Vildagliptin-insulin combination improves glycemic control in Asians with type 2 diabetes

AIM: To assess the efficacy and safety of vildagliptin 50 mg bid as add-on therapy to insulin in Asian patients with type 2 diabetes mellitus(T2DM).METHODS: This was a post hoc analysis of a subgroup of Asian patients from a multicenter,randomized,double-blind,placebo-controlled,parallel-group study in T2DM patients inadequately controlled by stable insulin therapy,with or without metformin.A total of 173 patients were randomized 1:1 to receive treatment with vildagliptin 50 mg bid(n = 87) or placebo(n = 86) for 24 *** in HbA1c and fasting plasma glucose(FPG),from baseline to study endpoint,were analyzed using an analysis of covariance *** from baseline to endpoint in body weight was summarized by *** and tolerability of vildagliptin was also ***: After 24 wk,the difference in adjusted mean change in HbA1c between vildagliptin and placebo was 0.82%(8.96 mmol/mol;P < 0.001) in Asian subgroup,0.85%(9.29 mmol/mol;P < 0.001) in patients also receiving metformin,and 0.73%(7.98 mmol/mol;P < 0.001) in patients without metformin,all in favor of *** was no significant difference in the change in FPG between *** was stable in both treatment groups(+0.3 kg and-0.2 kg,for vildagliptin and placebo,respectively).Overall,vildagliptin was safe and well tolerated with similarly low incidences of hypoglycemia(8.0% vs 8.1%) and no severe hypoglycemic events were experienced in either ***: In Asian patients inadequately controlled with insulin(with or without concomitant metformin),insulin-vildagliptin combination treatment significantly reduced HbA1c compared with placebo,without an increase in risk of hypoglycemia or weight gain.

Quantitative measures for assessment of the hydraulic excavator digging efficiency

In this paper,quantitative measures for the assessment of the hydraulic excavator digging efficiency are proposed and *** following factors are considered:(a) boundary digging forces allowed for by the stability of an excavator,(b) boundary digging forces enabled by the driving mechanisms of the excavator,(c) factors taking into consideration the digging position in the working range of an excavator,and(d) sign and direction of potential digging resistive force.A corrected digging force is defined and a mathematical model of kinematic chain and drive mechanisms of a five-member excavator configuration was developed comprising:an undercarriage,a rotational platform and an attachment with boom,stick,and *** the basis of the mathematical model of the excavator,software was developed for computation and detailed analysis of the digging forces in the entire workspace of the *** using the developed software,the analysis of boundary digging forces is conducted and the corrected digging force is determined for two models of hydraulic excavators of the same mass(around 17 000 kg) with identical kinematic chain parameters but with different parameters of manipulator driving *** results of the analysis show that the proposed set of quantitative measures can be used for assessment of the digging efficiency of existing excavator models and to serve as an optimization criterion in the synthesis of manipulator driving mechanisms of new excavator models.

Abnormalities of masseter inhibitory reflex in patients with episodic tension-type headache

Objective： To investigate the masseter inhibitory tension-type headache （TTH）. Methods： MIR was studied in 21 sex matched to the study cohort. Median age of patients was reflex （MIR） and its eventual changes in patients with episodic patients with episodic TTH and 30 healthy subjects, with age and 17.0 years （ranged 16-49 years）, median duration of disease 12 months （1-5 years）, and median frequency of headache 7.5 d per month. Results： The second period of suppression （$2） of MIR was reduced in intensity and duration in 10% of controls and 66.7% （confidence interval （CI） 45.3%-85%; P〈0.05） of patients with episodic TTH （X2=74.9; P〈0.001）. In 3 （14.3%） of patients with episodic TTH, S2 was completely absent. No significant correlation between the duration of disease and headache frequency was found. Conclusion： Our results confirm the link between episodic TTH and reduction or absence of S2. Teenage patients with episodic TTH may exhibit marked pathological changes in S2 in contrast to older individuals.

