Efficacy of acupuncture on cancer pain:A systematic review and meta-analysis

background:Pain associated with cancer is one of the greatest causes of reduced quality of life in patients.Acupuncture is one of the treatments used to address this issue,with the great advantage of having little or no side effects,especially when compared with pharmacological pain-killers.Objective:The aim of this systematic review and meta-analysis was to evaluate the current evidence regarding the efficacy of acupuncture for cancer pain.Search strategy:Six electronic databases(PubMed,EbSCO,Cochrane Library,Scielo,b-On and Scopus)were searched for relevant articles about pain relief in cancer patients from their beginning until 2022using MeSH terms such as“acupuncture,”“electroacupuncture,”“ear acupuncture,”“acupuncture analgesia,”“oncological pain,”and“cancer pain.”Inclusion criteria:Studies included were randomized controlled trials(RCTs)where acupuncture was compared with no treatment,placebo acupuncture or usual care.Data extraction and analysis:Three independent reviewers participated in data extraction and evaluation of risk of bias,and a meta-analysis was conducted.The primary outcome was pain intensity,measured with the visual analog scale,numeric rating scale,or brief pain inventory.Secondary outcomes also assessed were quality of life,functionality,xerostomia,pain interference,and analgesic consumption.Results were expressed as standardized mean difference(SMD)with 95%confidence interval(CI).Results:Sixteen RCTs with a total of 1124 participants were included in the meta-analysis,with the majority of the studies presenting a low or unclear risk of bias.Acupuncture was more effective in reducing pain than no treatment(SMD=–0.90,95%CI[–1.68,–0.12]),sham acupuncture(SMD=–1.10,95%CI[–1.59,–0.61])or usual care(SMD=–1.16,95%CI[–1.38,–0.93]).Conclusion:The results of this study suggest that acupuncture may be an effective intervention to reduce pain associated with cancer.Despite some limitations due to the low quality and small sample size of some included studies,as well as the different types and stages

Relationship between phase angle,steatosis,and liver fibrosis in patients coinfected with human immunodeficiency virus/hepatitis C virus

bACKGROUND Malnutrition,lipodystrophy,and dyslipidemia are prevalent characteristics in patients with human immunodeficiency virus(HIV)infection with or without previous treatment.Such a clinical condition can lead to the hypothesis of the presence of hepatic steatosis with possible progression to fibrosis and the risk of hepatocellular carcinoma.Notably,a low phase angle(PA),evaluated by bioelectrical impedance analysis(bIA),is an independent prognostic marker of clinical progression and survival in HIV-infected patients.AIM To evaluate the relationship between PA and body composition with steatosis and hepatic fibrosis in HIV/hepatitis C virus(HCV)-coinfected patients.METHODS A retrospective observational study by convenience sampling of coinfected HIV/HCV patients,in which all patients underwent transient elastography(Fibroscan)and bIA evaluation.Student’s t test was used for group comparisons,and Spearman’s or Pearson’s correlation test was used when appropriate.The significance level was set at 5%,and analyses were performed using SPSS version 21.0.RESULTS Forty-three patients who received antiretroviral therapy met the inclusion criteria,and 23(53.5%)were under treatment with protease inhibitors(PIs).There was no difference in Pa.b.tween those who used PIs and those who did not(P=0.635).There was no correlation between fibrosis grade and PA(P=0.355)or lean mass(P=0.378).There was a significant inverse correlation between the controlled attenuation parameter(CAP)and lean mass(P=0.378),positive correlation between PA and lean mass(P=0.378),and negative correlation between PA and fatty mass(P=0.378),although the CAP and PA were not correlated.When evaluated by sex,no significant correlations were found.CONCLUSION PA determines the muscle function of HIV/HCV-coinfected patients,and the CAP values reinforce the association with lean mass,suggesting that patients require early nutritional interventions.

