Energy storage and conservation are receiving increased attention due to rising global energy demands.Therefore,the development of energy storage materials is crucial.Thermal energy storage(TES)systems based on phase change materials(PCMs)have increased in prominence over the past two decades,not only because of their outstanding heat storage capacities but also their superior thermal energy regulation capability.However,issues such as leakage and low thermal conductivity limit their applicability in a variety of settings.Carbon-based materials such as graphene and its derivatives can be utilized to surmount these obstacles.This study examines the recent advancements in graphene-based phase change composites(PCCs),where graphene-based nanostructures such as graphene,graphene oxide(GO),functionalized graphene/GO,and graphene aerogel(GA)are incorporated into PCMs to substantially enhance their shape stability and thermal conductivity that could be translated to better storage capacity,durability,and temperature response,thus boosting their attractiveness for TES systems.In addition,the applications of these graphene-based PCCs in various TES disciplines,such as energy conservation in buildings,solar utilization,and battery thermal management,are discussed and summarized.

The Malaysian cataract surgery registry:incidence and risk factors of postoperative infectious endophthalmitis over a 7-year period

AIM：To report the incidence,risk factors and visual outcomes for postoperative endophthalmitis（POE）based on 7-year data from the Malaysian Ministry of Health Cataract Surgery Registry（MOH CSR）.METHODS：Data was collected from the web-based MOH CSR.All consecutive cataract surgery patients from 1st June 2008 to 31st December 2014 were identified.Exclusion criteria were traumatic cataract or previous ocular surgery.Demographic data,ocular co-morbidities,intraoperative details and postoperative visual acuity（VA）at final ophthalmological follow-up were noted.All eyes were taken for analysis.Subjects with POE were compared against subjects with no POE for risk factor assessment using multiple logistic regressions.RESULTS：A total of 163 503 subjects were screened.The incidence of POE was 0.08%（131/163 503）.Demographic POE risk factors included male gender（OR：2.121,95%CI：1.464-3.015）and renal disease（OR：2.867,95%CI：1.503-5.467）.POE risk increased with secondary causes of cataract（OR：3.562,95%CI：1.740-7.288）,uveitis（OR：11.663,95%CI：4.292-31.693）and diabetic retinopathy（OR：1.720,95%CI：1.078-2.744）.Intraoperative factors reducing POE were shorter surgical time（OR：2.114,95%CI：1.473-3.032）,topical or intracameral anaesthesia（OR：1.823,95%CI：1.278-2.602）,posterior chamber intraocular lens（PCIOL;OR：4.992,95%CI：2.689-9.266）and foldable IOL（OR：2.276,95%CI：1.498-3.457）.POE risk increased with posterior capsule rupture（OR：3.773,95%CI：1.915-7.432）and vitreous loss（OR：3.907,95%CI：1.720-8.873）.Postoperative VA of 6/12 or better was achieved in 15.27%（20/131）subjects with POE.CONCLUSION：This study concurs with other studies regarding POE risk factors.Further strengthening of MOH CSR data collection process will enable deeper analysis and optimization of POE treatment.

Barcelona Clinic Liver Cancer outperforms Hong Kong Liver Cancer staging of hepatocellular carcinoma in multiethnic Asians: Real-world perspective

AIM To compare the Barcelona Clinic Liver Cancer(BCLC) and Hong Kong Liver Cancer(HKLC) classification systems when applied to HCC patients from the largest tertiary-level centre in Singapore.METHODS One thousand two hundred and seventy hepatocellular carcinoma(HCC) patients prospectively enrolled in a tertiary-level centre registry in Singapore since 1988 were studied. Patients were grouped into their respective BCLC and HKLC stages. Data such as demography, aetiology of HCC and type of treatment were collected. Survival data was based on census with the National Registry of Births and Deaths on 31 st October 2015. Statistical analyses were done using SPSS version 21(Chicago, IL, United States). Survival analyses were done by the Kaplan-Meier method. Differences in survival rates were compared using the log-rank test.RESULTS The median age at presentation was 63 years(range 13-94); male 82.4%; Chinese 89.4%, Malay 7.1%, Indian, 2.8%. Hepatitis B was the predominant aetiology(75.0%; Hepatitis C 7.2%, Hepatitis B and C co-infection 3.8%, non-viral 14.0%). Both BCLC and HKLC staging systems showed good separation with overall log rank test confirming significant survival differences between stages in our cohort(P < 0.001). 206 out of the 240 patients(85.8%) assigned for curative treatment by the BCLC treatment algorithm received curative therapy for HCC [Stage 0 93.2%(68/73); Stage A 82.6%(138/167)]. In contrast, only 341/558(61.1%) patients received curative treatment despite being assigned for curative treatment by the HKLC treatment algorithm [Stage Ⅰ 72.7%(264/363); Stage Ⅱ 40.2%(66/164); Stage Va 35.5%(11/31)]. Patients who were assigned to curative treatment by HKLC but did not receive curative treatment had significantly poorer ECOG(P < 0.001), higher ChildPugh status(P < 0.001) and were older(median age 66 vs 61, P < 0.001) than those who received curative therapy. Median overall survival in patients assigned to curative treatment groups by BCLC and HKLC were 6.1 and 2.6 years respectively(P < 0.001). When

