Immediate effects of acupuncture on gait patterns in patients with knee osteoarthritis

Background Acupuncture has been shown to be effective in pain relief and anesthesia, and has been suggested for treating various kinds of functional disabilities in traditional Chinese medicine, including knee osteoarthritis （CA）. The study aimed to investigate the immediate effects of acupuncture on gait patterns in patients with knee CA. Methods Twenty patients with bilateral medial knee CA were assigned evenly and randomly to a sham group and an experimental group. During the experiment, the experimental group underwent a 30-minute formula electro-acupuncture treatment while the sham group received a sham treatment. Before and after treatment, each subject was evaluated for their knee pain using visual analog scales （VAS） and then their performance of level walking using gait analysis. For all the obtained variables, the independent t-test was used for between-group comparisons, while paired t-test was used to investigate the before and after changes. Results All the measured data before acupuncture treatment between the groups were not significantly different. The VAS scores were decreased significantly after acupuncture in both groups, and the mean change of the VAS values of the experiment group was 2 times greater than that of the sham group. After formula acupuncture stimulation, while no significant changes were found in all the gait variables in the sham group, the experimental group had significant increases in the gait speed, step length, as well as in several components of the joint angles and moments. Conclusions The results of the study suggest that significantly improved gait performance in the experimental group may be associated with pain relief after treatment, but the relatively small decrease of pain in the sham group was not enough to induce significant improvements in gait patterns. We discuss a unified model of quark confinement and new cosmic expansion with linear potentials based on a general（SU3）color×（U1）baryon symmetry. The phase functions in the usual gauge transformations are generalized to new ‘action integrals＇. The general Yang-Mills transformations have group properties and reduce to usual gauge transformations in special cases. Both quarks and ‘gauge bosons＇ are permanently confined by linear potentials. In this unified model of particle-cosmology, physics in the largest cosmos and that in the smallest quark system appear to both be dictated by the general Yang-Mills symmetry and characterized by a universal length. The basic force between two baryons is independent of distance. However, the cosmic repulsive force exerted on a baryonic supernova by a uniform sphere of galaxies is proportional to the distance from the center of the sphere. The new general YangMills field may give a field-theoretic explanation of the accelerated cosmic expansion. The prediction could be tested experimentally by measuring the frequency shifts of supernovae at different distances.

Based on a generalized Yang-Mills framework, gravitational and strong interactions can be unified in analogy with the unification in the electroweak theory. By gauging T （4） × [SU （3）] color in flat space-time, we have a unified model of chromo-gravity with a new tensor gauge field, which couples universally to all gluons, quarks and anti-quarks. The space-time translational gauge symmetry assures that all wave equations of quarks and gluons reduce to a Hamilton-Jacobi equation with the same ‘effective Riemann metric tensors’ in the geometric-optics （or classical） limit. The emergence of ef f ective metric tensors in the classical limit is essential for the unified model to agree with experiments. The unified model suggests that all gravitational, strong and electroweak interactions appear to be dictated by gauge symmetries in the generalized Yang-Mills framework.

Coupled with a petrographical study, I carried out an ion probe study of rare earth element microdistributions in mineral phases of silicate inclusions from the Colomera ⅡE iron meteorite. Most mineral grains have homogeneous REEs, but show considerable inter-grain variations by a factor of 2 to 100. The whole rock REE abundances for Colomera, estimated by combining REE data with modal abundances, are relatively LREE-enriched with REEs of -10’CI, which suggest that Colomera silicates were highly differentiated and might represent a low degree partial melt (-10%) of a chondritic source. REE geochemistry of Colomera silicate inclusions points to an origin that involves differentiation, dynamic mixing, remelting, reduction, recrystallization, and subsequent rapid cooling near the surface of a planetary body.

