Posterior reversible encephalopathy syndrome in patients with hematologic tumor confers worse outcome

Background:This study aimed to evaluate the clinical features of posterior reversible encephalopathy syndrome(PRES)in ***:The medical records of 31 patients from five medical centers who were diagnosed with PRES from 2001 to 2013 were retrospectively *** the 31 patients,16 were males,and 15 females,with a median age of 7 years(3-12 years).Patients younger than 10 years accounted for 74.2%of the 31 ***:Seizure,the most common clinical sign,occurred in 29 of the 31 *** disturbances were also observed in 20 *** imaging abnormalities were bilateral and predominant in the parietal and occipital white *** this series,three patients died in the acute phase of *** patient had resolution of neurologic presentation within one week,but no apparent improvement in radiological abnormalities was observed at eight *** patient showed gradual recovery of both neurologic presentation and radiological abnormalities during follow-up at eight *** patient developed long-term cortical *** of the PRES patients with hematologic tumor had a worse prognosis than those without hematologic ***:Seizure is a prevalent characteristic of children with *** prognosis can be seen in PRES patients with hematologic tumor.

Functional groups to modify g-C3N4 for improved photocatalytic activity of hydrogen evolution from water splitting

Rational modification by functional groups was regarded as one of efficient methods to improve the photocatalytic performance of graphitic carbon nitride(g-C3 N4).Herein,g-C3 N4 with yellow(Y-GCN)and brown(C-GCN)were prepared by using the fresh urea and the urea kept for five years,respectively,for the first *** results show that the H2 production rate of the C-GCN is 39.06μmol/h,which is about 5 times of the ***,in terms of apparent quantum efficiency(AQ.E)at 420 nm,C-GCN has a value of 6.3%and nearly 7.3 times higher than that of Y-GCN(0.86%).The results of XRD,IR,DRS,and NMR show,different from Y-GCN,a new kind of functional group of—N=CH—was firstly in-situ introduced into the C-GCN,resulting in good visible light absorption,and then markedly improving the photocatalytic *** calculation also confirms the effect of the—N=CH—group band structure of ***,XPS results demonstrate that the existence of—N=CH—groups in C-GCN results in tight interaction between C-GCN and Pt nanoparticles,and then improves the charge separation and photocatalytic *** present work demonstrates a good example of"defect engineering"to modify the intrinsic molecular structure of g-C3N4 and provides a new avenue to enhance the photocatalytic activity of g-C3N4 via facile and environmental-friendly method.

A DRM System Based on Mobile Agent for Digital Rights Redistribution

We propose a digital rights management （DRM） system based on mobile agent to protect the copyrights of content providers. In the system, the content provider creates a time limited blackbox out of an original agent and dispatches it to the user end to enforce DRM functions. The blackbox is an agent that can resist the attacks from the malicious user in a certain time interval. Owing to digital rights redistribution support, the user whose rights belong to redistribution category can transfer his rights to other users. Moreover, by introducing public key infrastructure （PKI） and certificate authority （CA） role, the security of the session can be ensured. An analysis of system security and performance and a comparison with traditional DRM system is given.

AIC AR and Decitabine Enhance the Sensitivity of K562 Cells to Imatinib by Promoting Mitochondrial Activity

Although the advent of tyrosine kinase inhibitors(TKIs)has dramatically improved the survival of patients with chronic myeloid leukaemia(CML),acquired drug resistance and TKI-insensitive leukaemic stem cells(LSCs)remain major obstacles to a CML *** recent years,the reprogramming of mitochondrial metabolism has emerged as a hallmark of cancers,including CML,and in turn may be exploited for therapeutic ***,we investigated the effects of several drugs on the mitochondrial function of the CML cell line K562 and found that 5-aminoimidazole-4-carboxamide ribotide(AICAR)and decitabine could effectively increase the ATP content and mitochondrial *** addition,these two drugs induced cell cycle arrest and a decrease in colony-forming capacity and promoted K562 cell ***,we demonstrated that treatment with AICAR or decitabine enhanced the sensitivity o f K562 cells to imatinib,as evidenced by a combination treatment ***,our findings indicate that TKIs combined with mitochondrial regulation may provide a therapeutic strategy for the treatment of CML.

Over-the-scope clip to close a gastrocutaneous fistula after esophagectomy

Over-the-scope clip(OTSC) system is becoming a new reliable technique which is available for the endoscopic closure of fistulas, bleeding, perforations and so on. We describe the case of a patient with a nonhealing gastrocutaneous fistula after esophagectomy for esophageal squamous cell carcinoma which was successfully closed using an OTSC system. This is the first report of the use of OTSC to treat a nonhealing gastrocutaneous fistula successfully after esophagectomy. We believe our experience will give such patients an ideal way to cure the fistula without suffering too much and also explore new application of OTSC.

