Inetetamab combined with tegafur as second-line treatment for human epidermal growth factor receptor-2-positive gastric cancer: A case report

BACKGROUND Human epidermal growth factor receptor-2(HER-2)plays a vital role in tumor cell proliferation and metastasis.However,the prognosis of HER2-positive gastric cancer is poor.Inetetamab,a novel anti-HER2 targeting drug independently developed in China,exhibits more potent antibody-dependent cell-mediated cytotoxicity than trastuzumab,which is administered as the first-line treatment for HER2-positive gastric cancer in combination with chemotherapy.In this case,the efficacy and safety of inetetamab combined with tegafur was investigated as a second-line treatment for HER2-positive gastric cancer.CASE SUMMARY A 52-year-old male patient with HER2-positive gastric cancer presented with abdominal distension,poor appetite,and fatigue two years after receiving six cycles of oxaliplatin combined with tegafur as first-line treatment after surgery,followed by tegafur monotherapy for six months.The patient was diagnosed with postoperative recurrence of gastric adenocarcinoma.He received 17 cycles of a combination of inetetamab,an innovative domestically developed anti-HER2 monoclonal antibody,and tegafur chemotherapy as the second-line treatment(inetetamab 200 mg on day 1,every 3 wk combined with tegafur twice daily on days 1–14,every 3 wk).Evaluation of the efficacy of the second-line treatment revealed that the patient achieved a stable condition and progression-free survival of 17 months.He tolerated the treatment well without exhibiting any grade 3-4 adverse events.CONCLUSION Inetetamab combined with chemotherapy for the treatment of metastatic HER2-positive gastric cancer demonstrates significant survival benefits and acceptable safety.

Occupancy of rooms in urban residential buildings by users in cold areas of China

Occupancy is used to represent the movements and locations of users among various zones of buildings,and it is the basis of all other daily energy consumption behaviors.This study investigated eight families in cold areas of China based on occupancy measurements obtained in four main rooms,i.e.,living room,bedroom,kitchen,and bathroom.In particular,we analyzed the duration of user occupancy and hourly mean occupancy,and characterized their regular and random features.According to the results,we developed an event-based occupancy model using an inhomogeneous Markov chain,where the rooms were modeled and daily events were divided into three categories according to their randomness.We established a new method for conversion between event characteristic parameters and a transition probability matrix,as well as an overlap avoidance method for active events.The model was then validated using real data.The results showed that the model performed well in terms of two evaluation criteria.The model should improve the accuracy of simulations of occupancy.

Significantly Enhanced Energy Storage Performances of PEI-based Composites Utilizing Surface Functionalized ZrO_(2) Nanoparticles for High-Temperature Application

Polymer dielectrics with a high energy density and an available energy storage capacity have been playing an important role in advanced electronics and power systems. Nevertheless, the use of polymer dielectrics in harsh environments is limited by their low energy density at high temperatures. Herein, zirconium dioxide(ZrO_(2)) nanoparticles were decorated with amino group utilizing 4,4-methylenebis(phenyl isocyanate)(AMEO) and successfully incorporated into polyetherimide(PEI) matrix. The dielectric properties, breakdown strength, and energy storage performances of PEI/ZrO_(2)-AMEO nanocomposites were investigated from 25 ℃ to 150 ℃. It is found that the combination of moderate bandgap ZrO_(2) with modest dielectric constant and polar groups at interface with deep trap can offer an available strategy to simultaneously increase the dielectric constant and breakdown strength of polymer dielectrics. As a result, the composites containing ZrO_(2)-AMEO exhibit excellent energy storage performance at elevated temperatures. Specially, the PEI-based composites with 3 vol% ZrO_(2)-AMEO display a maximum discharged energy density(U_(d)) of 3.1 J/cm^(3) at 150 ℃, presenting 90% higher than that of neat PEI. This study may help to better develop the polymer-based dielectric composite applied at elevated temperatures.

Dear Editor,COVID-19 inactivated vaccines have been extensively administered inChina.However,the majority of the Chinese population has experiencedbreakthrough infections from SARS-CoV-2 ancestral,Delta or Omicronvariants over the past three years,particularly during the wave of Omicron BA.5 and BA.7 variants at the end of 2022(Zhu et al.,2023).Subsequently,new Omicron variants,such as BQ.1,BQ.1.1,XBB,XBB.1/XBB.1.9,and XBB.1.5/XBB.1.9.1,are emerging in China(Yueetal.,2023;Zhu et al.,2023).Therefore,it is an urgentneed and a publichealth imperative to assess the extent of the immunoprotection established in this population.

The overdiagnosis of prostate cancer(PCa)caused by nonspecific elevation serum prostate-specific antigen(PSA)and the overtreatment of indolent PCa have become a global problem that needs to be solved urgently.We aimed to construct a prediction model and provide a risk stratification system to reduce unnecessary biopsies.In this retrospective study,clinical data of 1807 patients from three Chinese hospitals were used.The final model was built using stepwise logistic regression analysis.The apparent performance of the model was assessed by receiver operating characteristic curves,calibration plots,and decision curve analysis.Finally,a risk stratification system of clinically significant prostate cancer(csPCa)was created,and diagnosis-free survival analyses were performed.Following multivariable screening and evaluation of the diagnostic performances,a final diagnostic model comprised of the PSA density and Prostate Imaging-Reporting and Data System(PI-RADS)score was established.Model validation in the development cohort and two external cohorts showed excellent discrimination and calibration.Finally,we created a risk stratification system using risk thresholds of 0.05 and 0.60 as the cut-off values.The follow-up results indicated that the diagnosis-free survival rate for csPCa at 12 months and 24 months postoperatively was 99.7%and 99.4%,respectively,for patients with a risk threshold below O.05 after the initial negative prostate biopsy,which was significantly better than patients with higher risk.Our diagnostic model and risk stratification system can achieve a personalized risk calculation of csPCa.It provides a standardized tool for Chinese patients and physicians when considering thenecessity of prostatebiopsy.