Self-healing effect of ceria electrodeposited thin films on stainless steel in aggressive 0.5 mol/L NaCl aqueous solution

The self-healing effect of electrochemically deposited CeO2-Ce2O3 films on stainless steel OC404（SS） in 0.5 mol/L NaCl solution was studied. It was established that the corrosion potential of the steel, after covering it with CeO2-Ce2O3 layer and thermal treatment（i.e. potential of the system CeO2-Ce2O3/SSt.t.）, was shifted sharply to a considerably more positive value, while the corrosion current was reduced noticeably. The X-ray photoelectron spectroscopy（XPS）, energy dispersive spectroscopy（EDS） and scanning electron microscopy（SEM） analyses on the observed scratched surface area of the system CeO2-Ce2O3/SSt.t., after exposure of investigated specimens to 0.5 mol/L NaCl corrosion media, showed partial accumulation of ceria, as well as remarkable increase in the concentrations of oxides of Al, Cr and Fe, on the mechanically revealed steel surface. On the basis of the obtained results one could conclude that the secondary passive oxide/hydroxide films, formed after a definite time interval of exposure to corrosion media, acted as barriers, hindering the corrosion processes in active zones. A hypothesis was put forward about the mechanism of self-repairing oxide layers on the steel surface and their corrosion protection effect respectively.

We survey some results on travel time tomography. The question is whether we can determine the anisotropic index of refraction of a medium by measuring the travel times of waves going through the medium. This can be recast as geometry problems, the boundary rigidity problem and the lens rigidity problem. The boundary rigidity problem is whether we can determine a Riemannian metric of a compact Riemannian manifold with boundary by measuring the distance function between boundary points. The lens rigidity problem problem is to determine a Riemannian metric of a Riemannian manifold with boundary by measuring for every point and direction of entrance of a geodesic the point of exit and direction of exit and its length. The linearization of these two problems is tensor tomography. The question is whether one can determine a symmetric two-tensor from its integrals along geodesics. We emphasize recent results on boundary and lens rigidity and in tensor tomography in the partial data case, with further applications.

We establish several fundamental identities, including recurrence relations, degree elevation formulas, partition of unity and Marsden identity, for quantum Bernstein bases and quantum Bezier curves. We also develop two term recurrence relations for quantum Bernstein bases and recursive evaluation algorithms for quantum Bezier curves. Our proofs use standard mathematical induction and other elementary techniques.

Atypical presentation of pioderma gangrenosum complicating ulcerative colitis: Rapid disappearance with methylprednisolone

Piodermal gangrenosum (PG) is an uncommon ulcerative cutaneous dermatosis associated with a variety of systemic diseases, including inflammatory bowel disease (IBD), arthritis, leukaemia, hepatitis, and primary billiary cirrhosis. Other cutaneous ulceration resembling PG had been described in literature. There has been neither laboratory finding nor histological feature diagnostic of PG, and diagnosis of PG is mainly made based on the exclusion criteria. We present here a patient, with ulcerative colitis (UC) who was referred to the emergency section with a large and rapidly evolving cutaneous ulceration. Laboratory and microbiological investigation associated with histological findings of the ulcer specimen allowed us to exclude autoimmune and systemic diseases as well as immuno-proliferative disorders. An atypical presentation of PG with UC was diagnosed. Pulse boluses of i.v. methyl-prednisolone were started, and after tapering steroids, complete resolution of the skin lesion was achieved in 3 wk. The unusual rapid healing of the skin ulceration with steroid mono-therapy and the atypical cutaneous presentation in this patient as well as the risk of misdiagnosis of PG in the clinical practice were discussed.

Olfactory neuroblastoma (esthesioneuroblastoma, ЕNB) is a rare tumor arising from the olfactory neuroepithelium. We report a case of ЕNB located in inferior nasal concha, combined with thyroid gland carcinoma and gastrointestinal stromal carcinoma in a 77-year-old man. The tumor was resected endonasally. When the final diagnosis of olfactory neuroblastoma was confirmed by histopathologic examination and immunohistochemical staining, the PET/CT examination was performed. The imaging revealed a small focus of a moderately increased cancer activity in the thyroid region. A gastrointestinal stromal carcinoma was detected one year after the resection of the thyroid gland. We discuss the clinical appearance of ENB, staging systems, diagnosis and management. During the endonasal surgery, ENB was removed entirely. Seven days after operation, in order to monitor the postoperative result, PET/CT was performed and a papillary thyroid cancer was detected. One year after the thyroid surgery, gastroendoscopy showed a neoplastic formation in the stomach. In conclusion, we state that when identified as aggressive tumors such as ENB, it is necessary to provide regular examinations in order to detect distant ENB metastases or other neoplastic localisations.
