Trends and risk factors of elderly-onset Crohn’s disease: A nationwide cohort study

BACKGROUND The incidence of inflammatory bowel disease(IBD)is increasing in *** risk factors associated with IBD development have been *** To investigate trends and environmental risk factors of Crohn’s disease(CD)diagnosed in persons aged≥40 years in South *** Using the National Health Insurance Service database,a total of 14060821 persons aged>40 years who underwent national health screening in 2009 were followed up until December *** with newly diagnosed CD were enrolled and compared with non-CD *** was identified according to the International Classification of Diseases 10th revision and the rare/intractable disease registration program codes from the National Health Insurance Service *** mean follow-up periods was 7.39 ***,sex,diabetes,hypertension,smoking,alcohol consumption,regular exercise,body mass index,anemia,chronic kidney disease(CKD)and dyslipidemia were adjusted for in the multivariate analysis *** During the follow-up,1337(1.33/100000)patients developed *** in the middle-aged group(40-64 years)had a higher risk than women[adjusted hazard ratio(aHR)1.46,95%confidence interval(CI):1.29-1.66];however,this difference tended to disappear as the age of onset *** the middle-aged group,patients with a history of smoking[(aHR 1.46,95%CI:1.19-1.79)and anemia(aHR 1.85,95%CI:1.55-2.20)]had a significantly higher CD *** the elderly group(age,≥65 years),ex-smoking and anemia also increased the CD risk(aHR 1.68,95%CI:1.22-2.30)and 1.84(95%CI:1.47-2.30,respectively).Especially in the middle-aged group,those with CKD had a statistically elevated CD risk(aHR 1.37,95%CI:1.05-1.79).Alcohol consumption and higher body mass index showed negative association trend with CD incidence in both of the age groups.[Middle-aged:aHR 0.77(95%CI:0.66-0.89)and aHR 0.73(95% CI:0.63-0.84),respectively][Elderly-group:aHR 0.57(95% CI:0.42-0.78)and aHR 0.84(95%CI 0.67-1.04),respectively].For regular p

Immunolocalization of Ephexin-1 in the Developing Canine Cerebellum

Ephexin-1 functions as guanine nucleotide exchange factors for the Rho-type GTPases which have important roles in neuronal development including axon guidance, migration, morphogenesis, and plasticity of neurons. As little is known about ephexin-1 in the cerebellum, we investigated the immunolocalization of ephexin-1 in the developing canine cerebellum. While the cellular maturation was followed by the temporal pattern, the calbindin D-28k and ephexin-1 immunoreactivities gradually increased in developing canine cerebellum. When compared to the calbindin D-28k immunoreactivities, belated ephexin-1 immunolocalization was observed in the Purkinje cells which aligned a single layer during cerebellar development. These results suggest that ephexin-1 might play an important role in the development of the Purkinje cells during the first two postnatal weeks based on its immunolocalization in the present study.

Reference Coordinate System of Nonlinear Beam Element Based on the Geometrically Exact Formulation under Large Spatial Rotations and Deformations

Analysis of slender beam structures in a three-dimensional space is widely applicable in mechanical and civil engineering. This paper presents a new procedure to determine the reference coordinate system of a beam element under large rotation and elastic deformation based on a newly introduced physical concept: the zero twist sectional condition, which means that a non-twisted section between two nodes always exists and this section can reasonably be regarded as a reference coordinate system to calculate the internal element forces. This method can avoid the disagreement of the reference coordinates which might occur under large spatial rotations and deformations. Numerical examples given in the paper prove that this procedure guarantees the numerical exactness of the inherent formulation and improves the numerical efficiency, especially under large spatial rotations.

Magnesium citrate with a single dose of sodium phosphate for colonoscopy bowel preparation

AIM:To evaluate the efficacy and acceptability of magnesium citrate and a single dose of oral sodium phosphate(45 mL) solution for morning colonoscopy bowel preparation. METHODS:A total of 159 patients were randomly assigned to receive two split doses of 90 mg of sodium phosphate(GroupⅠ,n=79) or magnesium citrate(250 mL,the day before the procedure) followed by 45 mL of sodium phosphate(the day of procedure,GroupⅡ,n= 80) .The quality of bowel cleansing and the acceptability of each regimen were compared,including the satisfaction,taste,willing to repeat and adverse effects of each regimen. RESULTS:The quality of bowel cleansing of GroupⅡ was as good as that of GroupⅠ(An Aronchick scale score of good or excellent:70.9%vs 81.0%,respectively,P=0.34;the Ottawa system score:4.4±2.6 vs 3.8 ±3.0,respectively,P=0.76) .There was no statisticallysignificant difference between both groups with regard to acceptability,including the satisfaction,taste and willingness to repeat the regimen.A significantly greater number of older patients(over 65 years old) in Group Ⅱgraded the overall satisfaction as satisfactory(48.1% vs 78.1%,respectively;GroupⅠvs GroupⅡ,P=0.01) . There were no significant adverse reactions. CONCLUSION:Magnesium citrate and a single dose of sodium phosphate was as effective and tolerable as the conventional sodium phosphate regimen and is a satisfactory option.

