivity for epithelial markers, indicative of epithelial differentiation.