Historical relationships of areas of endemism of the brazilian Atlantic rain forest： a cladistic biogeographic analysis of harvestman taxa （Arachnida： Opiliones）

based on a cladistic biogeographic analysis of 6 species-level phylogenies of harvestman taxa, wesearched for congruence in the historical relationships of 12 areas of endemism of the brazilianAtlantic Rain Forest. We constructed general area cladograms using Primary brooks ParsimonyAnaJysis （bPA）, bPA of nodes, and paralogy-free subtree analysis. These analyses resulted in 6general area cladograms, that allow to infer a general pattern of the relationships among areas ofendemism from the brazilian Atlantic Rain Forest. Northern areas resulted related basally showingmain disjunctions at the Doce River Valley and Todos os Santos bay/S^o Francisco River Valley.The remaining areas of endemism were included in a southern and a southeastern block, sepa-rated by the Ribeira do Iguape Valley. Incongruence Length Differences tests showed no significantincongruence among the resulting cladograms and other matrix partitions. We concluded that tec-tonism and ancient marine transgressions were the probable processes responsible for the maindisjunctions, whereas Neogene refugia seem to have caused the more recent disjunctions. Thegeneral pattern and redundancy in area relationships suggest a model of main reiterative barriersin diversification at multiple times for the evolution of the Atlantic Rain Forest. The renewal of cla-distic biogeography and the search for common biogeographic patterns are discussed.

Minimally invasive surgery started spreading worldwide in 1987,when the first laparoscopic cholecystectomy was performed.Meanwhile,improvement of endoscopic equipment and instruments allowed gastroenterologists to attempt more aggressive endoluminal interventions,even beyond the wall barrier.The first transgastric peri-toneoscopy,in 2004,brought to light the concept of natural orifice transluminal endoscopic surgery(NOTES).The idea of incisionless surgery is attractive and has become a new goal for both surgeons and other people interested in this field of investigation.The authors present a review of all developments concerning NOTES,including animal studies and human experience.

Depression is a common,recurrent mental disorder and one of the leading causes of disability and global burden of disease worldwide.Up to 15%-40%of cases do not respond to diverse pharmacological treatments and,thus,can be defined as treatment-resistant depression(TRD).The development of biomarkers predictive of drug response could guide us towards personalized and earlier treatment.Growing evidence points to the involvement of the glutamatergic system in the pathogenesis of TRD.Specifically,the N-methyl-D-aspartic acid receptor(NMDAR)andα-amino-3-hydroxy-5-methyl-4-isoxazolepropionic acid receptor(AMPAR),which are targeted by ketamine and esketamine,are proposed as promising pathways.A literature search was performed to identify studies on the genetics of the glutamatergic system in depression,focused on variables related to NMDARs and AMPARs.Our review highlights GRIN2b,which encodes the NR2b subunit of NMDAR,as a candidate gene in the pathogenesis of TRD.In addition,several studies have associated genes encoding AMPAR subunits with symptomatic severity and suicidal ideation.These genes encoding glutamatergic receptors could,therefore,be candidate genes for understanding the etiopathogenesis of TRD,as well as for understanding the pharmacodynamic mechanisms and response to ketamine and esketamine treatment.

Sustainable graywater reuse for residential landscape irrigation-a critical review

The demand for potable water is rising rapidly due to an ever-increasing population,economic activities,and dwindling water supplies.To provide adequate water supplies in the future,understanding the issues and challenges in the reuse of water and developing appropriate strategies for reuse will be critical.One way to augment water supplies for residential use is to reuse graywater- the wastewater from kitchens,bathrooms,and laundries.In this article,we critically review the evolution of water reuse,the definition of graywater,graywater reuse practices,volumes and flow in different situations,and graywater characteristics.We then examine the issues associated with different graywater treatment methods and how using graywater for irrigation around homes affects soil quality and plant growth.The study concludes that graywater treatment costs,human health risks,and its effect on soil quality are some of the challenges that need to be addressed in the future for widespread and sustainable reuse of graywater for irrigation around homes.