Working as an ensemble method that establishes a committee of classifiers first and then aggregates their out- comes through majority voting, bagging has attracted consid- erable research interest and been applied in various applica- tion domains. It has demonstrated several advantages, but in its present form, bagging has been found to be less accurate than some other ensemble methods. To unlock its power and expand its user base, we propose an approach that improves bagging through the use of multi-algorithm ensembles. In a multi-algorithm ensemble, multiple classification algorithms are employed. Starting from a study of the nature of diver- sity, we show that compared to using different training sets alone, using heterogeneous algorithms together with different training sets increases diversity in ensembles, and hence we provide a fundamental explanation for research utilizing het- erogeneous algorithms. In addition, we partially address the problem of the relationship between diversity and accuracy by providing a non-linear function that describes the relation- ship between diversity and correlation. Furthermore, after re- alizing that the bootstrap procedure is the exclusive source of diversity in bagging, we use heterogeneity as another source of diversity and propose an approach utilizing heterogeneous algorithms in bagging. For evaluation, we consider several benchmark data sets from various application domains. The results indicate that, in terms of Fl-measure, our approach outperforms most of the other state-of-the-art ensemble meth- ods considered in experiments and, in terms of mean margin, our approach is superior to all the others considered in experiments.

Photoluminescence （PL） characteristics of Tb-doped silicon rich oxide （SRO） films prepared by DC-sputtering and post-annealing processes were studied. The silicon richness of the SRO film could be controlled by varying the sputtering power and oxygen concentration in the sputtering chamber. PL emission from the as-deposited sample was found to be composed of Th^3 ＋ intra 4f transition-related emission and the silicon nano particle-related broad bandwidth emission. Thermal annealing could significantly improve the material properties as well as the PL signals. PL properties depended strongly upon the annealing scheme and silicon richness. Annealing at high temperatures （900- 1050℃） enhanced Tbrelated emission and suppressed the silicon nano particle-related emission. For samples with different silicon richness, annealing at 950 ℃ was found to produce higher PL signals than at other temperatures. It was attributed more to lifetime quenching than to concentration quenching. Electroluminescent （EL） devices with a capacitor structure were fabricated, the optimized process condition for the EL device was found to be different from that of PL emission. Among the annealing schemes that were used, wet oxidation was found to improve device performance the most, whereas, dry oxidation was found to improve material property the most. Wet oxidation allowed the breakdown electrical field to increase significantly and to reach above 10 mV·cm^-1. The EL spectra showed a typical Th^3＋ emission, agreeing well with the PL spectra. The I-V measurements indicated that for 100 nm thick film, the Fowler-Nordheim tunneling started at an electrical field of around 6 mV·cm^-1 and the light emission became detectable at a current density of around 10-4 A·cm^-2 and higher. Strong electroluminescence light emission was detected when the electrical field was close to 10 mV·cm^-1.

BACKGROUND Majority of adhesive small bowel obstruction(SBO)cases can be managed ***,a proportion of patients failed non-operative *** To evaluate the predictors of successful non-operative management in adhesive *** A retrospective study was performed for all consecutive cases of adhesive SBO from November 2015 to May *** collated included basic demographics,clinical presentation,biochemistry and imaging results and management *** imaging studies were independently analyzed by a radiologist who was blinded to the clinical *** patients were divided into group A operative(including those that failed initial non-operative management)and group B non-operative for *** Of 252 patients were included in the final analysis;group A(n=90)(35.7%)and group B(n=162)(64.3%).There were no differences in the clinical features between both *** tests of inflammatory markers and lactate levels were similar in both *** the imaging findings,the presence of a definitive transition point[odds ratio(OR)=2.67,95%confidence interval(CI):0.98-7.32,P=0.048],presence of free fluid(OR=2.11,95%CI:1.15-3.89,P=0.015)and absence of small bowel faecal signs(OR=1.70,95%CI:1.01-2.88,P=0.047)were predictive of the need of surgical *** patients that received water soluble contrast medium,the evidence of contrast in colon was 3.83 times predictive of successful non-operative management(95%CI:1.79-8.21,P=0.001).CONCLUSION The computed tomography findings can assist clinicians in deciding early surgical intervention in adhesive SBO cases that are unlikely to be successful with non-operative management to prevent associated morbidity and mortality.
