Treatment satisfaction with rheumatoid arthritis in patients with different disease severity and financial burden:A subgroup analysis of a nationwide survey in China

Background:Despite the recent advances in treatments for rheumatoid arthritis (RA), there are still unmet needs in disease outcomes. This study aimed to analyze the satisfaction with drug therapies for RA according to the levels of disease severity (patientassessed) and proportions of treatment cost to household income.Methods:This was a subgroup study of a cross-sectional study in patients with RA and their physicians. The patients were subdivided into different subgroups based on their self-assessed severity of RA and on the proportions of treatment cost to household income (50%). The Treatment Satisfaction Questionnaire for Medication version II was used to assess patients’ treatment satisfaction.Results:When considering all medications, effectiveness, convenience, and global satisfaction scores were lower in the severe and moderate RA subgroups than those in the mild and extremely mild RA subgroups (all P 50% subgroup (all P 50% subgroups ( F = 12.646, P = 0.005). Global satisfaction score was higher in the <10% subgroup than that in the 31% to 50% subgroup ( F = 8.794, P = 0.032). Conclusion:Higher disease severity and higher financial burden were associated with lower patient satisfaction.

Pre-operative enhanced magnetic resonance imaging combined with clinical features predict early recurrence of hepatocellular carcinoma after radical resection

BACKGROUND Indentifying predictive factors for postoperative recurrence of hepatocellular carcinoma(HCC)has great significance for patient prognosis.AIM To explore the value of gadolinium ethoxybenzyl diethylenetriamine pentaacetic acid(Gd-EOB-DTPA)enhanced magnetic resonance imaging(MRI)combined with clinical features in predicting early recurrence of HCC after resection.METHODS A total of 161 patients with pathologically confirmed HCC were enrolled.The patients were divided into early recurrence and non-early recurrence group based on the follow-up results.The clinical,laboratory,pathological results and Gd-EOB-DTPA enhanced MRI imaging features were analyzed.RESULTS Of 161 patients,73 had early recurrence and 88 were had non-early recurrence.Univariate analysis showed that patient age,gender,serum alpha-fetoprotein level,the Barcelona Clinic Liver Cancer stage,China liver cancer(CNLC)stage,microvascular invasion(MVI),pathological satellite focus,tumor size,tumor number,tumor boundary,tumor capsule,intratumoral necrosis,portal vein tumor thrombus,large vessel invasion,nonperipheral washout,peritumoral enhancement,hepatobiliary phase(HBP)/tumor signal intensity(SI)/peritumoral SI,HBP peritumoral low signal and peritumoral delay enhancement were significantly associated with early recurrence of HCC after operation.Multivariate logistic regression analysis showed that patient age,MVI,CNLC stage,tumor boundary and large vessel invasion were independent predictive factors.External data validation indicated that the area under the curve of the combined predictors was 0.861,suggesting that multivariate logistic regression was a reasonable predictive model for early recurrence of HCC.CONCLUSION Gd-EOB-DTPA enhanced MRI combined with clinical features would help predicting the early recurrence of HCC after operation.

Sperm head morphology is crucial for male factor infertility diagnosis and assessment of male reproductive potential.Severalcriteria are available to analyze sperm head morphology,but they are limited by poor methodology comparability and populationapplicability.This study aimed to explore comprehensive and new normal morphometric reference values for spermatozoa heads infertile Asian males.An automated sperm morphology analysis system captured 23152 stained spermatozoa from confirmed fertilemales.Of these samples,1856 sperm head images were annotated by three experienced laboratory technicians as“normal”.Weemployed 14 novel morphometric features to describe sperm head size(head length,head width,length/width ratio,and girth),shape(ellipse intersection over union,girth intersection over union,short-axis symmetry,and long-axis symmetry),area(head,acrosome,postacrosomal areas,and acrosome area ratio),and degrees of acrosome and nuclear uniformity.This straight-forwardmethod for the morphometric analysis of sperm by accurate visual measurements is clinically applicable.The measured parameterspresent valuable information to establish morphometric reference intervals for normal sperm heads in fertile Asian males.Thepresented detailed measurement data will be valuable for interlaboratory comparisons and technician training.In vitro fertilizationand andrology laboratory technicians can use these parameters to perform objective morphology evaluation when assessing malefertilization potential.