Low serum albumin predicts early mortality in patients with severe hypoxic hepatitis

AIM To evaluate the incidence, etiology, and predictors of mortality of severe hypoxic hepatitis. METHODS We used computerized patient records to identify consecutive cases of severe hypoxic hepatitis admitted to a tertiary hospital in Singapore over a one-year period. We defined severe hypoxic hepatitis as elevation of serum transaminases more than 100 times upper limit of normal in the clinical setting of cardiac, circulatory or respiratory failure after exclusion of other causes of hepatitis. We used multivariable regression analysis to determine predictors for mortality. RESULTS We identified 75 cases of severe hypoxic hepatitis out of 71380 hospital admissions over one year, providing an incidence of 1.05 cases per 1000 admissions. Median age was 65 years(range 19-88); 57.3% males. The most common etiologies of severe hypoxic hepatitis were acute myocardial infarction and sepsis. Fifty-three patients(71%) died during the hospitalization. The sole independent predictive factor for mortality was serum albumin measured at the onset of severe hypoxic hepatitis. Patients with low serum albumin of less than 28 g/L have more than five-fold increase risk of death(OR = 5.39, 95%CI: 1.85-15.71).CONCLUSION Severe hypoxic hepatitis is uncommon but has a high mortality rate. Patients with low serum albumin are at highest risk of death.

Clinical applications, limitations and future role of transient elastography in the management of liver disease

Transient elastography(TE) is a reliable tool for the non-invasive assessment of liver fibrosis in routine clinical practice. TE is currently approved for use in Europe, Asia and the United States. The widespread adoption of this technology is certain to increase the use of TE worldwide. Although TE has been well validated in chronic viral hepatitis, its clinical role in other liver diseases remains less clear. The advent of new treatment for chronic hepatitis C and emerging prevalence of non-alcoholic steatohepatitis raises new questions on the role of TE in current clinical practice. This review aims to examine the clinical applications, limitations and future role of TE in current clinical practice in light of the changing epidemiology of liver diseases and new clinical management paradigms. In current clinical practice, TE is the most accurate noninvasive method for diagnosis of liver cirrhosis. TE is useful to rule out fibrosis and cirrhosis but does not have sufficient accuracy to discern between various stages of fibrosis. The clinical role of TE has evolved from cross-sectional point-in-time assessment of fibrosis and cirrhosis to the more relevant role of prediction of vital clinical end-points. This provides clinicians with the ability to modify treatment strategies based on the information provided by TE. TE has evolved over the past decade to become an essential tool to assist the clinician in the management of chronic liver disease.

Systematic review of the outcomes of surgical resection for intermediate and advanced Barcelona Clinic Liver Cancer stage hepatocellular carcinoma:A critical appraisal of the evidence

AIM To perform a systematic review to determine the survival outcomes after curative resection of intermediate and advanced hepatocellular carcinomas(HCC).METHODS A systematic review of the published literature was performed using the PubM ed database from 1 st January 1999 to 31 st Dec 2014 to identify studies that reported outcomes of liver resection as the primary curative treatment for Barcelona Clinic Liver Cancer(BCLC) stage B or C HCC. The primary end point was to determine the overall survival(OS) and disease free survival(DFS) of liver resection of HCC in BCLC stage B or C in patients with adequate liver reserve(i.e., Child's A or B status). The secondary end points were to assess the morbidity and mortality of liver resection in large HCC(defined as lesions larger than 10 cm in diameter) and to compare the OS and DFS after surgical resection of solitary vs multifocal HCC.RESULTS We identified 74 articles which met the inclusion criteria and were analyzed in this systematic review. Analysis of the resection outcomes of the included studies were grouped according to(1) BCLC stage B or C HCC,(2) Size of HCC and(3) multifocal tumors. The median 5-year OS of BCLC stage B was 38.7%(range 10.0-57.0); while the median 5-year OS of BCLC stage C was 20.0%(range 0.0-42.0). The collective median 5-year OS of both stages was 27.9%(0.0-57.0). In examining the morbidity and mortality following liver resection in large HCC, the pooled RR for morbidity [RR(95%CI) = 1.00(0.76-1.31)] and mortality [RR(95%CI) = 1.15(0.73-1.80)] were not significant. Within the spectrum of BCLC B and C lesions, tumors greater than 10 cm were reported to have median 5-year OS of 33.0% and multifocal lesions 54.0%.CONCLUSION Indication for surgical resection should be extended to BCLC stage B lesions in selected patients. Further studies are needed to stratify stage C lesions for resection.

