Minimally invasive thalamic hematoma drainage can improve the six-month outcome of thalamic hemorrhage

Objective To explore predictors of the 6-month clinical outcome ofthalamic hemorrhage, and evaluate if minimally invasive thalamic hematoma drainage （THD） could improve its prognosis. Methods A total of 54 patients with spontaneous thalamic hemorrhage were evaluated retrospectively. Clinical data, including demographics, stroke risk factors, neuroimaging variables, Glasgow Coma Score （GCS） on admission, surgical strategy, and outcome, were collected. Clinical outcome was assessed using a modified Rankin Scale, six months after onset. Univariate analysis and multivariate logistic regression analysis were performed to determine predictors of a poor outcome. Results Conservative treatnaent was performed for five patients （9.3%）, external ventricular drainage （EVD） for 20 patients （37.0%）, THD for four patients （7.4%）, and EVD combined with THD for 25 patients （46.3%）. At six months after onset, 21 （38.9%） patients achieved a favorable outcome, while 33 （61.1%） had a poor outcome. In the univariate analysis, predictors of poor 6-month outcome were lower GCS on admis- sion （P = 0.001）, larger hematoma volume （P 〈 0.001）, midline shift （P = 0.035）, acute hydrocephalus （P = 0.039）, and no THD （P = 0.037）. The independent predictors of poor outcome, according to the multivariate logistic regression analysis, were no THD and larger hematoma volume. Conclusions Minimally invasive THD, which removes most of the hematoma within a few days, with limited damage to perihematomal brain tissue, improved the 6-month outcome of thalamic hemorrhage. Thus, THD can be widely applied to treat patients with thalamic hemorrhage.

EFFECT OF LOSARTAN ON SLOWING PROGRESSION OF CHRONIC ALLOGRAFT NEPHROPATHY

Objective To investigate the effects of losartan, a specific angiotensin Ⅱ receptor blocker, on slowing progression of renal insufficiency in patients with biopsy-proven chronic allograft nephropathy （CAN） and the molecular mechanism of the therapy. Methods Twenty-two renal transplant recipients with biopsy-proven CAN （group A） were treated with losartan within two months after renal dysfunction for at least one year. Losartan was administered at a dose of 50 mg/d. Twenty-four recipients in the same fashion （group B） who never received angiotensin Ⅱ receptor antagonist were studied as control. The investigation time for each patient lasted one year. Renal functions and concentrations of plasma and urine transforming growth factor-betal （TGF-betal） were compared between the two groups at the initiation and end of the study. In group A, expressions of TGF-betal mRNA and immunofluorescence intensity of TGF-betal protein and pathological alterations in renal biopsy specimens were compared between before losartan therapy and after one year of the therapy. Results At the initiation of the investigation, no significant differences were found between group A and group B in clinical data such as donor age, cold-ischemia time, HLA mismatch, levels of creatinine clearance （Ccr）, plasma and urine TGF-betal concentrations. One year later, 14 of 22 （63.6%） patients showed stable or improved graft functions in group A, and 4 of 24 （16.7%） in group B. The difference was significant （P 〈 0.05）. At the end of the study, urine TGF-betal concentration was 273.8 ± 84.1 pg/*** in group A and 457.2 ± 78.9 pg/*** in group B. During one year study period, loss of Ccr was 6.6 ± 5.4 mL/min in group A and 16.2 ± 9.1 mL/min in group B. Both of the differences were significant between the two groups （P 〈 0.01）. No significant differences were found in plasma TGF-betal concentrations between the four values determined at the initiation and end of the study in the two groups （F= 2.56, P〉

Cosmological Constraints on Neutrino Masses in Light of JWST Red and Massive Candidate Galaxies

The overabundance of the red and massive candidate galaxies observed by the James Webb Space Telescope(JWST)implies efficient structure formation or large star formation efficiency at high redshift z~*** the scenario of a low or moderate star formation efficiency,because massive neutrinos tend to suppress the growth of structure of the universe,the JWST observation tightens the upper bound of the neutrino *** A cold dark matter cosmology and a star formation efficiency∈[0.05,0.3](flat prior),we perform joint analyses of Planck+JWST and Planck+BAO+JWST,and obtain improved constraints∑m_(ν)<0.196 eV and ∑m_(ν)+<0.111 eV at 95% confidence level,*** on the above assumptions,the inverted mass ordering,which implies ∑m_(ν)≥0.1 eV,is excluded by Planck+BAO+JWST at 92.7% confidence level.