Acute myeloid leukemia(AML)is a rapidly lethal blood cancer characterized by aberrant proliferation and differentiation of myeloid progenitors in the hematopoietic tissue.In recent decades,AML patients are significantly increased.In 2019 alone,21,450 new AML cases were diagnosed in the United States,with an estimated 10,920 deaths due to AML.What’s worse,AML occurs with an increasing incidence with advanced age,and the survival rate remains dismal with a median overall survival of only 5e10 months.Notwithstanding great advances in allogenic transplant and chemotherapy,the therapeutic outcomes are unfavorable due to the disease relapse and mortality rates.More importantly,the development of more specific and effective therapies was hampered in part by the lack of proper animal models for AML.Thus,management and elucidation of the pathological mechanism of AML remain an ongoing challenge,and the development of effective AML mouse models and treatment options are urgently needed.

Hapln1 promotes dedifferentiation and proliferation of iPSC-derived cardiomyocytes by promoting versican-based GDF11 trapping

Hyaluronan and proteoglycan link protein 1(Hapln1)supports active cardiomyogenesis in zebrafish hearts,but its regulation in mammal cardiomyocytes is unclear.This study aimed to explore the potential regulation of Hapln1 in the dedifferentiation and proliferation of cardiomyocytes and its therapeutic value in myocardial infarction with human induced pluripotent stem cell(hiPSC)-derived cardiomyocytes(CMs)and an adult mouse model of myocardial infarction.HiPSC-CMs and adult mice with myocardial infarction were used as in vitro and in vivo models,respectively.Previous single-cell RNA sequencing data were retrieved for bioinformatic exploration.The results showed that recombinant human Hapln1(rhHapln1)promotes the proliferation of hiPSC-CMs in a dose-dependent manner.As a physical binding protein of Hapln1,versican interacted with Nodal growth differentiation factor(NODAL)and growth differentiation factor 11(GDF11).GDF11,but not NODAL,was expressed by hiPSC-CMs.GDF11 expression was unaffected by rhHapln1 treatment.However,this molecule was required for rhHapln1-mediated activation of the transforming growth factor(TGF)-β/Drosophila mothers against decapentaplegic protein(SMAD)2/3 signaling in hiPSC-CMs,which stimulates cell dedifferentiation and proliferation.Recombinant mouse Hapln1(rmHapln1)could induce cardiac regeneration in the adult mouse model of myocardial infarction.In addition,rmHapln1 induced hiPSC-CM proliferation.In conclusion,Hapln1 can stimulate the dedifferentiation and proliferation of iPSC-derived cardiomyocytes by promoting versican-based GDF11 trapping and subsequent activation of the TGF-β/SMAD2/3 signaling pathway.Hapln1 might be an effective hiPSC-CM dedifferentiation and proliferation agent and a potential reagent for repairing damaged hearts.

Rapid prediction of damaged ship roll motion responses in beam waves based on stacking algorithm

Accurate modeling for highly non-linear coupling of a damaged ship with liquid sloshing in waves is still of considerable interest within the computational fluid dynamics(CFD)and AI framework.This paper describes a data-driven Stacking algorithm for fast prediction of roll motion response amplitudes in beam waves by constructing a hydrodynamics model of a damaged ship based on the dynamic overlapping grid CFD technology.The general idea is to optimize various parameters varying with four types of classical base models like multi-layer perception,support vector regression,random forest,and hist gradient boosting regression.This offers several attractive properties in terms of accuracy and efficiency by choosing the standard DTMB 5415 model with double damaged compartments for validation.It is clearly demonstrated that the predicted response amplitude operator(RAO)in the regular beam waves agrees well with the experimental data available,which verifies the accuracy of the established damaged ship hydrodynamics model.Given high-quality CFD samples,therefore,implementation of the designed Stacking algorithm with its optimal combination can predict the damaged ship roll motion amplitudes effectively and accurately(e.g.,the coefficient of determination 0.9926,the average absolute error 0.0955 and CPU 3s),by comparison of four types of typical base models and their various forms.Importantly,the established Stacking algorithm provides one potential that can break through problems involving the time-consuming and low efficiency for large-scale lengthy CFD simulations.

