Objective:Conservative approaches in muscle-invasive bladder cancer(MIBC)have been evolved to avoid aggressive surgery,but are limited to elderly,frail,and patients medically unfit for *** study aimed to assess the response rate of neoadjuvant chemotherapy(NACT)before radiotherapy(RT)in MIBC ***:Forty patients with urothelial carcinoma of stage T2-T4a,N0,M0 were enrolled between November 2013 and November 2015,and treated with three cycles of NACT with ***-NACT response was assessed using Response Evaluation Criteria in Solid Tumors(RECIST)*** who achieved complete response(CR)and partial response(PR)>50%were treated with radical RT,and those who had PR50%or CR,and were treated with ***(12.5%)patients who had PR<50%,SD,or PD underwent *** patients who received radiation showed CR after 6 *** follow-up was 43 months(range:10-66 months)and median overall survival(OS)was not ***-year OS,local control,and disease-free survival were 70.1%,60.9%,50.6%,respectively,and 50%of patients preserved their functioning ***-year OS rate was 88.9%in patients who achieved CR to NACT,73.1%in patients with PR≥50%and 40%in patients with PR<50%.Conclusion:NACT followed by RT provides a high probability of local response with bladder preservation in CR *** use of this treatment regimen in carefully selected patients may omit the need for morbid surgery.

A Novel Approach for Numerical Computation of Fokker-Planck Equation

This paper present an implementation of＂modified cubic B-spline differential quadrature method （MCB-DQM）＂ proposed by Arora ＆ Singh （Applied Mathematics and Computation Vol. 224（1）（2013） 161-177） for numerical computation of Fokker-Planck equations. The modified cubic B-splines are used as set of basis functions in the differential quadrature to compute the weighting coefficients for the spatial derivatives, which reduces Fokker-Planck equation into system of first-order ordinary differential equations （ODEs）, in time. The well known SSP-RK43 scheme is then applied to solve the resulting system of ODEs. The efficiency of proposed method has been confirmed by three examples having their exact solutions. This shows that MCB-DQM results are capable of achieving high accuracy. Advantage of the scheme is that it can be applied very smoothly to solve the linear or nonlinear physical problems, and a very less storage space is required which causes less accumulation of numerical errors.

Effect of limited access dressing on surface pH of chronic wounds

Aim:Changes in the pH of chronic wounds can inhibit the optimal activity of various enzymes in the wound environment,thereby delaying wound *** aim of the present study is to monitor the effect of limited access dressing(LAD)on the pH on the surface of chronic ***:A total of 140 patients with chronic wounds of more than 4 weeks duration were divided into two groups by simple randomization:a LAD group(n=64)and a conventional dressing group(n=76).Fifty-six participants(22 in the LAD group and 34 in the conventional dressing group)were lost to follow-up or withdrawn from the ***:In the LAD group(n=42),the mean age was 38.3±10.56 years(range:12-60 years),and the mean wound size at the time of admission was 28 cm^(2)(range:19-40 cm^(2)).In the conventional dressing group(n=42),the mean age was 35.3±14.0 years(range:17-65 years),and the mean wound size at the time of admission was 26 cm^(2)(range:18-39 cm^(2)).Patients treated with LAD showed a significant decrease in the mean±standard deviation pH when compared with the conventional dressing group(0.83±0.52 vs.0.41±0.26,P=0.048).Conclusion:LAD reduces the chronic wound surface pH to a level required for the optimal function of various *** could be a factor that exerts a beneficial effect on wound healing.

An Epidemiological Model Investigating the Association between Mothers Nutritional Status and Low Birth Weight in India

Introduction: Low birth weight (LBW) is the dominating risk factor for infant morbidity and mortality. LBW infants were three times more likely than normal birth weight infants to have neuro developmental complications and congenital abnormalities. The World Health Organization (WHO) has defined the term Low Birth Weight (LBW) as birth weight less than 2500 grams. Objective: To develop epidemiological model investigating the association between mother’s nutritional status and low birth weight in India. Data and Methods: Third round of the National Family Health Survey (NFHS-3) data collected during 2005-2006 is used for this study. This data provides a comprehensive picture of population and health conditions in India. To check the association between variables coefficient of contingency was calculated and multivariable logistic regression model was applied to check independent effect of covariates. Univariate, bivariate and multivariable logistics regression model has been developed to investigate the association between mother’s nutritional status and low birth weight in India. Adjusted odds ratios were calculated with 95% confidence interval. Conclusion: The prevalence of low birth weight was observed high among those women who were underweight, anemic, never visited for any ANC checkup. Emphasis needs to be given to maternal nutritional factors which are more persistent across India than the impact of other factors on birth weight. This can be done by selectively targeting interventions to improve nutrition.

Cope's sign and complete heart block secondary to acute cholecystitis: A case report

Rationale: Cope's sign is reflex bradycardia seen in the patient presenting with symptoms of acute cholecystitis. This bradycardia may be due to vagally mediated cardio-biliary reflex. Many of these reflexes due to acute cholecystitis have similar clinical features (some electrocardiographic changes like bradycardia, complete heart block, and asystole) mimicking that of acute coronary syndrome. Patient's concern: A 60-year old male presented with symptoms of acute cholecystitis and referred to the emergency department with complete heart block and abdominal pain with hypotension requiring an emergency temporary pacemaker. Diagnosis: Cope's sign and complete heart block. Intervention: Emergency temporary cardiac pacemaker insertion. Outcomes: The patient was discharged after three days with regular follow-up and advice for laparoscopic cholecystectomy. Lessons: Complete heart block or any symptomatic bradycardia associated with abdominal pain should be under consideration of cholecystitis that may be associated with either presence or absence of gall stones due to cardio biliary reflex.

