Background:Evidence on the health benefits of occupational physical activity(OPA)is *** examined the associations of baseline OPA and OPA changes with all-cause,cardiovascular disease(CVD),and cancer mortality and survival ***:This study included prospective and longitudinal data from the MJ Cohort,comprising adults over 18 years recruited in 1998-2016,349,248 adults(177,314 women)with baseline OPA,of whom 105,715(52,503 women)had 2 OPA measures at 6.3±4.2 years(mean±SD)*** were baseline OPA,OPA changes,and baseline leisure-time physical ***:Over a mean mortality follow-up of 16.2±5.5 years for men and 16.4±5.4 years for women,11,696 deaths(2033 of CVD and 4631 of cancer causes)in men and 8980 deaths(1475 of CVD and 3689 of cancer causes)in women *** moderately heavy/heavy baseline OPA was beneficially associated with all-cause mortality in men(multivariable-adjusted hazard ratio(HR)=0.93,95%confidence interval(95%CI):0.89-0.98 compared to light OPA)and women(HR=0.86,95%CI:0.79-0.93).Over a mean mortality follow-up of 12.5±4.6 years for men and 12.6±4.6 years for women,OPA decreases in men were detrimentally associated(HR=1.16,95%CI:1.01-1.33)with all-cause mortality,while OPA increases in women were beneficially(HR=0.83,95%CI:0.70-0.97)associated with the same *** or changes in OPA showed no associations with CVD or cancer ***:Higher baseline OPA was beneficially associated with all-cause mortality risk in both men and *** longitudinal OPA analyses partly confirmed the prospective findings,with some discordance between sex groups.

Treatment of Vulvovaginal Candidiasis with A Strategy Based on Phototherapy (M.A.C.^® Scar Acceleration Method)

Candidiasis, also known as candidiasis vulvovaginitis, is an infection caused by different types of Candida fungi, the most frequent being Candida albicans. The present study reports an effective strategy, which opens new avenues for the treatment of this public health problem. The MAC® Methodology, conventional laser light-emitting (LLLT)/LED) methods are based on the biphasic response demonstrated many times in LLLT research and as with other forms of drugs, a “drug” (irradiation parameters) and a “dose” (irradiation times) and the “Arndt-Schulz Law” is often cited as a suitable model to describe the dose-dependent effects of LLLT. This method uses photopharmaceuticals, cell markers and the use of correct parameters for each case to induce the acceleration of tissue repair. The present study shows a case of a 32-year-old patient diagnosed with recurrent candidiasis 4 years ago. Eighteen sessions were performed (every other day) using a photoactivated component (Methylene blue 1% + Clotrimazole 1%) and LED phototherapy (red, blue and violet) with emission times of 60 - 260 seconds for each applicator, according to the dose recommendations of the scar acceleration method (MAC®). At the sixth treatment session there was a noticeable decrease in the itching sensation reported by the patient. In session 11 she reported feeling a great improvement, indicating that she no longer felt itching in any area after 18 sessions. The present case demonstrates new methodologies to treat common problems in the population that have a positive impact on the quality of life. This methodology has a promising future because it is non-invasive and requires a great biological transformation for inflammatory, fungal and viral control.

*** risk of recurrence has limited the acceptability of conservative therapies of gallbladder *** aim of the present study was to determine the long-term rate of stone recurrence and its risk factors after successful extracorporeal shock-wave lithotripsy(ESWL).Material and *** study comprised a prospective ultrasound follow-up at yearly intervals or whenever biliary pain was reported.A total of 192 consecutive patients(primary single stones,n = 159;primary 2 or 3 stones,n = 33)were followed for up to 11.2 years after becoming stone-free and after termination of adjuvant treatment with ursodeoxycholic acid(UDCA).***-four patients developed recurrent stones after a median of 2.6 years(maximum = 8.8 years).The 108 patients without recurrence were followed for a median of 6.7 years(maximum = 11.2 years).By actuarial analysis,the cumulative recurrence rates for these 192 stone-free patients were 27%±3%,41%±4%and 54%±4%(observed ±SE)at 3,5 and 10 years,***’s regression analysis was used to identify the presence of slight calcification in the primary stone(s)as a protective feature against recurrence(p = 0.03).Conclusions.1)The risk of recurrence continues to increase over time,and although it rises less steeply after 5 years,it does not reach a plateau until at least 10 years.2)Having had slightly calcified stone(s)seems to be associated with a reduced risk of recurrence and might signal a "burnt out" lithogenic process.3)The long-term results are unsatisfactory and ESWL of gallbladder stones should be offered only in special cases.

