Early debridement and delayed primary vascularized cover in forearm electrical burns: A prospective study

AIM To look into the management options of early debridement of the wound, followed by vascularized cover to bring in fresh blood supply to remaining tissue in electrical burns. METHODS A total of 16 consecutive patients sustaining full thickness forearm burns over a period of one year were included in the study group. Debridement was undertaken within 48 h in 13 patients. Three patients were taken for debridement after 48 h. Debridement was repeated within 2-4 d after daily wound assessment and need for further debridement. RESULTS On an average two debridements(range 1-4) was required in our patients for the wound to be ready for definitive cover. Interval between each debridement ranged from 2-18 d. Fourteen patients were provided vascularized cover after final debridement(6 free flaps, 8 pedicled flaps). Functional assessment of gross hand function done at 6 wk, 2 mo, 3 mo and 6 mo follow-up. CONCLUSION High-tension electrical burns lead to significant morbi-dity. These injuries are best managed by early decompression followed by multiple serial debridements. The ideal timing of free flap coverage needs further investigation.

Nematodes are microscopic, soil-dwelling worms that navigate through soil particles in search of food or a suitable host. Most nematode species employ a myriad of physical and chemical cues that define their navigation strategies. Here, we demonstrate a microfluidic method to observe and characterize the physical aspects of nematode navigation at real-time. The microfluidic devices comprise a series of interconnected T-maze or cylindrical structures of varying geometry. At each physical intersection, nematodes are given the choice to migrate left or right. We found that this decision-making of nematodes is influenced by the angle of intersection of T-maze structures. We further showed that nematodes can be passively directed to move in a linear direction by carefully adjusting the position and spacing of cylindrical obstacles in its path. The experiments were conducted on two nematodes (non-parasitic C. elegans and pigparasitic Oesophagostomum dentatum) and in the absence of any chemical or electrical stimulants.

This paper presents a part count tool that predicts the part count for a particular product concept during the conceptual design phase. The part count tool will also aid in ranking the design concepts by the criterion of number of components for a product. This tool utilizes existing automated concept generation algorithms to generate the design concepts. It extracts the available data from the Design Engineering Lab Design Repository to determine an average number of parts per component type in the repository and then calculates an average part count for new concepts. The part count tool also uses an algorithm to determine how to connect two non-compatible components through the addition of mutually compatible components. While emphasis is placed on the average parts per product in evaluating designs, the overall functional requirement of the product is also considered.

Mid Trimester Transvaginal Ultrasound Assessment of Cervix for Prediction of Primary Caesarean Section

Objectives: To assess the role of mid-trimester transvaginal sonographic assessment of cervix in predicting the risk of primary cesarean section. Methods: Hundred pregnant women, who attended out-patient department of Tata Main Hospital for ante natal examination, from 1st December 2011 to 1st December 2012, were taken up for the study. Only those who had confirmed dates were included in the study. Transvaginal sonography was done at 18 - 26 week to measure cervical length. Parameters studied were gestational age at delivery, whether spontaneous or induced, duration of labor, pregnancy outcome and mode of delivery and indication for cesarean section. Results: Majority of LSCS were in the group with cervical length ≥4 cm (57.1%) with “p” value of <0.0001. Only 13.8% women who had vaginal delivery had cervical length ≥4 cm. Out of the 32 women with cervical length ≥4 cm, 26 (81.3%) required induction of labor and 10 (31.3%) delivered after 40 weeks. Mean cervical length for spontaneous onset of labor was 3.11 ± 0.85 cm and for induction of labor was 4.36 ± 1.11 cm (“p” value ≤ 0.001). Multiple logistic regression analysis was performed to study outcome variable of mode of delivery. Cervical length and induction to delivery interval were found to be independent predictor of mode of delivery. In the receiver operating characteristic (ROC) curve analysis, the best cut-off point for prediction of primary cesarean section was 40 mm for cervical length. Sensitivity, specificity, positive predictive value and negative predictive value of cervical length as a predictor of mode of delivery was 54.8%, 91.4%, 82.1% and 73.6% respectively. R2 (coefficient of determination) was 0.271. Conclusion: Transvaginal cervical length measurement at mid trimester can be used as a predictive tool to determine the risk of primary cesarean section as well as the need of induction of labor.

In the present paper dependence of geomagnetic activity on the solar-wind plasma and interplanetary magnetic field (IMF) parameters has been studied. We have taken interplanetary solar wind data at the instant of Dst minimum. Our study consists of 200 geomagnetic storms weighed by disturbance storm time (Dst) -50 nT, observed during solar cycle 23. The study suggests that the strength of the geomagnetic storm is strongly dependent on the total magnetic field Btotal. The correlation (-0.72) has been found reasonable. In perspective of previous studies, the strength of the geomagnetic storm is strongly dependent on the southward component (Bz) whereas in present study exposes that the correlation (0.22) is weak. This result indicates that solar wind southward magnetic field component Bz has significant growth particularly before the main phase of geomagnetic storm (not during the main phase). The present result implies that neither density nor temperature is significantly related to the variation of geomagnetic disturbance;rather the effects of the pressure and speed. However, a low plasma beta during highly geoeffective event seems to be an important criterion.
