Management of splenic artery aneurysm associated with extrahepatic portal vein obstruction

BACKGROUND: Splenic artery aneurysms although rare are clinically significant in view of their propensity for spontaneous rupture and life-threatening bleeding. While portal hypertension is an important etiological factor, the majority of reported cases are secondary to cirrhosis of the liver. We report three cases of splenic artery aneurysms associated with extrahepatic portal vein obstruction and discuss their management. METHODS: The records of three patients of splenic artery aneurysm associated with extrahepatic portal vein obstruction managed from 2003 to 2010 were reviewed retrospectively. The clinical presentation, surgical treatment and outcome were analyzed. RESULTS: The aneurysm was >3 cm in all patients. The clinical symptoms were secondary to extrahepatic portal vein obstruction (hematemesis in two, portal biliopathy in two) while the aneurysm was asymptomatic. Doppler ultrasound demonstrated aneurysms in all patients. A proximal splenorenal shunt was performed in two patients with excision of the aneurysm in one patient and ligation of the aneurysm in another one. The third patient had the splenic vein replaced by collaterals and hence underwent splenectomy with aneurysmectomy. All patients had an uneventful post-operative course. CONCLUSIONS: Splenic artery aneurysms are associated with extrahepatic portal vein obstruction. Surgery is the mainstay of treatment. Although technically difficult, it can be safely performed in an experienced center with minimal morbidity and good outcome.

As patients with carcinoma of the esophagus live longer, complications associated with the use of a gastric conduit are increasing. Ulcers form in the gastric conduit in 6.6% to 19.4% of patients. There are a few reports of perforation of a gastric conduit in the English literature. Almost all of these were associated with serious complications. We report a patient who developed a tension pneumothorax consequent to spontaneous perforation of an ulcer in the gastric conduit 7 years after the index surgery in a patient with carcinoma of the gastroesophageal junction. He responded well to conservative management. Complications related to a gastric conduit can be because of multiple factors. Periodic endoscopic surveillance of gastric conduits should be considered as these are at a higher risk of ulcer formation than a normal stomach. Long term treatment with proton pump inhibitors may decrease complications. There are no guidelines for the treatment of a perforated gastric conduit ulcer and the management should be individualized.

Pheochromocytoma and ganglioneuroma form rare composite tumours of the adrenal medulla comprising less than 3% of all sympathoadrenal tumours. We present a case of intraoperatively detected adrenal medullary tumour of composite pheochromocytoma and ganglioneuroma diagnosed on histopathology, in a normotensive patient. A 50-year-old male with a past history of chronic obstructive pulmonary disease presented with abdominal pain and significant weight loss since one month. Ultrasound and contrast-enhanced computed tomography abdomen revealed a large lobulated lesion in the distal body and tail of pancreas suggestive of solid and papillary neoplasm of body and tail of pancreas. Intra-operatively, a 15 cm × 10 cm solid lesion with cystic areas was seen arising from the left lower pole of the adrenal gland pushing the pancreas which appeared unremarkable. In our case, exploratory laparotomy with tumour excision was done. Extensive sectioning and microscopic examination of this adrenal tumour confirmed a diagnosis of composite Pheochromocytoma with Ganglioneuroma on histopathology. Immunophenotyping with S-100 further supported the diagnosis. The goal of this report is to increase the awareness of this rare disease and to further identify its variable presentation.

A Hybrid Deep Learning Model for COVID-19 Prediction and Current Status of Clinical Trials Worldwide

Infections or virus-based diseases are a significant threat to human societies and could affect the whole world within a very short *** Virus Disease-2019(COVID-19),also known as novel coronavirus or SARSCoV-2(Severe Acute Respiratory Syndrome-Coronavirus-2),is a respiratory based touch contiguous *** catastrophic situation resulting from the COVID-19 pandemic posed a serious threat to societies *** whole world is making tremendous efforts to combat this life-threatening *** taking remedial action and planning preventive measures on time,there is an urgent need for efficient prediction models to confront the COVID-19 outbreak.A deep learning-based ARIMA-LSTM hybrid model is proposed in this article for predicting the COVID-19 outbreak by utilizing real-time information from the WHO’s daily bulletin report as well as provides information regarding clinical trials across the *** evaluate the suitability and performance of our proposed model compared to other well-established prediction models,an experimental study has been *** estimate the prediction results,the three performance measures,i.e.,Root Mean Square Error(RMSE),Coefficient of determination(R2 Score),and Mean Absolute Percentage Error(MAPE)have been *** prediction results of fifty countries substantiated the fact that the proposed ARIMA-LSTM hybrid model performs very well as compared to other *** proposed model archives the lowest RMSE,lowest MAPE,and highest R2 Score throughout the testing,under varied selection criteria(country-wise).This article aims to contribute a deep learning-based solution for the wellbeing of livings and to provide the current status of clinical trials across the globe.