目的哮喘-慢性阻塞性肺疾病肺重叠症(Asthma-chronie obst ructive pulmonary disease overlap,A-CO)患者的生活质量较低,肺功能较差,病情经常加重。由于暴露于污染环境会加重其症状,增加发病率和病死率,环境管理对其至关重要。旨在通过系统性综述,寻找证据,以指导制订护理干预措施,促进ACO患者对空气质量的自我管理。方法2023年3月 ,系统检索CINAHL、Medline、Academie Search Complete .Scopus、WebofScience、Cochrane图书馆、乔安娜布里格斯研究所(JoannaBriggsInstitute,JBI)循证实践数据库,以及GoogleScholar。对纳人研究进行数据提取和综合,并对护理干预措施进行分类。采用JBI研究质量评估工具进行方法学质量评估。结果共纳入5篇来自不同国家的研究。促进ACO患者自我监控空气质量的干预措施包括:警觉性干预措施(医务人员定期巡诊、进行症状评估),监测干预措施(监测室内外诱发因素) ,教育干预措施(空气质量警报、避免接触过敏原),以及相关政策的影响,如无烟政策和改善空气质量的综合战略。尽管这些研究报告了不同的干预措施,方法质量也存在差异,但这些干预措施显示了预防患者病情恶化、尽量减少急诊就诊和住院治疗的潜力。这些关键点可作为护理干预措施的关键组成部分。结论有必要采取全面措施,让护士参与促进患者自我监控空气质量的多学科团队.提高认识、制订和实施护理干预,从而使ACO患者受益。同时,需要进--步开展研究,以扩大证据基础并完善这些干预措施。

Acute diverticulitis in younger patients:Any rationale for a different approach?

AIM:To compare the natural history and course of acute diverticulitis in a younger age group with an older population and to evaluate whether younger patients should be managed differently. METHODS:This study was a retrospective review of 157 patients treated with acute diverticulitis between January 1,2004 and December 31,*** was stratified according to the Hinchey classification. Patients were divided into 2 populations:group A≤ 50 years(n=31) ;group B>50 years(n=126) .Mean patient follow-up was 15 mo. RESULTS:The median age was 60 years.A significantly higher proportion of patients in group B presented with complicated diverticulitis(36.5%vs 12.9%,P=0.01) .Recurrence was more frequent in group A(25.8%vs 11.1%,P=0.03) and the mean time-torecurrence was shorter(12 mo vs 28 mo,P=0.26) . The most severe recurrent episodes of acute diverticulitis were classified as Hinchey stageⅠand none of the patients required emergency *** multivariate analysis,only age(P=0.024) was identified as an independent prognostic factor for recurrence. CONCLUSION:Based on the results of this study,we recommend that diverticulitis management should be based on the severity of the disease and not on the age of the patient.

Tetrasomy of the short arm of chromosome 9 constitutes a rare condition resulting in a well clinically recognized syndrome. In our case, in addition to the characteristic phenotype at birth, the existence of a hernia-type Bochdalek diaphragmatic defect was found. Cytogenetic analysis revealed a nonmosaic case of an isochromosome of the entire short arm of chromosome 9 with no involvement of the heterochromatic region of the long arm: 47, XX, + i (9p). Because chromosome 9 contains several gene locus for enzymes and receptors of the retinoid pathway, this case potentially contributes to retinoid hypothesis in the etiology of congenital diaphragmatic hernia.

The extraction of the physical parameters of long period gratings from the spectral response is not an easy process. We present a demodulation technique to synthesize the physical parameters of a long period grating recorded in an optical fibre. The demodulation is achieved through the implementation of a genetic algorithm. The extracted parameters are in agreement with the typical values known for long period gratings.

Fertility Care Provided by a Public Health Hospital to Viral-Infected Couples: A Case-Control Study

Objective: To evaluate the results of ART (clinical pregnancies and baby home rates) in couples infected with HBV, HCV, HIV or mixed in a Referral center. Patients: The study is a retrospective case-control study comprising 1587 cycles/1064 couples in the control group and 237 cycles/164 couples in the infected group, submitted to in vitro fertilization, intracytoplasmic sperm injection and embryo transfer. Two branches from the infected group were created: the HIV subgroup, comprising couples with one or both partners infected with HIV-1 (84 cycles/67 couples), and the HCV subgroup, including at least one partner infected with HCV (45 cycles/29 couples). Results: The infected group presented higher infertility duration (P P P P P = 0.01) subgroups. Oocytes and oocytes in metaphase II numbers were lower in the infected group, but not in the HIV and HCV subgroups. Clinical pregnancy rate was lower in the infected group (25.74%) when compared to the control (34.66%), probably due to the contribution of HCV individuals (17.78%), but not of HIV (28.57%).
