Is There Any Threshold for Vitamin D That Elevates Parathyroid Hormone

Objective: To evaluate vitamin D status in relation to serum levels for parathyroid hormone (PTH) and corrected calcium among endocrine outpatients. Methods: A total of 760 patients (mean(SD) age: 40.0(12.6) years, 94.6% were females) admitted to our endocrinology outpatient clinic were included and evaluated with respect to patient demographics, serum levels for 25-hydroxyvitamin D (25(OH) D, ng/mL), PTH (pg/mL) and corrected calcium (mg/dL) as well as the vitamin D status. Results: Vitamin D deficiency was determined in 65.0% and elevated PTH levels in 20.3% of patients. iPTH levels were significantly higher in females than in males (58.9(40.4) vs. 45.1(26.2) pg/mL, p = 0.031) and in summer than in winter cases (63.9(47.7) vs. 54.2(32.8) pg/mL, p = 0.002), whereas no difference was found in serum levels for 25(OH) D, corrected calcium and phosphate with respect to gender and season. Significantly higher levels for iPTH were noted in vitamin D deficient patients (60.7(43.9) pg/mL) than in normal (51.1(33.4) pg/mL) and vitamin D insufficient (57.1(26.0) pg/mL) cases (p = 0.03). iPTH levels were correlated positively with age (r = 0.116, p = 0.001) and negatively with corrected calcium (r = −0.097, p = 0.008), P (r = −0.224, p = 0.000) and 25(OH) D (r = −0.134, p = 0.000), whereas no correlation was noted between 25(OH) D and corrected calcium levels. Conclusion: Our findings indicated that vitamin D deficiency in 65.0%, whereas PTH elevation only in 20.3% of endocrine outpatients, despite the significantly negative correlation of PTH to 25(OH) D and significantly higher levels of PTH among vitamin D deficient cases than in vitamin D insufficient and sufficient cases. Gender’s and seasonal differences had influence on serum levels for PTH but not on either 25(OH) D or corrected calcium, and no correlation was evident between 25(OH) D and corrected calcium levels.

It is already well known that the “when, where and how strong” earthquake prediction problem cannot be solved by only analyzing the database from former earthquakes. A possible solution to this problem is proposed herein based on the analysis of the physicochemical processes as participants in earthquake preparation and on the characteristic rate of reflection of these processes on the Earth’s surface. The proposed procedure includes monitoring of correlation of electromagnetic fields variations with tidal waves.?This solution provides a way of selecting a complex of reliable earthquake precursors using the Inverse Problem Method for earthquakes which will occur in the region around the monitoring point (radial distance ≈ 700 km) in the next seven-day period [1].

目的:研究经计算机控制的Ishihara测试法诊断色盲的可靠性及其与经典Ishihara测试法的一致性,计算机控制的Ishihara测试法的敏感性与特异性;并探讨该测试方法在检测先天性色盲或色弱中的潜在价值。方法:用两种方法测试104名大学生的色觉,年龄20～23岁(中位数21岁)。分别采用经计算机控制的Ishihara色板和经典的Ishihara打印色板显露法对个体色觉进行检查。通过上述两种不同的方法评价每个学生对颜色的感知能力,借此探讨计算机控制的Ishihara测试法的特异性,准确性以及与经典法的一致性。结果:本实验共检测到6位男性色盲患者和1位女性色盲患者。调查该女性学生的家族史,发现其母亲为色盲基因携带者,而其父亲为色盲患者。本实验中男性色盲患病率为13.6%(6/44),女性为1.7%(1/60);整个人群总的患病率为6.7%(7/104)。进行本试验前,这些学生并未意识到自己存在色觉功能缺陷。为探讨这两种方法的一致性,分别对色觉正常和患色盲的学生的测试结果进行比较。结果表明:两种实验方法的特异性及敏感性均为100%,一致性达到100%。结论:计算机控制的Ishihara测试法是数字调控性的,但有关色觉测试的基本概念没有改变。该法总的测试时间和误差计分都已标准化,与手动测试法相比有明显的优势;而且该法与被认为是"金标准"的经典测试法的一致性达到100%。这些特征表明,计算机控制的Ishihara测试法是一种新颖和可靠的方法。

Comparison of myocardial perfusion scintigraphy and computed tomography (CT) angiography based on conventional coronary angiography

Coronary artery disease is one of the most common and important health problems in the world. Early diagnosis of this disease is very important to treat before severe myocardial damage occurred. Myocardial perfusion scintigraphy (MPS) and computed tomography coronary angiography (CTCA) which evaluates regional myocardial perfusion and coronary arteries, respectively, are reliable and non-invasive methods in terms of coronary artery disease. In this study we aimed to compare MPS and CTCA based on conventional coronary angiography (CCA). Totally 60 patients were included in the study. CCA and MPS were performed to 30 patients;CCA and CTCA were performed to the rest of the patients (30 patients). Lesions were classified as mild, moderate and severe in these imaging methods. MPS and CTCA were compared with CCA by using chi-square and Fisher’s exact test. MPS and CTCA’s p values were found for left anterior descending artery (LAD) p: 0, p: 0.271;for circumflex artery (Cx) p: 0.256, p: 0.08 and for right coronary artery (RCA) p: 0.033, p: 0.271, respectively. Furthermore MPS and CTCA’s sensitivity, specificity, accuracy, positive predictive value and negative predictive value were calculated 81% to 87%;70% to 49%;73% to 72%;54% to 72%;90% to 71%, respectively. CCA results were found more concordant with MPS for LAD and RCA lesions and more concordant with CTCA for Cx lesions. It was also found that positive predictive value of MPS and negative predictive value of CTCA were significantly higher than the others. As a result, MPS and CTCA were suggested as complementary techniques for the diagnosis of coronary artery disease, not as alternatives to each other.

