Neuropeptide Y (NPY), a metabolism-related cardiovascular factor, plays a crucial role in blood pressure(BP) regulation via peripheral and central pathways. The expression of NPY receptors (Y1R/Y2.) specific to baroreflex afferents impacts on the sexually dimorphic neural control of circulation. This study was designed to investigate the expression profiles of NPY receptors in the nodose ganglion (NG) and nucleus tractus solitary (NTS) under hypertensive conditions. To this end, rats with hypertension induced by NG-nitro-L-arginine methylester (L-NAME) or high fructose drinking (HFD), and spontaneously hypertensive rats (SHRs) were used to explore the effects/mechanisms of NPY on BP using functional, molecular, and electrophysiological approaches. The data showed that BP was elevated along with baroreceptor sensitivity dysfunction in model rats;Y1R was up-or down-regulated in the NG or NTS of male and female HFD/L-NAME groups,while Y2. was only down-regulated in the HFD groups as well as in the NG of the male L-NAME group. In SHRs,Y1R and Y2. were both down-regulated in the NTS, and not in the NG. In addition to NPY-mediated energy homeostasis, leptin-melanocortin activation may be essential for metabolic disturbance-related hypertension. We found that leptin and a-melanocyte stimulating hormone (aMSH) receptors were aberrantly down-regulated in HFD rats. In addition, a-MSH concentrations were reduced and NPY concentrations were elevated in the serum and NTS at 60 and 90 min after acute leptin infusion. Electrophysiological recordings showed that the decay time-constant and area under the curve of excitatory post-synaptic currents were decreased by Y1R activation in A-types, whereas, both were increased by Y2. activation in Ah-or C-types. These results demonstrate that sex-and afferent-specific NPY receptor expression in the baroreflex afferent pathway is likely to be a novel target for the clinical management of metabolism-related and essential hypertension.

Objective Several COVID-19 patients have overlapping comorbidities. The independent role of each component contributing to the risk of COVID-19 is unknown, and how some non-cardiometabolic comorbidities affect the risk of COVID-19 remains *** A retrospective follow-up design was adopted. A total of 1,160 laboratory-confirmed patients were enrolled from nine provinces in china. Data on comorbidities were obtained from the patients’ medical records. Multivariable logistic regression models were used to estimate the odds ratio(OR) and 95% confidence interval(95% CI) of the associations between comorbidities(cardiometabolic or non-cardiometabolic diseases), clinical severity, and treatment outcomes of *** Overall, 158(13.6%) patients were diagnosed with severe illness and 32.2.7%) had unfavorable outcomes. Hypertension(2.87, 1.30–6.32., type 2.diabetes(T2.DM)(3.57, 2.32.5.49),cardiovascular disease(CVD)(3.78, 1.81–7.89), fatty liver disease(7.53, 1.96–2..96), hyperlipidemia({2., 1.2.–3.67), other lung diseases(6.00, 3.01–11.96), and electrolyte imbalance(10.40, 3.00–2..10)were independently linked to increased odds of being severely ill. T2.DM(6.07, 2.89–12.75), CVD(8.47,6.03–11.89), and electrolyte imbalance(19.44, 11.47–32.96) were also strong predictors of unfavorable outcomes. Women with comorbidities were more likely to have severe disease on admission(5.46,3.2.–9.19), while men with comorbidities were more likely to have unfavorable treatment outcomes(6.58, 1.46–2..64) within two *** Besides hypertension, diabetes, and CVD, fatty liver disease, hyperlipidemia, other lung diseases, and electrolyte imbalance were independent risk factors for COVID-19 severity and poor treatment outcome. Women with comorbidities were more likely to have severe disease, while men with comorbidities were more likely to have unfavorable treatment outcomes.

目的运用世界卫生组织(WHO)健康服务构成要素的理论,从健康服务体系视域,对WHO有关康复服务的政策架构与行动战略进行系统的内容分析,探讨WHO国际康复政策的理论基础与方法、康复政策架构以及行动战略的核心内容。方法WHO发布的康复相关政策文件包括《健康服务体系中的康复》《健康服务体系中的康复:行动指南》《康复指标清单:康复监测和评估架构配套的工具》和《康复现状系统评估配套工具(STARS):康复信息收集模板(TRIC)》,形成国际康复政策体系。本研究运用内容分析方法,根据WHO健康服务构成的六大要素,即领导力和治理能力、筹资、健康人力资源、服务提供、医药技术、健康信息系统,详细分析国际康复政策的理论基础与方法、政策架构以及行动战略的核心内容与康复服务优先发展领域。结果现代康复服务政策核心依据WHO有关构建以人为本的健康服务、关注健康的社会决定因素的理论,依据《国际功能、残疾与健康分类》的功能、残疾和健康理论,明确发展康复是实现联合国可持续发展目标三,确保健康的生活方式,促进各年龄段人群的福祉,即“全民健康覆盖”的重要路径。系统分析了领导力和治理能力、筹资、健康人力资源、服务提供、医药技术、健康信息系统六大领域,WHO的国际政策架构、行动战略以及发展内容与优先领域。WHO倡导制定国家康复规划,建立与完善康复治理结构并发展康复治理能力,采取多种途径为发展康复服务筹资。将康复融入现代健康服务体系,在健康服务连续体中提供不同层次的康复服务,在健康服务体系中构建康复服务体系,培训专业人员,建立服务信息网络,提升服务质量和服务覆盖率,关注重点领域和优先项目,以满足不同人群的多样化需求,实现全民健康覆盖。将辅助技术作为医药技术领域纳入康复服务体系。在健康信息系统中,收集有关功能和康复需求的信息以及康复服务结果和影响的数据,并开展基于循证的康复政策系统研究。结论基于健康服务体系六大构成要素,从宏观、中观和微观三个层面,系统研究了国际康复政策架构。现代康复发展的理念是要实现全民健康覆盖。发展康复必须建立以人为中心的现代健康服务体系,并且关注社会决定因素。发展康复是实现联合国2.30年可持续发展目标的重要举措。国际康复的政策体系围绕康复领导力和治理、康复筹资、康复人力资源、康复服务提供、康复相关医药技术以及康复与健康信息六大领域,从理论与政策、政策行动、实施工具和路径(评价指标体系和实施效果评估)三个层级,构建了康复发展的政策和行动战略以及具体的实施方法与工具的体系。实施WHO康复政策,需要采取如下行动:加强康复服务领导力、治理、规划和协调能力;构建合理的康复筹资机制,为康复筹措必要资金;健全康复人力资源培养与保障机制;提升康复人员专业能力,提升康复服务提供能力并改善服务质量;提高辅助产品和辅助技术服务的质量与可及性;建立涵盖功能、残疾和康复的健康信息系统并开展康复科学研究。

