Correlation of transient elastography with hepatic venous pressure gradient in patients with cirrhotic portal hypertension: A study of 326 patients from India

AIM To study the diagnostic accuracy of transient elastography(TE) for detecting clinically significant portal hypertension(CSPH) in Indian patients with cirrhotic portal hypertension.METHODS This retrospective study was conducted at the Institute of Liver, Gastroenterology, and Panceatico-Biliary Sciences, Sir Ganga Ram Hospital, New Delhi, on consecutive patients with cirrhosis greater than 15 years of age who underwent hepatic venous pressure gradient(HVPG) and TE from July 2011 to May 2016. Correlation between HVPG and TE was analyzed using the Spearman's correlation test. Receiver operating characteristic ( ROC) curves were prepared for determining the utility of TE in predicting various stages of portal hypertension. The best cut-off value of TE forthe diagnosis of CSPH was obtained using the Youden index.RESULTS The study included 326 patients [median age 52(range 16-90) years; 81% males]. The most common etiology of cirrhosis was cryptogenic(45%) followed by alcohol(34%). The median HVPG was 16.0(range 1.5 to 30.5) mm Hg. Eighty-five percent of patients had CSPH. A significant positive correlation was noted between TE and HVPG(rho 0.361, P < 0.001). The area under ROC curve for TE in predicting CSPH was 0.740(95%CI: 0.662-0.818)(P < 0.01). A cut-off value of TE of 21.6 k Pa best predicted CSPH with a positive predictive value(PPV) of 93%.CONCLUSION TE has a fair positive correlation with HVPG; thus, TE can be used as a non-invasive modality to assess the degree of portal hypertension. A cut-off TE value of 21.6 k Pa identifies CSPH with a PPV of 93%.

Models of allopatric speciation within an island biogeographic framework suggest that the division of ancestral mainland populations leads to one or more allopatric island species predominantly through natural and sexual selection or genetic drift.Here we studied phenotypic divergence in a phylogenetic framework in the Dicrurus paradiseus allospecies complex in Sri Lanka,a continental island located in the Indian plate,to understand the complexity of phenotypic divergence on an island.Members of the genus Dicrurus are known as drongos and are conserved in morphology and plumage,but highly variable in vocalization due to vocal learning and mimicry.Two closely related drongos are found in Sri Lanka:the endemic D.lophorinus(or D.paradiseus lophorinus to many authors)found in the wet zone of the island and the widespread continental species D.paradiseus,which inhabits the dry zone.Sampling from all major populations,and voucher specimens from museums across their range in Sri Lanka,we examined phenotypic and genetic variation in this group.The phenotype showed two clusters:birds with a fish-like tail and erect crest(D.lophorinus),and birds with elongated tail streamers with backwardly curved crest(D.paradiseus).There was no significant difference in the vocal traits compared.The genetic variation was examined using two nuclear(Myo 2 and c-mos)and two mitochondrial(ND2 and Cytb)loci and the phylogenetic relationship was analyzed using the Bayesian inference coalescent-based species tree estimation method.The quantitative criteria for species delimitation provided a score sufficient to consider these two taxa as distinct species by considering measurements of body and plumage,acoustics,behaviour and distribution.The phylogeny supports distinct species status for the Sri Lanka Drongo(Dicrurus lophorinus)and that the D.lophorinus and D.paradiseus sister pair diverged since 1.35 mya.The variation in the crest and the tail plumage(components of phenotype)were the main contributors of the divergence,despite the similarity i

A rapid and sensitive ultra-performance liquid chromatography–tandem mass spectrometry（UPLC–MS/MS） method was developed and validated for the estimation of 17-desacetyl norgestimate in human plasma using solid-phase extraction technique. 17-desacetyl norgestimate D6 was used as the internal standard. Simple gradient chromatographic conditions and mass spectrometric detection enabled accurate and precise measurement of 17-desacetyl norgestimate at sub-picogram levels. The proposed method was validated for a linear range of 20–5000 pg/m L with a correlation coefficient Z 0.9988. The intra-run and inter-run precision and accuracy were within 10%. The overall recoveries for 17-desacetyl norgestimate and 17-desacetyl norgestimate D6 were 96.30% and 93.90%, respectively. The total run time was 4.5 min. The developed method was applied for the determination of the pharmacokinetic parameters of 17-desacetyl norgestimate following a single oral administration of a norgestimate and ethinyl estradiol0.250 mg/0.035 mg tablets in 35 healthy female volunteers.

