Delaying surgery after neoadjuvant chemoradiotherapy improves prognosis of rectal cancer

AIM To investigate the prognostic effect of a delayed interval between neoadjuvant chemoradiotherapy(CRT) and surgery in locally advanced rectal *** We evaluated 87 patients with locally advanced mid-or distal rectal cancer undergoing total mesorectal excision following an interval period after neoadjuvant CRT at ?i?li Hamidiye Etfal Training and Research Hospital,Istanbul between January 2009 and January *** were divided into two groups according to the intervalbefore surgery: < 8 wk(group Ⅰ) and ≥ 8 wk(group Ⅱ).Data related to patients,cancer characteristics and pathological examination were collected and *** When the distribution of timing between group Ⅰ(n = 45) and group Ⅱ(n = 42) was viewed,comparison of interval periods(median ± SD) of groups showed a significant difference of as 5 ± 1.28 wk in group Ⅰ and 10.1 ± 2.2 wk in group Ⅱ(P < 0.001).The median follow-up period for all patients was 34.5(9.9-81) *** Ⅱ had significantly higher rates of pathological complete response(p CR) than group Ⅰ had(19% vs 8.9%,P = 0.002).Rate of tumor regression grade(TRG) poor response was 44.4% in group Ⅰ and 9.5% in group Ⅱ(P < 0.002).A poor pathological response was associated with worse disease-free survival(P = 0.009).The interval time did not show any association with local recurrence(P = 0.79).CONCLUSION Delaying the neoadjuvant CRT-surgery interval may provide nodal down-staging,improve p CR rate,and decrease the rate of TRG poor response.

A Study to Determine the Impact of Movies on Travel Intention： Anime Movies and Japan Sample

The link between film/drama-induced tourism and the Internet to promote the destination apart from purchase intention has been studied for a long while. By using cinema hall and television, cost-efficient promotion tools were shaped. Anime consisting of digital drawing techniques has been one of the most remarkable examples in terms of promotion of destination and culture as well. In this paper, to evaluate the travel intention of watchers in Turkish anime forums to Japan, open ended questions based on the model of purchase intention were presented to members and followers in two popular anime forums via the Internet. The responses of 31 participants were analyzed. The results demonstrated that participants＇ purchase intention has been enhanced as the consequence of watching anime series.

Services provided at the premarital counseling center and characteristics of the clients, in kayseri, Turkey

Objectives: The study was conducted retro-spectively to evaluate the services offered at the Premarital Counseling Center (PCC) and the characteristics of the individuals who obtained counseling at the center in Kayseri. Methods: A retrospective review is based on data from the Premarital Counseling Center (PCC) in Kayseri. Seven hundred fifty four (754) individuals who attended the Center were included in the study sample. Data were collected from the Center’s records. Records of all people were reviewed by the researchers. Differences in proportions were analyzed by Chi square test and t test. Results: The age-average of the individuals who consulted the PCC was 26.2 ± 8.3, 91.1% of the individuals had Rh (+), 99.5% of them had VDRL (-), 3.9% had Hbs Ag (+), 0.8% had HCV (+) while none of the individuals was HIV (+) affected. It’s shown that 90.6% of the individuals who were between the ages of 15 - 19 were women, 9.4% of them were men. The difference between the age and the gender of the individuals who came to the center was considered significant. It was determined that the individuals who received counseling services at the PCC were not given any kind of educational and consulting services before.

Gallbladder polyps: Factors affecting surgical decision

AIM: To determine the factors affecting the decision to perform surgery, and the efficiency of ultrasonography (USG) in detecting gallbladder polyps (GP). METHODS: Data for 138 patients who underwent cholecystectomy between 1996 and 2012 in our clinic with a diagnosis of GP were retrospectively analyzed. Demographic data, clinical presentation, principal symptoms, ultrasonographic and histopathological findings were evaluated. Patients were evaluated in individual groups according to the age of the patients (older or younger than 50 years old) and polyp size (bigger or smaller than 10 mm) and characteristics of the polyps (pseudopolyp or real polyps). χ 2 tests were used for the statistical evaluation of the data. RESULTS: The median age was 50 (26-85) years and 91 of patients were female. Of 138 patients who underwent cholecystectomy with GP diagnosis, only 99 had a histopathologically defined polyp; 77 of them had pseudopolyps and 22 had true polyps. Twenty-one patients had adenocarcinoma. Of these 21 patients, 11 were male, their median age was 61 (40-85) years and all malignant polyps had diameters > 10 mm (P 10 mm, 22 had true polyps. The sensitivity of USG was 84.6% for polyps with diameters > 10 mm (P 10 mm.

