An analytical approach was presented for estimating the factor of safety(FS) for slope failure, with consideration of the impact of a confined aquifer. An upward-moving wetting front from the confined water was assumed and the pore water pressure distribution was then estimated and used to obtain the analytical expression of FS. Then, the validation of the theoretical analysis was applied based on an actual case in Hong Kong. It is shown that the presence of a confined aquifer leads to a lower FS value, and the impact rate of hydrostatic pressure on FS increases as the confined water pressure increases, approaching to a maximum value determined by the ratio of water density to saturated soil density. It is also presented that the contribution of hydrostatic pressure and hydrodynamic pressure to the slope stability vary with the confined aquifer pressure.

Mammalian target of rapamycin(m TOR) inhibitor as an attractive drug target with promising antitumor effects has been widely investigated. High quality clinical trial has been conducted in liver transplant(LT) recipients in Western countries. However, the pertinent studies in Eastern world are paucity. Therefore, we designed a clinical trial to test whether sirolimus can improve recurrence-free survival(RFS) in hepatocellular carcinoma(HCC) patients beyond the Milan criteria after LT. This is an open-labeled, single-arm, prospective, multicenter, and real-world study aiming to evaluate the clinical outcomes of early switch to sirolimus-based regimens in HCC patients after LT. Patients with a histologically proven HCC and beyond the Milan criteria will be enrolled. The initial immunosuppressant regimens are center-specifc for the frst 4-6 weeks. The following regimens integrated sirolimus into the regimens as a combination therapy with reduced calcineurin inhibitors based on the condition of patients and centers. The study is planned for 4 years in total with a 2-year enrollment period and a 2-year follow-up. We predict that sirolimus conversion regimen will provide survival benefts for patients particular in the key indicator RFS as well as better quality of life. If the trial is conducted successfully, we will have a continued monitoring over a longer follow-up time to estimate indicator of overall survival. We hope that the outcome will provide better evidence for clinical decision-making and revising treatment guidelines based on Chinese population data.

The Large sky Area Multi-Object Fiber Spectroscopic Telescope（LAMOST） general survey is a spectroscopic survey that will eventually cover approximately half of the celestial sphere and collect 10 million spectra of stars, galaxies and QSOs. Objects in both the pilot survey and the first year regular survey are included in the LAMOST DR1. The pilot survey started in October 2011 and ended in June 2012, and the data have been released to the public as the LAMOST Pilot Data Release in August 2012. The regular survey started in September 2012, and completed its first year of operation in June 2013. The LAMOST DR1 includes a total of 1202 plates containing 2 955 336 spectra, of which 1 790 879 spectra have observed signalto-noise ratio（SNR） ≥ 10. All data with SNR ≥ 2 are formally released as LAMOST DR1 under the LAMOST data policy. This data release contains a total of 2 204 696 spectra, of which 1 944 329 are stellar spectra, 12 082 are galaxy spectra and 5017 are quasars. The DR1 not only includes spectra, but also three stellar catalogs with measured parameters： late A,FGK-type stars with high quality spectra（1 061 918 entries）, A-type stars（100 073 entries）, and M-type stars（121 522 entries）. This paper introduces the survey design, the observational and instrumental limitations, data reduction and analysis, and some caveats. A description of the FITS structure of spectral files and parameter catalogs is also provided.

目的分析2015-2019年河南省输入性疟疾病例诊断情况,为开展消除疟疾后监测工作提供依据。方法通过中国疾病预防控制中心传染病监测信息系统和寄生虫病防治信息管理系统,收集2015-2019年河南省疟疾病例信息,对其诊断方式、诊断机构、诊断时间进行统计分析。结果 2015-2019年河南省共报告输入性疟疾病例952例,所有病例均为实验室确诊病例。疟疾快速诊断试纸条(RDT)阳性检出率为98.61%(779/790),高于镜检检出率(94.22%,897/952),差异有统计学意义(χ~2=22.773,P2015年的65.22%(120/184)升至2019年的81.50%(185/227)。238例在疾病预防控制(疾控)机构诊断的输入性疟疾病例中,71.01%(169/238)在县级疾控机构诊断;704例在医疗机构诊断的输入性疟疾病例中,仅8.38%(59/704)在县级医疗机构诊断。输入性疟疾病例从发病到确诊中位时间为3 d,其中从发病到初诊中位时间为1 d,各年从初诊到确诊时间差异有统计学意义(χ~2=24.956,P 2016年的4 d缩短为2019年的2d。输入性重症恶性疟病例从初诊到确诊中位时间(2d)长于非重症病例(1 d),差异有统计学意义(Z=7.557,P <0.05)。结论医疗机构在河南省输入性疟疾病例发现和监测中的作用越来越重要,但县级医疗机构诊断水平仍较低;应提高其诊断意识和能力,从而在消除疟疾后监测工作中发挥哨点医院作用。

目的探讨小剂量依托红霉素对哮喘患儿痰液中性粒细胞(PMN)及血清白介素-8(IL-8)、白介素-17(IL-17)水平的影响。方法 2007年1月至2009年1月,24例非嗜酸粒细胞哮喘儿童随机分为依托红霉素组及非依托红霉素组,各12例。两组患儿均接受正规阶梯治疗,包括规律性吸入糖皮质激素和间断应用β2受体激动剂。依托红霉素组在此基础上加用依托红霉素3～5 mg/(kg.d)口服,4周。两组患儿在急性期和恢复期均进行诱导痰细胞分类检查,ELISA方法检测血清IL-8和IL-17浓度,并进行肺功能检查。结果两组患儿缓解期诱导痰中的PMN明显低于急性发作期,差异有统计学意义(P均1)。依托红霉素组患儿缓解期的PMN明显低于非依托红霉素组,差异有统计学意义(P=0.001);两组患儿缓解期血清IL-8、IL-17浓度明显低于急性发作期,差异有统计学意义(P均1),依托红霉素组缓解期患儿的血清IL-8、IL-17浓度低于非依托红霉素组缓解期,差异有统计学意义(P1)。两组患儿缓解期的肺功能较急性发作期明显改善,差异有统计学意义(P1);依托红霉素组的改善幅度(△PEFR%)优于对照组,差异有统计学意义(Z=3.349;P=0.001)。结论小剂量依托红霉素可明显改善以PMN浸润为主的气道炎症性哮喘,可能是治疗儿童哮喘的一种新方法。[临床儿科杂志,2012,30(5):431-434]
