Efficacy and safety of perioperative therapy for locally resectable gastric cancer:A network meta-analysis of randomized clinical trials

BACKGROUND Gastric cancer(GC)is the fifth most commonly diagnosed malignancy worldwide,with over 1 million new cases per year,and the third leading cause of cancer-related death.AIM To determine the optimal perioperative treatment regimen for patients with locally resectable GC.METHODS A comprehensive literature search was conducted,focusing on phase II/III randomized controlled trials(RCTs)assessing perioperative chemotherapy and chemoradiotherapy in treating locally resectable GC.The R0 resection rate,overall survival(OS),disease-free survival(DFS),and incidence of grade 3 or higher nonsurgical severe adverse events(SAEs)associated with various perioperative regimens were analyzed.A Bayesian network meta-analysis was performed to compare treatment regimens and rank their efficacy.RESULTS Thirty RCTs involving 8346 patients were included in this study.Neoadjuvant XELOX plus neoadjuvant radiotherapy and neoadjuvant CF were found to significantly improve the R0 resection rate compared with surgery alone,and the former had the highest probability of being the most effective option in this context.Neoadjuvant plus adjuvant FLOT was associated with the highest probability of being the best regimen for improving OS.Owing to limited data,no definitive ranking could be determined for DFS.Considering nonsurgical SAEs,FLO has emerged as the safest treatment regimen.CONCLUSION This study provides valuable insights for clinicians when selecting perioperative treatment regimens for patients with locally resectable GC.Further studies are required to validate these findings.

Cell behaviors and functions show distinct contrast in different mechanical microenvironment.Numerous materials with varied rigidity have been developed to mimic the interactions between cells and their surroundings.However,the conventional static materials cannot fully capture the dynamic alterations at the bio-interface,especially for the molecular motion and the local mechanical changes in nanoscale.As an alternative,flexible materials have great potential to sense and adapt to mechanical changes in such complex microenvironment.The flexible materials could promote the cellular mechanosensing by dynamically adjusting their local mechanics,topography and ligand presentation to adapt to intracellular force generation.This process enables the cells to exhibit comparable or even higher level of mechanotransduction and the downstream‘hard’phenotypes compared to the conventional stiff or rigid ones.Here,we highlight the relevant studies regarding the development of such adaptive materials to mediate cell behaviors across the rigidity limitation on soft substrates.The concept of‘soft overcomes the hard’will guide the future development and application of biological materials.

RARRES2 regulates lipid metabolic reprogramming to mediate the development of brain metastasis in triple negative breast cancer

Background Triple negative breast cancer(TNBC),the most aggressive subtype of breast cancer,is characterized by a high incidence of brain metastasis(BrM)and a poor prognosis.As the most lethal form of breast cancer,BrM remains a major clinical challenge due to its rising incidence and lack of effective treatment strategies.Recent evidence suggested a potential role of lipid metabolic reprogramming in breast cancer brain metastasis(BCBrM),but the underlying mechanisms are far from being fully elucidated.Methods Through analysis of BCBrM transcriptome data from mice and patients,and immunohistochemical validation on patient tissues,we identified and verified the specific down-regulation of retinoic acid receptor responder 2(RARRES2),a multifunctional adipokine and chemokine,in BrM of TNBC.We investigated the effect of aberrant RARRES2 expression of BrM in both in vitro and in vivo studies.Key signaling pathway components were evaluated using multi-omics approaches.Lipidomics were performed to elucidate the regulation of lipid metabolic reprogramming of RARRES2.Results We found that downregulation of RARRES2 is specifically associated with BCBrM,and that RARRES2 deficiency promoted BCBrM through lipid metabolic reprogramming.Mechanistically,reduced expression of RARRES2 in brain metastatic potential TNBC cells resulted in increased levels of glycerophospholipid and decreased levels of triacylglycerols by regulating phosphatase and tensin homologue(PTEN)-mammalian target of rapamycin(mTOR)-sterol regulatory element-binding protein 1(SREBP1)signaling pathway to facilitate the survival of breast cancer cells in the unique brain microenvironment.Conclusions Our work uncovers an essential role of RARRES2 in linking lipid metabolic reprogramming and the development of BrM.RARRES2-dependent metabolic functions may serve as potential biomarkers or therapeutic targets for BCBrM.