In this paper, we present classical molecular dynamics （MD） simulations of model polymer/CNT composites constructed by embedding a single wall （10,10） CNT into two different amorphous polymer matrices： poly（methyl methacrylate） and poly{（m-phenylene-vinylene）-co- [（2,5-dioctoxy-p-phenylene） vinylene]}, respectively, with different volume fractions. The simulation results support the idea that it is possible to use CNTs to mechanically reinforce an appropri- ate polymer matrix, especially in the longitudinal direction of the nanotube. The comparison of the simulation results with the macroscopic rule-of-mixtures for composite systems showed that for strong interfacial interactions, there can be large deviations of the results from the rule-of- mixtures. In order to verify this study, results obtained have been compared with those given by Elliott and Han （2007）.

Habitat Range of two Alpine Medicinal Plants in a Trans-Himalayan Dry Valley,Central Nepal

Understanding of the habitat range of threatened Himalayan medicinal plants which are declining in their abundance due to high anthropogenic disturbances is essential for developing conservation strategies and agrotechnologies for cultivation. In this communication, we have discussed the habitat range of two alpine medicinal plants, Aconitum naviculare （Bruehl） Stapf and Neopicrorhiza scrophulariiflora （Pennel） Hong in a trans-Himalayan dry valley of central Nepal, Manang district. They are the most prioritized medicinal plants of the study area in terms of ethnomedicinal uses. A. naviculare occurs on warm and dry south facing slopes between 4090-4650 m asl along with sclerophyllous and thorny alpine scrubs, while N. scrophulariiflora is exclusively found on cool and moist north facing slope between 4000 and 4400 m asl where adequate water is available from snow melt to create a suitable habitat for this wetland dependent species. The soil in rooting zone of the two plants differs significantly in organic carbon （OC）, organic matter （OM）, total nitrogen （N） and carbon to nitrogen （C/N） ratio. Due to cool and moist condition of N. scrophulariiflora habitat, accumulation of soil OC is higher, but soil N content is lower probably due to slow release from litter, higher leaching loss and greater retention in perennial live biomass of the plant. The C/N ratio of soil is more suitable in A. navuculare habitat than that of N scrophulariiflora for N supply. Warm and sunny site with N rich soil can be suitable for cultivation ofA. naviculare, while moist and cool site with organic soil for N. scrophulariiflora. The populations of both the plants are fragmented and small. Due to collection by human and trampling damage by livestock, the population of A. naviculare was found absent in open areas in five of the six sampling sites and it was confined only within the bushes of alpine scrubs. Sulfamic acid（SA） is a bi-functional, cost-effective and reusable green catalyst for the synthesis of 4-（pyrazol-4-yl）methylenepyrazol-5（4H）-one derivatives by one-pot, three-component condensation of pyrazol-4-carbaxaldehydes, b-ketoesters and phenyl hydrazine（Route-I）. In addition to this method,another simple condensation of pyrazol-4-carbaxaldehydes with pyrazolone in the presence of SA under the solvent-free condition in good yield is reported. The merits of these protocols are mild conditions,non-aqueous workup, high yields, easy availability of the catalyst, no chromatographic separation and inexpensive solid acid catalyst. Furthermore, SA could be recycled and reused for five times without losing its catalytic activity.

Effect of benzaldehyde on the electrodeposition and corrosion properties of Ni–W alloys

The effect of different concentrations of benzaldehyde on the electrodeposition of Ni–W alloy coatings on a mild steel substrate from a citrate electrolyte was investigated in this study. The electrolytic alkaline bath(p H 8.0) contained stoichiometric amounts of nickel sulfate, sodium tungstate, and trisodium citrate as precursors. The corrosion resistance of the Ni–W-alloy-coated specimens in 0.2 mol/L H2SO4 was studied using various electrochemical techniques. Tafel polarization studies reveal that the alloy coatings obtained from the bath containing 50 ppm benzaldehyde exhibit a protection efficiency of 95.33%. The corrosion rate also decreases by 21.5 times compared with that of the blank. A higher charge-transfer resistance of 1159.40 ?·cm2 and a lower double-layer capacitance of 29.4 μF·cm-2 further confirm the better corrosion resistance of the alloy coating. X-ray diffraction studies reveal that the deposits on the mild steel surface are consisted of nanocrystals. A lower surface roughness value(Rmax) of the deposits is confirmed by atomic force microscopy.