Embryo Transfer Strategies for Women with Recurrent Implantation Failure During the Frozen-thawed Embryo Transfer Cycles:Sequential Embryo Transfer or Double-blastocyst Transfer?

Objective Both sequential embryo transfer(SeET)and double-blastocyst transfer(DBT)can serve as embryo transfer strategies for women with recurrent implantation failure(RIF).This study aims to compare the effects of SeET and DBT on pregnancy outcomes.Methods Totally,261 frozen-thawed embryo transfer cycles of 243 RIF women were included in this multicenter retrospective analysis.According to different embryo quality and transfer strategies,they were divided into four groups:group A,good-quality SeET(GQ-SeET,n=38 cycles);group B,poor-quality or mixed-quality SeET(PQ/MQ-SeET,n=31 cycles);group C,good-quality DBT(GQ-DBT,n=121 cycles);and group D,poor-quality or mixed-quality DBT(PQ/MQ-DBT,n=71 cycles).The main outcome,clinical pregnancy rate,was compared,and the generalized estimating equation(GEE)model was used to correct potential confounders that might impact pregnancy outcomes.Results GQ-DBT achieved a significantly higher clinical pregnancy rate(aOR 2.588,95%CI 1.267–5.284,P=0.009)and live birth rate(aOR 3.082,95%CI 1.482–6.412,P=0.003)than PQ/MQ-DBT.Similarly,the clinical pregnancy rate was significantly higher in GQ-SeET than in PQ/MQ-SeET(aOR 4.047,95%CI 1.218–13.450,P=0.023).The pregnancy outcomes of GQ-SeET were not significantly different from those of GQ-DBT,and the same results were found between PQ/MQ-SeET and PQ/MQ-DBT.Conclusion SeET relative to DBT did not seem to improve pregnancy outcomes for RIF patients if the embryo quality was comparable between the two groups.Better clinical pregnancy outcomes could be obtained by transferring good-quality embryos,no matter whether in SeET or DBT.Embryo quality plays a more important role in pregnancy outcomes for RIF patients.

Game is a universal being in the universe.Starting with human understanding of the game process,we discuss the existence and practice of gaming,expound challenges in multi-agent gaming,and put forward a theoretical framework for a multiagent evolutionary game based on the idea of evolution and system theory.Taking the next-generation early warning and detection system as an example,we introduce the applications of multi-agent evolutionary game.We construct a multi-agent selforganizing game decision-making model and develop a multi-agent method based on reinforcement learning,which are significant in studying organized and systematic game behaviors in a high-dimensional complex environment.

强化学习作为人工智能领域的重要分支,以其在多智能体系统决策中的卓越表现,成为当前主流方法.然而,传统的多智能体强化学习算法在面对维度爆炸、训练样本稀缺和难以迁移等方面仍然存在困难.为了克服这些挑战并提升算法性能,本文从学习机制的角度入手,深入研究学习机制与强化学习的深度融合,以推动多智能体强化学习算法的发展.首先,介绍了多智能体强化学习算法的基本原理、发展历程以及算法所面临的难点.随后,引入了基于学习机制的多智能体强化学习方法这一种新兴方向.这些学习机制,如元学习和迁移学习,被证明可以有效提升多智能体的学习速度,并缓解维度爆炸等问题.按照课程学习、演化博弈、元学习、分层学习、迁移学习等学习机制在多智能体强化学习中的应用进行了综述,通过罗列这些方法的研究成果,论述了各种方法的局限性,并提出了未来改进的方向.总结了这类融合算法在实际应用中取得的提升成果和实际应用,具体列举了基于学习机制的多智能体强化学习算法在交通控制、游戏领域的实际应用案例.同时,对这类融合算法未来在理论、算法和应用方面的发展方向进行了深入分析.这涵盖了对新颖理论的探索、算法性能的进一步优化,以及在更广泛领域中的推广应用.通过这样的综述和分析,为未来多智能体强化学习算法的研究方向和实际应用提供了有益的参考.

2023年是药物研发稳中求进的一年,小分子药物与生物药物的研发增长齐头并进。美国FDA药物评价和研究中心(CDER)在过去一年里共计批准了55款新药,相比2022年的37款新药获批数量提升明显,其中包括38款新分子实体、17款生物药物、5款基因疗法和2款细胞疗法。首创性(first-in-class)药物占比稳步增加,本年度获批13款小分子首创药物和7款生物首创药物,多数集中于肿瘤和罕见病领域。其中,多个首创性小分子药物具有突破性意义,例如首个神经激肽3(NK3)受体拮抗剂非唑奈坦(fezolinetant)、首个视黄酸受体(RIG-I)激动剂帕拉罗汀(palovarotene)、首个蛋白激酶B(AKT)抑制剂卡匹色替(capivasertib)、首个补体因子B抑制剂伊普可泮(iptacopan)等。首创性药物具有巨大的学术价值和商业价值,已成为当前学术界和工业界争相追寻的目标。然而,首创性药物不仅需要新靶标、新机制和新分子,更需要全面验证新靶标与疾病的因果性,研究新机制与药效之间的关联性,探索新分子与成药性之间的平衡关系,研发风险极大。本文通过浅析本年度3个首创性小分子药物的研发背景、研发过程和治疗应用,以期为更多的首创性药物提供研究思路与方法。