BACKGROUND Gastric cancer(GC)is one of the most aggressive malignancies with limited therapeutic options and a poor prognosis.Resveratrol,a non-flavonoid poly-phenolic compound found in a variety of Chinese medicinal materials,has shown excellent anti-GC effect.However,its exact mechanisms of action in GC have not been clarified.AIM To identify the effects of resveratrol on GC progression and explore the related molecular mechanisms.METHODS Action targets of resveratrol and GC-related targets were screened from public databases.The overlapping targets between the two were confirmed using a Venn diagram,and a“Resveratrol-Target-GC”network was constructed using Cyto-scape software version 3.9.1.The protein-protein interaction(PPI)network was constructed using STRING database and core targets were identified by PPI network analysis.The Database for Annotation,Visualization and Integrated A total of 378 resveratrol action targets and 2154 GC disease targets were obtained from public databases,and 181 intersection targets between the two were screened by Venn diagram.The top 20 core targets were identified by PPI network analysis of the overlapping targets.GO function analysis mainly involved protein binding,identical protein binding,cytoplasm,nucleus,negative regulation of apoptotic process and response to xenobiotic stimulus.KEGG enrichment analysis suggested that the involved signaling pathways mainly included PI3K-AKT signaling pathway,MAPK signaling pathway,IL-17 signaling pathway,TNF signaling pathway,ErbB signaling pathway,etc.FBJ murine osteosarcoma viral oncogene homolog(FOS)and matrix metallopeptidase 9(MMP9)were selected by differential expression analysis,and they were closely associated with immune infiltration.Molecular docking results showed that resveratrol docked well with these two targets.Resveratrol treatment arrested the cell cycle at the S phase,induced apoptosis,and weakened viability,migration and invasion in a dose-dependent manner.Furthermore,resveratrol could exhibit anti-GC effect by regula

BACKGROUND Rectus abdominis separation(DRA)affects pelvic stability and body image.No studies have explored the effects of manual massage on early postpartum DRA and postpartum depression.AIM To analyze the curative effect of massage on early postpartum DRA and its impact on postpartum depression and thus its ability promote the overall psychosomatic rehabilitation of postpartum women.METHODS Data were retrospectively collected on 70 primiparous women with postpartum DRA who underwent rehabilitation at the Postpartum Rehabilitation Center of Huzhou Maternal and Child Health Hospital from October 2022 to September 2023.The patients were divided into the Group S(35 cases,biomimetic electrical stimulation treatment)and Group L(35 cases,biomimetic electrical stimulation combined with manual massage treatment).Baseline data,the edinburgh postpartum depression scale(EPDS)score,and the visual analog scale(VAS)scores for rectus abdominis distance,waist circumference,and lower back pain before and after treatment were compared.RESULTS No significant differences were found in the baseline data,rectus abdominis distance,waist circumference,and VAS and EPDS scores between the two groups before treatment(P>0.05).After treatment,the distance between rectus abdominis and waist circumference in Group L were significantly smaller than those in Group S(P<0.05).Furthermore,lower back pain(VAS score)and the EPDS score in Group L were significantly lower than those in Group S(P<0.05).CONCLUSION Manual massage can significantly reduce early postpartum DRA,waist circumference,and back pain and improve the patient’s mental state and postpartum depression.

Rice spikelet rot disease not only threatens rice yields but also poses risks to humans and animals due to the production of the category 2B carcinogen fumonisins by the pathogen Fusarium proliferatum.Nitrogen(N)metabolism is known to have a significant influence on fungal growth and the synthesis of secondary metabolites.AreA is a global N regulatory gene belonging to the GATA transcription factor family.In this study,we observed that theΔAreA mutant exhibited a notable reduction in growth rate and conidium production.Pathogenicity experiments revealed thatΔAreA had almost lost its ability to infect rice spikelets.

Painful stimuli elicit first-line reflexive defensive reactions and,in many cases,also evoke second-line recuperative behaviors,the latter of which reflects the sensing of tissue damage and the alleviation of suffering.The lateral parabrachial nucleus(lPBN),composed of external-(elPBN),dorsal-(dlPBN),and central/superior-subnuclei(jointly referred to as slPBN),receives sensory inputs from spinal projection neurons and plays important roles in processing affective information from external threats and body integrity disruption.However,the organizational rules of lPBN neurons that provoke diverse behaviors in response to different painful stimuli from cutaneous and deep tissues remain unclear.In this study,we used region-specific neuronal depletion or silencing approaches combined with a battery of behavioral assays to show that slPBN neurons expressing substance P receptor(NK1R)(lPBNNK1R)are crucial for driving pain-associated self-care behaviors evoked by sustained noxious thermal and mechanical stimuli applied to skin or bone/muscle,while elPBN neurons are dispensable for driving such reactions.Notably,lPBNNK1R neurons are specifically required for forming sustained somatic pain-induced negative teaching signals and aversive memory but are not necessary for fear-learning or escape behaviors elicited by external threats.Lastly,both lPBNNK1R and elPBN neurons contribute to chemical irritant-induced nocifensive reactions.Our results reveal the functional organization of parabrachial substrates that drive distinct behavioral outcomes in response to sustained pain versus external danger under physiological conditions.