Abducens nerve palsy during pegvisomant therapy in an acromegalic patient

Pegvisomant is a recent treatment modality of acromegaly, which is most effective in regulating IGF-1 levels. Tumor growth has been reported in 5% of the acromegaly cases treated with pegvisomant. Herein we report a case of acromegaly resistant to surgical treatment, radiotherapy and medical therapy with a somatostatin analogue and cabergolin. Due to presence of pituitary tumor with cavernous sinus invasion and unremitting disease activity pegvisomant therapy was initiated. She developed left abducens nerve palsy after pegvisomant treatment. The objective of this report is to emphasize the value of close follow-up of the acromely cases under pegvisomant therapy, especially if they have cavernous sinus infiltrating tumor. A 44-year-old woman with acromegaly presented with visual defect and diplopia on left lateral gaze. Her complaint developed 3 years after initiation of pegvisomant therapy. Her neurologic examination was consistent with mild abducens nerve palsy on left side. Other causes for abducens nerve palsy were excluded. Pegvisomant was discontinued upon patient request. Although sella MRI did not reveal a tumor growth, even minimal growth within cavernous sinus can cause cranial nerve involvement. Therefore close and careful follow-up of cavernous sinus infiltrating tumors after pegvisomant therapy is crucial to early recognition of complications.

The relaxor behavior of PLZT ferroelectric ceramics has been analyzed in a wide frequency and temperature ranges,below and above the temperature for the formation of the so-called polar nano-regions(PNRs).An approximation to the dynamical behavior of the PNRs has been discussed using Cole-Cole’s relaxation model and Jonscher’s Universal Relaxation *** analysis considers both the dipolar contribution and those ones associated with DC and AC electric conductivities,this latter not being previously reported in the literature for relaxor *** effectiveness of the developed model has been verified from the agreement between the experimental data and the theoretical *** study also offers an indirect method to predict the DC component of the electrical conductivity.

Drug Resistance Trends and Patterns of Mycobacterium Tuberculosis Isolates from Pulmonary Tuberculosis Patients at a Tertiary Care Hospital in Turkey

Background: We aim to determine the proportions and patterns of resistance to first-line drugs: isoniazid (H), rifampicin (R), ethambutol (E) and streptomycin (S) among pulmonary tuberculosis patients. Methods: Strains were obtained from 1584 culture positive pulmonary tuberculosis patients. All specimens were inoculated into L?wenstein-Jensen media (LJ) and TK selective;drug susceptibility tests (DST) were performed for first-line drugs. Results: Multidrug resistant (MDR) were detected in 146 (9.2%) isolates. Three hundred (18.9%) isolates were resistant to H;220 (13.9%) to R;168 (10.6%) to S;137 (8.6%) to E. Any drug resistance was detected in 442 (27.9%) isolates. MDR rate was higher in male patients than females (P = 0.006). MDR rates were different according to the age groups (P = 0.02). The highest rate was in 35 - 44 years and the lowest rate was in 15 - 24 years. Conclusions: We found an association between middle age and male gender and MDR tuberculosis.

Nilotinib-mediated mucosal healing in a rat model of colitis

目的:调查无视网膜病变的糖尿病和非糖尿病患者接受普通白内障手术时,应用具有前房稳定环境(CASE)和加强控制和效率(ICE)的微脉冲超乳技术对患者超乳参数和黄斑中心凹厚度(CFT)变化的影响。方法:前瞻性研究。研究包含120例患者,其中60例患者为2型糖尿病患者,设为糖尿病组(无视网膜病变),另60例设为对照组。所有患者均接受普通白内障超声乳化术。术中记录超乳参数,包括超乳时间和有效超乳时间。术前和术后1、3mo检测记录CFT并计算每次检测中CFT的差异。结果:糖尿病组的平均超乳时间为1.40±0.43min,而对照组为1.44±0.32min,差异无统计学意义(P=0.85)。糖尿病组平均有效超乳时间为20.12±8.82s,对照组为19.24±9.02s,差异无统计学意义(P=0.964)。糖尿病组术前平均CFT为218.4±12.0μm,对照组为222.1±16.6μm,差异无统计学意义(P=0.168)。术后1mo糖尿病组CFT平均增加30.3±37.2μm,对照组平均增加13.1±12.5μm。术后1mo两组CFT明显增加,糖尿病组显著高于对照组(P=0.001)。术后3mo糖尿病组和对照组的平均CFT较术前分别增加12.5±12.4μm与4.6±9.7μm。糖尿病组CFT的增加显著高于对照组(P=0.00)。但分别比较糖尿病组和对照组术后1mo至3mo平均CFT变化,会发现两组均显著减少(P=0.00,P=0.03)。结论:普通超乳手术会使CFT显著增加。糖尿病组和对照组的超乳参数相似。糖尿病组CFT变化大于对照组,但这些症状大部分表现为亚临床,并且仅显示于光学相干断层扫描(OCT)的改变。术后3mo这种变化会恢复或消失,无需治疗。