Gallbladder(GB) wall thickening is a frequent finding caused by a spectrum of conditions. It is observed in many extracholecystic as well as intrinsic GB conditions. GB wall thickening can either be diffuse or focal. Diffuse wall thickening is a secondary occurrence in both extrinsic and intrinsic pathologies of GB, whereas, focal wall thickening is mostly associated with intrinsic GB pathologies. In the absence of specific clinical features, accurate etiological diagnosis can be challenging. The survival rate in GB carcinoma(GBC) can be improved if it is diagnosed at an early stage, especially when the tumor is confined to the wall. The pattern of wall thickening in GBC is often confused with benign diseases, especially chronic cholecystitis, xanthogranulomatous cholecystitis, and adenomyomatosis. Early recognition and differentiation of these conditions can improve the prognosis. In this minireview, the authors describe the patterns of abnormalities on various imaging modalities(conventional as well as advanced) for the diagnosis of GB wall thickening. This paper also illustrates an algorithmic approach for the etiological diagnosis of GB wall thickening and suggests a formatted reporting for GB wall abnormalities.

In recent years several studies have been made to understand the impact of Socially Responsible HRM practices on Organizational Performance.Employee progress,community and environment play an important role in the sustainable growth of an organization.Thus,organizations are always looking for the ways to improve the employee satisfaction vis-a-vis commitment to improve the performance.Recent studies have shown that as employees are important stakeholder,hence formulating proper Socially Responsible HRM practices may help organization to better the returns on assets.The main objective of the study is to identify the relationship among various dimensions of Socially Responsible HRM practices with dimensions of employee satisfaction,employee commitment and organizational performance for Indian manufacturing sector by using Bayesian Network approach.Results of the study establish the relationship between dimensions of Socially Responsible HRM and Organizational Performance.

Bearing ratio and leachate analysis of pond ash stabilized with lime and lime sludge

Recycling of industrial waste is one of the effective ways to overcome their disposal problem. Ash produced by thermal power plants and lime sludge produced by paper mills require huge disposal land and may create environmental problems such as dusting and leaching of harmful heavy metals. Stabilization of the ash can improve its engineering properties and address the environmental problems. This paper reports the laboratory test results of a Class F pond ash stabilized with lime(2%, 4%, 6% and 8% by weight)alone and in combination with lime sludge(5%, 10% and 15% by weight). The X-ray diffraction(XRD) and scanning electron micrograph(SEM) tests were also performed to identify the possible formation of crystalline phases after stabilization. The effects of lime sludge on the unsoaked and soaked bearing ratios of pond ash with different lime contents, after 7 d, 28 d and 45 d of curing, were observed. Test results indicated that the bearing ratio increased considerably up to a 4% lime content which can be taken as the optimum lime content. Further increase in lime content increased bearing ratio gradually but at a slower rate. The effect of lime sludge was more pronounced at the optimum lime content,particularly at a low curing period. Lime sludge improved the bearing ratio in soaked condition significantly. Leachate analysis of stabilized ash was performed using toxicity characteristic leaching procedure(TCLP-1311) method. The concentrations of toxic elements Zn, Cu, Cd, Ni and Cr in the stabilized mixes were lower than those in the unstabilized waste. The results indicated that the pond ash-lime-lime sludge mixes have potential application as road subbase material.

Natural gas has been considered as the best transition fuel into the future carbon constraint world.The ever-increasing demand for natural gas has prompted expanding research and development activities worldwide for exploring methane hydrates as a future energy resource.With its vast global resource volume(~3000 trillion cubic meter CH4)and high energy storage capacity(170 CH4 v/v methane hydrate),recovering energy from naturally-occurring methane hydrate has attracted both academic and industry interests to demonstrate the technical feasibility and economic viability.In this review paper,we highlight the recent advances in fundamental researches,seminal discoveries and implications from ongoing drilling programs and field production tests,the impending knowledge gaps and the future perspectives of recovering energy from methane hydrates.We further emphasize the current scientific,technological and economic challenges in realizing long-term commercial gas production from methane hydrate reservoir.The continuous growth of the corresponding experimental studies in China should target these specific challenges to narrow the knowledge gaps between laboratory-scale investigations and reservoir-scale applications.Furthermore,we briefly discuss both the environmental and geomechanical issues related to exploiting methane hydrate as the future energy resource and believe that they should be of paramount importance in the future development of novel gas production technologies.

Decreasing costs and favorable policies have resulted in increased penetration of solar photovoltaic(PV)power generation in distribution networks.As the PV systems penetration is likely to increase in the future,utilizing the reactive power capability of PV inverters to mitigate voltage deviations is being promoted.In recent years,droop control of inverter-based distributed energy resources has emerged as an essential tool for use in this study.The participation of PV systems in voltage regulation and its coordination with existing controllers,such as on-load tap changers,is paramount for controlling the voltage within specified limits.In this work,control strategies are presented that can be coordinated with the existing controls in a distributed manner.The effectiveness of the proposed method was demonstrated through simulation results on a distribution system.
