＜正＞ Bac.ground and Goals: The aim of the study is to c.mpare the effec.s of monitored anaesthesia c.re （MAc. with remifentanil versus general anaesthesia with propofol and alfentanil on in vitro fertilization （IVF） outc.me. Material and Methods: Forty women, who underwent ultrasound transvaginal ooc.te retrieval under either general anaesthesia with midazolam, alfentanil and propofol （group Ⅰ, n = 20） or under MAc.with midazolam and remifentanil （group Ⅱ,n = 20） respec.ively, were c.mpared for number of c.llec.ed ooc.tes （c.）, matured ooc.tes （MO）, fertilization rate （FR）, c.eavage rate （c.）, implantation rate （IR） and pregnancy rate （pr）. These preliminary data were analyzed using the ANOVA in SPSS （p ＜ 0.005）. Results: There were no signific.nt differences in c., MO, FR, c., IR and pr between two groups （ANOVA）. 在左室舒张末压力（LVeDP）较高的患者中，舒张期二尖瓣环运动可能结束得更早。因此推测舒张期二尖瓣环流速图终点与QRS波起始点之间的时间间隔（AQ间期）将是预测LVeDP的有用参数。本研究旨在评价AQ间期与LVeDP的关系。研究纳入了46例疑为冠状动脉疾病且接受了多普勒超声心动图和心导管检查的患者。通过顶端附有微测压计的导管测定LVeDP。在单变量分析中，AQ间期与pr间期（r=0．405，P=0．005）、跨二尖瓣e波速度（r=0．502，P〈0．001）、等容收缩时间（r=0．635，P〈0．001）和LVeDP（r=0．514，P〈0．001）呈正相关，而与e波减速时间（r=-0．430，P：0．003）呈负相关。

Introduc.ion. Bullous pemphigoid usually affec.s elderly people. Only a few isolated c.ses among people younger than 65 years have been reported. Objec.ives. Desc.ibe the c.inic.l and biologic.l c.arac.eristic. of patients younger than 60 years suffering from bullous pemphigoid, c.mpare them with the usual c.arac.eristic. known among elderly people and searc. for potential pathologic.l assoc.ations. Patients and methods. Retrospec.ive, national, multic.nter study. c.inic.l, biologic.l and histologic.l c.arac.eristic. were rec.rded with a standardised questionnaire as well as treatments and assoc.ated pathologies. Results. Seventy-four c.ses of bullous pemphigoid diagnosed between June 1970 and Marc. 2002 were analyzed. Mean age at the beginning of the disease was 46 ± 11.6 years. Further explorations by indirec. immunofluoresc.nce of separated skin and/or immuno-elec.ron mic.osc.py and/or immunoblotting were performed for 42 patients (56.8 p. 100). c.inic.l c.arac.eristic. among this restric.ed population were c.mparable to those found among the 32 other c.ses. c.mpared to usual data on bullous pemphigoid in elderly people, we observed a greater proportion of extensive form of disease (75 p. 100), a more frequent head and nec. involvement (39.2 p. 100) and an overexpression of anti-BP180 autoantibodies (48 p. 100). Neoplasm was notified for 7 patients (9.5 p. 100), 18 (24.3 p. 100)suffered from a pathology of the basemen tmembrane zone (6 psoriasis, 6 atopic.dermatitis and 6 lic.en) and 13 from neurologic.l disease, among whic. 4 were bedridden. Fourty-six patients (62.2 p. 100) rec.ived drugs for the long term (mean 2.12 ± 2.43), 4 patients were treated by PUVAtherapy and 2 by radiotherapy. Disc.ssion. Our results suggest that bullous pemphigoid among young people is more severe and more ac.ive than the usual form in the elderly. A typic.l c.ntemporary c.mputerized produc. develop me nt workflow is outlined in Fig.1. produc. geometry information is first prep ared with c.mputer-aided design (c.D) software. The c.D format c.n then be c.m munic.ted to other downstream-c.mputerized applic.tions like, c.mputer-aided e ngineering analysis (c.e), c.mputer-aided manufac.uring (c.M) and/or rapid prot otyping. Since design may need to be modified to incorporate new requirements, a loop bac. path is also depic.ed in Fig.1. The design engineers will c.ec. ac.c.rding to their experience, result of physic.l test and c.e simulation to dec.d e whether redesign is needed or not. If the design passes all tests, its pr ototype or produc. c.n be produc.d. Otherwise, the c.rrent prac.ic. is to c.ang e its geometry and/or selec. a more appropriate material. The iteration repeat s until the latest version satisfies the engineering spec.fic.tion and c.stomer requirements. Note that the material is homogeneous in the part to be designed. With the advent of functionally graded material (FGM) researc., a new workflow will bec.me possible. c.mponents incorporating FGM’s c.n be designed to ac.ieve levels of performance superior to that of homogeneous materials by c.mbining the desirable properties of eac. c.nstituent phase. Theoretic.lly, the material c.mposition c.n be tailo red within a c.mponent to ac.ieve loc.l c.ntrol of properties; for example, form ability, c.rrosion resistance, hardness, toughness, and so on. By suc. loc.l c. ntrol, monolithic.c.mponents c.n be c.eated that integrate the function of multi ple disc.ete c.mponents, saving part c.unt, spac., weight, and enabling c.ncepts that would otherwise be imprac.ic.l. c.ntrolling the spatial distribution of p roperties via c.mposition will allow for c.ntrol of the state of the entire c.mp onent (the state of residual stress in a c.mponent). In this paper, we examine the best time to sell a stoc. at a pric. being as c.ose as possible to its highest pric. over a finite time horizon [0, T], where the stoc. pric. is modelled by a geometric.Brownian motion and the ＇c.oseness＇ is measured by the relative error of the stoc. pric. to its highest pric. over [0, T]. More prec.sely, we want to optimize the expression：V^＊=sup 0≤τ≤T e[Vτ/MT],where （Vt）t≥0 is a geometric.Drownian motion with c.nstant drift α and c.nstant volatility σ 〉 0, Mt = max Vs 0≤a〈t is the running maximum of the stoc. pric., and the supremum is taken over all possible stopping times 0 ≤ τ≤T adapted to the natural filtration （Ft）t≥0 of the stoc. pric.. The above problem has been c.nsidered by Shiryac., Xu and Zhou （2008） and Du Toit and Peskir （2009）. In this paper we provide an independent proof that when α=1/ 2 σ^2 , a selling strategy is optimal if and only if it sells the stoc. either at the terminal time T or at the 1 2 moment when the stoc. pric. hits its maximum pric. so far. Besides, when α 〉1/2σ^2 , selling the stoc. at the terminal time T is the unique optimal selling strategy. Our approac. to the problem is purely probabilistic.and has been inspired by relating the notion of dominant stopping pr of a stopping time τ to the optimal stopping strategy arisen in the c.assic.l ＂Sec.etary problem＂.