This paper aims to identify and clarify the cyber security risks and their interaction with the power system in Smart Grid. The SCADA system and other communication networks interact with the power system on a real time basis, so it is important to understand the interaction between two layers to protect the power system from potential cyber threats. This paper has shown the risks of the open architecture SCADA in a quantitative method and proposed effective security measures through case studies.

BACKGROUND Clinical factors predicting graft survival(GS)after ABO-incompatible(ABOi)liver transplantation(LT),and differences between recipients with and without hepatocellular carcinoma(HCC)are *** To analyze the impact of serial serum tacrolimus trough concentration in recipients with or without(HCC)in ABOi living-donor liver transplantation(LDLT).METHODS We analyzed a historical cohort of 89 recipients who underwent ABOi LDLT,including 47 patients with *** The 1-,3-,5-,and 10-year GS rates were 85.9%,73.3%,71.4%,and 71.4%,respectively,and there were no significant differences between HCC and non-HCC *** multivariate Coxregression analyses,tacrolimus trough concentrations below 5.4 ng/mL at 24 wk post-LT,in addition to the antibody-mediated rejection(AMR)were associated with poor-graft *** HCC patients,AMR[hazard ratio(HR)=63.20,P7.3 ng/mL were significant predictive factors for HCC *** propensity score matching,patients with high tacrolimus concentrations at 4 wk had significantly poor recurrence-free *** Elevated tacrolimus levels at 4 wk after ABOi LDLT have been found to correlate with HCC ***,careful monitoring and control of tacrolimus levels are imperative in ABOi LT recipients with HCC.

A Study on Release Property through Self Coating in LSR Injection Process

This study explores the molding processes by using the Multi-component-injection molding (MCM) method, in which monolithic products are molded by attaching mixed liquid-silicone-rubbers (LSRs) to polybutylene terephthalate (PBT) that has the characteristic of self-binding while burning due to Pt catalysts. It is seen that this method increases the binding force between LSR and PBT. The surfaces coated with Polytetrafluoroethylene (PTFE), Cr, Ni, etc. have excellent peeling with plastic and such surfaces are compared with those treated with hot forging die and micro blasting without coating. When peeling tests are performed at the specified polymerization temperature and molding time after LSR molding on these surfaces of hot working die molds without coating, these surfaces show excellent peeling of molds and LSR products. In particular, they show better peeling after micro blasting than surfaces with Cr and Ni coating as well as surfaces without coating, and the peeling strength also decreases. The results of contact angle and XPS analysis indicate that the LSR binding force is enhanced as an effect of catalysts, though it is not found by SEM. According to the XPS analysis, the structures of the surfaces are close to methyl and vinyl materials that are produced while LSR molding.

Correlations between endoscopic and clinical disease activity indices in intestinal Behcet's disease

AIM:To develop a novel endoscopic severity model of intestinal Behcet's disease(BD) and to evaluate its feasibility by comparing it with the actual disease activity index for intestinal Behcet's disease(DAIBD).METHODS:We reviewed the medical records of 167 intestinal BD patients between March 1986 and April *** also investigated the endoscopic parameters including ulcer locations,distribution,number,depth,shape,size and margin to identify independent factors associated with *** endoscopic severity model was developed using significant colonoscopic variables identified by multivariate regression analysis and its correlation with the DAIBD was *** determine factors related to the discrepancy between endoscopic severity and clinical activity,clinical characteristics and laboratory markers of the patients were ***:A multivariate regression analysis revealed that the number of intestinal ulcers(≥ 2,P = 0.031) and volcanoshaped ulcers(P = 0.001) were predictive factors for the *** endoscopic severity model(Y) was developed based on selected endoscopic variables as follows:Y = 47.44 + 9.04 × non-Ileocecal area + 11.85 ×≥ 2 of intestinal ulcers + 5.03 × shallow ulcers + 12.76 × deep ulcers + 4.47 × geographicshaped ulcers + 26.93 × volcano-shaped ulcers + 8.65 ×≥ 20 mm of intestinal ***,endoscopic parameters used in the multivariate analysis explained only 18.9% of the DAIBD *** with severe DAIBD scores but with moderately predicted disease activity by the endoscopic severity model had more symptoms of irritable bowel syndrome(21.4% vs 4.9%,P = 0.026) and a lower rate of corticosteroid use(50.0% vs 75.6%,P = 0.016) than those with severe DAIBD scores and accurately predicted disease by the ***:Our study showed that the number of intestinal ulcers and volcano-shaped ulcers were predictive factors for severe DAIBD ***,the correlation between endoscopic severity and DAIBD(r = 0.434) was weak.