Mechanical performance of 22SiMn2TiB steel welded with lowtransformation- temperature filler wire and stainless steel filler wire

TX-80 low-transformation-temperature(LTT)welding wire was used to replace the traditional ER 307Si welding wire to realize the connection of 22SiMn2TiB armor steel in manual overlay welding.The previously existing issues,such as welding cracks,large welding deformation,and severe welding residual stress,were solved to ensure good strength and ductility requirements.In particular,with the same welding conditions,TX-80 LTT wire eliminates welding cracks.It reduces the welding deformation no matter the base pretreatment of pre-setting angle or no pre-setting angle.By comparison,it was found that the microstructure at the TX-80 weld is mainly composed of martensite and a small amount of retained austenite.In contrast,the microstructure of the ER 307Si weld consists of a large amount of austenite and a small amount of skeleton-like ferrite.The variation trend of residual stress and microhardness from the weld to the base were investigated and compared with the mechanical properties of base materials.The TX-80 and the ER 307Si tensile samples elongation is 6.76%and 6.01%,while the ultimate tensile strengths are 877 and 667 MPa,respectively.The average impact toughness at room temperature of the ER 307Si weld is 143.9 J/cm^(2),much higher than that of the TX-80 weld,which is only 36.7 J/cm^(2).The relationship between impact and tensile properties with microstructure species and distribution was established.In addition,the fracture surface of the tensile and the impact samples was observed and analyzed.Deeper dimples,fewer pores,larger radiation zone,and shear lips of TX-80 samples indicate better tensile ductility and worse impact toughness than those of ER 307Si weld.

CFD simulation of pumping ventilation in a three-story isolated building with internal partitioning:Effects of partition widths,heights and locations

Pumping ventilation(PV),a special single-sided ventilation(SSV),has been certified as an effective strategy to improve the air exchange rate of SSV.However,most studies targeted on the single space,and few studies have been focused on the effect of internal partitioning on PV.This paper aims to evaluate the ventilation performance of PV influenced by different configurations of internal partitioning.Computational fluid dynamics(CFD)simulation was used to predict the flow fields and ventilation rates.The width(w/H),height(h/H)and location(d/H)are the three main internal partition parameters considered in this study.The simulation results showed that the total,mean and fluctuating ventilation rates all decrease with wider internal partitions.The normalized total ventilation rate decreases by 7.6%when w/H is increased from 50%to 75%.However,the reduction rate is only 0.23%between w/H=0 and 25%,and only 0.61%between w/H=25%and 50%.The ventilation rate is hardly reduced by increasing the partition width when w/H50%.Increasing the partition height will reduce the mean ventilation rate but promote the fluctuating and total ventilation rate in some cases.An increase of total ventilation rate by 1.4%is observed from h/H=50%to 75%.The ventilation rate is larger when the internal partition is attached to the leeward or windward wall.The total,mean and fluctuating ventilation rates for d/H=50%are relatively higher than d/H=0 by 1.5%,3.1%and 0.8%,respectively.Hence the internal partition should be mounted attached to the windward wall so as to obtain the greatest pumping ventilation rate.The periodicity of pumping flow oscillation and pumping frequency are independent of the partition configurations.The peak power of pumping flow is the lowest for the widest internal partition and is negatively affected by the partition height,but it generally has a positive correlation with the distance between the partition and leeward wall.Present research will help to understand pump

Laser Oscillating of Nickel Alloyed Welds on Press‑Hardened Steel

Laser oscillating welding was employed to fabricate Al-Si coated press-hardened steel(PHS)to improve the element homogeneity in the fusion zone.Laser oscillating welding was employed with various oscillation amplitudes(0 mm,0.5 mm and 1.3 mm)in this present.Ni foil of 0.06 mm thickness was used as an interlayer between two tailored PHS welded.The weld morphology,elemental profile,microstructure and tensile strength of welded joints were studied.The results showed that full penetration weld without any weld defects were achieved for any oscillation amplitudes,and weld width increased with increasing oscillation amplitudes.With the oscillation amplitudes increased,Ni and Al had an uneven elemental profile due to strong stirring force,but the Ni and Al content in the weld was decreased and Ni had a sharp descent compared to Al element.Only fewδ-ferrite was presented in fusion line with the oscillation amplitudes increased to 1.3 mm.The oscillation amplitudes did not have an effect on the tensile properties,which was similar to that of base metal.But if keeping increasing the oscillation amplitudes or reducing the thickness of Ni interlayer,it has a potential risk to form more and moreδferrite such that deteriorate the mechnical properties of welded joints.