The paper deals with the biomass and productivity of sal (SF) and miscellaneous forests (MF) of Satpura plateau (Madhya Pradesh) India. These forest types were divided into four sites namely open miscellaneous (OMF, site-I), closed miscellaneous (CMF, site-II), open sal (OSF, site-III) and closed sal (CSF, site-IV). The degree of disturbance followed the order: III (0.70) 109 (III) > 79.80 (I) > 52.69 (II), while for NPPherb, the order of importance was, 109.50 (IV) > 73.27 (I) > (II), 71.75 (III) > 55.71 (II). NPPtotal was highest for closed forest stands than of the open ones. NPPteak was lower for high-disturbed site than of the less disturbed site. Photosynthetic/ non - photosynthetic ratio follows the order: 0.067 (II) > 0.030 (III) > 0.026 (IV) > 0.018 (I). Open forests showed lower values for this ratio. NEP was higher for SF than of the MF. Further closed forests showed higher values of NEP. OSF showed lower values of NEPsal than of the CSF. Disturbances in open forests not only reduced stand biomass of tree species, dominant species in particular, but also declined the tree productivity. So, gap filling plantation in side the forest is suggested to improve the productivity of open forests.

The Role of Combined OSR and SDF Method for Pre-Processing of Microarray Data that Accounts for Effective Denoising and Quantification

Microarray data is inherently noisy due to the noise contaminated from various sources during the preparation of microarray slide and thus it greatly affects the accuracy of the gene expression. How to eliminate the effect of the noise constitutes a challenging problem in microarray analysis. Efficient denoising is often a necessary and the first step to be taken before the image data is analyzed to compensate for data corruption and for effective utilization for these data. Hence preprocessing of microarray image is an essential to eliminate the background noise in order to enhance the image quality and effective quantification. Existing denoising techniques based on transformed domain have been utilized for microarray noise reduction with their own limitations. The objective of this paper is to introduce novel preprocessing techniques such as optimized spatial resolution (OSR) and spatial domain filtering (SDF) for reduction of noise from microarray data and reduction of error during quantification process for estimating the microarray spots accurately to determine expression level of genes. Besides combined optimized spatial resolution and spatial filtering is proposed and found improved denoising of microarray data with effective quantification of spots. The proposed method has been validated in microarray images of gene expression profiles of Myeloid Leukemia using Stanford Microarray Database with various quality measures such as signal to noise ratio, peak signal to noise ratio, image fidelity, structural content, absolute average difference and correlation quality. It was observed by quantitative analysis that the proposed technique is more efficient for denoising the microarray image which enables to make it suitable for effective quantification.

The hydrophobic effect is the major factor that drives a protein molecule towards folding and to a great degree the stability of protein structures. Therefore the knowledge of hydrophobic regions and its prediction is of great help in understanding the structure and function of the protein. Hence determination of membrane buried region is a computationally intensive task in bioinformatics. Several prediction methods have been reported but there are some deficiencies in prediction accuracy and adaptability of these methods. Of these proteins that are found embedded in cellular membranes, called as membrane proteins, are of particular importance because they form targets for over 60% of drugs on the market. 20-30% of all the proteins in any organism are membrane proteins. Thus transmembrane protein plays important role in the life activity of the cells. Hence prediction of membrane buried segments in transmembrane proteins is of particular importance. In this paper we have proposed signal processing algorithms based on digital filter for prediction of hydrophobic regions in the transmembrane proteins and found improved prediction efficiency than the existing methods. Hydrophobic regions are extracted by assigning physico-chemical parameter such as hydrophobicity and hydration energy index to each amino acid residue and the resulting numerical representation of the protein is subjected to digital low pass filter. The proposed method is validated on transmembrane proteins using Orientation of Proteins in Membranes (OPM) dataset with various prediction measures and found better prediction accuracy than the existing methods.

Interferon Regulatory Factor-2 (IRF-2) belongs to IRF family, was identified as a mammalian transcription factor involved in Interferon beta (IFNβ) gene regulation. Besides that IRF-2 is involved in immunomodulation, hematopoietic differentiation, cell cycle regulation and oncogenesis. We have done molecular sub-cloning and expression of recombinant murine IRF-2 as GST (Glutathione-S-Transferase)- IRF-2 fusion protein in E. coli/XL-1blue cells. Recombinant IRF-2 with GST moiety at N-terminus expressed as GST-IRF-2 (~66 kd) in E. coli along with different low molecular mass degradation products revealed approximately 30, 42, 60 and 62 kd by SDS-PAGE and Western blot, respectively. We further confirm that degradation takes place at C-terminus of the fusion protein not at N-terminus as anti-GST antibody was detecting all bands in the immunoblot. The recombinant IRF-2 was biologically active along with their degradation products in terms of their DNA binding activity as assessed by Electrophoretically Mobility Shift Assay (EMSA). We observed three different molecular mass DNA/protein complexes (1 - 3) with Virus Response Element (VRE) derived from human Interferon IFNβ gene and five different molecular mass complexes (1 - 5) with IRF-E motif (GAAAGT)4 in EMSA gel. GST only expressed from empty vector did not bind to these DNA elements. To confirm that the binding is specific, all complexes were competed out completely when challenged with 100-X fold molar excess of IRF-E oligo under cold competition. It means degradation products along with full-length protein are able to interact with VREβ as well as IRF-E motif. This means degradation products may regulate the target gene (s) activation/repression via interacting with VRE/IRF-E.