Intra-tumor heterogeneity(ITH) is a key challenge in cancer treatment, but previous studies have focused mainly on the genomic alterations without exploring phenotypic(transcriptomic and immune)heterogeneity. Using one of the largest prospective surgical cohorts for hepatocellular carcinoma(HCC)with multi-region sampling, we sequenced whole genomes and paired transcriptomes from 67 HCC patients(331 samples). We found that while genomic ITH was rather constant across stages, phenotypic ITH had a very different trajectory and quickly diversified in stage Ⅱ patients. Most strikingly, 30% of patients were found to contain more than one transcriptomic subtype within a single tumor. Such phenotypic ITH was found to be much more informative in predicting patient survival than genomic ITH and explains the poor efficacy of single-target systemic therapies in HCC. Taken together, we not only revealed an unprecedentedly dynamic landscape of phenotypic heterogeneity in HCC, but also highlighted the importance of studying phenotypic evolution across cancer types.

Network pharmacology used to decode potential active ingredients in Ferula assafoetida and mechanisms for the application to Alzheimer’s disease

Background:Alzheimer’s disease(AD)is a neurodegenerative disease that primarily manifests as progressive memory loss and cognitive impairment.Traditional herbal medicines may be helpful in the discovery of new anti-AD drugs.Studies have shown that Ferula assafoetida has neuroprotective and memory-enhancing effects,which may be beneficial for the treatment of AD.However,the combination of active ingredients and their mechanisms remain unclear.Therefore,we aimed to identify potential active ingredients in F.assafoetida and their mechanisms of action against AD by using network pharmacology.Methods:In our study,an integrated network pharmacological approach,that included adsorption,distribution,metabolism and excretion screening,target identification,network construction,topological analysis,gene ontology and Kyoto Encyclopedia of Genes and Genomes pathway enrichment analysis,and molecular docking,was used to predict the pharmacological material basis and potential mechanisms through which these ingredients may treat and prevent AD.Results:The results indicated that 12 key active ingredients,obtained by topological analysis(including farnesiferol a,conferol,farnesiferol b,ferulic acid,etc.),may be the primary pharmacological components that may ameliorate AD.The 2 key significant pathways identified are the cholinergic synapse signaling pathway(critical targets include ACHE,CHRM1,CHRM2,MAPK1,pik3CA,pik3CB,pik3CD,and pik3CG)and the AD signaling pathway(critical targets include APP,BACE1,GSK3B,MAPK1,NCSTN,NOS1,PSEN1).These critical targets are closely related to the regulation of three typical pathological features of AD[central nervous system(CNS)cholinergic hypofunction,amyloid-β(Aβ)plaques,and hyperphosphorylated tau proteins].Finally,14 critical targets in the 2 key significant pathways were validated by molecular docking analysis.Conclusion:F.assafoetida may be effective for alleviating AD symptoms,through multi-component,multi-target,and multi-pathway synergistic effects,associated with the multiple pathogenesis

用种子乳液聚合法制备了含氟乳液,借助AFM、XPS对所制备的自交联含氟乳液成膜进行表征、分析,利用goh模型求算出聚合物链段相互扩散系数,考察成膜温度对自交联含氟乳液成膜的影响。结果表明,成膜温度升高,有利于提高乳胶膜的交联密度和聚合物链段的扩散速度;而涂膜表面氟原子的含量在成膜温度小于50℃时随温度的升高而增大,在50℃以上时随温度的升高而减小,成膜温度并非越高越好。自交联含氟乳液成膜过程中,三维网状结构的形成、氟化组分迁移和聚合物链段相互扩散会出现相互影响和彼此的竞争。

目的:检测心房颤动(AF)病人与窦性心律者血清溶质载体家族7成员11(SLC7A11)、血清成纤维细胞生长因子23(FGF23)的水平,分析二者与AF之间的相关性及临床意义。方法:选取住院的AF病人118例作为观察组,根据相关指南分为阵发性AF组67例和非阵发性AF组51例。对照组选取窦性心律健康者96名。选择酶联吸附免疫实验法(ELISA)测出血清中SLC7A11、FGF23浓度;比较3组病人的临床资料及血清学指标,利用Pearson相关性分析血清SLC7A11、FGF23水平与超声心动图中左房内径(LAD)、左室舒张内径(LVD)、左心室射血分数(LVEF)和左心室缩短分数(FS)相关性。采用多元logsitic回归分析AF病人AF发生持续相关因素。结果:与对照组相比,血清SLC7A11在阵发性AF组和非阵发性AF组均下降(P<0.01),且非阵发性AF组中SLC7A11低于阵发性AF组(P<0.01),血清FGF23在阵发性AF组和非阵发性AF组均升高(P<0.01),且非阵发性AF组中FGF23高于阵发性AF组(P<0.01);Pearson相关性分析显示,LAD与血清SLC7A11呈负相关关系(r=-0.534,P<0.01),与血清FGF23呈正相关关系(r=0.532,P<0.01)。多元logsitic回归分析结果显示,SLC7A11是AF独立的保护因素(OR=0.231,P<0.01),而FGF23是独立危险因素(OR=1.097,P<0.01)。结论:SLC7A11、FGF23可能与AF的发病、进展有关。