Towards understanding bogus traffic service in online social networks

Critical functionality and huge infuence of the hot trend/topic page(HTP)in microblogging sites have driven the creation of a new kind of underground service called the bogus traffic service(BTS).BTS provides a kind of illegal service which hijacks the HTP by pushing the controlled topics into it for malicious customers with the goal of guiding public opinions.To hijack HTP,the agents of BTS maintain an army of black-market accounts called bogus trafic accounts(BTAs)and control BTAs to generate a burst of fake trafic by massively retweeting the tweets containing the customer desired topic(hashtag).Although this service has been extensively exploited by malicious customers,little has been done to understand it.In this paper,we conduct a systematic measurement study of the BTS.We first investigate and collect 125 BTS agents from a variety of sources and set up a honey pot account to capture BTAs from these agents.We then build a BTA detector that detects 162218 BTAs from Weibo,the largest Chinese microblogging site,with a precision of 94.5%.We further use them as a bridge to uncover 296916 topics that might be involved in bogus trafic.Finally,we uncover the operating mechanism from the perspectives of the attack cycle and the attack entity.The highlights of our findings include the temporal attack patterns and intelligent evasion tactics of the BTAs.These findings bring BTS into the spotlight.Our work will help in understanding and ultimately eliminating this threat.

A fungal CYP from Beauveria bassiana with promiscuous steroid hydroxylation capabilities

Hydroxylation of steroid core is critical to the synthesis of steroid drugs.Direct sp^(3) C-H hydroxylation is challenging through chemical catalysis,alternatively,fungal biotransformation offers a possible solution to this problem.However,mining and metabolic engineering of cytochrome P450 monooxygenases(CYPs)is usually regarded as a more eco-friendly and efficient strategy.Herein,we report the mining and identification of a new steroid CYP(CYP68BE1)from Beauveria bassiana by transcriptomics,heterologous expression,in vivo and in vitro functional characterization.The catalytic promiscuity of CYP68BE1 was explored,and CYP68BE1 showed promiscuously and catalytically versatile,which is qualified for monohydroxylation on C11α,C1α,C6βand dihydroxylation on C1β,11αand C6β,11αof six steroids,leading to the production of key steroid intermediates required in the industrial synthesis of some indispensable steroid drugs.Molecular dynamics simulations were performed,revealing the molecular basis of different binding orientations of CYP68BE1 with different substrates.The discovery of CYP68BE1 offers a promising biocatalyst for enriching the steroid structural and functional diversity,which also can be applied to biosynthesize valuable steroid drug intermediates.

A versatile model with three-dimensional triangular lattice for unconventional transport and various topological effects

The finite Berry curvature in topological materials can induce many subtle phenomena, such as the anomalous Hall effect(AHE), spin Hall effect(SHE), anomalous Nernst effect(ANE), non-linear Hall effect(NLHE) and bulk photovoltaic effects. To explore these novel physics as well as their connection and coupling, a precise and effective model should be developed. Here, we propose such a versatile model—a 3D triangular lattice with alternating hopping parameters, which can yield various topological phases,including kagome bands, triply degenerate fermions, double Weyl semimetals and so on. We reveal that this special lattice can present unconventional transport due to its unique topological surface states and the aforementioned topological phenomena, such as AHE, ANE, NLHE and the topological photocurrent effect. In addition, we also provide a number of material candidates that have been synthesized experimentally with this lattice, and discuss two materials, including a non-magnetic triangular system for SHE, NLHE and the shift current, and a ferromagnetic triangular lattice for AHE and ANE. Our work provides an excellent platform, including both the model and materials, for the study of Berry-curvature-related physics.

Photosynthetic response dynamics in the invasive species Tithonia diversifolia and two co-occurring native shrub species under fluctuating light conditions

To determine the invasiveness of invasive plants,many studies have compared photosynthetic traits or strategies between invasive and native species.However,few studies have compared the photosynthetic dynamics between invasive and native species during light fluctuations.We compared photosynthetic induction,relaxation dynamics and leaf traits between the invasive species,Tithonia diversifolia and two native species,Clerodendrum bungei and Blumea balsamifera,in full-sun and shady habitats.The photosynthetic dynamics and leaf traits differed among species.T.diversifolia showed a slower induction speed and stomatal opening response but had higher average intrinsic water-use efficiency than the two native species in full-sun habitats.Thus,the slow induction response may be attributed to the longer stomatal length in T.diversifolia.Habitat had a significant effect on photosynthetic dynamics in T.diversifolia and B.balsamifera but not in C.bungei.In shady habitat,T.diversifolia had a faster photosynthetic induction response than in full-sun habitat,leading to a higher average stomatal conductance during photosynthetic induction in T.diversifolia than in the two native species.In contrast,B.balsamifera had a larger stomatal length and slower photosynthetic induction and relaxation response in shady habitat than in full-sun habitat,resulting in higher carbon gain during photosynthetic relaxation.Nevertheless,in both habitats,T.diversifolia had an overall higher carbon gain during light fluctuations than the two native species.Our results indicated that T.diversifolia can adopt more effective response strategies under fluctuating light environments to maximize carbon gain,which may contribute to its successful invasion.