Mathematical simulation of radial heat transfer in packed beds by pseudohomogeneous modeling

Uniform flow regime and constant effective thermal conductivity inside packed beds are commonly accepted in the evaluation of the fluid dynamics and heat transfer in such systems. However, several authors have confirmed the presence of an oscillatory velocity profile caused by the effective contribu~ tion of porosity profile in the fluid dynamic behavior of packed beds, which directly influences the heat transfer inside the beds. This paper describes the application of a pseudo-homogeneous mathematical model for describing heat transfer in packed beds with oscillatory profiles of velocity and porosity, using a radius-dependent model for effective thermal conductivity kr. Several temperature profiles were obtained in a packed bed system with thermal source located on the wall. The simulated temperature and effective thermal conductivity obtained from simulations were compared with experimental data and calculation from a model based on uniform kr fitting. The results indicate that the proposed mathematical modeling was capable of better reDresenting the heat transfer in the packed bed.

bACKGROUND In 2019,cirrhosis accounted for 2.4%of global deaths.The projection for 2030 is an increase in this index.In recent years,hospitalization costs have escalated by 36%for compensated cirrhosis and 24%for decompensated cirrhosis.Therefore,it is necessary to identify a tool capable of predicting the mortality of these patients according to their clinical condition and consequently extending their survival time.Different studies have shown that the phase angle(PA)can be a feasible method in clinical practice,with the potential to guide assertive patient management in the therapeutic of chronic liver disease.AIM To evaluate the prognostic role of PA in cirrhotic patients over a 15-year followup period.METHODS Retrospective cohort study with 129 cirrhotic patients of both sexes over 18 years old.Diagnosis of cirrhosis by liver biopsy.The first year of data collection was 2007,and data regarding outcomes was collected in 2023.Data were gathered from medical records,such as esophageal varices(EV),EV bleeding,ascites,spontaneous bacterial peritonitis(SbP),encephalopathy,laboratory findings and PA.The cut-off value for the PA was 5.4°,a value described in 2012 by Fernandes et al for 129 patients evaluated in this study and the cut-off points for the brazilian population presented in percentiles(P),as described by Mattiello et al.The mortality was assessed using the PA percentile through Kaplan-Meier curves and multivariate binary logistic regression models.RESULTS Patients were divided into two groups according to the PA 5.4th(PA>5.4°,n=40;PA≤5.4°,n=89)PA percentile(ber of relevant complications such as ascites,SbP,liver encephalopathy and HCC.PA is strongly correlated with serum a.b.min(Pbilirubin(P=0.02)and direct bilirubin(P=0.003).PA is correlated with survival time(P<0.001)and length of stay(P=0.02).Logistic

Acute diverticulitis in younger patients:Any rationale for a different approach?

AIM:To compare the natural history and course of acute diverticulitis in a younger age group with an older population and to evaluate whether younger patients should be managed differently. METHODS:This study was a retrospective review of 157 patients treated with acute diverticulitis between January 1,2004 and December 31,2007.Diverticulitis was stratified according to the Hinchey classification. Patients were divided into 2 populations:group A≤ 50 years(n=31) ;group b>50 years(n=126) .Mean patient follow-up was 15 mo. RESULTS:The median age was 60 years.A significantly higher proportion of patients in group b presented with complicated diverticulitis(36.5%vs 12.9%,P=0.01) .Recurrence was more frequent in group A(25.8%vs 11.1%,P=0.03) and the mean time-torecurrence was shorter(12 mo vs 28 mo,P=0.26) . The most severe recurrent episodes of acute diverticulitis were classified as Hinchey stageⅠand none of the patients required emergency surgery.In multivariate analysis,only age(P=0.024) was identified as an independent prognostic factor for recurrence. CONCLUSION:based on the results of this study,we recommend that diverticulitis management should be based on the severity of the disease and not on the age of the patient.