目的探索体育融入现代健康服务体系的理论与方法。方法基于世界卫生组织现代健康服务体系的理论和《健康服务体系中的康复》政策指南,探讨在健康服务体系的六个构成要件方面,即领导力和治理能力、筹资、健康人力资源、服务提供、医药技术和健康信息系统,如何促进体育活动融入现代健康服务体系中,以及在健康服务连续体的预防、干预、康复和健康促进四个环节中,体育活动融入健康服务体系的重点内容与要求。结果提出了构建以人为本的跨行业、多学科的健康服务体系的目标,要求促进医疗与体育的融合,将运动干预作为健康干预的方法,发展体育与健康融合的服务技术与标准;培养掌握运动干预和运动康复的专业人才,发展信息系统,促进体育与健康服务的融合发展。结论体育活动是促进健康的重要手段,是现代健康服务的重要组成部分。从健康服务体系的构成要素入手,可以构建体育活动融入现代健康服务体系的理论与方法体系,从而促进实现覆盖全人群、全生命周期的健康服务体系,联合国“2.30议程”发展目标之三:确保健康的生活方式,促进各年龄段人群的福祉,实现“健康中国”相关目标。

目的:观察植物雌激素α-玉米赤霉醇对大鼠胸主动脉的舒张作用及其途径。方法:实验于2.05-09/12.首都医科大学病理生理学教研室完成。选取健康成年雌性Wistar大鼠17只,采用体外血管灌流的方法,用10-6mol/L苯肾上腺素预收缩血管,将10-6mol/L乙酰胆碱对血管环是否有舒张反应作为判断血管内皮是否完整的标准。观察α-玉米赤霉醇对血管内皮完整和去内皮胸主动脉的舒张作用,以及内皮型一氧化氮合酶抑制剂左旋硝精氨酸甲酯、鸟苷酸环化酶抑制剂亚甲蓝、钙激活钾通道阻断剂四乙胺、ATP敏感钾通道的阻断剂格列本脲、L-型钙通道激动剂(-)BayK8644(10-6mol/L)及雌激素受体阻断剂ICI182.780对这一过程的影响。各种药物对血管环的作用强度用最大舒张率表示,即加药后收缩幅度与加药前收缩幅度的百分比。结果:纳入大鼠17只,共取得胸主动脉环68个,共8个不合格予以剔除,其余60个胸主动脉环均用于实验。①α-玉米赤霉醇(10-10～10-5mol/L)可引起内皮完整的胸主动脉剂量依赖性的舒张效应;去内皮血管的最大舒张率显著低于同浓度内皮完整血管(P1)。②左旋硝精氨酸甲酯和亚甲蓝均可抑制α-玉米赤霉醇对胸主动脉的舒张作用,使两组胸主动脉环的最大舒张率显著降低(P1)。③四乙胺、格列本脲、BayK8644可以减弱α-玉米赤霉醇的舒张血管作用(P1),雌激素受体特异性阻断剂ICI182.780对10-6mol/Lα-玉米赤霉醇的舒张作用无显著影响(P>0.05)。结论:α-玉米赤霉醇可引起内皮依赖性的胸主动脉舒张,其作用与一氧化氮合酶/一氧化氮系统激活、钙激活钾通道和ATP敏感钾通道开放、钙通道抑制有关,但与雌激素受体无关。α-玉米赤霉醇的舒张血管效应可能是其心血管保护途径之一。

目的调查国内三级医院急诊科护士灾害护理能力现状及其影响因素。方法采用一般情况资料表、临床护士灾害护理能力评估量表对12.省市、18家三级医院的418名急诊科护士进行问卷调查,并对回收问卷数据进行统计分析。结果急诊护士灾害护理能力总体评分为(114.55±7.70)分,最高分和最低分别为159、93分。得分较高的灾害护理能力层面集中在灾难现场护理协作、心肺复苏、紧急转运、外伤处理、预检分诊、分级救护等;能力欠缺的护理层面主要集中在灾难基础及医学知识、灾害心理护理知识与技能、大规模人群感染控制、脆弱人群护理、灾难护理管理等。学历、年龄、工作年限、灾害救援经历是影响急诊护士灾害护理能力的独立因素。结论三级医院急诊护士灾难护理能力整体处于中等偏下水平,应通过多种形式开展灾害救援知识与护理技能培训,不断提升急诊护士对灾难护理的认知及技能水平,以增强其应对灾害的紧急救援与护理能力。
