Case report of tubercular spondylodiscitis with paraplegia managed by posterior transpedicular decompression and pedicle screw fixation

Pott’s paraplegia is still prevalent in this part of the world. Early onset paraplegia can be improved by timely surgical intervention under ATT cover. The disease mostly affects the thoraco-lumbar spine. Classically, the diseased area is addressed by anterior thoracic or thoraco-lumbar approach and after curettage of the diseased and necrotic material the anterior column is reconstructed by rib or fibular strut graft or metallic cage and supplemented by posterior instrumentation and fusion. Laminectomy, as a method of decompression, was greatly discouraged in spinal tuberculosis with compressive myelopathy except in posterior element involvement. We present a case of a 35 years old lady with Pott’s paraplegia treated by hemilaminectomy and transpedicular limited anterior decompression of the cord and pedicle screw fixation with fusion who improved vastly in terms of motor power.

Coracoid Process Fracture: A Report of Two Cases and Brief Review of Literature

Coracoid process of Scapula fracture is not a common entity. Because of its deep anatomic location, they hardly sustain any direct traumatic force. Most of the time, they get fractured by indirect force by the muscles and ligaments attached to them. They are usually associated with fracture of the acromion process of Scapula, acromio-clavicular dislocation and proximal humerus fracture or dislocation of the shoulder joint. They may be missed because more attention is drawn by the obvious injuries. We report two cases of coracoid process fracture, one is associated with fracture dislocation of the shoulder and the other with acromion process fracture.

A Rare Case of Fracture of Radius Associated with Dislocation of Both Distal and Proximal Radio-Ulnar Joint

Single bone forearm fractures are usually associated with dislocation of one radio-ulnar joint. The association of ulnar fractures with dislocation of proximal radio-ulnar joint and radius fractures with distal radio-ulnar joint is well described. Classically they are better known as Monteggia and Galeazzi fracture dislocations respectively. These peculiar presentations are attributed to intricate relationship of the forearm bones joined together as a unit by two radio-ulnar joints at proximal and distal ends and the interosseous membrane in the middle part. However, simultaneous dislocation of the both radio-ulnar joints associated with fracture of single bone is a very rare event. Literature search does not reveal a single case of such type. We are presenting here a case of fracture shaft of radius associated with dislocation of both proximal and distal radio-ulnar joints. The patient was managed by open reduction and fixation of the radius by a compression plate and the dislocations were reduced by closed method and stabilized with small K wires.