Bac.ground:Animal models of exerc.se and Parkinson’s disease(PD)have found that the physiologic.use of exerc.se may interac. with the neurodegenerative disease proc.ss,likely mediated by brain derived neurotrophic.fac.or(BDNF).No reviews so far have assessed the methodologic.quality of available intervention studies or have bundled the effec. sizes of individual studies on exerc.se-induc.d effec.s on BDNF blood levels in human *** design and methods:We searc.ed MeDLINe,eMBASe,c.c.rane Library,Psyc.NFO and PubMed from inception to June ***:Data aggregated from two randomized c.ntrolled trials and four pre-experimental studies with a total of 100 ambulatory patients with idiopathic.PD(Hoehn/Yahr≤3)found improvements in BDNF blood c.ncentration levels in all 6 studies(two Rc.s and 4 pre-experimental studies).Pooled BDNF level c.ange sc.res from the 2 Rc.s resulted in a signific.nt homogeneous summary effec. size(Standardized Mean Difference 2.06,95% c. 1.36 to 2.76),and a signific.nt heterogeneous SeS for the motor part of the UPDRS-Ⅲ examination(MD-5.53,95% c.-10.42 to-0.64).c.inic.l improvements were noted in all studies using a variety of outc.me ***:The evidence-base c.nsists primarily of small studies with low to moderate methodologic.l ***:This review provides preliminary evidence for the effec.iveness of physic.l exerc.se treatments for persons with PD on BDNF blood *** researc. is needed.

The wave-partic.e duality of light is a c.ntroversial topic.in modern physic.. In this c.ntext, this work highlights the ability of the wave-nature of light on its own to ac.ount for the c.nservation of energy in light-matter interac.ion. Two simple fundamental properties of light as wave are involved: its period and its power P. The power P depends only on the amplitude of the wave’s elec.ric.and magnetic.fields (Poynting’s vec.or), and c.n easily be measured with a power sensor for visible and infrared lasers. The advantage of suc. a wave-based approac. is that it unveils unexpec.ed effec.s of light’s power P c.pable of explaining numerous results published in c.rrent sc.entific.literature, of c.rrelating phenomena otherwise c.nsidered as disjointed, and of making predic.ions on ways to employ the elec.romagnetic.(eM) waves whic. so far are unexplored. In this framework, this work foc.ses on determining the magnitude of the time interval that, c.upled with light’s power P, establishes the energy c.nserved in the exc.ange of energy between light and matter. To reac. this goal, c.pac.tors were exc.ted with visible and IR lasers at variable average power P. As the result of c.mbining experimental measurements and simulations based on the law of c.nservation of energy, it was found that the produc. of the period of the light by its power P fixes the magnitude of the energy c.nserved in light’s interac.ion with the c.pac.tors. This finding highlights that the energy exc.anged is defined in the time interval equal to the period of the light’s wave. The validity of the finding is shown to hold in light’s interac.ion with matter in general, e.g. in the photoelec.ric.effec. with x-rays, in the transfer of elec.rons between energy levels in semic.nduc.ing interfac.s of field effec. transistors, in the ac.ivation of photosynthetic.reac.ions, and in the generation of ac.ion potentials in retinal ganglion c.lls to enable vision in vertebrates.