Intraoperative radiofrequency ablation combined with ^(125)iodine seed implantation for unresectable pancreatic cancer

AIM: To evaluate the feasibility, efficacy and safety of intraoperative radiofrequency ablation (RFA) combined with 125 iodine seed implantation for unresectable pancreatic cancer. METHODS: Thirty-two patients (21 males and 11 females) at the age of 68 years (range 48-90 years) with unresectable locally advanced pancreatic cancer admitted to our hospital from January 2006 to May 2008 were enrolled in this study. The tumor, 4-12 cm in diameter, located in pancreatic head of 23 patients and in pancreatic body and tail of 9 patients, was found to be unresectable during operation. Diagnosis of pancreatic cancer was made through intraoperative biopsy. Patients were treated with FRA combined with 125 iodine seed implantation. In brief, a RFA needle was placed, which was confirmed by intraoperative ultrasound to decrease the potential injury of surrounding vital structures, a 125 iodine seed was implanted near the blood vessels and around the tumor border followed by bypass palliative procedure (cholangio-jejunostomy and/or gastrojejunostomy) in 29 ***: The serum CA 19-9 level was decreased from 512 ± 86 U/mL before operation to 176 ± 64 U/mL, 108 ± 42 U/mL and 114 ± 48 U/mL, respectively, 1, 3 and 6 mo after operation (P < 0.05). The pain score on day 7 after operation, 1 and 3 mo after combined therapy was decreased from 5.86 ± 1.92 before operation to 2.65 ± 1.04, 1.65 ± 0.88 and 2.03 ± 1.16, respectively, after operation (P < 0.05). The rate of complete response (CR), partial response (PR), stable disease (SD), progressive disease (PD) in 32 patients was 21.8% (7/32), 56.3% (18/32), 15.6% (5/32) and 6.3% (2/32), respectively, 6 mo after operation, with a median overall survival time of 17. 5 mo. The median survival time of patients at stage Ⅲ was longer than that of those at stage Ⅳ (19 mo vs 10 mo, P = 0.0026). The median survival time of patients who received and did not receive chemotherapy after operation was 20 mo and 16 mo, respectively (P = 0.0176).

To address the challenge of treating complex pollutants containing heavy metals and organic compounds,a phenanthroline/TiO_(2)nanocomposite with rich oxygen vacancy defects was synthesized to integrate the functions of pollutant detection, adsorption, and photocatalytic degradation. The results showed that the nanocomposite could adsorb Cr^(3+)and the process could be transduced into a colorimetric signal for qualitative and quantitative detection. The adsorbed heavy metal also exhibited a synergistically enhanced photocatalytic degradation of a model organic pollutant under visible light. The simultaneous adsorption, detection, and photocatalysis could reduce the multifarious operations and high cost of traditional environmental remediation methods, indicating a strong application potential for the nanocomposite.

Dynamic functional and ultrastructural changes of gastric parietal cells induced by water immersion-restraint stress in rats

AIM： To investigate the dynamic functional and ultrastructural changes of gastric parietal cells induced by water immersion-restraint stress （WRS） in ***： WRS model of Sprague-Dawley （SD） rats was established. Fifty-six male SD rats were randomly divided into control group, stress group and post-stress group. The stress group was divided into 1, 2 and 4 h stress subgroups. The post-stress group was divided into 24, 48 and 72 h subgroups. The pH value of gastric juice, ulcer index （UI） of gastric mucosa and H^＋, K^＋- ATPase activity of gastric parietal cells were measured. Ultrastructural change of parietal cells was observed under transmission electron microscope （TEM）.RESULTS： The pH value of gastric juice decreased time-dependently in stress group and increased in post-stress group. The H^＋, K^＋-ATPase activity of gastric parietal cells and the UI of gastric mucosa increased time-dependently in stress group and decreased in poststress group. Compared to control group, the pH value decreased remarkably （P = 0.0001）, the UI and H^＋, K^＋- ATPase activity increased significantly （P = 0.0001, P = 0.0174） in 4 h stress subgroup. UI was positively related with stress time （r = 0.9876, P 〈 0.01） but negatively with pH value （r = -0.8724, P 〈 0.05）. The parietal cells became active in stress group, especially in 4 h stress subgroup, in which plenty of intracellular canalicular and mitochondria were observed under TEM. In post-stress group, the parietal cells recovered to resting ***： The acid secretion of parietal cells is consistent with their ultrastructural changes during the development and healing of stress ulcer induced by WRS and the degree of gastric mucosal lesions, suggesting gastric acid play an important role in the development of stress ulcer and is closely related with the recovery of gastric mucosal lesions induced by WRS.

Factors affecting occurrence of gastric varioliform lesions: A case-control study

AIM: To investigate the factors influencing the occurrence of gastric varioliform lesions(GVLs) and their possible link with gastric ***: A 1:1 matched case-control study was performed to retrospectively analyze data from 1638 chronic gastritis patients who had undergone gastroscopy at one of two Chinese hospitals between 2009 and 2014. Patients with GVLs(cases) were compared to those without such lesions(controls). Endoscopic and pathological findings were recorded, along with interview information on Helicobacter pylori(H. pylori) infection, medical, drug and family histories, lifestyle and eating habits. The association between each factor and the occurrence of GVLs was estimated, and then multivariate conditional logistic regression was used to evaluate the independent ***: The frequency and severity of glandular atrophy, intestinal metaplasia(IM) and low-grade intraepithelial neoplasia were significantly increased in the GVL group(P < 0.01). Overall analysis showed that H. pylori infection [3.051(2.157, 4.317), P <0.001], allergic respiratory diseases [3.636(2.183, 6.055), P < 0.001], work-related stress [2.019(1.568, 2.600), P < 0.001], irregular meals [2.300(1.462, 3.619), P < 0.001], high intake of spicy food [1.754(1.227, 2.507), P = 0.002] and high intake of fresh fruit [0.231(0.101, 0.529), P = 0.001] were significantly correlated with the occurrence of GVLs(positively, except for the latter). Stratified analyses indicated that pickled food consumption in patients over 50 years old [7.224(2.360, 22.115), P = 0.001] and excessive smoking in men [2.013(1.282, 3.163), P = 0.002] were also positively correlated, and that, for antral GVLs, vegetable consumption [0.491(0.311, 0.776), P = 0.002] was negatively correlated. CONCLUSION: Seven risk factors and two protective factors are determined for GVLs, which were found to be associated with premalignant abnormalities.