Lithium-ion batteries(LIBs)with extreme fast charging(XFC)capability are considered an effective way to alleviate range anxiety for electric vehicle(EV)*** to the high ionic and electronic conductivity of LiNi_(x)Co_(y)Mn_zO_(2)(x+y+z=1,NCM)cathodes,the inevitable Li plating of graphite in NCM|graphite cell is usually identified as a key bottleneck for XFC ***,the capacity decay mechanism of cathode materials under XFC has not been fully *** this work,three typical NCM cathode materials with different Ni fractions were chosen and their electrochemical performances under XFC associated with structural evolution were investigated.A faster capacity decay of NCMs under XFC conditions is observed,especially for Ni-rich ***-situ X-ray diffraction(XRD)reveals that the multiple caxis parameters appear at the high-voltage regions in Nirich NCMs,which is probably triggered by the larger obstruction of Li(de)***,NCMs with moderate Ni fraction also present a similar trend under XFC *** phenomenon is more detrimental to the structural and morphological stability,resulting in a faster capacity decay than that under low current *** work provides new insight into the degradation mechanism of NCMs under XFC conditions,which can promote the development of NCM cathode materials with XFC capability.

Optimal SOC Headroom of Pump Storage Hydropower for Maximizing Joint Revenue from Day-ahead and Real-time Markets Under Regional Transmission Organization Dispatch

In response to the increasing penetration of volatile and uncertain renewable energy,the regional transmission organizations(RTOs)have been recently focusing on enhancing the models of pump storage hydropower(PSH)plants,which are one of the key flexibility assets in the day-ahead(DA)and real-time(RT)markets,to further boost their flexibility provision *** by the recent research works that explored the potential benefits of excluding PSHs’cost-related terms from the objective functions of the DA market clearing model,this paper completes a rolling RT market scheme that is compatible with the DA ***,with the vision that PSHs could be permitted to submit state-of-charge(SOC)headrooms in the DA market and to release them in the RT market,this paper uncovers that PSHs could increase the total revenues from the two markets by optimizing their SOC headrooms,assisted by the proposed tri-level optimal SOC headroom ***,in the proposed tri-level model,the middle and lower levels respectively mimic the DA and RT scheduling processes of PSHs,and the upper level determines the optimal headrooms to be submitted to the RTO for maximizing the total revenue from the two *** case studies quantify the profitability of the optimal SOC headroom submissions as well as the associated financial risks.

Revisiting the Mg isotopic systematics of siliciclastic components of sediments and sedimentary rocks:A new geochemical proxy of continental weathering in Earth's history

Characteristics and Risk Factors for Pediatric Sepsis

Objective Sepsis is considered a major cause of health loss in children and had high mortality and ***,there is no reliable model for predicting the prognosis of pediatric patients with *** study aimed to analyze the clinical characteristics of sepsis in children and assess the risk factors associated with poor prognosis in pediatric sepsis patients to identify timely interventions and improve their *** This study analyzed the clinical indicators and laboratory results of septic patients hospitalized in the Pediatric Intensive Care Unit of Tongji Hospital,Tongji Medical College,Huazhong University of Science and Technology,China,from January 1,2019,to December 31,*** factors for sepsis were identified by logistic regression *** A total of 355 children with sepsis were enrolled,with 333 children(93.8%)in the good prognosis group,and 22 children(6.2%)in the poor prognosis *** them,there were 255 patients(71.8%)in the sepsis group,and 100 patients(28.2%)in the severe sepsis *** length of hospital stay in the poor prognosis group was longer than that in the good prognosis group(P0.05),and the platelet(PLT),albumin(ALB),and hemoglobin(Hb)levels were lower in the poor prognosis group(P<0.01).The IL-8 levels in the severe sepsis group were higher than those in the sepsis group(P<0.05).Multiple logistic regression analysis suggested that lower Hb levels,ALB levels,peak PLT counts,and higher IL-1βlevels were independent risk factors for poor prognosis in children with *** Lower Hb,ALB,and PLT counts and elevated IL-1βare independent risk factors for poor prognosis in children with sepsis.

Chinese Clinical Named Entity Recognition Using Multi-Feature Fusion and Multi-Scale Local Context Enhancement

Chinese Clinical Named Entity Recognition(CNER)is a crucial step in extracting medical information and is of great significance in promoting medical ***,CNER poses challenges due to the specificity of clinical terminology,the complexity of Chinese text semantics,and the uncertainty of Chinese entity *** address these issues,we propose an improved CNER model,which is based on multi-feature fusion and multi-scale local context *** model simultaneously fuses multi-feature representations of pinyin,radical,Part of Speech(POS),word boundary with BERT deep contextual representations to enhance the semantic representation of text for more effective entity ***,to address the model’s limitation of focusing just on global features,we incorporate Convolutional Neural Networks(CNNs)with various kernel sizes to capture multi-scale local features of the text and enhance the model’s comprehension of the ***,we integrate the obtained global and local features,and employ multi-head attention mechanism(MHA)extraction to enhance the model’s focus on characters associated with medical entities,hence boosting the model’s *** obtained 92.74%,and 87.80%F1 scores on the two CNER benchmark datasets,CCKS2017 and CCKS2019,*** results demonstrate that our model outperforms the latest models in CNER,showcasing its outstanding overall *** can be seen that the CNER model proposed in this study has an important application value in constructing clinical medical knowledge graph and intelligent Q&A system.

Enhanced boron neutron capture therapy(BNCT)through controlled drug release via boron-loaded nanofiber mats