Ultra Mobile Broadband Technology Overview and Competitive Advantages

Diagnostic dilemma: metastatic bone malignancy or primary hyperparathyroidism with brown tumor

Multiple osteolytic lesions are usually associated with metastatic involvement of the bone. However metabolic bone diseases should also take their place in differential diagnosis. Here, we describe a primary hyperparathyroidism case with full-blown osteolytic lesions wich was diagnosed at first sight with having metastatic bone involvement. PET CT scan and laboratory results excluded a metastatic bone malignancy. Elevated serum calcium of 13.16 mg/dl, decreased serum phoshorus of 1.4 mg/dl and high intact-PTH level of 1054.7 pg/ml pointed out primary hyperparathyroidism. Sonographic examination revealed two adenomas of 2.9 × 3.3 mmand 3.3 ×2.7 mmin the left superior and right inferior parathyroid glands, respectively. Scintigraphy confirmed the presence of adenoma on the left.

Background and aims:In this study,the relationship between perceived stress and psychological well-being was analyzed in liver transplant recipients having immunosuppressive ***:This study was performed as descriptive research with 134 patients who had liver transplantation at the organ transplantation institute of a training and research hospital(n?134).The Perceived Stress Scale and the Psychological Well-Being Scale were used for the data *** research data were analyzed by using the Statistical Package for Social Science ***:Of all participant liver transplant recipients,22.4%were aged 18–27 years and 48–57 years and 80.6%were ***,the participant liver transplant recipients used immunosuppressive drugs such as prednisolone(92.53%),tacrolimus(61.9%),and mycophenolate mofetil(52.98%).It was found that there was a statistically significant relationship between psychological well-being and perceived ***:In this study,it was discerned that perceived stress affected psychological well-being *** is of importance to realize the stress experienced by liver transplant recipients and enhance their psychological *** the process following transplant surgery,the liver transplant recipients should be carefully followed and efforts to improve patient outcomes should be made.

Economic Impact of Clinical Research to Research Centers and Opportunity Cost for the Reimbursement System in Turkey

Introduction: Clinical research is a key component of drug development with a significant economic value. It has been reported that the development of a new molecule requires 10 - 15 years and costs almost $1.3 billion. Around 75% of the cost is spent on the Phase I-IV clinical research process. This study aimed to show the economic value of clinical research in Turkey. Methods: Clinical trial budgets were estimated from the raw data of the Report of Istanbul Medical Faculty Clinical Research (ITFKAR). In the research, the estimated cost of drugs used in the clinical trials for the Turkish reimbursement agency (SGK) was calculated to show the cost of medicines acquired through clinical research. Results: The total budget for sponsored pharmaceutical research was $107 million in Turkey, and the government saved close to $311,096,130 during 2006-2010, due to not reimbursing the patients for the drugs in the clinical trials. Conclusion: Despite the limitations of the study, the findings are unique for Turkey. The results can lead to revisiting the importance and economic value of clinical trials in Turkey.

Multiple talon cusps, dens in dente and concrescence in a child with hypomelanosis of Ito

Hypomelanosis of Ito (HI) is a rare neurocutaneous disorder most likely caused by chromosomal mosaicism. HI patients may suffer from numerous clinical manifestations, but the expression of the disease is highly variable. Ophthalmologic, musculoskeletal, neurologic, and dental anomalies may be associated with the syndrome. The dental abnormalities found in HI include talon cusps, a single maxillary central in-cisor, enamel defects, hypodontia, and irregularly spaced teeth. The aim of this case report is to present multiple talon cusps, dens in dente and concrescence of maxillary permanent incisors in an 8-year-old boy affected with hypomelanosis of Ito. This unusual dental anomaly has been described in two previous reports of this neurocutaneous syndrome. HI can be difficult to diagnose, these dental findings may help to identify the syndrome in cases where other findings are minimal or atypical.
